Background: Patients undergoing surgery and anaesthesia are in a vulnerable situation, requiring advanced treatment and care. Nurse anaesthetists are specially trained to meet their needs when administering and monitoring anaesthesia, ensuring patient safety and comfort throughout the entire perioperative care. However, there is a growing concern that efficiency requirements might take precedence over humanistic care when having to prioritize. Most people have a limited understanding of the extended role of nurse anaesthetists in maintaining quality and safety in anaesthesia services. This study aimed to explore how nurse anaesthetists describe their practice as being nursing when caring for surgical patients. Methods: We applied a qualitative inductive design. Twenty nurse anaesthetists working in small or large hospitals in different regions of Norway were recruited. Semi-structured individual interviews were carried out. Data were analysed using qualitative content analysis. This study was conducted and reported in accordance with the Consolidated Criteria for Reporting Qualitative Research Checklist. Results: We identified three categories and eight subcategories. The category Continuously attending to physical and psychological needs involved reading the patient and responding accordingly and the ethical and moral obligation to maintain patients' well-being and dignity. The category Providing a concerned presence involved the importance of the nursing process and nurse anaesthetists' self-confidence in their professional abilities and readiness to act when caring for the surgical patient. The category Aspiring towards excellence involved expectations towards nurse anaesthetists as professionals who display professional courage and undertake professional development through continuous learning. Conclusion: Nurse anaesthetists experience themselves as being highly qualified to address the needs of surgical patients. Their nursing background and extensive training have prepared them to know how and argue why. Knowledge from this study could be expected to add to the field of describing and understanding nurse anaesthetists' practice in the perioperative context and support supervision in postgraduate education. This research is a descriptive and cross-sectional study aimed at examining the use of three different scales to assess the risk of pressure injury (PI) in surgical patients. This study was conducted between February 1 and July 1, 2022. The study included patients who had planned surgery in general surgery clinic. The sample size was 388 patients. Patients who agreed to participate in the study were asked to fill out the " Introductory Information Form " and perform a risk assessment using the Braden, Munro and 3S Scales. The statistics program IBM SPSS Statistics 25.0 packaged software was used in the analyses of data. It is evident that the relationship between the scales and the diagnosis of PI, as well as the strength of this relationship, are statistically significant (p < 0.05). The Munro Scale exhibited the highest value of Phi value and validity coefficient c, indicating a stronger association with the diagnosis of PI, thus suggesting its greater effectiveness in discrimination. According to the validity coefficients found, it can be stated that the correct classification percentages for Munro, Braden, and 3S Scales were 91 %, 71 %, and 66 %, respectively. The findings of this study indicate that both the Braden, Munro, and 3S Scales can be used for PI risk assessment in surgical patients, but the Munro Scale exhibits superior predictive validity compared to the Braden and 3S Scales in terms of overall sensitivity and specificity. • The risk of PI in surgical patients should be assessed with a valid and reliable risk assessment tool. • There is no clear evidence regarding which PI risk assessment tool should be used in surgical patients. • Munro and 3S Scale are among the PI risk assessment tools in surgical patients. The aim of this study was to determine and compare non-pharmacologic methods for the management of postoperative sore throat in adult patients undergoing endotracheal intubation. This study used a systematic review of clinical trials. Articles published between 2010 and 2022 in PubMed, Scopus, Web of Science, MEDLINE, EBSCOHost databases were included. The review was organized according to Cochrane Collaboration guidelines and reported using Preferred Reporting Items for Systematic Reviewsand Meta-Analyses. A total of 857 articles were retrieved in the initial search. After reviewing the articles according to the inclusion and exclusion criteria, a final set of seven articles was evaluated. It was observed that ASA I-II patients in whom elective surgery was planned were mostly included in the studies. It was determined that cold vapor, ice cube, licorice gargle, luo han guo (Monk fruit) herbal tea and tube warming were used as non-pharmacological methods. Interventions were performed preoperatively in five of the studies and postoperatively in two. Numeric pain scale was frequently used to assess sore throat. Patients’ sore throat was most commonly evaluated in the second, fourth and 24th hours after extubation. Cold vapor, licorice gargle, luo han guo herbal tea and tube warming were found to be effective in reducing sore throat. Several reliable non-pharmacological methods are available for managing a sore throat in patients undergoing endotracheal intubation. However, more research is needed to determine the most effective non-pharmacological approach.

Relevance. One of the tasks of modern surgery is to provide high-quality medical care with a significant impact on the patient, which should lead to an improvement in the quality of life. Quality of life assessment is becoming increasingly relevant in healthcare and is associated with improved decision-making, higher satisfaction and better treatment outcomes.The purpose of the study. To identify criteria for an objective assessment of the quality of life of patients in the postoperative period.Materials and methods. In the course of this study, the most relevant domestic and foreign literary sources were analyzed, reflecting the problem of assessing the quality of life of patients in the postoperative period.Results. To assess the quality of life, various forms of questionnaires for patients are usually used. Questionnaires can be both general and specific for a particular disease. After analyzing numerous questionnaires to determine the quality of life, it became possible to combine and classify objective criteria for the quality of life.Conclusion. The use of general and special questionnaires makes it possible to perform a full-scale assessment of both the main components of the quality of life and its specific aspects in this disease. At the same time, it is important to take into account the influence of various factors, including gender, age, and the initial quality of life, on the change in indicators over time. This approach is the most correct and corresponds to modern recommendations for analyzing quality of life data that changes over time and is associated with various factors.

