Objectives: Muscles and their tendons present a considerable diversity of morphological variations. The aim of this study was to explore variants of muscles and tendons from compartments of the thigh and to raise awareness about potential problems during ultrasound examination. Materials and Methods: This comprehensive review of the literature was created on the basis of scientific articles sourced from PubMed. The search included all relevant papers related to the topic, ensuring that the most up-to-date studies were incorporated. In order to achieve these results, we created the exclusion criteria and extracted papers that did not meet the requirements of our review. Relevant papers were incorporated, and tracking of citations was fulfilled. The described method allowed for a broad yet detailed understanding, ensuring that the review of the literature covers all key aspects of the presented research. Results: Various aspects of thigh muscle anomalies were already undertaken; however, as this study has shown, current knowledge, while valuable, is insufficient to draw definitive conclusions regarding the prevalence and clinical implications of these muscle variations. A more robust body of ultrasound-based research is essential to accurately characterize these anomalies, establish their frequency, and assess their impact on clinical decision-making, including diagnostic accuracy, surgical planning, and therapeutic interventions. Conclusions: Numerous anatomical variations of the thigh muscles and tendons that were described in literature over the years might have clinical implications and could lead to mistakes during diagnosis by ultrasound imaging. Degloving muscle injury was described for the rectus femoris where the inner bipennate component is dissociated from its superficial unipennate component. The semimembranosus muscle displays a distinctive dual morphology, featuring both unipennate and bipennate muscle fibers. Nevertheless, this specific tear pattern has not been previously documented. Conversely, the adductor longus muscle showcases an elongated intramuscular tendon segment, indicating a multipennate morphology. We present two separate cases of previous undescribed degloving injuries of the semimembranosus and the adductor longus in teenage soccer players with MRI and ultrasound diagnosis, ultrasound-guided hematoma aspiration, and recovery timelines for return-to-play. Purpose: Resistance training may be empirically believed to increase passive muscle stiffness. Meanwhile, a recent study showed that the passive stiffness of a specific hamstring muscle acutely decreased after eccentric-only resistance exercise at long muscle lengths with a long contraction duration (LL). To extend this finding, the present study investigated the chronic effects of eccentric-only resistance training with LL at different weekly frequencies on the passive stiffness of the biarticular hamstring muscles. Methods: Thirty-six healthy young males were assigned into two training groups with two and three weekly sessions (W2 and W3; n = 12, respectively) and a control group (CON; n = 12). The participants in both training groups performed eccentric-only stiff-leg deadlift at 50%–100% of exercise range of motion (0% = upright position) with 5 s per repetition for 10 wk. Before and after the intervention period, the shear moduli of the biarticular hamstring muscles, maximal voluntary isometric torque of knee flexion, and volumes of the individual hamstring muscles were measured. Results: In W3, the shear modulus of the semimembranosus (−11.4%) significantly decreased, whereas those of the other biarticular muscles did not change. There were no significant changes in the shear moduli of the biarticular hamstring muscles in W2 or CON. The isometric torque (20.3% and 26.2%, respectively) and semimembranosus volume (5.7% and 7.4%, respectively) were significantly increased in W2 and W3. Conclusions: Passive stiffness of a specific muscle could be chronically decreased when eccentric-only resistance training with LL is performed at a relatively high weekly frequency with a high total training volume. Abstract The meniscal position within the knee is critical to maintain normal knee function. The joint capsule might dynamically coordinate the medial meniscus (MM) by transmitting a semimembranosus action. However, their interrelationships in vivo are unclear. We aimed to determine relationships among the MM, joint capsule, and semimembranosus during passive tibial external-internal and isometric tibial internal rotation at the medial and posteromedial knees of 10 healthy individuals in vivo using ultrasound. We analyzed images of the MM and joint capsule locations at the medial and posteromedial knee and the velocity waveform similarity of each structure during rotational tasks. Both isometric internal rotation with semimembranosus action and passive tibial external rotation displaced the MM inward at the medial knee. The MM and joint capsule during these MM displacements coordinately moved with more than moderate cross-correlation coefficients (passive external and isometric internal rotations, ≥ 0.54 and ≥ 0.90, respectively). The movements of the MM and joint capsule to the semimembranosus during isometric internal rotation also coordinated with moderate cross-correlation coefficients (≥ 0.62). Therefore, the joint capsule might dynamically coordinate the MM by transmitting semimembranosus action. Whether increased tibial internal rotation or semimembranosus shortening causes MM extrusion awaits further investigation.

Background: To determine if there is an association between semimembranosus tendinosis and medial meniscal tears using MRI. Methods: A retrospective review of knee 3T MRI scans was performed to determine the presence or absence of medial meniscal tears in patients with semimembranosus tendinosis. All studies were interpreted by two musculoskeletal radiologists. Univariate association for the presence of semimembranosus tendinosis and medial meniscal tears was performed with a Chi-square test followed by logistic regression modelling among statistically significant associations. Results: A total of 150 knee MRI scans were reviewed (age 32.8 ± 7.1 years; 70 females). Semimembranosus tendinosis was present in 66 knees (44%) in the patient population. Semimembranosus tendinosis was present in 81% of patients with meniscal tears versus 36% of patients without meniscal tears (p < 0.0001). This association remained statistically significant when adjusted for age and sex with an adjusted odds ratio of 7.0 (p < 0.0003). Models adjusted for the above covariates and containing the interaction term produced an adjusted odds ratio of 13.0 (p < 0.0001) in men, while in women this association was non-significant with an adjusted odds ratio of 2.0 (p = 0.42). Conclusions: Subjects with semimembranosus tendinosis were seven times more likely to have medial meniscal tears even when adjusting for sex and age. Background: Medial meniscal ramp lesions have recently been an area of interest because of their recognized prevalence in association with anterior cruciate ligament (ACL) ruptures. Anatomically, the medial meniscal ramp is composed of the meniscocapsular ligament in continuity with the semimembranosus muscle and the meniscotibial ligament. Diagnosis of ramp semimembranosus complex (RSC) injuries remains challenging, and their prevalence is likely to be underestimated in comparison with ramp lesions. Purpose: To determine the prevalence of RSC lesions after a complete ACL rupture. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A retrospective database analysis was performed. The cohort consisted of the first 100 patients with complete ACL rupture confirmed by magnetic resonance imaging (MRI) who underwent knee arthroscopy for ACL reconstruction in 2019. The semimembranosus lesions were identified using MRI by 2 independent radiologists specializing in musculoskeletal imaging. The ramp lesions were initially diagnosed using MRI and then confirmed during arthroscopy by an experienced knee surgeon. The magnitude of rotatory instability was recorded using the pivot-shift test. A multivariate analysis was used to determine the lesions associated with the semimembranosus complex. Results: Of 100 patients, 53 showed lesions of the RSC; among them, 30 ramp lesions were confirmed after arthroscopic evaluation, and 40 semimembranosus lesions (23 without ramp lesion and 17 with ramp lesion) were found using MRI. A positive pivot shift was present in 57% of patients with combined RSC injury (P =.04) compared with 36% in patients without an RSC lesion. Conclusion: Lesions of the RSC were found in more than half of ACL ruptures in this retrospective cohort. Rotational instability could be associated with combined ACL and RSC injury. Soccer teams integrate specific exercises into their typical workout programs for injury prevention. This study examined the effects of hamstring exercise on paraspinal and hamstring stiffness. These findings can inform training and rehabilitation programs to improve muscle health and prevent injuries. Fifteen young, healthy males performed passive and active (submaximal) knee flexion efforts from 0°, 45°, to 90° angle of knee flexion from the prone position. Using shear-wave elastography (SWE) and surface electromyography, we measured the elastic modulus and root mean square (RMS) signal of the erector spinae (ES), multifidus (MF), semitendinosus (ST), and semimembranosus (SM) during different knee flexion angles. Passive SWE modulus at 0° was 12.44 ± 4.45 kPa (ES), 13.35 ± 6.12 kPa (MF), 22.01 ± 4.68 kPa (ST), and 21.57 ± 5.22 kPa (SM) and it was greater (p < 0.05) compared to 45° and 90°. The corresponding values during knee flexion contractions at 0° increased to 18.99 ± 6.11 kPa (ES), 20.65 ± 11.31 kPa (MF), 71.21 ± 13.88 kPa (ST), and 70.20 ± 14.29 kPa (SM) and did not differ between angles (p > 0.05). Compared to rest, the relative increase in the SWE modulus during active contraction had a median value (interquartile range) ranging from 68.11 (86.29) to 101.69 (54.33)% for the paraspinal muscles and it was moderately to strongly correlated (r > 0.672) with the corresponding increase of the hamstring muscles [ranging from 225.94 (114.72) to 463.16 (185.16)%]. The RMS signal was greater during active compared to passive conditions, and it was lower at 90° compared to 45° (for SM/ST) and 0° (for all muscles). The association between paraspinal and hamstring passive muscle stiffness indicates a potential transmission of forces through myofascial connections between the lumbar spine and the lower limbs. Purpose: The hamstrings muscles are innervated by sciatic nerve branches. However, previous studies assessing which and how many branches innervate each muscle have yielded discrepant results. This study investigated the innervation patterns of hamstrings. Materials and methods: Thirty-five cadaver limbs were investigated. The average age of subjects was 78.6 ± 17.2 years, with 48.6% male and 51.4% female, while 57.1% were right limbs and 42.9% left. The sciatic nerve, hamstrings and associated structures were dissected. The number of nerve branches for each muscle and the level where they penetrated the muscle were recorded. Results: The sciatic nerve was connected by a fibrous band to the long head of the biceps femoris. This muscle was innervated by either one or two branches, which penetrated the muscle into its superior or middle third. The short head of the biceps femoris was innervated by a single nerve that usually penetrated its middle third, but sometimes inferiorly or, less commonly, superiorly. The semitendinosus was always innervated by two branches, the superior branch penetrating its upper third, the inferior mostly the middle third. The semimembranosus usually was innervated by a single nerve branch that penetrated the muscle at its middle or lower third. Four specimens revealed common nerves that innervated than one muscle. Conclusions: We have characterized hamstring innervation patterns, knowledge that is relevant to neurolysis, surgery of the thigh, and other procedures. Moreover, a mechanical connection between the sciatic nerve and biceps femoris long head was identified that could explain certain neuralgias. [ABSTRACT FROM AUTHOR]

Purpose: To study the morphology and the morphometry of the oblique popliteal ligament (OPL). Methods: Thirty cadaver knees were dissected to study the morphology and morphometry of the OPL. For the measurement of the morphology of the OPL a standard tape and the vernier callipers were used. Result: Out of 30 specimens 14 were Y shaped, 10 were band shaped, and 6 were Z shaped observed. Total length was measured on both the limbs, on the right side it was 4.5 ± 0.4 cm and on the left side 4.5 ± 0.5 cm was recorded. Width at the medial attachment was also measured on both the limbs, on the right side it was 4.6 ± 0.5 cm and on the left side 4.7 ± 0.5 cm was recorded. And width at the lateral attachment was recorded too, on the right side it was 4 ± 0.3 cm and on the left side it was 4 ± 0.3 cm. Width at the midpoint was recorded as on the right side 3.5 ± 0.2 cm and on the left side 3.5 ± 0.2 cm. Conclusion: The OPL is a thick ligament that arises as an extension of the semimembranosus, and it exists in various morphology which includes band, Y, Z, complex shapes. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study is to compare the patient‐reported outcomes and return to sports of the conservative and surgical treatment of distal hamstring tendon injuries. Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses guidelines, two reviewers searched PubMed, Scopus and Virtual Health Library databases in January 2023. Clinical studies evaluating conservative or surgical management outcomes of distal hamstring tendon injuries were considered eligible for this systematic review if predefined criteria were fulfilled: (1) published in English or Spanish; (2) evaluated any of the following: patient‐reported outcomes, return‐to‐sports rate (RTS‐R) or return‐to‐sports time (RTS‐T). Data were presented in tables using absolute values from individual studies and derived pooled percentages. Results: Eighteen studies were included for 67 patients and 68 distal hamstring tendon injuries. Initially, 39 patients (58.2%) underwent surgical treatment, whereas 28 (41.8%) were treated conservatively. Among conservative treatment patients, 15 failed and had to be operated on (53.6%), all with distal semitendinosus tendon injuries. Anchor fixation was the technique of choice in 20 lesions (36.4%), tenodesis in 16 (29.1%), tenectomy in 14 (25.5%) and sutures were preferred in five (9%). Thirteen out of 28 patients (46.4%) undergoing initial conservative treatment returned to sports at a mean of 3.6 months (range 1 week to 12 months), in contrast to surgical treatment, in which 36 out of 39 patients (92.3%) returned at a mean of 4.2 months (range 6 weeks to 12 months). Additionally, 14 of 15 patients (93.3%) converted to surgical treatment after failed conservative treatment returned to sports at a mean of 7.6 months after injury. Conclusion: Initial surgical treatment of distal hamstring tendon injuries yields a high RTS‐R (92.3%) at a mean of 4.2 months. Furthermore, 15 out of 28 patients (53.6%) initially treated conservatively had to be operated on, delaying the RTS‐T (mean 7.6 months after injury) without affecting their RTS‐R. The purpose of this study was to examine the effect of passive and active knee flexion efforts on the stiffness of the thoracolumbar (TLF), semitendinosus (STF), and semimembranosus fascia (SMF). Fourteen young healthy males participated in this study. Using ultrasound shear‐wave elastography, fascia elastic modulus was measured at rest (passive condition) and during submaximal isometric knee flexion efforts (active condition) with the hip at neutral position and the knee flexed at 0°, 45°, and 90°. Analysis of variance designs indicated that when the knee was passively extended from 90° to 0°, shear modulus of the TLF, SMF, and STF increased significantly (p < 0.05). Similarly, active knee flexion contractions caused a significant increase in TLF, SMF, and STF shear modulus (p < 0.001). Compared to hamstring fascia, the TLF showed greater thickness but a lower shear modulus (p < 0.05) while STF modulus was greater compared that to SMF during active contraction (p < 0.05). There is a growing interest in using by-products rich in polyphenols, such as carob pulp (Cp, Ceratonia siliqua L.), as a dietary source of antioxidants for animals. This study assesses the effects of including Cp in lambs' diet and meat display time (0, 7, 9, and 11 days) in modified atmosphere packaging on meat colour, fatty acid (FA) composition, tocopherol levels, and lipid oxidation values in the Semimembranosus muscle of 40 light lambs. The lambs were fed with concentrates supplemented with increasing Cp levels (0, 150, and 300 g/kg) for 45 days before slaughter. Metmyoglobin (MMb) and malondialdehyde (MDA) contents increased linearly with display time (p < 0.05), regardless of diet (p > 0.05). At 11 days of display, MMb (28 ± 0.8%) and MDA (0.6 ± 0.1 mg MDA/kg of meat) contents remained within the acceptable limits. The α-tocopherol content was lower in the 30% Cp group and meat (p < 0.05). Total saturated and monounsaturated FA contents (934 ± 64 and 823 ± 65 mg/100 g of meat, respectively) did not differ significantly among the groups. However, the meat from lambs fed with 30% Cp showed reduced levels of branched-chain FAs, while polyunsaturated FAs increased (p < 0.05) compared to the control lambs. The inclusion of Cp in the lamb's diet, up to 30%, did not lead to meat deterioration and improved certain quality parameters, including a healthier FA profile. Background: The semimembranosus (SM) tendon acts as a secondary dynamic stabilizer of the knee. It restrains external rotation and anterior translation of the medial compartment. Its role in the mechanism of injury during anterior cruciate ligament (ACL) rupture is unknown. Hypothesis: The bone bruise (BB) often detected at the posteromedial tibia in association with acute ACL tear may be related to the traction force from the SM tendon insertion. Magnetic resonance imaging (MRI) alterations can be detectable at the direct arm of the SM tendon in association with acute ACL injury. Study Design: Cross-sectional study: Level of evidence, 3. Methods: In the first study phase, 36 noninjured patients underwent knee MRI. The anatomic appearance of the SM tendon was evaluated. An imaging score for evaluating the SM tendon was developed for the purpose of the study. The intensity (in the axial or sagittal plane), morphology, and thickness of the distal SM tendon was evaluated and scored (4 total points). In the second study phase, 52 patients undergoing acute ACL reconstruction were included. Preoperative MRI was examined and scored, with documentation of BB at the posteromedial tibial plateau. Finally, arthroscopic diagnosis of a ramp lesion was confirmed. Logistic regression analysis was carried out for correlation between an altered MRI scoring system and the presence of BB at the posteromedial tibial plateau, the presence of a ramp lesion, or both. Results: Interrater agreement of 100% was obtained in the noninjured cohort (ie, no alteration found in any patient). The score validation in the cohort of patients with acute ACL injury showed a Cohen κ of 0.78 (interrater agreement, 82.7%). The direct arm of the SM tendon was altered in 35 of 52 patients (67.3%). A ramp lesion of the medial meniscus was arthroscopically detected in 21 patients (40.4%). The presence of BB at the posteromedial tibial plateau was detected in 33 patients (63.5%) and at the posterior medial femoral condyle in 1 (1.9%). Correlation analysis showed a significant association of a pathologic SM score with the presence of BB at the posteromedial tibial plateau (odds ratio = 2.7; P =.001). Conversely, no correlation was observed between the pathologic score and the presence of a ramp lesion (odds ratio = 0.88; P =.578). Conclusion: The prevalence of pathologic findings in the direct arm of the SM tendon insertion was high in the acutely injured cohort with ACL rupture and is correlated with the presence of BB at the posteromedial tibial plateau. Hamstring injuries are among the most common muscle injuries. They have been reported in many different sports, such as running, soccer, track and field, rugby, and waterskiing. However, they are also present among the general population. Most hamstring injuries are mild strains, but also moderate and severe injuries occur. Hamstring injuries usually occur in rapid movements involving eccentric demands of the posterior thigh. Sprinting has been found to mainly affect the isolated proximal biceps femoris, whereas stretching-type injuries most often involve an isolated proximal injury of the semimembranosus muscle. The main cause of severe 2- or 3-tendon avulsion is a rapid forceful hip flexion with the ipsilateral knee extended. Most hamstring injuries are treated non-surgically with good results. However, there are also clear indications for surgical treatment, such as severe 2- or 3-tendon avulsions. In athletes, more aggressive recommendations concerning surgical treatment can be found. For a professional athlete, a proximal isolated tendon avulsion with clear retraction should be treated operatively regardless of the injured tendon. Surgical treatment has been found to have good results in severe injuries, especially if the avulsion injury is repaired in acute phase. In chronic hamstring injuries and recurring ruptures, the anatomical apposition of the retracted muscles is more difficult to be achieved. This review article analyses the outcomes of surgical treatment of hamstring ruptures. The present study confirms the previous knowledge that surgical treatment of hamstring tendon injuries causes good results with high satisfaction rates, both in complete and partial avulsions. Early surgical repair leads to better functional results with lower complication rates, especially in complete avulsions.KEY MESSAGEsSurgical treatment of hamstring tendon ruptures leads to high satisfaction and return to sport rates.Both complete and partial hamstring tendon ruptures have better results after acute surgical repair, when compared to cases treated surgically later.Athletes with hamstring tendon ruptures should be treated more aggressively with operative methods.

