Obiective. Stabilirea nivelului seroprevalenței markerilor hepatitelor virale A, B, C și E la lucrătorii medicali din secția de urologie a Spitalului Clinic Republican în vederea ajustării măsurilor de profilaxie a acestor infecții la ei. Metode. A fost realizat un studiu epidemiologic descriptiv transversal în baza probelor de sânge prelevate de la lucrătorii medicali, care au fost testate prin metoda imuno-enzimatică la markerii hepatitelor virale: anti-HAV, anti-HAV IgM, AgHBs, anti-HBcor, anti-HBs, anti-HCV, anti-HEV IgG și anti-HEV IgM. În total, au fost examinați 49 lucrători medicali, efectuându-se 392 de investigații de laborator. Rezultate. În cadrul lucrătorilor din secția de urologie, s-au identificat următoarele seroprevalențe pentru markerii virali: AgHBs - 2,0±2,0%; anti-HBcor - 38,8±7,0%; anti-HBs - 51,0±7,1%; anti-HCV - 4,1±2,8%; anti-HAV - 100%; anti-HAV IgM - 0%; anti-HEV IgG - 12,2±4,7%; anti-HEV IgM - 12,2±4,7%. Grupurile cele mai afectate au fost persoanele de gen feminin, asistentele medicale sau personalul auxiliar și cei cu o experiență de muncă de ≥30 ani. Concluzii. Rezultatele obținute indică faptul că lucrătorii medicali din secția de urologie pot fi considerați un grup cu risc crescut de infectare cu virusurile hepatitelor B, C și E.

La survenue de hernies chez le sujet âgé est favorisée par la faiblesse pariétale acquise et les troubles urinaires du bas appareil fréquents à cet âge. Le traitement dans ce contexte répond à plus d'impératifs car devant aussi éviter l'aggravation de pathologies chroniques souvent préexistantes. Notre objectif était de décrire les particularités diagnostiques, thérapeutiques et évolutives des hernies inguinales du sujet âgé. Il s'agissait d'une étude rétrospective descriptive, portant sur tous les hommes âgés de plus de 60 ans opérés pour hernie inguinale entre le 1er janvier 2017 au 31 mars 2018 au Service d´Urologie de l´Hôpital Aristide Le Dantec de Dakar. Les paramètres étudiés étaient l´âge, les comorbidités, le type de hernie, la technique chirurgicale et les résultats. Il y avait 77 patients. L´âge moyen était de 69,8 ans. Une hypertrophie bénigne de la prostate (HBP) était notée dans 45,4%. Les patients étaient classés ASA I dans 62,3%, ASA II dans 18,2% et ASA III dans 19,5%. La durée moyenne d'évolution des signes était de 28,2 mois. Sur le plan anatomique, 45,4% de hernies étaient indirectes et 54,6% de hernies étaient directes. Le taux de PSA (Prostate-Specific Antigen) était entre 0 et 4 dans 75%, entre 4 et 10 dans 12,5% et supérieur à 10 dans 12,5%. L'intervention était faite sous rachianesthésie dans 98,7% des cas (n=76). Sur le plan thérapeutique, il a été réalisé une cure selon Bassini dans 84,4% des cas. La durée moyenne d'hospitalisation était de 1,3 jours. Il n'y a pas eu de décès. Le délai du suivi était en moyenne de 15,3 mois. Nous avons noté une récidive dans 7,8%. L'évaluation de la douleur chronique ne montrait aucune douleur dans 93,5%. La hernie inguinale est une affection bénigne mais potentiellement grave chez le sujet âgé. Le traitement chirurgical en un temps de la hernie et des pathologies associées est faisable avec une préférence pour les cures sans prothèse pour diminuer le risque infectieux.