Summary: This study was conducted to determine the effects of the use of ergonomic sleep mask on sleep quality and comfort in intensive care patients. This randomised controlled experimental study was completed with 128 surgical intensive care patients (control = 64, experimental = 64). During the second night of their stay in the unit, ergonomic sleep masks were given to the patients in the experimental group, and earplugs and eye masks were given to the patients in the control group. A Patient information form, Visual analogue scale for discomfort, and the Richard‐Campbell sleep questionnaire were used to collect data. While 51.6% of the patients were female, the mean age of the patients was 63.87 ± 14.94 years. The highest rates of patients had undergone cardiovascular surgery (28.9%) and general anaesthesia (57.8%). It was determined that the sleep quality of the patients in the experimental group was statistically and clinically significantly higher after the intervention (50.86 ± 21.46 vs 37.64 ± 14.97, t = −5.355, Cohen's d = 0.450, p < 0.001). Likewise, the patients who used ergonomic sleep masks had a statistically significantly lower mean VAS for Discomfort score, and their comfort level was higher (p < 0.001), but the difference was not clinically significant (Cohen's d = 0.208). The results of this study showed that the use of ergonomic sleep masks in surgical intensive care patients had a more positive effect on both the sleep quality and comfort levels of patients compared with earplugs and eye masks. The use of an ergonomic sleep mask is recommended in the early period to facilitate sleep and rest in surgical intensive care patients. [ABSTRACT FROM AUTHOR]

Methods CiteSpace software is a Java application that supports knowledge research, data visualization and analysis of literature databases. The following data were retrieved: title, abstract, year, keywords, author, academic institution, journal and citations. The included literature was analysed using CiteSpace visualization software, knowledge mapping, collaborative network analysis, cluster analysis and highlighting keywords for visualization. Results A total of 1288 articles related to hypothermia in surgical patients were included, with the largest number of articles published in the United States (448), followed by in Germany (104) and China (102). Background: Guaranteeing and preserving individuality in care is conceivable with individual planning and implementation of interventions in helping to solve the problems determined by the evaluation of all dimensions of the patient and with the contribution of the patient in the care pronouncements. Aim: To investigate the association between surgical patients' awareness of individualized care and their satisfaction with nursing care. Methods: The cross-sectional, descriptive, and correlational study conducted with 112 patients hospitalized in a state hospital where they agreed to participate between May and August 2019. Participants completed Patient information forms, "Individualized Care Scale" and "The Newcastle Satisfaction with Nursing Scale". Findings: The mean age of the surgical patients contributing to the study was 41.28±19.82 (min-max: 18-84) and the mean duration of hospitalization was 3.35±3.19 (min-max: 1-23). Moreover, the mean score of Recognizing the Nursing Action was 3.43 ± 0.94, and the mean score of Awareness of Individuality was 3.66 ± 0.81, which is the subscale of Individualized Care Scale; the mean Newcastle Satisfaction with Nursing Scale score is 69.80 ± 15.51. It was shown that there was a positive correlation between the patients' mean Recognizing the Nursing Action, Awareness of Individuality, and Newcastle Satisfaction with Nursing Scale scores (p < 0.01). Conclusion: The satisfaction level increases when the level of awareness of individualized care of surgical patients increases. Despite preoperative anxiety having many negative consequences on post-operative physical and mental health, no adequate information on the degree to which the preoperative period exposed clients to preoperative anxiety and its associated factors.Objective: To assess the prevalence of preoperative anxiety and associated factors among adult surgical patients in Debre Berhan Comprehensive Specialized Hospital, Ethiopia.Methods: Institution-based cross-sectional study was conducted from June 1, 2020, to August 30, 2020. State-Trait Anxiety Inventory Scale (S-STAI) was used to measure the level of pre-operative anxiety. A systematic random sampling technique was employed to collect the data. The data were entered to Epi-Data version 4.6 and exported to SPSS version 25 for data analysis. A statistically significant association was declared at a p-value less than 0.05.Results: A total of 330 patients were enrolled in the study with a response rate of 93.2%. The prevalence of a high level of preoperative anxiety was 53.6% (95% CI (49.6– 58.7)). Being female 3.2 (2.19, 3.71) illiterate 4.1 (2.01, 15.39), fear of death 2.12 (1.30, 3.44), results of operation 2.60 (1.75, 4.18), postoperative pain 2.35 (1.37, 4.02), and surgical complication 1.9 (1.03, 12.13) were statistically significant factors.Conclusion: Preoperative anxiety affects more than half of surgical patients which is a high and serious health burden. Additionally, this study revealed that being female, illiterate, fear of death, having a history of surgical complications and fear of complications, and fear of postoperative pain were significantly associated with preoperative anxiety. Assessing during the preoperative period helps to detect and ameliorate the problem. Use of anaesthesia is