Uit onderzoek blijkt dat onbehandelde cariës in het blijvende gebit op nummer 1 staat van de meest voorkomende aandoeningen wereldwijd. En dat terwijl cariës goed te behandelen en te ‘managen’ is. Toch zien we nog grote verschillen tussen sociale klassen in ontwikkelde landen, maar ook tussen ontwikkelde landen en ontwikkelingslanden. Voorlichting en zelfzorg spelen een zeer belangrijke rol. Overheden, vooral in de ontwikkelingslanden, zouden zich meer faciliterend kunnen opstellen om projecten die de (mond)gezondheid bevorderen te laten slagen. Het gebruik van de traditionele miswak kan ook helpen bij het introduceren van de gewoonte om dagelijks mondverzorging toe te passen.

De laatste jaren zijn er veel veranderingen ontstaan op zowel het gebied van de financiering, als van de organisatie van mondzorgpraktijken, de taakverdeling tussen de verschillende zorgverleners en het zorgaanbod. De kwaliteit van de mondzorg lijkt voor zowel patiënten als zorgverzekeraars een punt van zorg en er worden vraagtekens geplaatst bij de transparantie ervan. Voor de Gezondheidsraad waren deze ontwikkelingen aanleiding om vooral de wetenschappelijke onderbouwing van de mondzorg grondig te onderzoeken.

Aim: The present study assessed whether gingivitis and plaque scores of 8- to 11-year-old school children who participated in a SBTB programme for 2 years were lower than those of children who did not participate in the programme.Material and methods: The present study was performed using an examiner-blind, parallel group design and was performed in Burma (Myanmar) in 2006. Three of the five schools where daily SBTB programmes took place after lunch and which were performed under teacher supervision were randomly selected; three non-participating schools (non-SBTB) from the same area were assigned as controls. Twenty-five children per school were examined for gingivitis (bleeding on marginal probing) and plaque (Quigley & Hein).Results: In total, 150 8- to 11-year-old children participated, with 75 children in either group. The test group (SBTB) exhibited an overall mean bleeding score of 0.76. For the control group (non-SBTB), this score was 0.83. With respect to the overall mean plaque scores, the test group exhibited a score of 2.93, whereas the control group exhibited a score of 2.91. No statistically significant differences between the test and the control group were observed.Conclusion: The present study did not reveal a statistically significant effect of daily SBTB programmes in 8- to 11-year-old school children with respect to gingivitis and plaque scores.

Natuurlijk heeft voeding invloed op het optreden van cariës. Maar het is de vraag of die invloed groot genoeg is om de stelling te onderschrijven.

For the dental care of parents and children, people in the Netherlands relyespecially on the advice of the Ivory Cross. The basis of this advice is plaqueremoval with fluoride toothpaste. W hen this offers insufficient protection,one usually finds irregular and careless dental hygiene. Information andinstruction concerning daily and careful dental hygiene should receive thehighest priority of dental care professionals. If a patient’s own dental carecannot be brought up to standard or if this cannot be done immediately, then(temporary) support can be provided by dental professionals in the form ofpreventive treatment. Doing this, however, without improving the self care ofpatients has to be regarded an inadequate treatment modality.

Voor de gebitsverzorging door ouder en kind worden in Nederland veelal de adviezen van het Ivoren Kruis gehanteerd. De basis van dit advies is plaqueverwijdering met fluoridetandpasta. Wanneer dit onvoldoende bescherming biedt, is er meestal sprake van onregelmatig en onzorgvuldig gebruik. Voorlichting en instructie over dagelijkse en zorgvuldige mondhygiëne dient een eerste prioriteit te zijn bij de zorgverlener. Als zelfzorg niet of niet meteen op peil kan worden gebracht, kan (tijdelijke) ondersteuning plaatsvinden met professionele preventieve behandelingen. Alleen professionele preventieve behandeling zonder aandacht voor verbetering van zelfzorg is onvoldoende en moet als een kunstfout in de behandelingsstrategie worden gezien.

This thesis will explore the most meaningful aspects which are considered to be of interest regarding the efficacy of toothbrushes. One could discuss efficacy by means of plaque removal as well as by means of the effect on gingival inflammation. Plaque removal may be considered as a surrogate effect whereas effects on gingival inflammation can be considered more as an effect related to oral health. Some of the following papers in this thesis focus on instant effects such as plaque removal and may also assess gingival abrasion as a means to assess safety to soft oral tissue.The aim of this PhD research is to provide an overview on the efficacy of toothbrushing. Efficacy relates to numerous factors such as the time spend to brush, the frequency of brushing, the brushhead type, filament type and arrangement, the age of the toothbrush, and with regard to powered toothbrushes, the mode of action.Specific objectives were:1. The toothbrush: Outdated or state of the art? 2. Effects of school-based brushing on gingivitis. 3. Toothbrush age: Is an old brush less effective as compared to a new brush? 4. Brushhead design: Is a multi-level toothbrush more effective than a flat-trimmed toothbrush. 5. Manual versus Powered. Are powered toothbrushes more effective than manual brushes? 6. Powered versus Powered. Which mode of action of powered toothbrushes is more effective? 7. Plaque removal efficacy. On average, how much plaque is removed after a single brushing episode using a powered toothbrush?

This thesis presents a sequence of papers to illustrate selected aspects of the neglect of global oral health, highlights new approaches to describing the extent and impact of dental caries, explains the difficulties related to quality assurance of fluoride toothpastes; and, finally, describes a successful model for school health, which integrates oral health. Moreover, the important role of advocacy in different contexts is illustrated.