Abstract Temporal biomechanical and physiological responses to physical activity vary between individual hamstrings components as well as between exercises, suggesting that hamstring muscles operate differently, and over different lengths, between tasks. Nevertheless, the force-length properties of these muscles have not been thoroughly investigated. The present review examines the factors influencing the hamstrings’ force-length properties and relates them to in vivo function. A search in four databases was performed for studies that examined relations between muscle length and force, torque, activation, or moment arm of hamstring muscles. Evidence was collated in relation to force-length relationships at a sarcomere/fiber level and then moment arm-length, activation-length, and torque-joint angle relations. Five forward simulation models were also used to predict force-length and torque-length relations of hamstring muscles. The results show that, due to architectural differences alone, semitendinosus (ST) produces less peak force and has a flatter active (contractile) fiber force-length relation than both biceps femoris long head (BFlh) and semimembranosus (SM), however BFlh and SM contribute greater forces through much of the hip and knee joint ranges of motion. The hamstrings’ maximum moment arms are greater at the hip than knee, so the muscles tend to act more as force producers at the hip but generate greater joint rotation and angular velocity at the knee for a given muscle shortening length and speed. However, SM moment arm is longer than SM and BFlh, partially alleviating its reduced force capacity but also reducing its otherwise substantial excursion potential. The current evidence, bound by the limitations of electromyography techniques, suggests that joint angle-dependent activation variations have minimal impact on force-length or torque-angle relations. During daily activities such as walking or sitting down, the hamstrings appear to operate on the ascending limbs of their force-length relations while knee flexion exercises performed with hip angles 45–90° promote more optimal force generation. Exercises requiring hip flexion at 45–120° and knee extension 45–0° (e.g. sprint running) may therefore evoke greater muscle forces and, speculatively, provide a more optimum adaptive stimulus. Finally, increases in resistance to stretch during hip flexion beyond 45° result mainly from SM and BFlh muscles.

Purpose: To introduce a new magnetic resonance imaging (MRI) sign, termed the Cobra sign, and identify its diagnostic metrics. The secondary aim was to demonstrate that this sign can be a source of false evaluation of tendon retraction in patients with proximal hamstring avulsion injury., Method: This retrospective cohort study targeted patients surgically treated for proximal hamstring avulsion injury from January 2019 to June 2023. The MRI Cobra sign was defined as a wavy curved T2-hypointense band with the free end folding distally over itself, resembling a cobra head. The primary outcome measure was the characterization of the Cobra sign in patients with proximal hamstring avulsion injury. The secondary outcome was the association of this sign with tendon retraction. The study included 81 proximal hamstring avulsion injury patients (mean age of 45.7, SD = 13.9), with 41 (50.6%) complete avulsions, 33 semimembranosus, and 7 conjoint tendons., Results: The MRI Cobra sign was found in 25 patients (17 semimembranosus and 8 complete). It was confirmed surgically only in semimembranosus cases. It demonstrated 51.5% sensitivity and 83.3% specificity for isolated semimembranosus avulsions, with a significant positive likelihood ratio of 3.0. MRI retraction was 10.05 cm (±3.0), reducing to 7.9 cm (±2.5) on surgical measurement (mean difference = 2.0 cm, p < 0.001). The regression analysis confirmed MRI retraction's influence on the Cobra sign, with a 1.4 odds increase per unit (p < 0.001). In linear regression analysis, each unit increase in MRI retraction corresponded to a 79% increase in surgical retraction (coefficient 0.7, t = 11.1, p < 0.001)., Conclusion: The Cobra sign demonstrated acceptable diagnostic accuracy for isolated semimembranosus avulsion, with a high specificity of 83.3%, a low sensitivity of 51.5%, and a positive likelihood ratio of 3.0. Hamstring injuries are among the most common muscle injuries. They have been reported in many different sports, such as running, soccer, track and field, rugby, and waterskiing. However, they are also present among the general population. Most hamstring injuries are mild strains, but also moderate and severe injuries occur. Hamstring injuries usually occur in rapid movements involving eccentric demands of the posterior thigh. Sprinting has been found to mainly affect the isolated proximal biceps femoris, whereas stretching-type injuries most often involve an isolated proximal injury of the semimembranosus muscle. The main cause of severe 2- or 3-tendon avulsion is a rapid forceful hip flexion with the ipsilateral knee extended. Most hamstring injuries are treated non-surgically with good results. However, there are also clear indications for surgical treatment, such as severe 2- or 3-tendon avulsions. In athletes, more aggressive recommendations concerning surgical treatment can be found. For a professional athlete, a proximal isolated tendon avulsion with clear retraction should be treated operatively regardless of the injured tendon. Surgical treatment has been found to have good results in severe injuries, especially if the avulsion injury is repaired in acute phase. In chronic hamstring injuries and recurring ruptures, the anatomical apposition of the retracted muscles is more difficult to be achieved. This review article analyses the outcomes of surgical treatment of hamstring ruptures. The present study confirms the previous knowledge that surgical treatment of hamstring tendon injuries causes good results with high satisfaction rates, both in complete and partial avulsions. Early surgical repair leads to better functional results with lower complication rates, especially in complete avulsions. Surgical treatment of hamstring tendon ruptures leads to high satisfaction and return to sport rates. Both complete and partial hamstring tendon ruptures have better results after acute surgical repair, when compared to cases treated surgically later. Fractures of the posteromedial tibial plateau are infrequent, for this reason also their clinical description and their treatment is poorly represented in the literature. Only six cases in the literature describe the avulsion fracture of the posterior medial tibial plateau at the insertion of the semimembranous muscle. The mechanism of injury, described in the various articles, however, does not appear univocal, as well as the type of treatment. The analysis of the literature revealed the need for adequate imaging and stable surgical synthesis, in order to obtain an optimal functional recovery.