essential in clinical procedures, be it minor or major. Without anaesthesia, the procedure may be technically infeasible. Though the use of anaesthetic drugs help in painless and smooth conduction of medical procedures, yet it is not risk free. There are many factors that influence the performance of anaesthesia and so the associated risks. Among all factors, health of a person has the greatest impact on the performance of anaesthesia. In this study, we computationally analyse the impact of nutritional status of a person on the performance of anaesthesia in terms of required time and quantity. We formulate mathematical equations to relate all the three situations of nutritional status with the performance of anaesthesia. Our experimental results show that under-nutritional status and over-nutritional status have significant impact on the performance of anaesthesia while normal-nutritional status doesn't have much impact (negative or positive). This computational analysis can add an extra flavour to the pre-operative and post-operative preparations and decision making of the clinicians which can mitigate the associated risks. Abstract Background The relationship between the health care provider and the patient is an indispensable element of medical care. The existence of a proper therapeutic relationship between the health care provider and the patient can increase patients’ trust and willingness to communicate, improve adherence to medical recommendations, enhance continuing care, and promote patient satisfaction. However, little is known in developing countries including Ethiopia what the patient health care provider relationship looks like. This study aimed to assess the health care provider-patient relationship during preoperative care in obstetric and gynecologic surgeries at Jimma Medical Center, Jimma, Ethiopia. Methods Institution-based cross-sectional study was conducted from April 1 to May 30, 2020, at Jimma Medical Center. A total of 372 surgical patients were selected using a systematic random sampling method. The collected data were coded, entered into Epi data version 3.1, and analyzed using the statistical package for social science (SPSS) version 25. Bivariate and multivariable regression was carried out to determine the association between the outcome variable and the independent variable. The strength of association of dependent and independent variables was presented by crude and adjusted odds ratio at a 95% confidence interval. Variables with a p value of < 0.05 were considered statistically significant. Results The proportion of good patient to health care provider relationship in this study was 179 (48%) and it had a significant association with patient marital status AOR = 0.29 (95% CI 0.147–0.580), consent form available AOR = 0.162 (95% CI 0.035–0.750), the profession of healthcare providers who request the consent AOR = 0.305 (95% CI 0.117–.794), mode of decision-making AOR = 0.165 (95% CI 0.039–.709), and patient’s satisfaction AOR = 5.34(95% CI 3.1–9.16). Conclusions The proportion of patient-to healthcare providers’ relationship was low. More than half of the respondents did not have good patient–health care provider relationship. Hence, health care providers should be concerned about their relationship with their patients to increase the quality of medical care. The health care providers should bear in mind that patients may refuse to seek care from a provider whose relationship is not strong, even if the provider is skilled in preventing and managing complications.

IntroductionCritical care is a serious global healthcare burden. Although a high number of surgical patients are being admitted to the surgical intensive care unit (SICU), the mortality remained high, particularly in low and middle-income countries. However, there is limited data in Ethiopia. Therefore, this study aimed to investigate the survival status and predictors of mortality in surgical patients admitted to the SICUs of Addis Ababa governmental hospitals, Ethiopia.MethodsA multicenter retrospective cohort study was conducted on 410 surgical patients admitted to the SICUs of three government hospitals in Addis Ababa selected using a simple random sampling from February 2017 to February 2020. Aim:There is a need to assess the pain fear levels of surgical patients simply and appropriately before the surgery. This study aimed to develop and test the psychometrics of an instrument to evaluate the fear of postoperative pain.Methods:This methodological study was conducted at four surgical clinics including general surgery, orthopedics and traumatology, neurosurgery, heart and vessel surgery of a university hospital between 28 March and 19 October 2018. Totally, 150 patients who were scheduled for elective surgery and at their preoperative day were included. This scale contained 10 items related to the postoperative pain sources and aimed to identify the pain fear of patients preoperatively. To measure the sampling adequacy, Kaiser-Meyer-Olkin index and Bartlett’s test of sphericity was used. Explanatory and confirmatory factor analyses were conducted to evaluate the construct validity. The internal consistency of the scale was evaluated by Cronbach’s alpha calculation. All hypotheses were tested in two directions.Results:The total variance explained 55.5% of the variance for one factor structure consisting of 10 items. The model fit index values through the confirmatory factor analysis were found to support this structure.Conclusion:The scale is appropriate to be used in clinical settings to quickly evaluate the elective surgical patients’ fear level of postoperative pain preoperatively.

