4. Treatment of Heart Failure with Sodium-glucose Cotransporter Type 2 Inhibitors

Heart failure is a chronic disease that affects millions of people worldwide and is a leading cause of death. Sodium-glucose cotransporter type 2 inhibitors are a new class of drugs that have demonstrated cardiovascular benefits in patients with type 2 diabetes. The aim of the research was to gain insight into the efficacy of these drugs in the treatment of heart failure. To achieve this, a systematic search was conducted in the PubMed, Embase and Cochrane Library databases from 2018 to 2023. Randomized clinical trials comparing sodium-glucose cotransporter type 2 inhibitors with placebo or standard treatment in patients with heart failure were included. Ten clinical trials were identified that met the inclusion criteria. The results showed that the treatment significantly reduced the risk of hospitalization for heart failure and cardiovascular mortality compared with placebo or standard care, and that sodium-glucose cotransporter type 2 inhibitors improved cardiac function and reduced levels of B-type natriuretic peptide, a biomarker of cardiac stress. The results of this study support the inclusion of sodium-glucose cotransporter type 2 inhibitors in the treatment of heart failure in patients with or without type 2 diabetes. Further studies are needed to evaluate the long-term safety and efficacy of sodium-glucose cotransporter type 2 inhibitors in patients with heart failure.

5. Rare genetic cardiovascular diseases: descriptive epidemiological data in a Mexican third-level cardiology hospital outpatient clinic.

Introduction: rare cardiovascular diseases are conditions with low prevalence in the general population that affect the cardiovascular system. Most have a genetic etiology with a heritable potential denominated as Rare Genetic Cardiovascular Diseases (RGCVD). Currently, there are scarce epidemiological studies on RGCVD, and the overall prevalence is unknown. Objective: descriptive epidemiological analysis of RGCVD, emphasizing the overall prevalence estimation in a cardiovascular hospital. In addition, the cardiovascular phenotype distribution and other related demographic characteristics were analyzed. Material and methods: the study consists of a retrospective descriptive epidemiological analysis from January to December 2019 in the outpatient clinic of a cardiovascular third-level hospital in Mexico City. RGCVD patients were identified with an exhaustive review of all clinical records. The overall prevalence of RGCVD was estimated. In addition, the number of diseases, cardiovascular phenotype distribution, and other demographic data of these diseases were analyzed. Results: RGCVD comprised 794 of 31,487 patients in the outpatient clinic, corresponding to 104 diseases. The overall prevalence of RGCVD was 2.5% (95%CI, 2.3-2.7) patients. The prevalence of monogenic and chromosomal disorders was 2.1 and 0.4%, respectively. Congenital heart diseases were the most frequent cardiovascular phenotype (42.4%), and the less frequent were cardiac tumor disorders (0.9%). Conclusions: the study identified that almost one in 40 patients in the outpatient clinic of a cardiology hospital had an RGCVD. 6. Demanda cardiovascular do beach tennis recreacional em mulheres normotensas e hipertensas de meia-idade

Objetivo: O objetivo do estudo foi analisar e comparar as respostas cardiovasculares de uma única sessão de beach tennis (BT) em mulheres de meia-idade com pressão normal (NTN) e hipertensão (HTN). Métodos: A amostra foi composta por 26 mulheres (13 NTN e 13 HTN; ≈ 47 ± 7 anos) que foram submetidas a uma sessão de BT composta por 5-min de aquecimento, seguidos por 3 jogos de 12-min com intervalos de 2-min entre elas. Todas os jogos foram realizados em uma quadra de BT regular e com regras oficiais. A pressão arterial (PA) e a frequência cardíaca foram avaliadas antes (Pré), durante a sessão (imediatamente após cada jogo) e após a sessão por 30-min. Resultados: Para a frequência cardíaca, não houve diferença entre os grupos em nenhum dos momentos avaliados (p > 0,050). Em comparação com o valor basal, a PA sistólica nas NTN aumentou após o primeiro (23 mmHg, p < 0,001), segundo (23 mmHg, p < 0,001) e terceiro jogo (14 mmHg, p = 0,013). Em contraste, nas HTN, a PA sistólica aumentou apenas após o primeiro jogo (19 mmHg, p = 0,009). Após o exercício, nas NTN, a PA sistólica diminuiu pós-30’ (-11 mmHg, p = 0,025). Nas HTN, a PA sistólica diminuiu tanto no pós-15’ (-19 mmHg, p < 0,001) quanto no pós-30’ (-19 mmHg, p < 0,001). Ao comparar os valores delta entre os grupos em cada ponto de recuperação, as HTN apresentaram uma PA sistólica inferior às NTN no pós-15’ (-16 mmHg, p < 0,001) e pós-30’ (-9 mmHg, p = 0,046). Conclusão: Em conclusão, durante o exercício, as mulheres HTN e NTN apresentaram um aumento na demanda cardiovascular em comparação aos valores pré-sessão. Além disso, uma única sessão de BT reduziu agudamente a PA em mulheres HTN e NTN, com uma redução mais acentuada observada nas HTN.

Antecedentes: Una visión de la medicina espacial aplicada a los viajes al espacio profundo es un reto. A medida que las misiones espaciales aumentan su duración en el espacio y se extienden más allá de la órbita de la Tierra, también aumentarán los riesgos que conlleva trabajar en estas condiciones extremas y aisladas. Los efectos que sufren los astronautas en el espacio los circunscribimos en dos variables; la radiación y la microgravedad. Los peligros para la salud van desde una mayor exposición a la radiación, disminución de la masa corporal, el alargamiento de los telómeros, la inestabilidad del genoma, la distensión de la arteria carótida, el aumento del grosor de la íntima-media, cambios en el microbioma de la piel entre otros. Si un astronauta supera un vuelo en microgravedad de 3 a 6 meses desarrollará adaptaciones fisiológicas que conducen a la intolerancia ortostática. Todo lo anterior es necesario interpretarlo y reconocer que la gravedad cero y un viaje prolongado puede causar problemas en el cuerpo de un astronauta aun por reconocer. Objetivo: Evidenciar las condiciones de los astronautas en órbita baja descritas por estudios científicos en medicina aeroespacial. Métodos: Se realizó una revisión bibliográfica y se encontraron 58 documentos. Los artículos fueron obtenidos de las bases de datos NASA, Pubmed, Nature Riviews. Para la gestión y organización de la información se utilizó el programa Mendeley de libre acceso.