Temporal biomechanical and physiological responses to physical activity vary between individual hamstrings components as well as between exercises, suggesting that hamstring muscles operate differently, and over different lengths, between tasks. Nevertheless, the force-length properties of these muscles have not been thoroughly investigated. The present review examines the factors influencing the hamstrings' force-length properties and relates them to in vivo function. A search in four databases was performed for studies that examined relations between muscle length and force, torque, activation, or moment arm of hamstring muscles. Evidence was collated in relation to force-length relationships at a sarcomere/fiber level and then moment arm-length, activation-length, and torque-joint angle relations. Five forward simulation models were also used to predict force-length and torque-length relations of hamstring muscles. The results show that, due to architectural differences alone, semitendinosus (ST) produces less peak force and has a flatter active (contractile) fiber force-length relation than both biceps femoris long head (BFlh) and semimembranosus (SM), however BFlh and SM contribute greater forces through much of the hip and knee joint ranges of motion. The hamstrings' maximum moment arms are greater at the hip than knee, so the muscles tend to act more as force producers at the hip but generate greater joint rotation and angular velocity at the knee for a given muscle shortening length and speed. However, SM moment arm is longer than SM and BFlh, partially alleviating its reduced force capacity but also reducing its otherwise substantial excursion potential. The current evidence, bound by the limitations of electromyography techniques, suggests that joint angle-dependent activation variations have minimal impact on force-length or torque-angle relations. Introduction: The oblique popliteal ligament (OPL), chief posterior support of the knee, is an extension of the semimembranosus (SM) tendon and runs superolaterally to the lateral femoral condyle. Due to variations and immense functional value of the OPL, the present study was undertaken to explore the OPL morphology and morphometry. Materials and methods: Thirty cadaveric lower limbs were dissected to observe the OPL in the floor of the popliteal fossa, and its general shape, number of bands and extensions were noted. The morphometry was done using digital callipers in which the length of the upper and lower margins as well as the width of the OPL at the medial and lateral attachments were measured. The width of bands and extensions were also recorded. The mean and standard deviation (SD) of all parameters have been also calculated. Results: The morphological types of the OPL were categorized into four types, which were found in 40%, 50%, 6.6% and 3.3% of specimens, respectively. Types 1 and 2 OPL were band like. Type 1 showed an accessory upper band and type 2, a broad medial attachment. Type 3 was cord like, whereas type 4 was fan like and showed two upper bands. The mean length (upper), length (lower), width of the SM tendon expansion, width at attachment to the medial femoral condyle and width at the lateral attachment were 33.4 mm, 38.41 mm, 8.58 mm, 12.46 mm and 21.42 mm, respectively. Discussion: The different morphological types of OPL may produce minor alterations in the stability of the knee, especially in trauma. Thirty non-castrated Dorper x Santa Inês lambs, with an average of 24.9 ± 3.6 kg and four months of age were confined for 60 days in collective stalls and distributed in a completely randomized design with six replications. After slaughter, by means of contrasts, the averages of the parameters of the semimembranous and semitendinosus muscles were analyzed. The cottonseed increased cooking loss and ash, and reduced muscle weight, water holding capacity and red intensity. The disintegration of the cottonseed reduced the shear force in diets without LignoCaSO3, increased the protein and the loss by cooking and reduced the pH in the diets with the additive. The luminosity values increased with the disintegration of the cottonseed in diets with and without LignoCaSO3. The addition of LignoCaSO3 increased the weight of the muscle, protein, ash, pH, shear strength and the intensity of red. Moisture, lipids and yellow intensity were not influenced by the diets. Even changing the physical-chemical characteristics, the cottonseed with or without LignoCaSO3 does not change the quality of the meat

A 30-year-old male sustained a road traffic accident and presented to our trauma centre with injuries to his pelvis and right knee. Radiology showed closed fractures of the right posterior wall and posterior column of the acetabulum and PCL bony avulsion with posteromedial tibial plateau osteochondral fracture, without any distal neurovascular deficit. He was managed with surgical intervention for both injuries. His hip and knee healed well with a good functional range of motion at 12 months of follow-up. The aim of highlighting this case is that it underscores the rarity of concurrent PCL and semimembranosus (SM) avulsion injuries, emphasizing the importance of comprehensive evaluation and tailored surgical management. Utilizing CT imaging proves instrumental in identifying the associated posteromedial osteochondral fragment. Purpose: To compare outcome of operative and non-operative treatment of avulsion fractures of the hamstring origin, with minor (< 1.5 cm) and major (≥ 1.5 cm) displacement, and early (≤ 4 weeks) and delayed (> 4 weeks) surgery. Methods: A systematic literature search was performed using PubMed, Cochrane, Embase, CINAHL and SPORTDiscus. A quality assessment was performed using the Physiotherapy Evidence Database (PEDro) scale. Results: Eight studies with 90 patients (mean age: 16 years) were included. All studies had low methodological quality (PEDro score ≤ 5). Operative treatment yielded a return to preinjury activity rate (RTPA) of 87% (95% CI: 68–95), return to sports (RTS) rate of 100% (95% CI: 82–100), Harris hip score (HHS) of 99 (range 96–100) and a University of California Los Angeles activity scale (UCLA) score of 100%. Non-operative treatment yielded a RTPA rate of 100% (95% CI:68–100), RTS rate of 86% (95% CI: 69–94), HHS score of 99 (range 96–100), and non-union rate of 18% (95% CI: 9–34). All patients with minor displacement were treated non-operatively (RTPA: 100% [95% CI: 21–100], RTS: 100% [95% CI: 51–100]). For major displacement, operative treatment led to RTPA and RTS rates of 86% (95% CI: 65–95) and 100% (95% CI: 84–100), and 0% (0/1, 95% CI: 0–79) and 100% (95% CI: 51–100) for non-operative treatment. Early surgery yielded RTPA and RTS rates of 100% (95% CI: 34–100 & 57–100) compared to 100 (95% CI: 72–100) and 90% (95% CI: 60–98) for delayed repair. Conclusion: All included studies have high risk of bias. There is only low level of evidence with a limited number of included patients to compare outcome of operative and non-operative treatment. Overall outcome was satisfactory. There is a treatment selection phenomenon based on displacement, with acceptable outcome in both groups. Abstract Background The presentation of patients with knee injuries which can have a significant impact on their ability to work and perform activities of daily living is constantly rising. The posterolateral corner of the knee has a complex anatomy of muscles, tendons, and ligaments, with huge variation in the population. The fabella is one such structure, found in the posterolateral corner of the knee, which can serve as a common origin point of various ligaments. Case report We present a case report of a 53-year-old white man who presented with atraumatic, posterior knee pain and was found to have a congenital, anomalous band originating from the fabella, causing semimembranosus impingement. This was diagnosed with magnetic resonance imaging; he underwent division of the anomalous band, which resulted in complete resolution of his symptoms. Conclusion We propose that patients who present with posterior knee pain, without any history of trauma, and have no abnormalities on plain radiographs, should undergo magnetic resonance imaging of their knees. This will help in assessing the ligament complex in the posterior compartment of the knee, and exclude impingement of the semimembranosus as an, albeit rare, cause of posteromedial knee pain.