The present opioid epidemic in the United States is a significant cause for concern in healthcare. In 1995, the concept of pain was introduced as the fifth vital sign. Since then, the sales of opioids have increased dramatically, as have the number of opioid deaths. The misuse and diversion of retained opioids following surgical procedures contribute to the problem. The objective of this project was to review the latest scholarly work and evaluate the findings related to patient education and disposal of opioid medications to decrease opioid misuse and increase disposal. A systematic review. The systematic search strategy included PubMed, Ovid Technologies (OVID), and Cumulative Index of Nursing and Allied Health Literature (CINAHL) electronic databases. A total of 4 randomized controlled trials (RCTs), 2 quasi-experimental studies, and 2 quality improvement projects met the criteria for inclusion. The studies found that as many as 92% of patients had leftover unused opioids. The retention rate of opioids among surgical patients was found to be 33 to 95%. When educational material was provided about disposal, the studies found that the disposal rate was as high as 71%. Patient education about opioid misuse, diversion, and disposal are essential topics that need to be addressed with patients and caregivers. [ABSTRACT FROM AUTHOR]

Objective: This article explores the effect of preoperative health education, in the form of animation videos, on postoperative self-reported pain levels and anxiety in femoral fractures. Methods: Ninety cases of femoral fracture were divided at random into the oral instruction group, the recorded video group, and the animation video group, with 30 cases in each group. Sociodemographic data were collected the day before surgery. Health education was then offered in one of three ways: orally, using a recorded video, or using an animation video. On days 2, 4, and 7 after surgery, the state-trait anxiety inventory (STAI) and the visual analog scale (VAS) were used to assess postoperative anxiety and pain levels, respectively, in the participants. Results: At different time points during the evaluation, total anxiety scores in the animation and recorded video groups were significantly lower than in the oral instruction group (P < 0.01), and the pairwise comparisons indicated statistically significant differences (F = 11.04, 10.06, 10.37, P < 0.01). However, the levels of postoperative pain in the animation and recorded video groups were not significantly different (P > 0.05). STAI scores in the three groups were found to have significant interactions with the measurement time (F = 6.74, P < 0.01). However, there were no apparent interactions between the VAS score and the measurement time (F = 1.31, P > 0.05) in the three groups. Conclusion: Preoperative health education with the aid of multimedia is more effective than oral instruction in lowering patients' postoperative anxiety and pain levels. In addition, animation videos are superior to recorded videos in mitigating postoperative anxiety. Background: cognitive impairment is known to be very common in patients with hip fractures, but studies are needed to help understand the relationship between both events. Our goal was to determine the relationship between moderate or severe cognitive impairment and hip fractures during the six months following that episode. Methods: a retrospective longitudinal study was conducted on a sample of 665 people over 65 years of age. The main variable of the study was cognitive impairment at six months of fracture, assessed using the Pfeiffer scale (PS). Other data related to clinical features were also collected for further analysis. Results: binary logistic regression analyses showed that the main factors related to moderate or severe cognitive impairment at the sixth month of the fracture were age (OR = 1.078), initial cognitive impairment (OR = 535.762), and discharge (OR = 547.91), cognitive worsening at the sixth month with respect to the time of admission (OR = 7.024), moderate dependence on admission (OR = 15.474) and at six months (OR = 8.088), poor ambulation at discharge (OR = 5.071) and institutionalization prior to admission (OR = 5.349) or during the first semester after fracture (OR = 6.317). Conclusions: this research provides evidence about the clinical factors that predict moderate or severe cognitive decline at the sixth month in patients undergoing surgery for a hip fracture. Background: Family has a pivotal impact on the individuals' health status and the society as well. Family involvement in care provision has been investigated in nursing literature. Aim: To investigate family role in care provision to surgical patient. Methodology: A review of the contemporary literature was conducted on the database PubMed/Medline and the search engine Google Scholar with the keywords: «family influence», «family role», «surgical patient» and «surgery». Data were extracted according to eligibility criteria, analyzed and synthesized. Results: From the analysis of the results of the studies included it was found that the active family involvement contributes to the management of perioperative pain in pediatric patients, while, at the same time, ensures the follow up process and an effective communication with the health care professionals postoperatively. Among family members, spouses/husbands provide psychological support to the adult surgical patients and have a significant impact on the decision-making process regarding to the surgical procedures. Conclusions: Family members support and enhance the decision making process related to surgical procedures of the adult surgical patients in the context of a partnership - the family act as a partner in surgical care provision. Moreover, the essential and active involvement of the family facilitates the care provided to pediatric surgical patients so the family acts as a facilitator in surgical care provision. Hasilnya diperoleh lima artikel kuantitatif dan dua artikel kualitatif. Berdasarkan studi literatur yang dilakukan didapatkan bahwa nyeri, kebisingan, lingkungan (suhu, dan pencahayaan), dan faktor psikologi merupakan faktor – faktor yang mempengaruhi kualitas tidur pasien pasca pembedahan. Nyeri merupakan faktor terbanyak penyebab gangguan tidur. Pengetahuan perawat tentang pentingnya kualitas tidur dan faktor yang mempengaruhi sangat diperlukan untuk mengantisipasi terjadinya gangguan tidur yang dapat menghambat proses pemulihan pasien pasca pembedahan. ABSTRACT Sleep disturbance can reduce patients’ pain tolerance, and interrupt post-operative rehabilitation. This study aims to explore literature relating to factors that affecting sleep quality in post-operative orthopaedic patients undergoing hospitalization. The method used is the study of literature through Universitas Indonesia online database (ScienceDirect, Clinica Key, Sage journals) and PubMed by using sleep disturbance, factors affecting sleep, sleep quality among orthopaedic patients, sleep on surgical patients, insomnia, sleep, and hospitalization as keywords. The acquired journal was measured using Critical Appraisal Skills Program (CASP), with article criteria as follow: cross-sectional design, cohort study, Randomized Controlled Trial (RCT), Case-control/series and Qualitative Research, full text, open access, written in English and published years between 2014 and 2019. The results were obtained 5 quantitative journals and two qualitative journals. Based on literature studies conducted, pain, noise, environment (temperature, and lighting), and psychological factors are factors that affecting sleep quality among surgical patients. Pain is the dominant factor that affects sleep. Nurses' knowledge about the importance of sleep quality and factors that cause sleep disturbance are needed to anticipate sleep disturbance in the future that can affect post-operative rehabilitation.