The present study aimed to compare the effect of 24 sessions of stretching exercises performed before resistance exercises versus resistance exercise sessions on the hemodynamic responses of sedentary individuals. Thirty volunteers (26.76±6.29 years old, 81.56±10.66 kg body weight, 1.74±0.07 m height, and 26.74 ±.82 kg/m2 body mass index) participated in this study. After performing the test and retest of 10 maximum repetitions (10RM), the volunteers were divided into 3 groups: a) stretching exercise + resistance exercise (SE+RE); b) resistance exercise (RE) and c) control group (CG). The RE was composed of 3 sets of 10 repetitions at 80% of 10RM with a 2-minute interval between sets of RE preceded or not by 3 sets of 30 seconds of SE. Blood pressure (BP) was assessed using an automatic oscillometric device at session 0, session 12 and session 24 of training. Systolic BP (SBP), diastolic BP (DBP), mean BP (MAP), heart rate (HR), double product (DP) and oxygen saturation (SpO2) were measured after 10 minutes of rest. Comparison between groups was performed by Kruskal Wallis test with Bonferroni correction and 5% significance (p < 0.05). DBP showed significant difference when comparing session 0 vs. session 24 (p = 0.006) for the RE protocol. MAP decreased significantly after session 24 for the RE protocol compared to the other protocols (p = 0.016) and difference between session 0 vs. session 24 (p = 0.022). Significant difference was found in SE + RE protocol (p = 0.029) compared to session 0 vs. session 24 GC. This study showed that RE improved the DBP and MAP response and that the combination of SE+RE improved the SpO2 response after 24 training sessions. Heart failure (HF) is a clinical syndrome characterized by signs and symptoms secondary to deteriorated cardiac function. Congestive heart failure (CHF) is a compensatory mechanism through eccentric remodeling that leads to maladaptive hemodynamic decompensation. Materials and Methods: Observational, descriptive, and cross-sectional study. A descriptive analysis of the hospital discharge rate (HDR) for CHF between the years 2019-2022 in Chile, according to the variables sex, age group, and length of hospital stay. Data were collected from the Department of Health Statistics and Information and the 2017 Census and processed using Microsoft Excel 2019. Evaluation by an ethics committee was not required. Results: A total of 28,833 hospital discharges were studied, determining a period rate of 41.02 cases per 100,000 inhabitants. Regarding the sex variable, a higher prevalence was observed in men and in those over 80 years of age. The average length of hospital stay was 8.79 days, with a notable decrease in hospital stay in men to 5 days in the year 2022. Discussion: The study reveals differences in discharge incidence and hospitalization days, with an increase in discharges in older men, attributable to the disease that increases admissions as age increases. Gender discrepancy may be explained by a higher incidence of ischemic heart disease in men and more comorbidities in women. In 2022, more people were hospitalized for fewer days, possibly due to the pandemic, contrasting with 2019. [ABSTRACT FROM AUTHOR]

Objective: Measuring a sure cardiorespiratory arrest skills and handling of the automatic external defibrillator by resident nurses, through the "Basic Resuscitation Skills Self-Efficacy Scale." Methods: A Cross-sectional study, census, quantitative and analytical cohort study developed with 13 resident nurse professionals from a university hospital in Minas Gerais. Individual data collection in each sector of work at the beginning or end of the shifts, being located according to the list provided by the secretariat of the offering institution. Results: Being the average age of participants equal to 26.38 ± 2.81, with predominance of females (86.4%) with specific course in cardiorespiratory arrest (69.2%) and report of having witnessed cardiorespiratory arrest in real context (84.6%). Regarding the performance of resuscitation maneuvers, 61.5% never performed, and 92.3% never manipulated the automatic external defibrillator. Spearman's correlation between "Basic Resuscitation Skills Self-Efficacy Scale" and general self-efficacy pops with positive correlation, of strong magnitude (r =0.685) and significant (p= 0.010). Conclusion: The instrument in question is of quick application and demonstrated the ability to measure resuscitation skills, as well as the self-efficacy of the individual. This study was unprecedented when evaluating the skills of nursing residents in the performance in cardiorespiratory arrest, using this recently validated instrument for use in the Brazilian population. Objective: To evaluate the relationship between birth weight and the autonomic nervous system in adulthood through a systematic review. Data source: This is a systematic review of publications without limitation of year and language. We included studies involving the autonomic nervous system and birth weight in adults. Manuscripts were selected based on electronic searches of Medical Literature Analysis and Retrieval System Online (MEDLINE), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Web of Science Cochrane Library and Scopus databases, using “Autonomic Nervous System” OR “Heart Rate” OR “Heart Rate Variability” AND “Birth Weight” as a search strategy. This review is registered on the International Prospective Register of Systematic Reviews — PROSPERO (ID: CRD42020165622). Data synthesis: We found 894 articles; 215 were excluded for duplicity. Of the remaining 679 studies, 11 remained. Two were excluded because they did not specifically treat the autonomic nervous system or birth weight. There were nine publications, two cohort and seven cross-sectional studies. The main findings were that extreme, very low, low or high birth weight may have some impact on the autonomic nervous system in adult life. Conclusions: Birth weight outside the normality rate may have a negative influence on the autonomic nervous system, causing autonomic dysfunction and increasing the risk of cardiovascular diseases in adult life. Thus, the importance of the follow-up of health professionals from pregnancy to gestation and throughout life, with preventive care being emphasized. [ABSTRACT FROM AUTHOR]

