The changing demography of European populations mandates a vital role for internists in caring for patients in each level of healthcare. Internists in the tertiary or academic setting are highly ranked in terms of their responsibilities: they are clinicians, educators, researchers, role models, mentors and administrators. Contrary to the highly focused approach of sub-specialties, general internists working in academic settings can ensure that coordinated care is delivered in the most cost-conscious and efficient way. Moreover, internal medicine is one of the most appropriate specialties in which to teach clinical reasoning skills, decision-making and analytical thinking, as well as evidence based, patient oriented medicine. Internists deal with challenging patients of the new millennium with a high burden of chronic diseases and polypharmacy; practice personalised medicine with a wide scientific background and so they are the perfect fit to establish and implement new tools for scientific research. In conclusion, internal medicine is developing a new identity as a specialty in its own right. The European Federation of Internal Medicine supports the concept of academic internists and calls upon the member countries to construct academic (general) internal medicine departments in their respective countries. As 'internal medicine is the cornerstone of every national healthcare system', academic (general) internal medicine should strive to be the cornerstone of every integrated, patient-centred, modern medical care and training system., (Copyright © 2014 European Federation of Internal Medicine. Background: Channelopathies, defined as diseases that are caused by mutations in genes encoding ion channels, are associated with a wide variety of symptoms and have been documented extensively over the past decade. In contrast, despite the important role of chloride in serum, textbooks in general do not allocate chapters exclusively on hypochloremia or hyperchloremia and information on chloride other than channelopathies is scattered in the literature., Study Design: To systematically review the function of chloride in man, data for this review include searches of MEDLINE, PubMed, and references from relevant articles including the search terms "chloride," "HCl," "chloride channel" "acid-base," "acidosis," "alkalosis," "anion gap" "strong anion gap" "Stewart," "base excess" and "lactate." In addition, internal medicine, critical care, nephrology and gastroenterology textbooks were evaluated on topics pertaining the assessment and management of acid-base disorders, including reference lists from journals or textbooks., Conclusion: Chloride is, after sodium, the most abundant electrolyte in serum, with a key role in the regulation of body fluids, electrolyte balance, the preservation of electrical neutrality, acid-base status and it is an essential component for the assessment of many pathological conditions. When assessing serum electrolytes, abnormal chloride levels alone usually signify a more serious underlying metabolic disorder, such as metabolic acidosis or alkalosis. Chloride is an important component of diagnostic tests in a wide array of clinical situations. In these cases, chloride can be tested in sweat, serum, urine and feces. Background and Objectives: Clinical trials have demonstrated that initial outpatient treatment is safe and effective in patients with deep vein thrombosis (DVT). Considering the relative lack of literature-based evidence on outpatient low molecular weight heparin (LMWH) treatment in daily practice this study prospectively evaluated the implementation of a protocol for full outpatient treatment of DVT in a non-teaching hospital., Design and Methods: Consecutive patients with objectively demonstrated DVT were treated on an outpatient basis with subcutaneous nadroparin injections for at least 5 days and oral anticoagulant treatment for at least 3 months., Results: In 294 of 309 (95%) consecutive patients with proven DVT, nadroparin could be started on a fully outpatient basis. During initial LMWH treatment one patient had to be hospitalized because of objectively proven pulmonary embolism (PE), and one patient developed a major bleeding complication. Overall, during 3 months follow-up recurrent venous thromboembolism (VTE) occurred in nine patients (3.1%; 95 CI 1.1 to 5.1), four patients experienced a major non-fatal hemorrhage (1.4%; 95 CI 0.04 to 2.7) and ten patients died (3.4%; 95% CI 1.3 to 5.5) of whom seven with disseminated malignancy, but none of fatal PE., Interpretation and Conclusions: Out of hospital initiation of anticoagulant treatment with LMWH is safe and effective in the overall majority of patients (95%) with objectively proven DVT. We believe that these results are relevant to both clinicians and health care providers in view of the feasibility of home treatment in nearly all patients.

Objective: To evaluate a standard protocol for ambulant treatment of patients with deep venous thrombosis (DVT) in a leg with low molecular weight heparins (LMWH)., Design: Prospective., Method: A protocol was established within a regional context which provided for all patients in or around Veghel, the Netherlands, with demonstrated DVT in the period 1 March 1997-31 July 1999 to receive ambulant treatment with LMWH and oral anticoagulants. After diagnosing the treatment was started in the emergency department of the Sint Joseph Hospital in Veghel, the subsequent LMWH injectons were administered by the general practitioner in his practice., Results: Of the 130 patients with demonstrated DVT, six were hospitalized because of pulmonary embolism, carcinoma or pregnancy, while 124 (95%) were eligible for ambulant treatment (56 males with a median age of 57 years (range 16-98) and 68 females with a median age of 51 years (18-89). During the acute period of LMWH treatment, none of the 124 patients developed a recurrence of thrombosis, pulmonary embolism or major haemorrhage. During 3 months' follow-up one patient developed a thrombosis in the other leg, this patient died after 2 months from disseminated carcinoma of the colon., Conclusion: Ambulant treatment of patients with DVT of the leg with LMWH in a daily practice setting is feasible and can be carried out effectively and safely in 95% of the patients in a regional organization.