Hypertension is the most common medical co morbidity encountered in a perioperative patient. It often causes a dilemma in management and results in postponement of surgery. Although the consequences of uncontrolled hypertension have significant adverse out come in a surgical patient, there are no national guidelines or protocols on how to manage such a situation. Having a clear understanding about the evaluation of such patients; target blood pressure levels; selection and timing of appropriate antihypertensive medication would improve the outcome of these patients. This review article would focus on a practical guide to the clinicians in evaluating and managing a patient with perioperative hypertension in a Sri Lankan setting.

ObjectiveThis article explores the effect of preoperative health education, in the form of animation videos, on postoperative self-reported pain levels and anxiety in femoral fractures.MethodsNinety cases of femoral fracture were divided at random into the oral instruction group, the recorded video group, and the animation video group, with 30 cases in each group. Sociodemographic data were collected the day before surgery. Health education was then offered in one of three ways: orally, using a recorded video, or using an animation video. On days 2, 4, and 7 after surgery, the state-trait anxiety inventory (STAI) and the visual analog scale (VAS) were used to assess postoperative anxiety and pain levels, respectively, in the participants.ResultsAt different time points during the evaluation, total anxiety scores in the animation and recorded video groups were significantly lower than in the oral instruction group (P < 0.01), and the pairwise comparisons indicated statistically significant differences (F = 11.04, 10.06, 10.37, P < 0.01). However, the levels of postoperative pain in the animation and recorded video groups were not significantly different (P > 0.05). STAI scores in the three groups were found to have significant interactions with the measurement time (F = 6.74, P < 0.01). However, there were no apparent interactions between the VAS score and the measurement time (F = 1.31, P > 0.05) in the three groups.ConclusionPreoperative health education with the aid of multimedia is more effective than oral instruction in lowering patients’ postoperative anxiety and pain levels. In addition, animation videos are superior to recorded videos in mitigating postoperative anxiety. Whether the two approaches differ in reducing postoperative pain in bone fractures remains to be further tested.

Introduction: Hypercalcemia in critically ill patients is associated with an increased severity of illness and mortality that becomes worse as the levels rise. Pamidronate was evaluated for the treatment of hypercalcemia in critically ill surgical patients., Methods: This retrospective study evaluated 30 critically ill surgical patients who developed hypercalcemia (ionized calcium (iCa)≥1.25 mmol/L) while in the surgical ICUs over three years. Patients were case-matched 1:1 for age and severity of illness., Results: In the 30 patients with an overall Acute Physiology and Chronic Health Evaluation (APACHE) II of 22±9, mechanical ventilation was required in 27/30 (90%) and hemodialysis in 11/30 (37%). Within four days of pamidronate, iCa declined from a mean of 1.46±0.14 to 1.15±0.14 mmol/L, p=0.004; however, the blood urea nitrogen (BUN) and serum creatinine (SCr) increased significantly in patients with renal impairment. An iCa≥1.35 mmol/L increased mortality from 0% to 26%., Conclusions: Hypercalcemia in critically ill surgical patients is associated with increased severity of illness. Background: The relationship between the health care provider and the patient is an indispensable element of medical care. The existence of a proper therapeutic relationship between the health care provider and the patient can increase patients' trust and willingness to communicate, improve adherence to medical recommendations, enhance continuing care, and promote patient satisfaction. However, little is known in developing countries including Ethiopia what the patient health care provider relationship looks like. This study aimed to assess the health care provider-patient relationship during preoperative care in obstetric and gynecologic surgeries at Jimma Medical Center, Jimma, Ethiopia. Methods: Institution-based cross-sectional study was conducted from April 1 to May 30, 2020, at Jimma Medical Center. A total of 372 surgical patients were selected using a systematic random sampling method. The collected data were coded, entered into Epi data version 3.1, and analyzed using the statistical package for social science (SPSS) version 25. Bivariate and multivariable regression was carried out to determine the association between the outcome variable and the independent variable. The strength of association of dependent and independent variables was presented by crude and adjusted odds ratio at a 95% confidence interval. Variables with a p value of < 0.05 were considered statistically significant. Results: The proportion of good patient to health care provider relationship in this study was 179 (48%) and it had a significant association with patient marital status AOR = 0.29 (95% CI 0.147–0.580), consent form available AOR = 0.162 (95% CI 0.035–0.750), the profession of healthcare providers who request the consent AOR = 0.305 (95% CI 0.117–.794), mode of decision-making AOR = 0.165 (95% CI 0.039–.709), and patient's satisfaction AOR = 5.34(95% CI 3.1–9.16). Conclusions: The proportion of patient-to healthcare providers' relationship was low. More than half of the respondents did not have good patient–health