Background: Cumulative exposure to violence can change the regulation of epigenetic and physiological markers. Although violence has been associated with accelerated cellular aging, little is known about associations with cardiac autonomic activity. Objective: The current study aimed to investigate the relationship of exposure to community and domestic violence (CDV) with vagal activity and epigenetic aging acceleration. Methods: A total of 86 adolescents (57% female) were evaluated and interviewed at two time-points in São Gonçalo (2014–2019), a Brazilian city with high levels of violence. Exposure to CDV was assessed in both time-points. GrimAge acceleration was calculated from saliva DNA methylation using Infinium HumanMethylation450K (Illumina) collected in the first assessment. Heart rate variability (HRV) was collected during two stress tasks at the second assessment. Results: The exposure to violence witnessed or directly experienced at home and in the community increased significantly (t = 4.87, p <.01) across two-time points, and males had reported higher violence exposure (t = 2.06, p =.043). Violence at 1st assessment was significantly associated with GrimAge acceleration (B =.039, p value =.043). Violence at both assessments were associated with HRV measured during the narration of the worst trauma (traumaHRV) (B =.009, p value =.039, and B =.007, p value =.024, 1st and 2nd assessment respectively). GrimAge acceleration was significantly associated with traumaHRV (B =.043, p value =.049), and HRV measured during a 3D roller coaster video (B =.061, p value =.024). Conclusions: We found relevant evidence that experiencing violence during adolescence is associated with epigenetic aging and stress-related vagal activity. Understanding these factors during this period could contribute to the development of early interventions for health promotion. HIGHLIGHTS Higher exposure to Community and domestic violence is associated with increased GrimAge acceleration. En este estudio descriptivo-exploratorio, se incluyó 55 hipertensos, obteniendo como resultados: a) la edad predominante fue de 23 - 83 años (56 + 7,47); b) 50,90% fueron femeninos; c) 3,63% pertenecían al estrato II, 3,63% estrato III, 87,27% estrato IV y 5,45% estrato V; d) 81,81% se encontraron con prehipertensión; e) 18,18% presentaron hipertensión arterial grado I; e) estuvieron presentes factores de riesgo cardiovascular no modificables: Edad/Sexo 69,09% (41,81% mujeres > 45 años, 27,27% hombres > 55 años), antecedente familiar de enfermedad cardiovascular 89,09%; y modificables: sedentarismo 83,63%, hábitos alimentarios inadecuados 54,54%, diabetes mellitus 29,09%, dislipidemia 23,63%, hábito tabáquico 21,81%, obesidad 3,63%. Finalmente se determinó el riesgo para enfermedad cardiovascular: < 10% 89,09%, 10 - 20% 7,27%, 20 - 30% 3,63%. Se concluye que la totalidad de los casos estudiados presentaron factores de riesgos modificables y no modificables para enfermedad cardiovascular, y el riesgo predominante para padecer esta patología en 5-10 años fue de menos de 10%.

Objective. To analyze the spatial distribution and temporal trend of hospital discharges associated with ischemic heart disease (IHD) in Chile for the period 2002-2020. Materials and methods. Mixed ecological study. Cases of IHD >=18 years registered in Chile between 2002 and 2020 were included. Cartographic representation at the communal level according to sex and residence. Spatial autocorrelation was used applying local Moran’s I. univariate by sex. Time trend was evaluated using the segmented Poisson (JP) model. Results. 434 061 total cases. The minimum hospitalization rates were 53 x 100 000 inhabitants for males and 32 x 100 000 inhabitants for females. The maximum rates determined correspond to 638 x 100 000 inhabitants for males and 364 x 100 000 inhabitants for females. Counties register Moran’s I. values for men that tend to disperse as the analyzed period progresses (0.247 to 0.334) and in the case of women they tend to cluster (0.244 to 0.223). Trend reflects increasing behavior in the age ranges from 18 to 24 years and from 25 to 34 years for females. Conclusion. There is a transition from low to high rates during the period studied. It is possible to group communes with high values in the central and southern zone of Chile, and low values in the northern zone of the country. The existence of a hospital, on average, affects utilization rates for both genders. [ABSTRACT FROM AUTHOR]

Utilizando como descriptores: troponin; covid-19; cardiovascular system. Resultados: 23 estudios revelaron alteraciones cardiacas en pacientes hospitalizados por COVID-19 mediante el examen del biomarcador cardioespecífico troponina, además de relacionarlo con altas tasas de mortalidad. Los estudios han demostrado una fuerte relación entre la lesión miocárdica aguda, evidenciada por la elevación de la troponina, y un peor pronóstico durante la hospitalización. Conclusión: Se mapearon alteraciones cardiovasculares en pacientes hospitalizados con Covid-19 que progresaron a enfermedad grave. Este estudio puede contribuir a la planificación asistencial mediante protocolos dirigidos al manejo clínico, específicos para pacientes con la enfermedad, tanto en la fase aguda como en la fase post-Covid-19.

24. Long-term post-COVID-19 sequelae. A review study

At present, investigations are more frequent in which it is reported that survivors of the SARS-CoV-2 infection do not regain their state of health prior to the viral process. The study collected information about the various sequelae recorded in original research indexed in databases such as Scielo, Scopus, and Pubmed, 11 articles were found from patients who had COVID-19, who participated in various studies in the post-acute stage of the acute disease process. the disease, some of them were even evaluated after 6 months of having started the symptoms, however, they continued presenting symptoms after the remission of the disease. In other articles, patients were evaluated by imaging tests such as computed axial tomography and cardiac magnetic resonance imaging. The results indicated sequelae in the respiratory system such as dyspnea, fatigue, lesions compatible with pulmonary fibrosis; while, in the cardiovascular system, sequelae such as tachycardia, palpitations, arterial hypertension, myocarditis and pericardial effusion were reported. Likewise, sequelae such as headache, ageusia, anosmia, insomnia, amnesia and concentration deficit were evidenced in the nervous system; In the musculoskeletal system, arthralgia, myalgia, difficulty moving, and, finally, in mental health, the conditions recorded were anxiety, post-traumatic stress disorder, depression, apathy, dysphoria, and feelings of inferiority.

