The impact of mosquito-borne diseases on human and veterinary health is being exacerbated by rapid environmental changes caused mainly by changing climatic patterns and globalization. To gain insight into mosquito-borne virus circulation from two counties in eastern and southeastern Romania, we have used a combination of sampling methods in natural, urban and peri-urban sites. The presence of 37 mosquito-borne viruses in 16,827 pooled mosquitoes was analyzed using a high-throughput microfluidic real-time PCR assay. West Nile virus (WNV) was detected in 10/365 pools of Culex pipiens (n = 8), Culex modestus (n = 1) and Aedes vexans (n = 1) from both studied counties. We also report the first molecular detection of Sindbis virus (SINV) RNA in the country in one pool of Culex modestus. WNV infection was confirmed by real-time RT-PCR (10/10) and virus isolation on Vero or C6/36 cells (four samples). For the SINV-positive pool, no cytopathic effectwas observed after infection of Vero or C6/36 cells, but no amplification was obtained in conventional SINV RT-PCR. Phylogenetic analysis of WNV partial NS5 sequences revealed that WNV lineage 2 of theCentral-Southeast European clade, has a wider circulation in Romania than previously known.

In order to determine the prevalence of equine infectious anemia virus (EIAV), Usutu virus (USUV), and West Nile virus (WNV) in eastern Algerian drylands, 340 sera from distinct equids have been collected from 2015 to 2017. Serological analysis for the presence of antibodies against EIAV and flaviviruses was performed using commercially available ELISAs. Sera detected positive, doubtful, or negative close to the doubtful threshold in flavivirus ELISA were tested by the virus neutralization test (VNT), using WNV and USUV strains. The prevalence of WNV antibodies with ELISA was 11.47% (39/340) against 13.53% (46/340) by WNV VNT. EIAV antibodies were not detected in any samples. WNV seroprevalence varies with species, breed and location of horses. Only, one equid was positive for both WNV and USUV neutralizing antibodies. This is the first screening on equids sera of EIAV and USUV in Algeria. This study indicate that WNV and possibly USUV have circulated/are circulating in the Algerian equine population, unlike EIAV does not seem to be present.

Le virus du Nil occidental (VNO) est un arbovirus principalement transmis par les moustiques. Le cycle de transmission enzootique implique généralement une variété d’espèces d’oiseaux sur lesquelles se nourrissent de façon préférentielle divers moustiques, les principaux vecteurs enzootiques au Québec étant les moustiques du genre Culex (c’est-à-dire Culex pipiens et Cx. restuans). Des cas d’infection humaine par le VNO surviennent sporadiquement chaque année au Québec et on sait peu de choses sur les facteurs qui déterminent l’ampleur des éclosions chez les humains. Malgré l’existence de nombreuses études sur le virus du Nil occidental (VNO) aux États-Unis, y compris sur le rôle de réservoir des espèces d’oiseaux, et sur les changements alimentaires du moustique Culex (des oiseaux vers les mammifères), peu d'études équivalentes existent dans les régions voisines du Canada où le VNO est endémique. Ce travail vise à déterminer l’importance relative des différentes espèces d’oiseaux dans la transmission du VNO dans le sud du Québec. Nous nous sommes concentrés sur les espèces aviaires du corridor de migration de l’Atlantique Est (selon la base de données Avibase). Les espèces d’oiseaux ont ensuite été triées et sélectionnées en fonction de ce qui suit : 1) les taux de mortalité aviaire dus au VNO signalés dans les données de surveillance locale du Réseau canadien pour la santé de la faune, 2) la séroprévalence et la compétence des réservoirs du VNO en Amérique du Nord (d’après la littérature), 3) les estimations locales de la densité des oiseaux (d’après les données ebird) et 4) les données d’une étude sur les repas sanguins de Culex pipiens / Cx. restuans saisies entre 2008 et 2013 chez des moustiques de la région de Montréal. L’analyse a généré une liste de 67 espèces d’oiseaux potentiellement impliquées dans la circulation du VNO dans notre zone d’étude et nous avons émis des hypothèses quant à la contribution relative à l’amplification du VNO. Cette étude permettra de mieux comprendre les facteurs susceptibles d’influencer la circulation du VNO dans le sud du Québec et sera utile aux futures études de modélisation de la transmission enzootique du VNO., West Nile virus (WNV) is an arbovirus that is primarily transmitted by mosquitoes. The enzootic transmission cycle typically involves a variety of bird species and mosquitoes that preferentially feed