4. p64 flow diverter: Results in 108 patients from a single center

Abstract

Background and purposeFlow diverters are increasingly used to treat intracranial aneurysms. We report the safety and efficacy of the p64 flow diverter, a resheathable and detachable device for intracranial aneurysms.Materials and methodsWe retrospectively reviewed 108 patients with 109 aneurysms treated with the p64 between March 2014 and July 2019. There were 87 women and 21 men, mean age 57 years. Of 109 aneurysms, 74 were discovered incidentally, 12 were symptomatic, 18 were previously treated, and five were ruptured dissection aneurysms. A total of 10 aneurysms were located in the posterior circulation. The mean aneurysm or remnant size was 8.1 mm.ResultsHemorrhage by perforation with the distal guidewire occurred in two patients with permanent neurological deficits in one. In one patient, acute in-stent occlusion caused infarction with a permanent deficit. Permanent morbidity was 1.9% (2 of 108, 95%CI 0.1-6.9%); there was no mortality. During follow-up, three in-stent occlusions occurred, all asymptomatic. There were no delayed hemorrhagic complications. At six months, 77 of 96 aneurysms (80.2%) were completely occluded, and at last follow-up, this increased to 93 of 96 aneurysms (96.9%). In-stent stenosis at any degree occurred in 11 patients, progressing to asymptomatic complete occlusion in one. In the other patients, stenosis resolved or improved at further follow-up.ConclusionThe p64 offers an effective and safe treatment option. Aneurysm occlusion rate was 97% at last follow-up, mostly achieved with a single device. There were no delayed hemorrhagic complications. Delayed in-stent stenosis infrequently progresses to occlusion but remains a matter of concern.

5. The Woven EndoBridge for unruptured intracranial aneurysms: Results in 95 aneurysms from a single center

Background and purposeThe Woven EndoBridge (WEB) is an intrasaccular flowdisruptor that is increasingly used for the treatment of (wide-necked) aneurysms. We present our experience with the WEB for unruptured aneurysms.Materials and methodsBetween April 2014 and August 2019, 93 patients with 95 unruptured aneurysms were primarily treated with the WEB. There were 69 women and 24 men, mean age 61 years (median 58, range 37-80).ResultsOf 95 aneurysms, 86 had been discovered incidentally, 3 were symptomatic and 6 were additional to another ruptured aneurysm. Location was anterior communicating artery 33, middle cerebral artery 29, basilar tip 19, carotid tip 8, posterior communicating artery 4, posterior inferior cerebellar artery 1, superior cerebellar artery 1. Mean aneurysm size was 6 mm (median 6, range 3-13 mm).In one aneurysm additional coils were used and in another, a stent was placed. There was one procedural rupture without clinical sequelae. There were two thrombo-embolic complications leading to permanent deficit in one patient (mRS 2). Morbidity rate was 1.0% (1 of 93, 95%CI 0.01-6.5%) and mortality was 0% (0 of 93, 95%CI 0.0-4.8%). Angiographic follow-up at six months was available in 85 patients with 87 aneurysms (91%). Of 87 aneurysms, 68 (78%) were completely occluded, 14 (16%) had a neck remnant and 5 were incompletely occluded. Four aneurysms were retreated. Retreatment rate was 4.5% (4 of 87, 95%CI 1.7-13.6%).ConclusionWEB treatment of unruptured aneurysms is safe and effective. Additional devices are needed only rarely and retreatment at follow-up is infrequent.

6. The p48 flow diverter: First clinical results in 25 aneurysms in three centers

Abstract

BackgroundThe novel low-profile p48 flow diverter has been designed to treat aneurysms on small vessels of 1.75-3mm. We report our first clinical experiences.MethodsBetween March 2018-January 2020, 22 patients with 25 aneurysms were treated with the p48 in 3 centers. One patient had 3 aneurysms covered by one p48 and one patient had 2 aneurysms. There were 5 men, 17 women, with a mean age of 55 years (median 59, range 29-73 years).ResultsIn 25 aneurysms, 24 p48 flow diverters were placed. In 1 patient additional coils were placed in the aneurysm. Procedural vessel rupture by the micro guidewire occurred in 2 patients and vessel rupture during p48 balloon dilatation occurred in 1 patient. Overall, the permanent morbidity rate was 13.6% (3 of 22, 95%CI 3.9-34.2%) and mortality was 4.5% (1 of 22, 95%CI <0.01-23.5%). Most complications were procedure-related and not device-specific. Of 22 patients with 25 aneurysms treated with p48, 18 patients with 20 aneurysms had angiographic follow-up after 5-18 months. Of 19 aneurysms, 10 were occluded and 7 showed a remnant. Two aneurysms were open after 6 months. Three aneurysms were still not occluded after 12, 14, and 18 months and these 3 were retreated. Retreatment rate was 16% (3 of 19) and the adequate occlusion rate was 90% (17 of 19).ConclusionsTreatment of aneurysms in small-caliber vessels with the p48 is feasible and effective but is not without complications. More data is needed to establish indications, safety, and efficacy more accurately.