25. Alterações Longitudinais nos Níveis de Atividade Física e Parâmetros de Risco Cardiovascular em Pacientes com Doença Arterial Periférica Sintomática

Resumo Fundamento: Estudos transversais anteriores demonstraram que a atividade física está associada a menor risco cardiovascular em pacientes com doença arterial periférica (DAP). No entanto, não é possível estabelecer causalidade e estudos com desenho longitudinal são necessários. Objetivo: Analisar as alterações nos parâmetros de risco cardiovascular e níveis de atividade física após 2 anos de acompanhamento em pacientes com DAP sintomática. Métodos: O presente estudo teve início em 2015. Na primeira fase, foram incluídos 268 pacientes. Na segunda fase, após 2 anos (mediana = 26 meses), foram reavaliados 72 pacientes. Parâmetros de risco cardiovascular, como pressão arterial, modulação autonômica cardíaca e rigidez arterial, e níveis de atividade física foram medidos na linha de base e após 2 anos de acompanhamento. A associação entre as alterações delta (valores após 2 anos – valores da linha de base) na atividade física e nos parâmetros cardiovasculares foi analisada por meio de regressão linear múltipla. O nível de significância foi estabelecido em p < 0,05 com DAP. Resultados: Pacientes reduziram seus níveis totais de atividade física em comparação com a linha de base (linha de base = 2.257,6 ± 774,5 versus acompanhamento = 2.041 ± 676,2 min/semana, p = 0,001). Após o acompanhamento, o índice tornozelo-braquial (0,62 ± 0,20 versus 0,54 ± 0,20, p = 0,003) e o desvio padrão de todos os intervalos RR (43,4 ± 27,0 versus 25,1 ± 13,4 ms, p < 0,001) foram menores, enquanto a velocidade da onda de pulso carotídeo-femoral foi maior (9,0 ± 3,0 versus 10,7 ± 3,4 m/s, p = 0,002) em relação aos valores basais. Não observamos associação entre os valores delta dos níveis de atividade física e os parâmetros de risco cardiovascular. Conclusão: Pacientes com DAP tiveram níveis reduzidos de atividade física e comprometimento em relação aos parâmetros de risco cardiovascular após 2 anos de acompanhamento.

Background: there are several studies that show a relationship between cardiovascular hyperreactivity and some cardiovascular risk factors, but it is necessary to continue showing this relationship. Objective: to determine the association of cardiovascular hyperreactivity and obesity between sedentary and active individuals in Cienfuegos. Methods: a descriptive, cross-sectional and correlational study was carried out in a universe of 80 individuals between 18 and 30 years of age in the Cienfuegos municipality, during 2020. The variables analyzed were: age, sex, skin color, level of sedentary lifestyle, pressure baseline blood pressure with the sustained weight test, body mass index and abdominal waist. Pearson's Chi Square was used to describe the association of the study variables between sedentary and active individuals, as well as the T test for the comparison of means. Significance level p˂0.05. The results were expressed in charts. Results: ages 18-24 years old and white skin color predominated. 76.3 % have a sedentary lifestyle over assets. 65.6 % of the sedentary were female. In the anthropometric tests, more than 30 % were overweight-obese and 20 % had altered abdominal waist, for both the condition of being sedentary coexisted. The sedentary subjects presented mean values of the hemodynamic variables higher than the active subjects. 73.8 % of the sedentary subjects were cardiovascular hyperreactive, as opposed to the normoreactive cardiovascular state that was present in 94.7 % of the active subjects. Conclusions: the condition of sedentary lifestyle increases the values of the hemodynamic and anthropometric variables studied.

El electrocardiograma (ECG) es el principal medio diagnóstico para identificar las alteraciones en la conducción eléctrica del corazón. Por lo tanto, interpretar correctamente el ECG es una competencia esencial pero altamente compleja para los estudiantes de ciencias de la salud. Con el objetivo de facilitar el aprendizaje de electrocardiografía en nuestros estudiantes, se desarrolló un aplicativo móvil de acceso gratuito y contenido validado. El proceso de creación se completó en cuatro fases: creación de contenidos, validación con expertos, desarrollo del software y prueba de usabilidad. El contenido fue validado por cinco expertos, alcanzando un coeficiente de validez de contenido de .91 en claridad, .95 en precisión y .97 en relevancia. La prueba de usabilidad se realizó aplicando la versión en español del Computer System Usability Questionnaire (CSUQ) a 33 sujetos del programa de enfermería, quienes puntuaron: calidad del sistema 6.41/7, calidad de la información 6.07/7, calidad de la interfaz 6.55/7 y la satisfacción general 6.52/7. El ECG para enfermería consta de cuatro módulos de aprendizaje y un simulador del ritmo cardiaco para la identificación de las principales arritmias. Se publicó en 2021 y ha llegado a más de cuarenta países.