upon them. The main enzootic vectors in Québec are mosquitoes in the genus Culex (i.e., Culex pipiens and Cx. restuans). Cases of WNV occur sporadically each year in Québec and little is known about the factors that drive the magnitude of human outbreaks. Despite many studies on West Nile Virus (WNV) in the US, including the reservoir role of bird species and the summer shifts of the Culex mosquito, feeding from birds to mammals, there have been few equivalent studies in the neighboring regions of Canada where WNV is endemic. Birds are considered the main hosts of WNV amplification and dispersion. In this study, we aim to determine the relative importance of different bird species in WNV transmission in southern Quebec. We focused on the East Atlantic migration corridor of different avian species (according to the Avibase database). Bird species were then sorted and selected according to : 1) rates of avian mortality from WNV reported in local surveillance data from the Canadian Wildlife Health Cooperative; 2) seroprevalence and reservoir competence for WNV in North America (from the literature); 3) local bird density estimates (from ebird data), and 4) data from a blood meal analysis study on Culex pipiens/Cx.restuans mosquitoes captured between 2008 and 2013 in Montreal region. The analysis generated a list of 67 bird species potentially involved in the circulation of WNV in our study area and hypotheses were generated regarding the relative contribution to WNV amplificationThis study will improve our understanding of the factors that may impact WNV circulation in southern Québec and will be useful to future modelling studies of enzootic WNV transmission.

Background West Nile virus (WNV) and Usutu virus (USUV), two closely related flaviviruses, mainly follow an enzootic cycle involving mosquitoes and birds, but also infect humans and other mammals. Since 2010, their epidemiological situation may have shifted from irregular epidemics to endemicity in several European regions; this requires confirmation, as it could have implications for risk assessment and surveillance strategies. Aim To explore the seroprevalence in animals and humans and potential endemicity of WNV and USUV in Southern France, given a long history of WNV outbreaks and the only severe human USUV case in France in this region. Methods We evaluated the prevalence of WNV and USUV in a repeated cross-sectional study by serological and molecular analyses of human, dog, horse, bird and mosquito samples in the Camargue area, including the city of Montpellier, between 2016 and 2020. Results We observed the active transmission of both viruses and higher USUV prevalence in humans, dogs, birds and mosquitoes, while WNV prevalence was higher in horses. In 500 human samples, 15 were positive for USUV and 6 for WNV. Genetic data showed that the same lineages, WNV lineage 1a and USUV lineage Africa 3, were found in mosquitoes in 2015, 2018 and 2020. Conclusion These findings support existing literature suggesting endemisation in the study region and contribute to a better understanding of USUV and WNV circulation in Southern France. Our study underlines the importance of a One Health approach for the surveillance of these viruses.

International audience; Mosquito-borne diseases have a significant impact on humans and animals and this impact is exacerbated by environmental changes. However, in Tunisia, surveillance of the West Nile virus (WNV) is based solely on the surveillance of human neuroinvasive infections and no study has reported mosquito-borne viruses (MBVs), nor has there been any thorough serological investigation of anti-MBV antibodies in horses. This study therefore sought to investigate the presence of MBVs in Tunisia. Among tested mosquito pools, infections by WNV, Usutu virus (USUV), and Sindbis virus (SINV) were identified in Cx. perexiguus. The serosurvey showed that 146 of 369 surveyed horses were positive for flavivirus antibodies using the cELISA test. The microsphere immunoassay (MIA) showed that 74 of 104 flavivirus cELISA-positive horses were positive for WNV, 8 were positive for USUV, 7 were positive for undetermined flaviviruses, and 2 were positive for tick-borne encephalitis virus (TBEV). Virus neutralization tests and MIA results correlated well. This study is the first to report the detection of WNV, USUV and SINV in Cx. perexiguus in Tunisia. Besides, it has shown that there is a significant circulation of WNV and USUV among horses, which is likely to cause future sporadic outbreaks. An integrated arbovirus surveillance system that includes entomological surveillance as an early alert system is of major epidemiological importance.

