Abstract

BACKGROUND: Portal vein embolization (PVE) is performed to reduce the risk of liver failure and subsequent mortality after major liver resection. Although a cut-off value of 2·7 per cent per min per m2 has been used with hepatobiliary scintigraphy (HBS) for future remnant liver function (FRLF), patients with perihilar cholangiocarcinoma (PHC) potentially benefit from an additional cut-off of 8·5 per cent/min (not corrected for body surface area). Since January 2016 a more liberal approach to PVE has been adopted, including this additional cut-off for HBS of 8·5 per cent/min. The aim of this study was to assess the effect of this approach on liver failure and mortality. BACKGROUND: Pancreatic resection is a major abdominal operation with 50% risk of postoperative complications. A common complication is pancreatic fistula, which may have severe clinical consequences such as postoperative bleeding, organ failure and death. The objective of this study is to investigate whether implementation of an algorithm for early detection and minimally invasive management of pancreatic fistula may improve outcomes after pancreatic resection. METHODS: This is a nationwide stepped-wedge, cluster-randomized, superiority trial, designed in adherence to the Consolidated Standards of Reporting Trials (CONSORT) guidelines. Abstract

Purpose: Irreversible electroporation (IRE) uses short duration, high-voltage electrical pulses to induce cell death via nanoscale defects resulting from altered transmembrane potential. The technique is gaining interest for ablations in unresectable pancreatic and hepatobiliary cancer. Metal stents are often used for palliative biliary drainage in these patients, but are currently seen as an absolute contraindication for IRE due to the perceived risk of direct heating of the metal and its surroundings. This study investigates the thermal and tissue viability changes due to a metal stent during IRE. Methods: IRE was performed in a homogeneous tissue model (polyacrylamide gel), without and with a metal stent placed perpendicular and parallel to the electrodes, delivering 90 and 270 pulses (15-35 A, 90 μsec, 1.5 cm active tip exposure, 1.5 cm interelectrode distance, 1000-1500 V/cm, 90 pulses/min), and in-vivo in a porcine liver (4 ablations). Temperature changes were measured with an infrared thermal camera and with fiber-optic probes. Tissue viability after in-vivo IRE was investigated macroscopically using 5-triphenyltetrazolium chloride (TTC) vitality staining. Results: In the gel, direct stent-heating was not observed. Contrarily, the presence of a stent between the electrodes caused a higher increase in median temperature near the electrodes (23.2 vs 13.3°C [90 pulses]; p = 0.021, and 33.1 vs 24.8°C [270 pulses]; p = 0.242). In-vivo, no temperature difference was observed for ablations with and without a stent. Tissue examination showed white coagulation 1mm around the electrodes only. A rim of vital tissue remained around the stent, whereas ablation without stent resulted in complete tissue avitality. Conclusion: IRE in the vicinity of a metal stent does not cause notable direct heating of the metal, but results in higher temperatures around the electrodes and remnant viable tissue. Abstract

The first aim of this thesis is to investigate the diagnostic value of needle-based, percutaneous optical coherence tomography (OCT) in the differentiation of renal masses. Secondly, the feasibility and safety of percutaneous, irreversible electroporation (IRE) are assessed for the treatment of SRMs. Chapter 2 reports on the comparison between OCT and conventional RMB for the differentiation of renal masses. Diagnostic yield (total percentage of biopsies that lead to diagnosis) and accuracy, (sensitivity, specificity, negative predictive value, and positive predictive value) will be assessed for both OCT and RMB in a prospective cohort of 95 patients. In chapter 3 a literature overview is presented of the novel ablative technologies, including irreversible electroporation (IRE), microwave ablation (MWA), and stereotactic ablative radiation therapy (SABR), for the treatment of localised RCC. They are compared to the currently established ablative techniques, cryoablation and radiofrequency ablation (RFA). In chapter 4 a literature overview on IRE in several organs is provided including the liver, pancreas, kidney, and prostate. Additionally, the principle and the procedure of IRE is described. Chapter 5 reports on the study protocol regarding IRE for the ablation of localised RCC in humans. It aims at feasibility and safety, functional outcomes, and clinical efficacy. Additionally, the protocol has a specific focus on several imaging techniques (MRI, CT, and contrast-enhanced ultrasound) used in the follow-up after IRE. Chapter 6 reports on the feasibility and safety results of the IRE study, as well as the functional outcomes including postoperative pain and renal function. Chapter 7 describes the imaging characteristics, enhancement, and the volume of the ablated tumour after renal IRE ablation using contrast-enhanced CT and MRI.

Abstract

Dit proefschrift geeft een overzicht van de introductie van irreversibele electroporese (IRE) als behandeloptie voor lokaal irresectabel pancreascarcinoom (LAPC). De prognose voor patiënten met pancreascarcinoom is erg slecht. Omdat de tumor vaak rondom bloedvaten en vitale structuren groeit is operatieve verwijdering vaak niet mogelijk, zelfs als er nog geen uitzaaiingen zijn. Een nieuwe methode om dit soort tumoren te behandelen is IRE, waarbij de tumor lek gemaakt wordt met stroomstootjes. In dit proefschrift wordt deze nieuwe techniek onderzocht, waarbij gekeken wordt naar de warmteontwikkeling, het effect op specifieke weefsels, en wat deze techniek kan betekenen voor patiënten met LAPC.

50. Optimizing preoperative portal vein embolization for liver resection

Abstract

The unique capacity of the liver to regenerate has been a topic of interest for many of researchers. The exact mechanisms still remain unclear. In the field of liver surgery, a future remnant liver volume of at least 25% of the total liver volume is necessary to prevent postoperative liver failure. A well-established method to enhance both liver volume and liver function preoperatively, is portal vein embolization (PVE). With PVE, a compensatory regeneration response of the non-embolized liver lobe is induced, resulting in increased volume and function of the future remnant liver, which contributes to performing safer resection. The search for an absorbable embolization material in rabbits for PVE to prevent complications that can occur due to the use of permanent embolization is presented in this thesis. This thesis also focuses on 99mTc-mebrofenin hepatobiliary scintigraphy (HBS) as a quantitative liver function test which can be used to predict the hypertrophy response 3 weeks after PVE. Bile acids have been identified to play an important role in the early phase of liver regeneration by activating bile salt-activated transcription factor farnesoid X-receptor (FXR). We evaluated the effect of a potent FXR agonist (obeticholic acid, OCA) on liver growth following portal vein embolization in a rabbit model. This thesis explores several conditions of preoperative liver augmenting procedures along with aspects of perioperative care in patients undergoing major liver resection in order to improve postoperative outcomes in terms of postoperative morbidity and mortality.