This study investigated the effect of hip joint position on the activity level of individual hamstring muscles during stiff-leg deadlift. Fourteen male collegiate sprinters performed stiff-leg deadlift in the adducted (ADD), neutral (NT), abducted (ABD), internally rotated by 20° (IN20), and externally rotated positions by 20° (EX20) and by 40° (EX40) of the hip joint. Surface electromyogram (EMG) was recorded from the proximal and distal regions of the biceps femoris long head (BFlh), semitendinosus (ST), and semimembranosus (SM). Root mean square value of EMG (RMS-EMG) data were calculated for the concentric and eccentric phases of deadlift and was normalized by RMS-EMG during maximal voluntary contraction of isometric knee flexion as an activity level. Results revealed that the activity level of BFlh during the concentric phase was higher in EX20 (p50.008, difference56.3%) and EX40 (p50.001, difference59.4%) than in NT. Semimembranosus showed a higher activity level in IN20 than in EX40 during the concentric (p 5 0.004, difference 5 4.3%) and eccentric phases (p 5 0.023, difference 5 4.1%). In addition, the activity level was higher in ABD than in NT for BFlh (p 50.015, difference 5 4.6%), ST (p 5 0.047, difference 5 3.8%), and SM (p 5 0.005, difference 5 3.9%) during the concentric phase of deadlift. In conclusion, the deadlift in the hip-abducted position requires high activation of the individual hamstrings and that in the hip externally and internally rotated positions needs high activation of BFlh and SM, respectively. Background: Some authors have suggested that the semimembranosus tendon is involved in the pathophysiology of ramp lesions. This led us to conduct a gross and microscopic analysis of the posterior horn of the medial meniscus and the structures inserted on it. Hypothesis: (1) The semimembranosus tendon has a tendinous branch inserting into the posterior horn of the medial meniscus, and (2) the meniscotibial ligament is inserted on the posteroinferior edge of the medial meniscus. Study Design: Descriptive laboratory study. Methods: In total, 14 fresh cadaveric knees were dissected. From each cadaveric donor, a stable anatomic specimen was harvested en bloc, including the medial femoral condyle, medial tibial plateau, whole medial meniscus, cruciate ligaments, joint capsule, and distal insertion of the semimembranosus tendon. The harvested blocks were cut along the sagittal plane to isolate the distal insertion of the semimembranosus tendon on the posterior joint capsule and the posterior horn of the medial meniscus in a single slice. Histological slides were made from these samples and analyzed under a microscope. Results: In all knees, gross examination revealed a direct branch of the semimembranosus and a tendinous capsular branch ending behind the posterior horn of the medial meniscus. This capsular branch protruded over the joint capsule, over the meniscotibial ligament below and the meniscocapsular ligament above, but never ended directly in the meniscal tissue. The capsular branch was 14.3 ± 4.4 mm long (mean ± SD). The direct tendon inserted 11 ± 2.8 mm below the articular surface of the tibial plateau. The meniscotibial ligament inserted on the posteroinferior edge of the medial meniscus, and the meniscocapsular ligament insertion was on its posterosuperior edge. Highly vascularized adipose tissue was found, delimited by the posterior horn of the medial meniscus, meniscotibial ligament, meniscocapsular ligament, and capsular branch of the semimembranosus tendon. Conclusion: In all knees, our study found a capsular branch of the semimembranosus tendon inserted behind the medial meniscus. The meniscotibial ligament was inserted on the posteroinferior edge of the medial meniscus. Histological analysis of this area revealed that this ligament inserted differently from the insertion previously described in the literature. Clinical Relevance: This laboratory study provides insight into the pathophysiology of ramp lesions frequently associated with anterior cruciate ligament injury. Limited information is available on whether stiffness is different within and between the constituents of the hamstring, that is, the biceps femoris long head (BFlh), semitendinosus (ST), and semimembranosus (SM). Therefore, understanding of hamstring injuries and stretching effect on hamstring stiffness is difficult. The present study primarily aimed to identify whether passive muscle stiffness differs between the BFlh, ST, and SM and between the proximal, middle, and distal sites within each muscle. Secondly, the effect of stretching exercise on the heterogeneity in passive muscle stiffness was examined. In the lengthened hamstring positions by extending the knee joint or flexing the hip joint, passive muscle shear modulus (a measure of stiffness) at the proximal, middle, and distal sites of the BFlh, ST, and SM was measured by using ultrasound shear wave elastography. Furthermore, before and after five repetitions of 90‐seconds static stretching for the hamstring, passive muscle shear modulus at the proximal and distal sites of the SM was measured. The shear modulus was significantly higher in the SM than in the BFlh and ST and higher at the distal site than the proximal site in all muscles. After the stretching, the higher shear modulus at the distal site of the SM compared to the proximal site was still observed (pre‐stretching: +80%, post‐stretching: +81%). These findings indicate that passive muscle stiffness varies within the hamstring regardless of performing stretching exercise and that passive muscle stiffness is not highest at the proximal site of the SM where a stretching‐type hamstring strain typically occurs. Background: Nonoperative management of proximal semimembranosus injuries is associated with prolonged periods of convalescence and high risk of recurrence. To our knowledge, the outcomes of acute surgical repair for complete, nonavulsion proximal semimembranosus injuries have not been previously reported. Hypothesis: Acute surgical repair of complete, nonavulsion proximal semimembranosus injuries enables early return to sporting activity with low risk of recurrence. Study Design: Case series: Level of evidence, 4. Methods: This prospective single-surgeon study included 20 professional athletes undergoing acute primary surgical repair of complete, nonavulsion proximal semimembranosus injuries confirmed on preoperative magnetic resonance imaging. All study patients underwent a standardized postoperative rehabilitation program. Predefined outcomes were recorded at regular intervals after surgery. Mean follow-up time was 27.6 months (range, 24.0-34.6 months) from date of surgery. Results: Of the 20 patients, 19 (95%) returned to their preinjury level of sporting activity. Mean ± SD time from surgical repair to full sporting activity was 11.9 ± 5.7 weeks. No patients had recurrence of the primary injury. At 3 months after surgery, patients had improved mean passive straight leg raise (71.5° ± 5.9° vs 31.1° ± 7.2°; P <.001); increased mean isometric hamstring muscle strength at 0° (83.8% ± 5.9% vs 48.4% ± 8.3%; P <.001), 15° (77.6% ± 6.0% vs 52.3% ± 14.7%; P <.001), and 45° (88.6% ± 5.4% vs 66.7% ± 13.1%; P <.001); higher mean lower extremity functional scores (64.8 ± 4.6 vs 34.4 ± 5.1; P <.001); and improved Marx activity rating scores (10.7 ± 1.6 vs 5.5 ± 2.0; P <.001) as compared with preoperative values. High patient satisfaction and functional outcome scores were maintained at 1 and 2 years after surgery. Conclusion: Acute surgical repair of complete, nonavulsion proximal semimembranosus injuries is associated with high patient satisfaction, increased muscle strength, improved functional outcome scores, and high return to preinjury level of sporting activity with low risk of recurrence at short-term follow-up. The passive material properties of a muscle, as evaluated by shear or Young’s modulus, can vary depending on the muscle cross-sectional area (CSA), since intramuscular compositions such as adipose tissue and connective tissue change with muscle hypertrophy and atrophy. However, the relationship remains unclear between the passive material properties and CSA of a muscle. Therefore, we examined whether passive muscle shear modulus is related to muscle CSA in the human hamstring. The shear modulus and anatomical CSA of the biceps femoris, semitendinosus, and semimembranosus were measured in 71 healthy young males with the hip flexed at 70° and knee fully extended. The shear modulus of each muscle was measured using ultrasound shear wave elastography. Muscle CSA was assessed using panoramic ultrasound imaging. There were individual differences in muscle shear modulus as well as muscle CSA. The coefficients of variation (CVs) of muscle shear modulus were 41.4% (range: 8.8 - 54.3 kPa), 33.0% (range: 8.4 - 57.3 kPa), and 41.1% (range: 18.2 - 107.1 kPa) for the biceps femoris, semitendinosus, and semimembranosus, respectively. There were no significant correlations between shear modulus and CSA in each or overall hamstring muscle (absolute r < 0.113). The present findings indicate that, although the passive material property of muscle is not the same among individuals, it is substantially independent of the muscle CSA, at least in the hamstring.

We aimed to compare dynamic exercise performance between hip extension exercises with different knee angles and between knee flexion exercises with different hip angles, and to investigate the recruitment pattern of the hamstrings in each exercise. Seven men performed 4 isokinetic exercises (3 maximal concentric contractions at 30°/s (peak torque) and 30 maximal concentric contractions at 180°/s (total work)): hip extension with the knee fully extended (HEke) and with the knee flexed at 90° (HEkf) and knee flexion with the hip fully extended (KFhe) and with the hip flexed at 90° (KFhf). The recruitment pattern of the hamstrings was evaluated in each exercise using magnetic resonance imaging (T2 calculation). The HEke condition showed significantly greater peak torque than the HEkf condition (p < 0.05). The KFhf condition had significantly greater peak torque and total work values than the KFhe condition (p < 0.05). Although the biceps femoris long head, semitendinosus, and semimembranosus had significantly increased post-exercise T2 values in the HEke (p < 0.05), KFhe, and KFhf conditions (p < 0.01), the T2 increase values were significantly greater under the KFhf than the HEke condition (p < 0.05). The semitendinosus showed a significantly greater T2 increase value than other muscles under both KFhe and KFhf conditions (p < 0.05). Performance of hip extension and knee flexion exercises increases when the hamstring muscles are in a lengthened condition. The purpose of this study was to determine the reliability and validity of ultrasonography for measurement of hamstring muscle and semitendinosus (ST) tendon cross-sectional area (CSA). On two consecutive days, muscle anatomical CSA (ACSA) and ST tendon CSA were measured at standardized positions (30%-80% of thigh length; half the distance from the distal muscle-tendon junction to the popliteal crease) on 12 legs using ultrasonography and compared with corresponding magnetic resonance imaging measures. Inter-day intraclass correlation coefficients were good-to-excellent (0.882-0.996) for all assessed muscle and tendon sites. The limits of agreement widths were narrowest (range: 17%-52%) when muscle ACSA was large but were wide at sites with relatively small ACSA (≤184%) and for ST tendon CSA (range: 72%). Results suggest ultrasound-based measures of individual hamstring muscle maximal ACSA are reliable and valid and ST tendon CSA measures are reliable but require comparison with cadaveric or intra-operative measurements to verify validity. Variations of the anatomy of the hamstring muscle complex are uncommon. In particular, absence of some or all of the hamstring muscles is rarely described in the literature. This report presents a case of absent semimembranosus musculature detected incidentally on knee magnetic resonance imaging (MRI), then found to be bilateral on MRI of both thighs performed shortly thereafter. The objective of the trial was to study the qualitative parameters of two muscles of Aberdeen Angus bulls with 250-300 kg carcass weight. After slaughter, longissimus thoracis et lumborum (LD) muscle and unseparated semimembranosus and adductor femoris (SMA) muscles were removed from the chilled carcasses. Muscles were vacuum-packed and wet aged at +2 °C for 10, 14, 18 and 20 days. Meat pH, electrical conductivity, shear force and colour were measured in all ageing times. Two thermal treatment methods (sous-vide (SV) and grilling) were used to determine cooking losses. The effects of muscles, ageing times and muscles by ageing times interaction was found with two-factorial analysis of variance. The effects of muscles, ageing times and muscle groups by ageing time interaction for raw and SV treated meat shear force was significant. Ageing decreased SV treated meat shear force from day 10 (40.8 N) to 18 (29.7 N). Fresh and SV treated LD muscle was tougher compared to the SMA muscle group, but SM showed a better response to the tenderness within 20 days of ageing. Redness and yellowness value was higher in the SM group in comparison to LD. Muscles showed good colour (lightness, redness and yellowness) stability within ageing for 20 days. No interactions were found between muscle groups and ageing times for SV treated and grilled beef cooking losses. However, SV treated meat lost more weight than grilled meat slices. The present study suggests that the optimal ageing time for meat is 18 days when the grilled meat cooking loss is the lowest. [ABSTRACT FROM AUTHOR]