International audience; Mosquito-borne flaviviruses with an enzootic transmission cycle like Japanese encephalitis virus (JEV) and West Nile virus (WNV) are a major public health concern. The circulation of JEV in Southeast Asia is well-documented, and the important role of pigs as amplification hosts for the virus is long known. The influence of other domestic animals especially poultry that lives in high abundance and close proximity to humans is not intensively analyzed. Another understudied field in Asia is the presence of the closely related WNV. Such analyses are difficult to perform due to the intense antigenic cross-reactivity between these viruses and the lack of suitable standardized serological assays. The main objective of this study was to assess the prevalence of JEV and WNV flaviviruses in domestic birds, detailed in chickens and ducks, in three different Cambodian provinces. We determined the flavivirus seroprevalence using an hemagglutination inhibition assay (HIA). Additionally, we investigated in positive samples the presence of JEV and WNV neutralizing antibodies (nAb) using foci reduction neutralization test (FRNT). We found 29% (180/620) of the investigated birds positive for flavivirus antibodies with an age-depended increase of the seroprevalence (OR = 1.04) and a higher prevalence in ducks compared to chicken (OR = 3.01). Within the flavivirus- positive birds, we found 43% (28/65) with nAb against JEV. We also observed the expected cross-reactivity between JEV and WNV, by identifying 18.5% double-positive birds that had higher titers of nAb than single-positive birds. Additionally, seven domestic birds (10.7%) showed only nAb against WNV and no nAb against JEV. Our study provides evidence for an intense JEV circulation in domestic birds in Cambodia, and the first serological evidence for WNV presence in Southeast Asia since decades. These findings mark the need for a re-definition of areas at risk for JEV and WNV transmission, and the need for further and intensified surveillance of mosquito-transmitted diseases in domestic animals.

Le virus West Nile a été identifié pour la première fois en 1937 en Ouganda, il est transmis essentiellement par des moustiques du genre Culex sp. C'est un arbovirus de la famille des Flaviviridae infectant les oiseaux. Ce virus fait parler de lui dans les pays du pourtour méditerranéen depuis les années 1990. En 1999 ce virus fait son apparition en Amérique du Nord, il se répand sur l'ensemble du territoire nord-américain durant les dix années qui vont suivre. Ce virus va devenir la principale cause d'encéphalites arbovirales aux Etats-Unis. La sévérité des symptômes et sa transmission via transfusion et transplantations d'organes ont nécessité la mise en place de tests de dépistages et des réseaux de surveillance, pour mieux comprendre la biologie, l'écologie et l'épidémiologie de ce virus. En Europe, les épidémies sont plus limitées, mais la récurrence de ces épidémies dans certaines régions est inquiétante. L'infection est souvent asymptomatique, mais des symptômes grippaux et des désordres neurologiques plus graves pouvant entraîner la mort peuvent survenir. Des études sont réalisées afin de mettre au point des solutions thérapeutiques qui sont actuellement absentes et des mesures prophylactiques.