Coronaviruses are a large family of single-stranded ARN viruses that infect a wide variety of animals, including humans. The SARS-CoV-2 virus, which is responsible for the disease called COVID-19, has infected 27,249,308 people and caused 890,971 deaths worldwide until September 7, 2020. Considering the genetic similarities between SARSCoV- 2 and the epidemic coronaviruses SARS-CoV and MERS, presumably they share tropisms for specific cell lines and systemic conditions. The clinical and paraclinical characteristics of this new virus have been described in detail at the pulmonary level, although there is increasing evidence that it is a multisystemic agent. In the present work, we describe the extrapulmonary manifestations of COVID-19 reported to date, including hematological, cardiovascular, neurological, renal, muscular, ophthalmological, endocrine-metabolic, gastrointestinal, hepatobiliary, cutaneous and in special populations: pediatric (including multisystemic inflammatory syndrome) and pregnant women. It is essential to know the systemic complications of SARS-CoV-2 infection when managing these patients, given the potential risk to life of the most serious manifestations. Objetivo: Investigar os efeitos do treinamento dos esportes de combate, tendo como foco analisar especificamente as modalidades de Judô e Jiu Jitsu sob a frequência cardíaca e pressão arterial. Materiais e Métodos: Foi realizado uma revisão da literatura sistemática, utilizando na pesquisa as bases de dados Pubmed e Scielo. Resultados: Foram identificados 1945 artigos e após análise do material foram selecionados 04 manuscritos obedecendo os critérios de exclusão e inclusão com finalidade de serem trabalhados neste estudo. Ambas as modalidades selecionadas apresentaram efeitos positivos frente ao sistema cardiovascular. Discussão: Os principais efeitos são a redução da pressão arterial e frequência cardíaca, contatando que a prática do judô gera efeitos moduladores do sistema, diminuindo o risco de doenças cardiovasculares. Conclusão: Tanto o Judô quanto Jiu Jitsu são capazes de produzir modulações positivas no sistema nervoso autônomo, sendo uma excelente estratégia na prevenção de doenças cardiovasculares.

Indivíduos com lesão medular (LM) podem apresentar alterações no sistema cardiovascular, como diminuição da variabilidade da frequência cardíaca (VFC), decorrente da perda parcial e ou total da inervação autonômica. Nessa população, o exercício físico tem sido colocado como uma intervenção que pode minimizar essas disfunções cardiovasculares. Assim, o objetivo do presente estudo foi apresentar, por meio da revisão de literatura, o comportamento da VFC em indivíduos com LM em resposta ao exercício físico agudo e crônico. A busca foi realizada nas bases de dados PubMed, Web of Science, Scielo e BVS. Foram utilizados os descritores: lesão medular AND variabilidade da frequência cardíaca AND exercício físico (spinal cord injury, heart rate variability e physical exercise). Após a busca nas bases de dados foram encontrados 109 artigos sendo 65 no PubMed, 20 no Web of Science, 2 no Scielo e 22 na BVS. Desses, foram selecionados 12 artigos que preencheram os critérios de inclusão, sendo que, apenas 7 estudos avaliaram o efeito do exercício físico crônico na VFC e 5 artigos que avaliaram o efeito agudo. Indivíduos com LM preservam a VFC no repouso, no entanto, apresentam alguns déficits na capacidade de realizar ajustes cardiovasculares quando submetidos à sobrecarga imposta pelo exercício físico agudo. E a maioria dos estudos não encontrou diferenças na VFC ao efetuar a análise após o exercício físico crônico, independentemente do tipo, duração e intensidade do exercício físico.

Várias personalidades ao longo da história contribuíram para melhorar a acurácia diagnóstica. Contudo, o erro diagnóstico é ainda comum e causa um grande impacto nos sistemas de saúde. Para lidar com esse desafio, vários modelos de raciocínio clínico surgiram para sistematizar o processo de pensamento clínico. Este artigo descreve a história do raciocínio clínico e os métodos atuais de raciocínio diagnóstico, propõe um novo modelo de raciocínio clínico chamado Raciocínio Integrativo, e traz perspectivas sobre o futuro do raciocínio clínico.

Introduction and objectives: Studies assessing the effects of caffeine (CAF) on the cardiovascular system have demonstrated that CAF can delay cardiac recovery following exercise. This study intended to assess the impact of CAF intake before physical exercise on heart rate variability (HRV) and cardiovascular parameters. Methods: This is a prospective, crossover, controlled clinical trial conducted at the University of Pernambuco, Petrolina, PE, Brazil. The experimental protocol was split into three stages with a minimum of 48 hours between them. Exercises intensity was standardized based on the one repetition maximum test (1RM), obtaining the load of each volunteer for the intensity of 75% of 1RM. In the second and third phases, the control protocols were applied and 300 mg caffeine was given 45 minutes before training. HRV indices were determined at the subsequent times: 0 to 5 minutes of rest (before) and during 30 minutes of recovery (Rec) (after exercise), divided into six intervals, each of 5 minutes. Results: The final sample involved 30 volunteers. CAF delayed HRV recovery after resistance exercise. In general, CAF impaired recovery of HRV after resistance exercise. Significant changes were observed in the RMSSD, SDNN, TINN, SD1, low frequency and high frequency indices between the control and CAF group. Conclusion: CAF protocol delayed parasympathetic regulation of heart rhythm following exercise, slowing recovery of HR, blood pressure and HRV indices after exercise. Resumo: Introdução e objetivos: Estudos avaliando os efeitos da cafeína (CAF) sobre o sistema cardiovascular mostraram que a CAF pode retardar a recuperação dos batimentos cardíacos ao estado de repouso após o exercício. Este estudo teve como objetivo avaliar o impacto da ingestão de cafeína (CAF) antes do exercício físico de força sobre variabilidade da frequência cardíaca (VFC) na recuperação. Metodologia: Trata-se de um ensaio clínico prospetivo, controlado por cruzamento, realizado na Universidade de Pernambuco, em Petrolina, PE. Está registrado no ClinicalTrials.gov (Number NCT03899675). O procedimento experimental foi dividido em três etapas com um mínimo de 48 horas entre elas. A intensidade do exercício foi padronizada com base no teste de 1 repetição máxima (1RM), obtendo-se a carga de cada voluntário para a intensidade de 75% de 1RM. Na segunda e terceira fase da pesquisa, os protocolos de controle e cafeína (300 mg) foram aplicados. A ingestão de cafeína aconteceu 45 minutos antes do treinamento. Os índices de VFC foram determinados nos seguintes tempos: 0 a 5 minutos de repouso (antes) e durante 30 minutos de recuperação (Rec) (após o exercício), divididos em seis intervalos de 5 minutos cada. Resultados: A amostra final foi composta por 30 voluntários. No geral, a cafeína prejudicou a recuperação da VFC no período de recuperação após o exercício resistido. Diferenças significativas foram observadas nos índices RMSSD, SDNN, TINN, SD1, LF e HF entre o grupo controle e o grupo CAF. Conclusão: O protocolo CAF foi capaz de intensificar reduções na atividade parassimpática após o exercício, causando atraso na recuperação da FC, pressão arterial e índices de VFC após o exercício.

