Abstract

Exciting new developments—pharmacologic, endoscopic, and surgical—have arisen for the treatment of many esophageal diseases. Refractory gastroesophageal reflux disease presents a therapeutic challenge, and several new options have been proposed to overcome an insufficient effectiveness of proton pump inhibitors. In patients with distal esophageal spasm, drugs and endoscopic treatments are the current mainstays of the therapeutic approach. Treatment with proton pump inhibitors (or antireflux surgery) should be considered in patients with Barrett's esophagus, since a recent meta-analysis demonstrated a 71% reduction in risk of neoplastic progression. Endoscopic resection combined with radiofrequency ablation is the standard of care in patients with early esophageal adenocarcinoma. Mucosal squamous cancer may also be treated endoscopically, preferably with endoscopic submucosal dissection. Patients with upper esophageal cancer often refrain from surgery. Robot-assisted, thoracolaparoscopic, minimally invasive esophagectomy may be an appropriate option for these patients, as the robot facilitates a good overview of the upper mediastinum. Induction chemoradiotherapy is currently considered as standard treatment for patients with advanced squamous cell carcinoma, while the role of neoadjuvant therapy for adenocarcinoma remains controversial. A system for defining and recording perioperative complications associated with esophagectomy has been recently developed and may help to find predictors of mortality and morbidity.

Abstract

In het eerste- en tweede IJkdijk ontwikkelprogramma is binnen de verschillende activiteiten, (onder meer) de LiveDijken en veldproeven veel kennis ontwikkeld. Dit betreft kennis over faalmechanismen aan keringen en de (door)ontwikkeling van innovatieve toepassingen en technieken voor de beheerderspraktijk. Binnen deze best practices rapportage is de meerwaarde van de projecten bekeken vanuit drie primaire processen van de waterbeheerder: beheer en onderhoud, beoordelen (voorheen toetsen) en versterken. De gedachte hierachter is dat betrouwbare technische informatie over keringen, bereikt door analyse van rationeel en gestructureerd meten en monitoren aan deze keringen een bijdrage levert aan het inzicht in de toestand van de dijken. Deze informatiecyclus van de kering stelt de beheerder in de gelegenheid om op ieder gewenst moment (tot real-time) inzicht in zijn beheersgebied te hebben. Hiermee wordt aangesloten op trends en behoeften als assetmanagement, continue inzicht en de zorgplicht. Specifiek kan dit daarnaast bijdragen aan het beoordelingsproces (door meer inzicht minder afkeuren of beter geprioriteerd voor versterking) en bij het scherper ontwerpen (binnen de versterkingsopgave). Ten slotte bieden de beschreven meet- en monitoringstechnieken inzicht en handvatten in het geval van calamiteuze situaties, informatie uit monitoringssystemen kan worden benut voor een betere (crisis)besluitvorming.

Abstract

In de eerste- en tweede IJkdijk ontwikkelprogramma’s is binnen de verschillende activiteiten (onder meer de) LiveDijken en veldproeven, veel kennis ontwikkeld. Dit betreft kennis over faalmechanismen aan keringen en de (door)ontwikkeling van innovatieve toepassingen en technieken voor de beheerderspraktijk. Deze Lessons Learned rapportage is gebaseerd op veertien interviews met betrokken partijen afkomstig uit de zogenaamde Gouden Driehoek (overheid, bedrijven en kennisinstellingen). Tijdens de interviews is gereflecteerd – vanuit technisch-, bestuurlijk- en procesmatig perspectief - op de ervaringen van deze partijen ten aanzien van de acht geselecteerde projecten. De meeste lessen zijn geleerd op het gebied van procesorganisatie: procesinrichting, -begeleiding en implementatie van meet-, visualisatie- en monitoringssystemen binnen de eigen organisatie. Respondenten geven aan dat op dit vlak dan ook de meeste winst te behalen valt, teneinde de doorwerking van de resultaten van de projecten en proeven binnen beleidskaders en interne processen te optimaliseren.