Innovative vaccine approach based on the use of novirhabdovirus-derived platform. Novirhabdoviruses such as the Viral Hemorrhagic Septicemia Virus (VHSV) or the Infectious Hematopoietic Necrosis Virus (IHNV) are rhabdoviruses infecting fish at low temperature. The use of reverse genetics system developed in our laboratory allowed, by manipulating the viral genome, the recovery of recombinant virus expressing at the viral envelope various antigens of interest against animal or human viruses to vaccinate mammalian. In the current article, we present the generation of a new vaccine against the West Nile Virus, a Flavivirus and against influenza virus. In the latter example, we have produced recombinant Novirhabdoviruses expressing the hemagglutinin HA from H1N1 influenza virus. These recombinant viruses, when injected in mice, induce a complete protection of the immunized mice against a lethal challenge with influenza virus. That new vaccine platform offers several advantages i) The recombinant viruses are easy and fast to be mass produced ii) They can incorporate any antigen of interest at the viral envelope iii) Immunizations do not need adjuvant addition iv) They are naturally inactivated at temperature higher than 20° C, thus do not require any inactivation process., Les Novirhabdovirus, tels que le Virus de la Septicémie Hémorragique Virale (vSHV) ou le virus de la Nécrose Hématopoiétique Infectieuse (vNHI) sont des rhabdovirus infectant les poissons à basse température. L’utilisation de systèmes de génétique inverse développés dans notre laboratoire ont permis, en manipulant le génome viral, de produire des virus recombinés exprimant à leur surface différents antigènes d’intérêt pour la vaccination contre des virus affectant les mammifères, dont l’homme. Dans cette communication, nous présentons le développement de vaccins contre le virus du Nil Occidental, un Flavivirus et contre un virus grippal. Dans ce dernier exemple, nous avons produit des Novirhabdovirus exprimant l’hémagglutinine HA, très immunogène, d’un virus grippal H1N1. Ces virus recombinés injectés chez la souris induisent une protection totale de ces souris immunisées face à une épreuve létale avec le virus grippal. Cette plateforme vaccinale présente de nombreux avantages : i) les virus recombinés sont faciles et rapides à produire en masse, ii) ils peuvent incorporer n’importe quel antigène d’intérêt à leur surface, iii) ils ne nécessitent pas d’adjuvant lors des immunisations, iv) ils sont naturellement inactivés au-delà de 20° C, rendant ainsi inutile le processus d’inactivation., Brémont Michel. Depuis quelques décennies, les maladies vectorielles, maladies transmises par des arthropodes hématophages, sont en émergence ou en réémergence à travers le monde. La gestion des maladies vectorielles repose principalement sur la réduction des contacts entre l’homme et le vecteur, ce qui nécessite de connaître la distribution spatio-temporelle des populations de vecteurs infectés. Dans un contexte d’émergence, l’estimation du risque entomologique , représenté par la distribution des populations de vecteurs en quête d’un hôte, est souvent la seule manière d’estimer le risque pour l’homme dans la mesure où la circulation de l’agent pathogène et la détection des cas humains sont encore faibles. En s’appuyant sur l’émergence de la maladie de Lyme et du virus du Nil occidental (VNO) au Québec, ce travail de recherche avait pour objectif de comprendre la représentativité du risque entomologique, estimé par la surveillance ou la modélisation, par rapport à la distribution réelle des vecteurs et par rapport au risque pour l’homme, en fonction de l’écologie des vecteurs d’intérêt dans un contexte d’émergence. Dans un premier temps, nous avons étudié la distribution spatio-temporelle du risque entomologique estimé par la surveillance entomologique de la maladie de Lyme. La première étude a montré une importante progression des populations de tiques, en termes de présence et d’abondance, entre 2007 et 2014 dans le sud du Québec. Ces résultats sont en faveur d’une surveillance à long terme pour réévaluer régulièrement le risque entomologique et identifier les nouvelles zones à risque. La deuxième étude s’est intéressée à la distribution locale des populations de tiques à différentes échelles spatiales. L’hétérogénéité de la distribution mise en évidence à chaque échelle, en lien avec l’écologie des tiques, implique une variation locale du risque. Ce résultat incite à la réflexion sur l’utilisation et l’interprétation du risque entomologique, notamment dans les zones où la présence de tiques n’a pas pu être confirmée par la surveillance, et sur le choix de l’échelle géographique utilisée pour présenter ce risque. La compréhension de la variation spatiale du risque à une fine échelle peut être un atout pour les communautés locales, les gestionnaires de parcs ou les citoyens, pour interpréter correctement les informations véhiculées à une plus large échelle par les autorités de santé publique et adapter leur comportement à la situation locale. La troisième étude s’est intéressée à la relation entre le risque entomologique et le risque pour l’homme estimé par le nombre de cas humains. Nos résultats suggèrent que le risque entomologique peut être un bon estimateur du risque pour l’homme en fonction de l’indicateur choisi. Dans notre étude, la surveillance entomologique passive est un meilleur prédicteur du nombre de cas humains dans une municipalité que la surveillance entomologique active. Nous avons ainsi pu développer un indicateur, le nombre de personnes ayant soumis une tique en surveillance passive, pour discriminer les municipalités ayant déclaré au moins trois cas humains. Cet indicateur a été inclus dans la définition des niveaux de risque au Québec en 2017. Mais la relation entre