28. Pathologies urologiques liées à une consommation de cannabis.

Bien qu'illégale, la consommation de cannabis est assez fréquente en France et son rôle dans les pathologies urologiques est peu connu. L'objectif était de rapporter une revue des effets du cannabis dans les pathologies urologiques. Une revue de la littérature a été réalisée sur la base de données PubMed en mars 2020 en utilisant les mots clés « marijuana », « urologic neoplasms », « male infertility » et « lower urinary tract symptom ». Le cannabis aurait un effet myorelaxant sur le détrusor et pourrait soulager les symptômes du bas appareil urinaire (SBAU) de la phase de remplissage. Cette propriété a conduit à son évaluation dans le traitement de l'hyperactivité vésicale chez les patients atteints de sclérose en plaque avec des résultats encourageants. Il semblerait qu'une consommation excessive et prolongée de cannabis serait associée à un risque accru de tumeurs germinales testiculaires notamment non séminomateuses. Le rôle propre du cannabis sur le cancer de vessie est controversé mais les produits liés à sa combustion sont des carcinogènes urothéliaux comme pour le tabac. Les données cliniques sont faibles concernant les autres cancers urologiques et à ce jour, aucune association ne peut être établie. Enfin, le cannabis perturberait la fonction reproductive des hommes avec une altération des paramètres spermatiques. Une consommation régulière de cannabis serait corrélée à de nombreuses pathologies urologiques oncologiques et fonctionnelles. Il convient d'interroger nos patients sur leur usage actuel et passé de cannabis afin de les informer sur les pathologies urologiques auxquelles ils s'exposent. The illegal consumption of marijuana is not exceptional in France and its role in urological diseases is few known. The aim was to report a review of marijuana effects in urological diseases. A review of the literature was performed with the PubMed database in March 2020 using the keywords "marijuana", "urological neoplasms", "male infertility" and "lower urinary tract symptom". Marijuana could have a muscle relaxant effect on the detrusor and may improves lower urinary tract symptoms (LUTS). This property led to its evaluation in overactive bladder treatment in patients with multiple sclerosis with encouraging results. It seems that excessive and prolonged use of marijuana is associated with an increased risk of testicular cancer, particularly in non-seminomatous germ cell tumors. The cannabis proper role in bladder cancer is controversial, but the products linked to its combustion are urothelial carcinogens like tobacco. Clinical data are few for other urological cancers and to date no association can be established. Finally, marijuana would disrupt male reproductive function by sperm parameters alteration. Regular consumption of marijuana is correlated with many oncological and functional urological diseases. Our patients should be asked about their current and past use of marijuana in order to inform them of which urological pathologies they are exposed to. [ABSTRACT FROM AUTHOR]

Les aminosides sont une classe d'antibiotiques que tout urologue doit savoir utiliser dans sa pratique courante. Ils sont à réserver essentiellement aux cas d'infections urinaires sévères, d'obstruction de la voie excrétrice supérieure et aux situations d'allergies notamment aux β-lactamines. Il est actuellement recommandé de privilégier les traitements de courte durée et à posologie élevée. L'objectif est de maintenir leur efficacité liée à leurs caractéristiques pharmacologiques et au faible taux de résistance bactérienne tout en se prémunissant de toxicités potentiellement graves pour le patient. Aminoglycosides are a class of antibiotics that every urologist should know how to use in everyday practice. They are essentially reserved for severe urinary tract infections, upper tract obstruction and in cases of beta-lactam allergy. It is recommended to use higher dosages and shorter treatment duration. The objective is to maintain their effectiveness, which is linked to their pharmacologic characteristics and low rate of bacterial resistance while minimizing potential toxicity for the patient. Background: Intraoperative eye protection is generally important during surgery in patients under general anesthesia. There are, however, challenges that arise when a laser is used intraoperatively. In gynecological and urological diseases lasers are increasingly being used for treatment but a lege artis eye cover or protection has not yet been investigated in the international literature.Objective: According to nursing experts, how is a lege artis eye coverage performed during gynecological and urological laser surgery?Material and Methods: In order to answer the research question, standardized expert interviews were carried out via telephone, direct face to face interviews and e‑mails with experts from specialized clinics in Austria. The results of a nonsystematic literature search formed the basis for the interview guidelines.Results: In this study 11 experts agreed to participate in a qualitative survey. The result of the interviews recommended the use of skin-friendly plasters in combination with a vitamin D eye ointment, special plasters/covers and/or laser safety goggles with a vitamin D eye ointment and NaCl dressings. In addition to the direct protection of the eye, a periodic intraoperative eyelid closure check must be performed.Conclusion: The findings from the expert interviews are to be understood as practice recommendations. - Abstract