At present, investigations are more frequent in which it is reported that survivors of the SARS-CoV-2 infection do not regain their state of health prior to the viral process. The study collected information about the various sequelae recorded in original research indexed in databases such as Scielo, Scopus, and Pubmed, 11 articles were found from patients who had COVID-19, who participated in various studies in the post-acute stage of the acute disease process. the disease, some of them were even evaluated after 6 months of having started the symptoms, however, they continued presenting symptoms after the remission of the disease. In other articles, patients were evaluated by imaging tests such as computed axial tomography and cardiac magnetic resonance imaging. The results indicated sequelae in the respiratory system such as dyspnea, fatigue, lesions compatible with pulmonary fibrosis; while, in the cardiovascular system, sequelae such as tachycardia, palpitations, arterial hypertension, myocarditis and pericardial effusion were reported. Likewise, sequelae such as headache, ageusia, anosmia, insomnia, amnesia and concentration deficit were evidenced in the nervous system; In the musculoskeletal system, arthralgia, myalgia, difficulty moving, and, finally, in mental health, the conditions recorded were anxiety, post-traumatic stress disorder, depression, apathy, dysphoria, and feelings of inferiority. Introduction: at the beginning of professional Physiotherapy training in the Pulmonary Cardiovascular field, in Colombia, and its evolution who was parallel to the scientific and technological advances so that, at the end of the 20th century revealed a greater knowledge from the evaluation, treatment and improving life expectancy in patients with cardiovascular and chronic respiratory disease. Colombian universities that lead the undergrade programs in physiotherapy work jointly with the Colombian Association of Physiotherapy (ASCOFI) gave the support in continuing education, with specific seminars and certificates in the area, with national and international speakers, in the context of informal, which lead to a great subject reform at the credit level, content and objectives within the curriculum, in addition, clinical rotations, the aim was to form a profile optimizing insertion labor and social development with specialized skills and competencies at the pulmonary cardiovascular level that favor the integration, positioning and impact the physiotherapist role in the Intensive Care Units (ICU), emergencies, hospitalization and external consultation. The effectiveness of this alliance was the pillar for the creation of specializations in Critical Care, Cardiac Rehabilitation, Pulmonary Rehabilitation and Cardiorespiratory Physiotherapy in our country. Development: the article is based on the reflection of the training and professional performance of the Colombian Physiotherapist within the Pulmonary Cardiovascular Domain (PCD). Their advances, scope and demands to defend their professional practice in this area, and likewise the development and dynamics of the physiotherapist in our country, which reflected in the autonomy, the introduction of a large number of actions and behaviors of specialized care and comprehensive approach between the PCD, neuro-musculoskeletal and integumentary domains which have turned the physical therapist an essential resource in all hospital and health care centers, favoring the optimization of resources, the timely and effective response to the growing demand for respiratory and cardiovascular disease. Conclusion: although the balance in the PCD in Colombia is positive, the demand continues in an exponential grow, it is important to consider the distribution of specialist physiotherapists or those with optimal skills in the area in different regions of the country, who can respond to the community health care needs and their epidemiological profile, propose alliances, actions and discussions with scientific, governmental and educational associations that allow regulating the quality of postgraduate training programs and continuing education that arise permanently. Objetivo: o presente estudo tem como objetivo revisar os dados disponíveis na literatura para verificar se a periodontite apical (PA) é um fator de risco para as doenças cardiovasculares (DCVs). Materiais e Métodos: foi realizada uma busca nas bases de dados do Pubmed e Google Acadêmico sobre a possível associação da PA com o desenvolvimento de problemas cardiovasculares. Após os critérios de seleção, foram incluídos 8 artigos científicos para serem detalhadamente abordados. Revisão de literatura: embora a PA seja um processo inflamatório local, sua patogênese pode influenciar no desenvolvimento de complicações sistêmicas. Partindo do princípio que uma resposta inflamatória localizada é capaz de acarretar à liberação e ativação de mediadores pró¬-inflamatórios sistêmicos, como as citocinas interleucinas e fatores de necrose tumoral, alguns autores sugerem uma possível associação entre a PA crônica e as DCVs, devido o potencial de surgimento de danos endoteliais e formação de ateromas. Discussão: embora ainda seja questionável a correlação existente entre lesões inflamatórias periapicais e doenças sistêmicas, devido principalmente ao descrédito em que o assunto era tratado no passado e na quantidade limitada de informações conclusivas disponíveis atualmente, os dados revisados neste trabalho propõem que existe uma associação positiva entre a PA e as DCVs, podendo-se verificar uma maior prevalência de PA em pacientes com problemas cardiovasculares. Conclusão: tem-se disponível cientificamente uma quantidade limitada de achados. Portanto, sugere-se a realização de mais pesquisas sobre o tema, objetivando a busca por evidências relevantes e bem fundamentadas sobre esta associação.