le risque entomologique et le risque pour l’homme comporte des biais, notamment en raison de l’hétérogénéité spatio-temporelle des populations de vecteurs, de la mobilité des vecteurs, de la distribution de la population humaine et du comportement humain. Dans un deuxième temps, nous avons exploré la représentativité du risque entomologique estimé à partir de la modélisation des populations de vecteurs, en s’appuyant sur l’exemple du VNO. Nos modèles donnent une estimation de l’abondance quotidienne de Culex pipiens-restuans et Aedes vexans au cours de l’été en fonction des conditions météorologiques, ce qui permet de prévoir les périodes les plus à risque de transmission, lorsque la densité de vecteurs est élevée. Cependant, des facteurs environnementaux, tels que le type d’habitat ou l’utilisation de traitements larvicides, apportent des variations locales de l’abondance des populations de vecteurs. Ces facteurs peuvent néanmoins être pris en compte dans les modèles pour ajuster la valeur moyenne estimée à partir de la température et des précipitations. Cette étude a également souligné l’importance de l’écologie des vecteurs qui nécessite d’adapter les modèles à chaque espèce de vecteur étudiée. Ce travail de thèse a permis de s’interroger sur l’effet de l’écologie des vecteurs et du contexte d’émergence sur l’interprétation et l’utilisation du risque entomologique. Ceci apportera aux différents acteurs en santé publique des connaissances pour optimiser l’utilisation des outils à leur disposition pour la gestion des maladies vectorielles émergentes., In recent decades, vector-borne diseases, diseases transmitted by bloodsucking arthropods, have been emerging or re-emerging around the world. The management of vector-borne diseases is mainly based on the reduction of contacts between the human and the vector, which requires knowledge of the spatiotemporal distribution of vector populations, i.e. the entomological risk. For emerging vector-borne diseases, the estimation of the entomological risk may be the only way to estimate the risk to humans since the circulation of the pathogen and the detection of human cases may be weak. Based on the emergence of Lyme disease and West Nile Virus (WNV) in Québec, the aim of this research was to understand the representativeness of the entomological risk, estimated by surveillance or modeling, in relation to the vector distribution and the risk to humans, according to the ecology of the vectors of interest and in a context of emergence. In the first part of the thesis, I explored the spatial and temporal distribution of entomological risk of Lyme disease as measured by entomological surveillance of its tick vector. The first study showed a significant increase in tick populations in terms of presence and abundance between 2007 and 2014 in southern Québec. These results support long-term monitoring to regularly reassess entomological risk and identify new risk areas. The second study looked at the spatial distribution of tick populations at different spatial scales. The spatial heterogeneity highlighted at each scale implies a local spatial variation of the risk associated with the ecology of ticks. This result asks question about the use and interpretation of information on entomological risk, particularly in areas where the presence of ticks could not be confirmed by surveillance; and about the representation of the risk and the choice of the geographic scale used. Understanding spatial variation in risk can be an asset for local communities, park managers or citizens by allowing interpretation of the information provided on a wider scale by public health authorities and adaptation to the local situation. The third study looked at the relationship between the measured entomological risk and the risk to humans estimated by the number of human cases. Our results suggest that the quality of the estimate of the risk to humans based on the entomological risk depends on the chosen entomological indicator. In our study, passive entomological surveillance was a better predictor of the number of human cases in a municipality than active entomological surveillance. We were thus able to develop an indicator, the number of people who submitted a tick in passive surveillance, to discriminate municipalities with at least three reported human cases. This indicator was included in the official definition of risk levels in Québec in 2017. Despite the success of this indicator, the relationship between entomological risk and risk to humans remains biased because of the spatial and temporal heterogeneity of vector populations, the mobility of vectors, the distribution of the human population and human behavior. In the second part of the thesis, I examined the representativeness of the entomological risk estimated from the modeling of vector populations, using the example of WNV. The resulting models provide a daily estimate of the abundance of Culex pipiens-restuans and Aedes vexans during the summer depending on weather conditions, which made it possible to predict the periods of highest WNV transmission risk due to higher vector density. However, environmental factors, such as the habitat, as well as the use of larvicide treatments, drive local variation in vector population abundance. These factors may nevertheless be taken into account in models to adjust the estimated mean value. This study also highlighted the importance of vector ecology which requires adaptation of the models to each vector species of interest. This thesis has questioned the effect of vector ecology and emergent context on the interpretation and the use of entomological risk. This will provide knowledge to the various actors in public health to optimize the use of the tools at their disposal for the management of emerging vector-borne diseases.