Objective: To study infections associated with care in the urology department of the Gabriel Touré University Hospital. Patients and methods: This was a prospective, descriptive study from April 1 to September 31, 2016 in the Urology Department of CHU Gabriel TOURE. Included in this study were all inpatients (operated and non-operated) in the department. Not included are non-consenting patients and patients admitted for necrosectomy. The following parameters were studied: age, sex, origin, nationality, ASA score, BMI, WHO performance index, duration and type of intervention, ECBU, ECB, GE, antibiotics, antiseptics used, mortality, morbidity. Results: We collected 203 patients, 30 (15%) of whom had healthcare-associated infections. The mean age of patients with a healthcare-associated infection was 58 years with a standard deviation of 24.5983 and a sex ratio of 5,2. ASA patients had a healthcare-associated infection in 70% of cases and ASA2 in 52% and ASA3 in 100% of cases. We recorded 13 patients infected with NNISS score=0 or 9%, 12 cases infected with NNISS score=1 or 24% and 3 cases infected with NNISS score=2 or 27%. Fever appeared in 35% of patients from the 4th day; fever was recorded from the 5th, 6th, 7th and 11th days, i.e. 5 to 30%. Organ suppurations dominated with 52%, followed by superficial suppurations of 38%, and 10% deep suppurations. The majority of patients had a suppuration between the 4th and 6th day of the operation, i.e. 65%. Germs are in order of frequency: E. coli (40.74%); Klebsiella pneumoniae (37.03%); Enterococcus faecalis (14.81%); Pseudomonas aeroginosa, (3.70%) and Acinetobacter baumannii (3.70%). Nitrofurantoin has been effective on E. coli, K. pneumoniae, and E. faecalis. Conclusion: Care-associated infections are potentially serious and sometimes fatal, prolonging the duration of hospitalization with a definite impact on patients' post-operative rehabilitation. The introduction of the surgical robot resulted in increasing technical difficulty of these robot assisted surgeries (RAS). The qualification and certification of RAS skills are still in a preliminary phase within all surgical specialties, also in urology. New methods of training for novice surgeons (residents and fellows) in these highly specialised techniques should be implemented to guarantee patient safety. The challenge for novice surgeons is how to learn new surgical procedures and once the procedure is learned and they become experts how to analyse past performances and subsequently use this as a lesson for the future in order to improve postoperative outcome. In the first part of this these we explored the best methods to educate surgeons in robotic surgery. By using different modalities of training, (i.e., hands-on training combined with theoretical information of learning) we discovered (chapter 2) novice robot surgeons were too positive in the self-assessment of their own surgical skills after a training in RAS. To prevent overconfidence biases the novice surgeons should be provided with feedback (proctoring) to inform them about their results to enhance learning and inform them of their competence levels. Although results (chapter 3) show there is no significant difference in the acquisition of surgical skills between different types of proctoring provided to novice surgeons the participant satisfaction seems to be higher in the “human proctoring” group. Which could result in additional motivation for the participant to continue training in RAS. Additional research (chapter 4) shows residents in urology are allowed to perform RAS during their residency, but criteria for starting RAS differ significantly among the teaching hospitals. This shows there is a need for the implementation of a (multi-step) training and certification program in the Dutch urology residency curriculum. The results of the survey showed that most respondents still perform RAS after the fellowship, which matches results from other research into the impact of RAS fellowships.34,35 A remarkable finding of this study is that a large proportion of respondents is unaware of the oncological (33%) and functional outcomes (66%) of their patients. Thus, participants of RAS fellowships should be coached to review their own results to learn from their past performance and reduce their learning curve. The second part of the thesis focuses the assessment of a robotic surgeon’s surgical performance. Different methods were used to identify which factors should be assed in surgical video analysis in order to ascertain a surgeon’s surgical skills. In robot assisted surgery. (Chapter 6 and 8). In addition, a research protocol is provided by which the non-technical skills can be assed in both laparotomic surgery and robot assisted surgery in order to discover the difference in non-technical skills between the two surgical modalities. The third part of the thesis focusses on the relation between a surgeon’s performance and postoperative outcome of patients. In this part multiple methods of surgical video analysis were used to identify factors which could influence the postoperative outcome of patients (Chapters 9, 10, 11, and 12). Results show the length of the urethral stump after dissection of the prostate could be a significant factor in the recovery of continence after prostatectomy.