care provider relationship. Hence, health care providers should be concerned about their relationship with their patients to increase the quality of medical care. The health care providers should bear in mind that patients may refuse to seek care from a provider whose relationship is not strong, even if the provider is skilled in preventing and managing complications. The widespread use of high-dose oxygen, to avoid perioperative hypoxemia along with WHO-recommended intraoperative hyperoxia to reduce surgical site infections, is an established clinical practice. However, growing pathophysiological evidence has demonstrated that hyperoxia exerts deleterious effects on many organs, mainly mediated by reactive oxygen species. The purpose of this narrative review was to present the pathophysiology of perioperative hyperoxia on surgical wound healing, on systemic macro and microcirculation, on the lungs, heart, brain, kidneys, gut, coagulation, and infections. We reported here that a high systemic oxygen supply could induce oxidative stress with inflammation, vasoconstriction, impaired microcirculation, activation of hemostasis, acute and chronic lung injury, coronary blood flow disturbances, cerebral ischemia, surgical anastomosis impairment, gut dysbiosis, and altered antibiotics susceptibility. Clinical studies have provided rather conflicting results on the definitions and outcomes of hyperoxic patients, often not speculating on the biological basis of their results, while this review highlighted what happens when supranormal PaO2 values are reached in the surgical setting. Based on the assumptions analyzed in this study, we may suggest that the maintenance of PaO2 within physiological ranges, avoiding unnecessary oxygen administration, may be the basis for good clinical practice.

Aim:Falls are a patient safety problem and cause physical, psychosocial, and economic problems in surgical patients during postoperative period. The aim of this research is to evaluate fall risk in patients in the perioperative period.Materials and Methods:This longitudinal research was conducted between May and August 2017, at thoracic and cardiovascular surgery clinics of a university hospital, with 50 patients who underwent planned surgery. Data were collected preoperatively, on the day of surgery and on the first postoperative day using a patient identification form and Itaki Fall Risk Scale. Statistical analysis was completed in the SPSS 20.0 program using descriptive analyses, Chi-square tests, and Spearman’s Rho.Results:It was determined that 48% of the patients had high fall risk preoperatively, 100% had high fall risk on the day of surgery and 82% had high fall risk on the first postoperative day. Aim: There is a need to assess the pain fear levels of surgical patients simply and appropriately before the surgery. This study aimed to develop and test the psychometrics of an instrument to evaluate the fear of postoperative pain. Methods: This methodological study was conducted at four surgical clinics including general surgery, orthopedics and traumatology, neurosurgery, heart and vessel surgery of a university hospital between 28 March and 19 October 2018. Totally, 150 patients who were scheduled for elective surgery and at their preoperative day were included. This scale contained 10 items related to the postoperative pain sources and aimed to identify the pain fear of patients preoperatively. To measure the sampling adequacy, Kaiser-Meyer-Olkin index and Bartlett's test of sphericity was used. Explanatory and confirmatory factor analyses were conducted to evaluate the construct validity. The internal consistency of the scale was evaluated by Cronbach's alpha calculation. All hypotheses were tested in two directions. Results: The total variance explained 55.5% of the variance for one factor structure consisting of 10 items. The model fit index values through the confirmatory factor analysis were found to support this structure. Conclusion: The scale is appropriate to be used in clinical settings to quickly evaluate the elective surgical patients' fear level of postoperative pain preoperatively. [ABSTRACT FROM AUTHOR]

Background. The concept of the sense of coherence (SOC) - the global orientation of life, created by Aaron Antonovsky - is increasingly popular. This study within the field of health psychology examines the situation of patients awaiting surgery in Poland and Ireland. Objectives. To investigate the relationship between the strength of the SOC and its components (comprehensibility, manageability and meaningfulness) and the level of satisfaction with life (SWL) of patients hospitalized in surgical departments in hospitals in Poland and Ireland. Materials and methods. The research was conducted in a group of 60 surgical patients, including 30 hospitalized in Poland and 30 in Ireland. The tools utilized were the Sense of Coherence Questionnaire for Adults SOC-29 and the Satisfaction with Life Scale (SWLS). Polish versions of both questionnaires were also used. Results. We obtained the following results: - for comprehensibility: in patients undergoing surgery in hospitals in Poland, the mean (M) = 46.3, standard deviation (SD) = 9.8, minimum value (Min) = 28, and maximum value (Max) = 63; in Irish patients, M = 50.8, SD = 9.2, Min = 33, and Max = 71; - for manageability: in patients undergoing surgery in hospitals in Poland, the M = 49.7, SD = 5.07, Min = 37, and Max = 58; in Irish patients, M = 49.3 SD = 6.39, Min = 38, and Max = 63; - for meaningfulness: in patients undergoing surgery in hospitals in Poland, M = 45.5, SD = 4.24, Min = 37, and Max = 54; in Irish patients, M = 44.9 SD = 5.74, Min = 34, and