Purpose: To demonstrate the feasibility of a novel, ellipse fitting approach, named PLANET, for simultaneous estimation of relaxation times T1 and T2 from a single 3D phase-cycled balanced steady-state free precession (bSSFP) sequence. Methods: A method is presented in which the elliptical signal model is used to describe the phase-cycled bSSFP steadystate signal. The fitting of the model to the acquired data is reformulated into a linear convex problem, which is solved directly by a linear least squares method, specific to ellipses. Subsequently, the relaxation times T1 and T2, the banding free magnitude, and the off-resonance are calculated from the fitting results. Results: Maps of T1 and T2, as well as an off-resonance and a banding free magnitude can be simultaneously, quickly, and robustly estimated from a single 3D phase-cycled bSSFP sequence. The feasibility of the method was demonstrated in a phantom and in the brain of healthy volunteers on a clinical MR scanner. The results were in good agreement for the phantom, but a systematic underestimation of T1 was observed in the brain. Conclusion: The presented method allows for accurate mapping of relaxation times and off-resonance, and for the reconstruction of banding free magnitude images at realistic signalto- noise ratios. Purpose: Ongoing discussions occur to translate the safety restrictions on MR scanners from specific absorption rate (SAR) to thermal dose. Therefore, this research focuses on the accuracy of thermal simulations in human subjects during an MR exam, which is fundamental information in that debate. Methods: Radiofrequency (RF) heating experiments were performed on the calves of 13 healthy subjects using a dedicated transmit-receive coil while monitoring the temperature with proton resonance frequency shift (PRFS) thermometry. Subject-specific models and one generic model were used for electromagnetic and thermal simulations using Pennes' bioheat equation, with the blood equilibration constant equaling zero. The simulations were subsequently compared with the experimental results. Results: The mean B1+ equaled 15 μT in the center slice of all volunteers, and 95% of the voxels had errors smaller than 2.8 μT between the simulation and measurement. The intersubject variation in RF power to achieve the required B1+ was 11%. The resulting intersubject variation in median temperature rise was 14%. Thermal simulations underestimated the median temperature increase on average, with 34% in subject-specific models and 28% in the generic model. Conclusions: Although thermal measures are directly coupled to tissue damage and therefore suitable for RF safety assessment, insecurities in the applied thermal modeling limit their estimation accuracy.

Transcranial magnetic stimulation (TMS) is an emerging technique that allows non-invasive neurostimulation. However, the correct validation of electromagnetic models of typical TMS coils and the correct assessment of the incident TMS field (BTMS) produced by standard TMS stimulators are still lacking. Such a validation can be performed by mapping BTMS produced by a realistic TMS setup. In this study, we show that MRI can provide precise quantification of the magnetic field produced by a realistic TMS coil and a clinically used TMS stimulator in the region in which neurostimulation occurs. Measurements of the phase accumulation created by TMS pulses applied during a tailored MR sequence were performed in a phantom. Dedicated hardware was developed to synchronize a typical, clinically used, TMS setup with a 3-T MR scanner. For comparison purposes, electromagnetic simulations of BTMS were performed. MR-based measurements allow the mapping and quantification of BTMS starting 2.5 cm from the TMS coil. For closer regions, the intra-voxel dephasing induced by BTMS prohibits TMS field measurements. For 1% TMS output, the maximum measured value was ~0.1 mT. Simulations reflect quantitatively the experimental data. These measurements can be used to validate electromagnetic models of TMS coils, to guide TMS coil positioning, and for dosimetry and quality assessment of concurrent TMS-MRI studies without the need for crude methods, such as motor threshold, for stimulation dose determination.