The French press of the Belle Époque abounded with ads promising their readers mental invigoration through testicular beautification. Where did this link between the appearance of men’s nether regions and mental health come from? This article examines the reasons behind French physicians’ belief, at the eve of the 20th century, that in/sanity and testicular health were correlated. The first part reveals how, in the psychiatric field, certain organicist views led to the idea that inspecting testicles was key to understanding the male psyche. The second part explores how urologists integrated these findings when they treated “false urinary” patients—patients who suffered from mixed symptoms that associated testicular and mental ailments. Perceptions of “testicular angst” therefore prompted a dialogue between two seemingly distant fields—urology and psychiatry—and contributed to shaping new representations of manhood that stressed the vulnerability of male bodies and minds rather than the fortitude of the “stronger sex”. La presse de la Belle Époque regorge de publicités dans lesquelles on promet de vivifier l’esprit des hommes en soignant la beauté de leurs testicules. Mais d’où venait cette idée d’un lien entre apparence des testicules et tranquillité d’esprit ? Cet article éclaire cette question en retraçant la façon dont les médecins français ont corrélé santé mentale et santé testiculaire à l’orée du xxe siècle. La première partie montre comment, dans le champ des savoirs psy, l’avènement de certaines perspectives organicistes conduisit à penser qu’examiner les testicules offrait des clés pour comprendre la psyché masculine. La seconde partie révèle comment les urologues intégrèrent ces idées pour aborder les patients « faux urinaires » qui présentaient des symptômes mixtes associant troubles testiculaires et mentaux. Ces représentations communes de « l’angoisse testiculaire » permirent ainsi à deux spécialités a priori éloignées (l’urologie et la psychiatrie) de dialoguer, tout en façonnant de nouvelles images de la masculinité qui insistaient sur la vulnérabilité des corps et des esprits masculins plutôt que sur la vigueur du « sexe fort ».

Objectif: identifier les causes des décès enregistrés dans un service d’urologie d’Afrique subsaharienne. Patients et méthodes: Il s’agissait d’une étude rétrospective colligeant tous les décès entre février 2010 et février 2014. Les paramètres étudiés étaient l’âge au moment du décès, le sexe, les facteurs de co-morbidité, la durée d’hospitalisation et la cause du décès. Résultats: Le nombre de décès était de 132 dont 7 femmes et 125 hommes. La mortalité hospitalière était de 2,9%. L’âge moyen des patients décédés était de 63,6 ans (médiane à 66 ans). La tranche d’âge la plus intéressée était celle entre 61 et 80 ans. La mortalité était opératoire dans 0,8%. Il existait des facteurs de co-morbidité dans 28% des cas. Le cancer était la cause du décès dans 51,5% des cas. Le cancer de la prostate était la cause de décès dans 32 cas (47,1%), le cancer de la vessie dans 16 cas (23,5%), le cancer du rein dans 14 cas (20,6%), le cancer du testicule dans 4 cas (5,8%) et le cancer de la verge dans 2 cas (2,9%). La mortalité par les affections non cancéreuses était dominée par la rétention chronique d’urine et les phlegmons des organes génitaux externes. Conclusion: Dans notre centre, la mortalité spécifique reste élevée et les causes sont dominées par les cancers urologiques.