Max = 56. Conclusions. The results of the study confirmed the assumption that a general SOC correlates with SWL. Introduction: Introduction: disease-related malnutrition has a negative impact on the outcome in surgical patients. Our objective was to assess the prevalence of nutritional risk in the field of vascular surgery, as well as its consequences on patient outcome and health expenditure. Patients and methods: this is a prospective, observational study conducted during 6 months in a vascular surgery ward at the University Hospital of León, Spain. The Malnutrition Universal Screening Tool was used to obtain data on admission and then every 7 days until hospital discharge. Clinical variables, surgical intervention performed, medical-surgical complications, hospital stay, healthcare costs, and early readmissions were studied. Results: a total of 104 patients, 84.6 % males, with a mean age of 69 (SD: 13) years were enrolled. Of these, 46.2 % were admitted due to peripheral arterial disease; 10.6 % had a positive MUST at the time of admission and 19.2 % at discharge; 100 % of malnourished patients at admission remained in the same situation at discharge. During hospitalization, in 29 patients (27.9 %) the nutritional situation worsened. In all, 81.25 % of patients who experienced worsening of their MUST score had been admitted urgently (p < 0.05). Patients who required urgent surgery significantly worsened in terms of their nutritional status (p < 0.001). Patients with worsening nutritional status obtained higher rates for: surgical reintervention (p < 0.05), pharmaceutical expense (p = 0.017), total hospital expense (€1,000/patient/admission), transfers to chronic care centers (p = 0.0002), and number of early readmissions (p = 0.017). Conclusion: patients with nutritional risk suffered an increase in medical-surgical complications, hospital stay, healthcare costs, and re-admission rates. Patient satisfaction is an indicator of quality and well-being that, at the same time, facilitates, promotes, and guarantees high levels of health care provided by the nursing staff. General objective: To determine the satisfaction of surgical patients with nursing care in the surgical area of the Homero Castanier Crespo Hospital in the city of Azogues. Method: Quantitative, prospective, descriptive, cross-sectional, and non-experimental study, the study population was all patients of both sexes, aged between 18 and 65 years, with surgical pathologies, in the period from February to March 2021 to whom the Caring Assessment Instrument (CARE-Q) called the Caring Satisfaction Questionnaire was applied, which was indicated and used in several countries such as the United States, Australia, China, Taiwan, validated in Spanish by Nurse Gloria Sepúlveda. Utilizing the Likert scale the patient gave an opinion of the perception of agreement and disagreement and each category had a designated score from 1 to 4 points, respectively. The results reveal that the nursing staff provides optimal health care where patients express feeling satisfied, however, it is evident that there is greater satisfaction with nursing care in young women of the third level as opposed to other educational levels. COVID-19 is a novel viral infection with a wide variety of clinical manifestations ranging from asymptomatic cases to severe respiratory illness. Laryngospasm is a spontaneous sustained closure of the laryngeal muscles leading to acute airway obstruction. We report a case of a 40-year-old male with a history of nephrolithiasis who underwent laparoscopic pyeloplasty and developed laryngospasm as a consequence of contracting COVID-19. The case was further complicated by the development of pneumothorax and pleural effusion during the postoperative period. The patient was managed with supplemental oxygen, antibiotics, antiviral therapy, and close monitoring. He recovered without any additional complications. Introduction: A Norwegian-Danish research team identified a gap in research regarding how surgical patients felt about their post-operative care needs being met in hospitals. A study was subsequently developed to understand their subjective assessments of how they value the perceived fulfilment of their actual care needs. The study was further informed by international calls to focus on the fundamentals of care practice. Our aim was to determine the extent to which surgical patients receiving elective treatment experience the physical environment, atmosphere and collaboration with staff as supportive of their care and treatment, and what this means for them after treatment. In addition, we aimed to document the extent to which patients experienced being understood and having influence in their care., Methods: A descriptive observation study using a cross-sectional design. The validated Perioperative User Participation Perspectives (POUP) questionnaire was completed on the day of discharge by 194 adult (male and female) patients (mean age: 56 years) who had undergone elective surgery on gynaecological, internal medicine or orthopaedic wards. Agreement between the subjective importance of nursing care for patients and the perceived reality was determined., Results: Agreement regarding the physical environment, a clean bed and clean surroundings was between 91.7 and 96.2%, and agreement with regard to a good relational atmosphere with staff it was 94.2 to 96.7%. In terms of the relational aspects of care, being understood and having influence the agreement was calculated to be 89.4 to 94.4%. However, 42.6% of the patients reported they were involved in drawing up a care plan. For those patients who valued collaborating in their care planning