Purpose: Dynamic contrast enhanced (DCE) imaging is a widely used technique in oncologic imaging. An essential prerequisite for obtaining quantitative values from DCE-MRI is the determination of the arterial input function (AIF). However, it is very challenging to accurately estimate the AIF using MR. A comprehensive model, which uses complex data instead of either magnitude or phase, was developed to improve AIF estimation. Theory and Methods: The model was first applied to simulated data. Subsequently, the accuracy of the estimated contrast agent concentration was validated in a phantom. Finally the method was applied to existing DCE scans of 13 prostate cancer patients. Results: The complex signal method combines the complementary strengths of the magnitude and phase method, increasing the precision and accuracy of concentration estimation in simulated and phantom data. The in vivo AIFs show a good agreement between arterial voxels (standard deviation in the peak and tail equal 0.4 mM and 0.12 mM, respectively). Furthermore, the dynamic behavior closely followed the AIF obtained with DCE-CT in the same patients (mean correlation coefficient: 0.92). Conclusion: By using the complex signal, the AIF estimation becomes more accurate and precise. PURPOSE: The high precession frequency in ultrahigh field MRI coincides with reduced RF penetration, increased RF power deposition and consequently can lead to reduced scan efficiency. However, the shorter wavelength enables the use of efficient antennas rather than loop coils. In fact, ultrathin monopole antennas have been demonstrated at 7 T, which fit in natural cavities like the rectum in the human body. As the RF field generated by the antenna provides an extremely nonuniform B1 field, the use of conventional RF pulses will lead to severe image distortions and highly nonuniform contrast. However, using the two predominant dimensions (orthogonal to the antenna), 2D RF pulses can be designed that counteract the nonuniform B1 into uniform flip angles. In this study the authors investigate the use of an ultrathin antenna not only for reception, but also for transmission in 7 T MRI of the rectum. METHODS: The 2D radially compensating excitation (2D RACE) pulse was designed in matlab. SAR calculations between the 2D RACE pulse and an adiabatic RF pulse (BIR-4) have been obtained, to visualize the gain in decreasing the SAR when using the 2D RACE pulse instead of an adiabatic RF pulse. The authors used the 7 T whole body MR system in combination with an internally placed monopole antenna used for transceiving and obtained 3D gradient echo images with a conventional sinc pulse and with the 2D RACE pulse. For extra clarity, they also reconstructed an image where the receive field of the antenna was removed. RESULTS: Comparing the results of the SAR simulations of the 2D RACE pulse with a BIR-4 pulse shows that for low flip angles (θ < 41°) the SAR can be decreased with a factor of 4.8 or even more, when using the 2D RACE pulse. Relative to a conventional sinc excitation, the 2D RACE pulse achieves more uniform flip angle distributions than a BIR-4 pulse with a smaller SAR increase (16 × versus 64 ×). CONCLUSIONS: The authors have shown that the 2D RACE pulse provides more homogeneous flip angles for gradient echo sequences when compared to a conventional sinc pulse albeit at increased SAR. However, when compared to adiabatic RF pulses, as shown by simulations, the SAR of the 2D RACE pulse can be an order of magnitude less. Phantom and in vivo human rectum images are obtained to demonstrate that the 2D RACE pulse can provide a uniform excitation while transmitting with a single ultrathin endorectal antenna at 7 T. The combination of thin rectal antennas with efficient uniform transmit can open up new possibilities in high resolution imaging of rectal cancer.

PURPOSE: One of the main safety concerns in MR is heating of the subject due to radiofrequency (RF) exposure. Recently was shown that local peak temperatures can reach dangerous values and the most prominent parameter for accurate temperature estimations is thermoregulation. Therefore, the goal of this research is testing the feasibility of measuring thermoregulation in vivo using MR methods. THEORY AND METHODS: The calves of 13 volunteers were scanned at 3 tesla. A Proton Resonance Frequency Shift method was used for temperature measurement. Arterial Spin Labeling and phase contrast scans were used for perfusion and flow measurements respectively. The calves were monitored during extreme RF exposure (20 W/kg, 16 min) and after physical exercise. RESULTS: Temperature increases due to RF absorption (range of the 90th percentile of all volunteers: 1.1-2.5°C) matched with the reference skin temperature changes. Parallel imaging is essential for the acceleration of abdominal and pelvic 2D multi-slice imaging, in order to reduce scan time and mitigate motion artifacts. Controlled Aliasing In Parallel Imaging Results IN Higher Acceleration (CAIPIRINHA) accelerated imaging has been shown to increase the signal-to-noise ratio (SNR) significantly compared with in-plane parallel imaging with similar acceleration. We hypothesize that for CAIPIRINHA-accelerated abdominal imaging the consistency of image quality and SNR is more difficult to achieve due to the subject-specific coil sensitivity profiles, caused by (1) flexible coil placement; (2) variations in anatomy; and (3) variations in scan coverage along the superior-inferior direction. To test this, a mathematical framework is introduced that calculates the (retained) SNR for in-plane and simultaneous multi-slice (SMS)-accelerated acquisitions. Moreover, this framework was used to optimize the sampling pattern by maximizing the local SNR within a region of interest (ROI) through non-linear, RF-induced CAIPIRINHA slice shifts. The framework was evaluated on 14 healthy subjects and the optimized sampling pattern was compared with in-plane acceleration and CAIPIRINHA acceleration with linear slice shifts, which are primarily used in brain imaging. We demonstrate that the field of view (FOV) in the superior-inferior direction, the coil positioning and the individual anatomy have a large impact on the image SNR (changes up to 50% for varying coil positions and 40% differences between subjects) and image artifacts for simultaneous multi-slice acceleration. Consequently, sampling patterns have to be optimized for acquisitions employing different FOVs and ideally on an individual basis. Optimization of the sampling pattern, which exploits non-linear shifts between slices, showed a considerable SNR increase (10-30%) for higher acceleration factors. The framework outlined in this article can be used to optimize sampling patterns for a broad range of accelerated body acquisitions on an individual basis.