45. Effect of 24 sessions of stretching exercises performed before resistance exercises on hemodynamic responses

- Abstract
The present study aimed to compare the effect of 24 sessions of stretching exercises performed before resistance exercises versus resistance exercise sessions on the hemodynamic responses of sedentary individuals. Thirty volunteers (26.76±6.29 years old, 81.56±10.66 kg body weight, 1.74±0.07 m height, and 26.74 ±.82 kg/m2 body mass index) participated in this study. After performing the test and retest of 10 maximum repetitions (10RM), the volunteers were divided into 3 groups: a) stretching exercise + resistance exercise (SE+RE); b) resistance exercise (RE) and c) control group (CG). The RE was composed of 3 sets of 10 repetitions at 80% of 10RM with a 2-minute interval between sets of RE preceded or not by 3 sets of 30 seconds of SE. Blood pressure (BP) was assessed using an automatic oscillometric device at session 0, session 12 and session 24 of training. Systolic BP (SBP), diastolic BP (DBP), mean BP (MAP), heart rate (HR), double product (DP) and oxygen saturation (SpO2) were measured after 10 minutes of rest. Comparison between groups was performed by Kruskal Wallis test with Bonferroni correction and 5% significance (p < 0.05). DBP showed significant difference when comparing session 0 vs. session 24 (p = 0.006) for the RE protocol. MAP decreased significantly after session 24 for the RE protocol compared to the other protocols (p = 0.016) and difference between session 0 vs. session 24 (p = 0.022). Significant difference was found in SE + RE protocol (p = 0.029) compared to session 0 vs. session 24 GC. This study showed that RE improved the DBP and MAP response and that the combination of SE+RE improved the SpO2 response after 24 training sessions., El presente estudio tuvo como objetivo comparar el efecto de 24 sesiones de ejercicios de estiramiento realizados antes de ejercicios de resistencia versus sesiones de ejercicios de resistencia sobre las respuestas hemodinámicas de individuos sedentarios. Treinta voluntarios (26.76±6.29 años de edad, 81.56±10.66 kg de peso corporal, 1.74±0.07 m de altura y 26.74±.82 kg/m2 de índice de masa corporal) participaron en este estudio. Después de realizar la prueba y la retest de 10 repeticiones máximas (10RM), los voluntarios se dividieron en 3 grupos: a) ejercicios de estiramiento + ejercicios de resistencia (EE+ER); b) ejercicios de resistencia (ER) y c) grupo de control (GC). El ER se compuso de 3 series de 10 repeticiones al 80% de 10RM con un intervalo de 2 minutos entre series de ER precedidas o no por 3 series de 30 segundos de EE. La presión arterial (PA) se evaluó utilizando un dispositivo oscilométrico automático en la sesión 0, sesión 12 y sesión 24 de entrenamiento. La presión arterial sistólica (PAS), la presión arterial diastólica (PAD), la presión arterial media (PAM), la frecuencia cardíaca (FC), el doble producto (DP) y la saturación de oxígeno (SpO2) se midieron después de 10 minutos de reposo. La comparación entre grupos se realizó mediante la prueba de Kruskal Wallis con corrección de Bonferroni y un 5% de significancia (p < 0.05). La PAD mostró una diferencia significativa al comparar la sesión 0 vs. la sesión 24 (p = 0.006) para el protocolo ER. La PAM disminuyó significativamente después de la sesión 24 para el protocolo ER en comparación con los otros protocolos (p = 0.016) y la diferencia entre la sesión 0 vs. la sesión 24 (p = 0.022). Se encontró una diferencia significativa en el protocolo EE+ER (p = 0.029) en comparación con la sesión 0 vs. la sesión 24 del GC. Este estudio mostró que el ER mejoró la respuesta de la PAD y la PAM, y que la combinación de EE+ER mejoró la respuesta de SpO2 después de 24 sesiones de entrenamiento.

Based on previous work developed by EETAC research groups, we want to work on a wireless measurement system for mechanical and electrical signals from the cardiovascular system that can be acquired with sensors placed on clothing or footwear that allow them to be measured comfortably and continuously without requiring any action by the user. In this work we will develop a system for acquiring these signals by means of a microcontroller (preferably low power consumption) that will subsequently send them via Bluetooth to a PC or smartphone where the data received can be observed and stored. The monitoring of physiological parameters in a comfortable and non-invasive way outside hospital environments (in this case applied to aircraft pilots) is a topic of great interest nowadays in the field of the search for new medical applications of engineering., A partir del trabajo previo desarrollado por los grupos de investigación de la EETAC, se quiere trabajar sobre un sistema de medida inalámbrico de las señales de origen mecánico y eléctrico provenientes del sistema cardiovascular y que se pueden adquirir con sensores colocados sobre la ropa o en el calzado que permiten medirlos de forma cómoda y continúa sin requerir ninguna acción por parte del usuario. En este trabajo se desarrollará un sistema de adquisición de estas señales mediante un microcontrolador (preferentemente de bajo consumo) que posteriormente los enviará vía Bluetooth hacia un PC o un smartphone en el que se podrán observar y almacenar los datos recibidos. La monitorización de parámetros fisiológicos de forma cómoda y no invasiva fuera de entornos hospitalarios (en este caso aplicada a los pilotos de aeronave) es un tema de gran interés actualmente en el campo de la búsqueda de nuevas aplicaciones médicas de la ingeniería.