the congruency was 80%., Conclusion: A conducive atmosphere and a keen eye for the patients' wishes and needs is of particular importance at the time of discharge after elective surgery., (Copyright © 2024. Background: The surgical patient is often at risk of malnutrition, especially when he is affected by gastrointestinal pathology. When such risk is confirmed its common to intervene with an additional nutritional support. Whenever possible, the enteric route should be privileged, once it is the most physiological, it maintains the structural and functional integrity of the intestinal barrier and it helps to stimulate the secretion of intestinal hormones, preventing the atrophy of the intestinal mucosa. Objective: To access the benefits and the safety of early enteral nutrition in the postoperative of gastrointestinal surgery. Method: Review of the literature that used the PICO methodology to compile the research question. The research was performed in CINAHL Complete, Cochrane Central Register of Controlled Trials; Cochrane Database of Systematic Reviews and Pubmed databases to identify studies published between 2015 and 2019. Nine randomized clinical trials and six systematic reviews were selected. Results: Most of the articles included in this review point to the benefits of early enteral in people undergoing gastrointestinal surgery, namely the rapid recovery of intestinal function and reduction in hospital stay, which in turn reduces hospital costs. Only two articles considered this nutritional approach not viable/safe due to the increase in postoperative complications in its participants. Conclusion: This review reveals that early enteral nutrition is a beneficial and safe intervention in the postoperative of gastrointestinal surgery. However, due to some limitations of the studies analyzed and given the wide range of scenarios and surgical interventions, further studies are suggested. [ABSTRACT FROM AUTHOR]

The objective for the present study was to compare the collapsibility (IcIVC) and distensibility (IdIVC) indices of the inferior vena cava with pulse pressure variation (PPV) and determine the accuracy and cutoff points of IcIVC and IdIVC that best predict response to intravenous fluid therapy in surgical patients. Observational, prospective, nonblinded, single center. Hospital do Servidor Público Estadual de São Paulo, in São Paulo, Brazil. Volunteer surgical patients. This prospective study evaluated adult surgical patients before and after they underwent mechanical ventilation. IcIVC and IdIVC measurements were obtained with echocardiography and PPV through arterial catheterization. Twenty-two patients with a mean age of 55.7 ± 10.9 years were included; 31.8% of the study participants had PPV values >13% and were shown to be responsive to fluid. A good correlation was detected between PPV and icIVC (R2 = 0.71; p < 0.001) and between PPV and idIVC (R2 = 0.79; p < 0.001). The area under the receiver operating characteristic curve was 0.98 for icIVC (95% confidence interval 0.81-0.99; p < 0.001) and 0.88 for idIVC (95% confidence interval 0.67-0.98; p < 0.001). PPV was found to have good correlation with the inferior vena cava diameter variation using echocardiography in surgical patients undergoing spontaneous and artificial ventilation. Background. Alcoholism is a public health problem. Chronic ethanol consumption determines physiological and morphological changes, reflected by an abnormal response to regular medication and nutrients, with consequences on the efficiency of anesthesia and the surgical act. Material and methods. A retrospective study was conducted on the latest 1750 cases (2016-2018), hospitalized and operated, in which the preoperative identification of alcohol abuse was made by dosing biochemical markers (gamma-glutamyl transferase, poor carbohydrate transferrin and medium erythrocytic corpuscular volume), by standard questionnaires and, of course, by preanesthetic exam. Results and discussions. The study showed a rate of 8% (140 cases) of alcohol abuse among these surgical patients. The paper reviews the somatic complications of alcoholism: neurological, gastrointestinal, cardiovascular, metabolic, hematological, immunological and hormonal, and discusses the effectiveness of the study method. Aim: The aim of the study was to describe the current state of pain assessment in surgical patients in the South Bohemia region. Design: A quantitative descriptive study. Methods: Two sets of questionnaires were used to determine the state of pain assessment. The sample included 253 nurses and 205 patients from the South Bohemia region. Results: Nurses use visual analog scales (129) and verbal pain assessment (89) to assess pain. A more precise verbal description of pain assessment would be appreciated by 44 of the respondents, and 42 would appreciate confirmation of assessments by doctors. According to nurses, the main pain manifestations in patients are: limited mobility (156), verbal expressions (134), lack of cooperation (129) and limited independence (128). Direct physical manifestations of pain mentioned by patients include: limited mobility (140), limited independence (97), and mimic manifestation (46). Patients would like pain assessments complemented by more accurate verbal assessments (84), use of pictures to illustrate pain (40), and increased frequency of assessment by doctors (29). This cross-sectional study evaluates the clinical accuracy, relevance, clarity, and emotional sensitivity of responses to inquiries from patients undergoing surgery provided by large language models (LLMs), highlighting their potential as adjunct tools in patient communication and education.