Introduction: Physical examination appears to be a limiting factor of teleconsultation (TC). We evaluated the feasibility of self-percussion of the lumbar fossae (sPLF) and TC for simple renal colic (SRC) in comparison with a face-to-face consultation (FC)., Material and Methods: We performed a comparative prospective study in two steps. First: evaluation of the quality of an sPLF on a standardized patient in TC, without and with tutorial. Secondarily: evaluation of a TC and a FC for a SRC with a standardized patient in real conditions. Evaluation using objective clinical scores and qualitative scales by an observer, the standardized patient and the practitioner himself., Results: Forty-two practitioners were included in the study. In the absence of a tutorial, the sPLF was most often "poorly done". The tutorial led to a significant improvement in the quality of sPLF. There was no difference in diagnostic and therapeutic performance among senior physicians between TC and FC. The therapeutic performances of the interns were significantly lower in TC without his being aware of it. The qualitative scores were significantly lower in TC vs FC according to the practitioners, the standardized patient and the observer., Conclusion: An sPLF is feasible but its practice should be taught. Unlike interns, senior physicians were able to perform a TC comparable to FC for the management of SRC. Objectives: To describe a concept of standardized preoperative one-day evaluation before urinary reconstructive or diversion surgery for the treatment of neurogenic lower urinary tract (LUT) dysfunction, and to evaluate its feasibility and its impact on the care pathway., Materials and Methods: All patients who underwent a one-day standardized evaluation before a urinary reconstructive or derivation surgery for the treatment of neurogenic LUT dysfunction between January 2017 and December 2021 in our institution were included. Data were collected retrospectively from standardized reports. The main outcome was the rate of completion of the tests and consultations planned during this evaluation. Secondary outcomes included the findings from the one-day evaluation and changes in the urological surgical strategy at different time points within one year., Results: One hundred and thirty-one patients benefited from this one-day standardized evaluation. The overall completeness rate of the data collected was 77.5%, increasing from 62.3% in 2017 to 89.3% in 2021. The urological surgical plan was modified for 19.1% of patients following this preoperative evaluation. The indication was then confirmed for 114 patients (87.0%) by the multidisciplinary meeting and was carried out unchanged during the following year for 89 patients (67.9%). An associated colostomy procedure was proposed for 18.3% of patients and was finally performed in 11.5%., Conclusion: A standardized multidisciplinary preoperative one-day evaluation before performing reconstructive or diversion surgery for the treatment of neurogenic LUT dysfunction seems feasible and makes it possible to optimize the surgical plan and adapt the course of care., (Copyright © 2023 Elsevier Masson SAS. Professor Antoni Jurasz was one of the most eminent Polish surgeons in the period between the two World Wars. He founded the Polish School of Medicine at the University of Edinburgh and the Polish Paderewski Hospital in Edinburgh in 1941. Having served in the Prussian Army he was also a co-founder of the military health service of the Polish Armed Forces. He was an educator of great format and a fierce patriot. This is an opportunity to remember a man and surgeon who devoted the last years of his life to the cause of closer rapprochement between nations in the name of well-conceived service to Poland and to humanity. The establishment of a Polish School of Medicine within the framework of Edinburgh University is a magnificent example of international academic collaboration. Extensive research was undertaken to collect the literature and documents in Polish, German and British libraries and archives in order to prepare this study. [ABSTRACT FROM AUTHOR]

The urologist Oswald Schwarz (1883-1949) was not only a pioneer of psychosomatic and sexual medicine, he also made important contributions to the development of medical anthropology. That National Socialism put a sudden end to his scientific career can only be described as tragic. Nevertheless, many of his ideas and reflections can be found in today's psychosomatic and sexological literature. In view of his achievements, Oswald Schwarz deserves a prominent place in the history of urology as well as in sexual medicine. This article gives an overview of his life and works.