PURPOSE: MR safety at 7 Tesla relies on accurate numerical simulations of transmit electromagnetic fields to fully assess local specific absorption rate (SAR) safety. Numerical simulations for SAR safety are currently performed using models of healthy patients. These simulations might not be useful for estimating SAR in patients who have large lesions with potentially abnormal dielectric properties, e.g., brain tumors. THEORY AND METHODS: In this study, brain tumor patient models are constructed based on scans of four patients with high grade brain tumors. Dielectric properties for the modeled tumors are assigned based on electrical properties tomography data for the same patients. Simulations were performed to determine SAR. RESULTS: Local SAR increases in the tumors by as much as 30%. However, the location of the maximum 10-gram averaged SAR typically occurs outside of the tumor, and thus does not increase. In the worst case, if the tumor model is moved to the location of maximum electric field intensity, then we do observe an increase in the estimated peak 10-gram SAR directly related to the tumor. CONCLUSION: Peak local SAR estimation made on the results of a healthy patient model simulation may underestimate the true peak local SAR in a brain tumor patient. The purpose of this work was to show that the overall peak power of RF pulses for CAIPIRINHA excitation can be substantially reduced by applying interslice phase relaxation. The optimal phases are scan dependent and can be quickly calculated by the proposed method. The multi-band RF pulse design is implemented as the minimization of a linear objective function with quadratic constraints. The interslice phase is considered to be a variable for optimization. In the case of a phase cycling scheme (CAIPIRINHA), the peak power is considered over all pulses. The computation time (about 1s) is compatible with online RF pulse design. It is shown that the optimal interslice phases depend on the CAIPIRINHA scheme used and that RF peak power is reduced when the CAIPIRINHA phase cycling is taken into account in the optimization. The proposed method is extremely fast and results in RF pulses with low peak power for CAIPIRINHA excitation. The MATLAB implementation is given in the appendix; it allows for online determination of scan-dependent phase parameters. Furthermore, the method can be easily extended to pTx shimming systems in the context of multi-slice excitations, and this possibility is included in the software. PURPOSE: MR thermometry (MRT) is a noninvasive method for measuring temperature that can potentially be used for radio frequency (RF) safety monitoring. This application requires measuring absolute temperature. In this study, a multigradient-echo (mGE) MRT sequence was used for that purpose. A drawback of this sequence, however, is that its accuracy is affected by background gradients. In this article, we present a method to minimize this effect and to improve absolute temperature measurements using MRI. THEORY: By determining background gradients using a B0 map or by combining data acquired with two opposing readout directions, the error can be removed in a homogenous phantom, thus improving temperature maps. METHODS: All scans were performed on a 3T system using ethylene glycol-filled phantoms. Background gradients were varied, and one phantom was uniformly heated to validate both compensation approaches. Independent temperature recordings were made with optical probes. RESULTS: Errors correlated closely to the background gradients in all experiments. Temperature distributions showed a much smaller standard deviation when the corrections were applied (0.21°C vs. 0.45°C) and correlated well with thermo-optical probes. CONCLUSION: The corrections offer the possibility to measure RF heating in phantoms more precisely. This allows mGE MRT to become a valuable tool in RF safety assessment.