- Abstract
Aims: To study the role of lipocalin-2 (Lcn2) and the effect of Lcn2 blockade via anti-Lcn2 antibody in the development of abdominal aortic aneurysm (AAA). Methods and results: Expression mRNA and protein levels of Lcn2 and its human orthologue neutrophil gelatinase-associated lipocalin (NGAL) in aortic wall samples from experimental mouse and human AAA samples, respectively, were analysed by real-time PCR and immunohistochemistry. Experimental AAA was induced by aortic elastase perfusion in wild-type mice (WT) and Lcn2-deficient mice (Lcn2-/-). NGAL/Lcn2 mRNA and protein levels in human and murine AAA samples were increased compared with healthy aortas. Decreased AAA incidence and reduced aortic expansion were observed in Lcn2-/- mice or mice preoperative treated with a polyclonal anti-Lcn2 antibody compared with WT mice or mice treated with control IgG, respectively, at Day 14 after elastase perfusion. Moreover, immunohistochemical analysis of AAA tissues from Lcn2-/- or anti-Lcn2-treated mice showed diminished elastin damage, reduced microvessels and polymorphonuclear neutrophil (PMN) infiltration, and enhanced preservation of vascular smooth muscle cells compared with WT aortas. Fluorescent molecular tomography revealed decreased MMP activity in AAA of Lcn2-/- mice compared with WT controls. Therapeutic administration of anti-Lcn2 antibody to WT mice 3 days after elastase perfusion decreased aortic dilatation and PMN infiltration compared with WT mice treated with control IgG. Conclusion: Either Lcn2 deficiency or anti-Lcn2 antibody blockade limits AAA expansion in mice by decreasing PMN infiltration in the aorta. Introdução: O Diabetes Mellitus é uma das doenças crônicas mais complexas do ponto de vista psicológico, pois os aspectos psicológicos, incluindo o tipo de personalidade, podem influenciar em um pior prognóstico da doença, comprometendo a adesão ao tratamento medicamentoso, bem como mudanças nos hábitos de vida. Objetivo: Analisar a associação entre personalidade e risco cardiovascular em pessoas com diabetes tipo 2 atendidas na Atenção Primária a Saúde. Método: Revisão sistemática realizada nas bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE via PubMed), Web of Science, Scopus, Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS), Excerpta Medica Database (EMBASE) e PsycINFO, sem restrição de data, nos idiomas português, inglês e espanhol, e busca adicional na lista de referências dos estudos selecionados. Foram incluídos estudos observacionais realizados com a população adulta de ambos os sexos com diabetes tipo 2. O risco de viés nos estudos foi avaliado pela escala Newcastle-Ottawa. Resultados: Foram incluídos sete estudos publicados entre 1998 e 2019, a maioria originários dos Estados Unidos, com predomínio de estudos transversais. A amostra variou de 95 a 15.029 participantes, em sua maioria mulheres com idade de 41,8 anos. Os estudos transversais foram de baixa qualidade, caracterizando alto risco de viés. A personalidade está relacionada ao sofrimento emocional, pior percepção de saúde, depressão, menor apoio social e prevalência do tipo D. Considerações finais: A personalidade impacta a adesão aos comportamentos de saúde, que influenciam o desenvolvimento de complicações cardiovasculares em pessoas com diabetes tipo 2., Introduction: Diabetes Mellitus is one of the most complex chronic diseases from a psychological point of view, as psychological aspects, including personality type, can influence a worse prognosis of the disease, compromising adherence to medication treatment, as well as changes in habits of life. Objective: To analyze the association between personality and cardiovascular risk in people with type 2 diabetes treated in Primary Health Care. Method: Systematic review carried out in the databases Medical Literature Analysis and Retrieval System Online (MEDLINE via PubMed), Web of Science, Scopus, Latin American and Caribbean Literature in Health Sciences (LILACS), Excerpta Medica Database (EMBASE) and PsycINFO, without date restrictions, in Portuguese, English and Spanish, and additional search in the list of references of selected studies. Observational studies carried out with the adult population of both sexes with type 2 diabetes were included. The risk of bias in the studies was assessed using the Newcastle-Ottawa scale. Results: Seven studies published between 1998 and 2019 were included, most originating in the United States, with a predominance of cross-sectional studies. The sample ranged from 95 to 15,029 participants, the majority of whom were women aged 41.8 years. The cross-sectional studies were of low quality, characterizing a high risk of bias. Personality is related to emotional distress, worse health perception, depression, less social support and prevalence of type D. La personalidad se relaciona con malestar emocional, peor percepción de salud, depresión, menor apoyo social y prevalencia del tipo D. Consideraciones finales: La personalidad impacta la adherencia a conductas de salud, que influyen en el desarrollo de complicaciones cardiovasculares en personas con diabetes tipo 2.

50. Hypertension alters the function of nitrergic and sensory innervation in mesenteric arteries from female rats

- Abstract
Objectives To investigate whether hypertension could modify the function of adrenergic, nitrergic, and sensory innervations involved in the electrical field stimulation-induced response in mesenteric arteries from female rats. Methods Vascular reactivity experiments were performed in endothelium-denuded mesenteric arteries from normotensive, Wistar–Kyoto and spontaneously hypertensive female rats; protein expression was measured by western blot; nitric oxide release was measured by fluorometry; calcitonin gene-related peptide and noradrenaline release were determined by enzyme immunoassay. Results The electrical field stimulation-induced contractions were significantly lower in segments from spontaneously hypertensive rats than those of Wistar–Kyoto rats. Hypertension did not modify either the response or release of noradrenaline. Preincubation with the nitric oxide synthase inhibitor NG-nitro-L-arginine methyl ester increased the electrical field stimulation-induced contractions only in segments from Wistar–Kyoto rats. The relaxation induced by the nitric oxide donor sodium nitroprusside was similar in segments from both strains. The electrical field stimulation-induced nitric oxide release was decreased in segments from spontaneously hypertensive rats. The calcitonin gene-related peptide receptor antagonist CGRP(8–37) did not alter the electrical field stimulation-induced contractions in segments from Wistar–Kyoto rats but increased them in segments from spontaneously hypertensive rats. The calcitonin gene-related peptide-induced relaxation was increased in segments from spontaneously hypertensive rats. The expression of the 15-kDa active form of RAMP1 was increased in segments from spontaneously hypertensive rats. Conclusion In contrast to male rats, electrical field stimulation-induced contractions are decreased in hypertensive female rats.