Most suckling kids are raised on farms for cheese production, and many goat farmers rear kids with milk replacers. The aim of this work was to study the influence of the use of milk replacers on the color of four muscles. A total of 246 suckling kids of eight breeds were slaughtered to achieve carcasses of 5 kg. The color of the biceps femoris, semimembranosus, semitendinosus, and longissimus thoracis muscles was measured with a spectrophotometer, and CIELab coordinates were registered. In addition, the pH of longissimus thoracis was measured. The effect of the rearing system (RS) on the color of the studied muscles is strongly modulated by breed. In general terms, there are two groups of kids according to the color of meat. The first group has great lightness and hue angle including Malagueña, Palmera, and Tinerfeña fed natural and artificial milk. The second group with great redness includes Retinta, Payoya, and Verata fed natural and artificial milk together with Florida fed natural milk and Cabra del Guadarrama fed milk replacers. Hence, farms should consider selecting a breed and RS together. Most of the kid meat with high pH comes from kids raised on milk replacers. Because artificial RSs use very early weaning, which might induce a high pH and dark meat, two artificial rearing strategies can be proposed. The first strategy is to choose less sensitive breeds that produce meat with a normal pH. The second strategy is to restrict suckling of natural milk but minimize separation from the mother. Muscle morphology is an important contributor to hamstring muscle injury and malfunction. The aim of this study was to examine if hamstring muscle-tendon lengths differ between various measurement methods as well as if passive length changes differ between individual hamstrings. The lengths of biceps femoris long head (BFlh), semimembranosus (SM), and semitendinosus (ST) of 12 healthy males were determined using three methods: Firstly, by identifying the muscle attachments using ultrasound (US) and then measuring the distance on the skin using a flexible ultrasound tape (TAPE-US). Secondly, by scanning each muscle using extended-field-of view US (EFOV-US) and, thirdly, by estimating length using modelling equations (MODEL). Measurements were performed with the participant relaxed at six combinations of hip (0°, 90°) and knee (0°, 45°, and 90°) flexion angles. The MODEL method showed greater BFlh and SM lengths as well as changes in length than US methods. EFOV-US showed greater ST and SM lengths than TAPE-US (p < 0.05). SM length change across all joint positions was greater than BFlh and ST (p < 0.05). Hamstring length predicted using regression equations is greater compared with those measured using US-based methods. The EFOV-US method yielded greater ST and SM length than the TAPE-US method. SM showed the highest change in length at different hip and knee joint positions.

The posterior compartment of the thigh is composed of three major muscles collectively known as the hamstring muscles. These consist of the biceps femoris short and long head, semimembranosus and semitendinosus. Excluding the short head of biceps femoris, the hamstrings contribute to the movement of the hip and the knee joints as they span across both joints. Our hypothesis is that the nature of the conjoint tendon -ischial angle predisposes to an increased risk of tearing in this hamstring component. We therefore aim to look at the anatomy of the hamstring origin at the ischial tuberosity and spatial relationship between the long head of biceps, semitendinosus and semimembranosus in the form of vector angles. 100 consecutive pelvic MRIs in patients under the age of 40 years were reviewed by musculoskeletal radiology fellow and a consultant musculoskeletal radiologist with more than 10 years' experience in musculoskeletal radiology and measured the angle of origin of conjoined tendon and semimembranosus at its ischial origin. P value using a paired t -test was less than 0.0001 confirming that the difference in the vector angle of the different hamstring components was statistically significant. The median angle of origin of conjoined tendon was 12° and for semimembranosus was 6°. Applying the concept of Newton's second law to the angles calculated we demonstrated that an increase of 9% force applied to the conjoint tendon origin when compared to the semimembranosus tendon. We hypothesis that the difference in the angle of origin of the components of hamstrings might be one of the reasons for the difference in the incidence and patterns of the injuries of the various muscles of the hamstrings. Background: Little is known about the dynamic restraints of the semimembranosus muscle (SM). Purpose and Hypothesis: The goal of the present study was to elucidate the role of (1) passive and (2) active restraints to medial-side instability and to analyze (3) the corresponding tightening of the posteromedial structures by loading the SM. It was hypothesized that points 1 to 3 will significantly restrain medial knee instability. This will aid in understanding the synergistic effect of the semimembranosus corner. Study Design: Controlled laboratory study. Methods: Nine knees were tested in a 6 degrees of freedom robotic setup and an optical tracking system. External rotation (ER; 4 N·m), internal rotation (4 N·m), anteromedial rotation (4-N·m ER and 89-N anterior tibial translation), and valgus rotation (8 N·m) were applied at 0°, 30°, 60°, and 90°, with and without an SM load of 75 N. Sequential cutting of the medial collateral ligament and posterior oblique ligament was then performed. At the intact state of the knee and after each cut, the aforementioned simulated laxity tests were performed. Results: The medial collateral ligament was found to be the main passive stabilizer to ER and anteromedial rotation, resulting in 9.3° ± 6.8° (P < .05), 8.1° ± 3.6° (P < .05), and 7.6° ± 4.2° (P < .05) at 30°, 60°, and 90°, respectively. Conversely, after the posterior oblique ligament was cut, internal rotation instability increased significantly at early flexion angles (9.3° ± 3.2° at 0° and 5.2° ± 1.1 at 30°). Loading the SM had an overall effect on restraining ER (P < .001) and anteromedial rotation (P < .001). This increased with flexion angle and sectioning of the medial structures and resulted in a pooled 2.8° ± 1.7° (not significant), 5.4° ± 2° (P < .01), 7.5° ± 2.8° (P < .001), and 8.3° ± 4.4° (P < .001) at 0°, 30°, 60°, and 90° when compared with the unloaded state. Conclusion: The SM was found to be a main active restraint to ER and anteromedial rotation, especially at higher flexion angles and in absence of the main passive medial restraints. The calculated tensioning effect was small in all flexion angles for all simulated laxity tests.