22. Percutaneous Vertebroplasty and Procedural Pain

Abstract

BACKGROUND AND PURPOSE: No consensus exists regarding pain management during PV. In this study, we evaluated the effectiveness of local infiltration anesthesia as the only pain medication. MATERIALS AND METHODS: From September 2008 to March 2009, 44 consecutive patients (35 women and 9 men; mean age, 74 years) with symptomatic OVCFs were included in the study. Lidocaine was infiltrated to the skin and the periosteum of the pedicle. After PV, patients indicated pain sensation on a VAS. In addition, patients indicated the most painful moment during the procedure: lidocaine infiltration, placing the needles, or cement injection. Finally, patients were asked whether pain medication during the procedure was sufficient. After the procedure the surgeon was asked to judge the expected VAS score of the patient. RESULTS: Mean VAS score was 5.7 (median, 6; range, 1–10). Seventeen of 44 patients (39%) indicated that lidocaine infiltration was insufficient for procedural pain reduction. The mean VAS score of these patients was 7.3 (range, 5–10). Placing the needles was specified as most painful moment in 29 patients (66%), lidocaine infiltration in 11 (25%), and cement injection in 4 (9%). Surgeons' expectations of patients' VAS scores were a mean of 3.3 (median, 3; range, 1–6). CONCLUSIONS: For a substantial proportion of patients, local infiltration anesthesia was not sufficient for pain reduction during PV. The severity of pain experienced by the patient is usually not appreciated correctly by the surgeon.

24. Complications of Particle Embolization of Meningiomas: Frequency, Risk Factors, and Outcome

Abstract

BACKGROUND AND PURPOSE: Particle embolization is widely used in the treatment of meningiomas. We assessed the frequency and outcome of complications of embolization of meningiomas and tried to identify risk factors. MATERIALS AND METHODS: Between 1994 and 2009, a total of 198 patients with 201 meningiomas underwent embolization. Indication for embolization was preoperative in 165 meningiomas and adjunctive to radiosurgery in 8. In the remaining 28 meningiomas, embolization was initially offered as a sole therapy. There were 128 women and 70 men with a mean age of 54.4 years (median age, 54 years; range, 15–90 years). Complications were defined as any neurologic deficit or death that occurred during or after embolization. Logistic regression was used to identify the following possible risk factors: age above median, female sex, tumor size above median, meningioma location in 5 categories, use of small particle size (45–150 μm), the presence of major peritumoral edema, and arterial supply in 3 categories. RESULTS: Complications occurred in 11 patients (5.6%; 95% confidence interval [CI], 3.0%–9.8%). Ten complications were hemorrhagic, and 1 was ischemic. Six of 10 patients with hemorrhagic complications underwent emergency surgery with removal of the hematoma and meningioma. Complications of embolization resulted in death in 2 and dependency in 5 patients (7/198, 3.5%; 95% CI, 1.6%–2.0%). The use of small particles (45–150 μm) was the only risk factor for complications (odds ratio [OR], 10.21; CI, 1.3–80.7; P = .028). CONCLUSIONS: In this series, particle embolization of meningiomas had a complication rate of 5.6%. We believe that the use of small polyvinyl alcohol (PVA) particles (45–150 μm) should be discouraged.

Abstract

Coiling is increasingly used as treatment for intracranial aneurysms with favorable short-term outcome. Concern exists about long-term reopening and the inherent risk of recurrent subarachnoid hemorrhage (SAH), and long-term imaging follow-up is advocated. It is unknown for how long and how often coiled aneurysms need to be followed and what subgroups carry a higher or lower risk for reopening. Recently, new data have become available that concern the designation of an optimal long-term follow-up protocol. Three studies focused on a special subgroup, the coiled aneurysms that are adequately occluded at 6 months. This subgroup comprises about 80% of all coiled aneurysms. In these aneurysms, the risk of reopening that needs retreatment during the next 5–10 years is very low. In addition, the risk of recurrent SAH is almost zero, even lower than that after clipping. Within the first 5 years after coiling, the incidence of the development of de novo aneurysms or enlargement of existing untreated aneurysms is low, with an extremely low risk of recurrent SAH from these aneurysms. These data suggest that aneurysms that are adequately occluded at 6 months after coiling, in general, do not need prolonged imaging follow-up in the next 5–10 years. More data are needed to identify possible subgroups with adequately occluded coiled aneurysms at 6 months that behave differently on longer follow-up, such as giant aneurysms, partially thrombosed aneurysms, or patients with a positive family history.

Abstract

SUMMARY: Giant serpentine aneurysms are fusiform partially thrombosed aneurysms with a separate outflow tract to normal distal cerebral vessels. Three patients with giant serpentine aneurysms of the anterior and middle cerebral arteries were treated with endovascular occlusion of the aneurysmal lumen with coils or glue after balloon test occlusion of the involved vessel. In all 3 patients, leptomeningeal collateral circulation was sufficient to prevent distal ischemia.