Les preuves s'accumulent sur les effets des changements climatiques. Étant donné leurs impacts sur la santé, en particulier sur les maladies à transmission vectorielle, il est nécessaire de concevoir des stratégies efficaces pour adapter les programmes de gestion de risque de ces maladies. Les changements climatiques constituent un problème complexe, impliquant de multiples parties prenantes et comportant beaucoup d’inconnues. Des approches qui prennent en compte cette complexité sont ainsi nécessaires afin de faire avancer la recherche sur l'adaptation aux changements climatique basée autant sur des données scientifiques que sur des données provenant de différents intervenants. Les approches fondées sur les données probantes sont de plus en plus recherchées dans les politiques de santé et la prise de décisions, dans le but d'améliorer la santé des populations. Ces approches sont apparues en réaction aux approches improvisées, développées suite aux crises liées aux problèmes de santé publique. Elles doivent être systématiques et transparentes, et faire appel aux meilleures preuves disponibles. L'aide à la décision multicritère délibérative constitue une de ces approches. L'objectif de cette thèse était d'étudier les principales préoccupations en matière de décisions relatives à l'adaptation au risque des maladies vectorielles influencées par les changements climatiques dans deux contextes : le Québec, où les maladies vectorielles sont présentes, mais ne constituent pas la principale préoccupation, et le Burkina Faso, où les maladies vectorielles sont au contraire très préoccupantes. Les fondements théoriques de cette étude sont basés sur la science post-normale, l'adaptation aux changements climatiques et une approche d'analyse de décision multicritère. La recherche sur l'adaptation aux changements climatiques vise à influencer les politiques cherchant à réduire les risques et les impacts associés à ces changements. Dans ce cadre et dans le contexte de l'adaptation aux maladies vectorielles, trois questions clés se posent : 1) de quelles maladies nous préoccupons-nous ? 2) qui est le plus vulnérable à ces maladies ? et 3) quelles sont les mesures recommandées pour s'adapter à ces maladies ? Cette thèse contribuera à répondre à ces trois questions dans le but de faire avancer l'adaptation face aux maladies vectorielles. Pour répondre à la première question, nous avons identifié des préoccupations d'importance pour la priorisation des maladies liées aux changements climatiques au Québec et au Burkina Faso grâce à l’utilisation d’une approche délibérative multicritère d'aide à la décision. Les résultats ont démontré que, alors que des préoccupations générales sont partagées entre ces deux régions, des préoccupations plus spécifiques aux maladies diffèrent quant à elles selon le contexte, tant sur des aspects scientifiques que sur d’autres aspects partagés par les parties prenantes. Pour répondre à la deuxième question, les connaissances actuelles et les comportements de la population québécoise quant au virus du Nil occidental ont été explorés, comme étape préliminaire pour évaluer la capacité d'adaptation au risque de maladies causées par les moustiques. Nous avons considéré que la réponse au risque perçu de maladies transmises par les moustiques constituait une forme d'adaptation. Les résultats ont montré que les connaissances globales et les niveaux d'adoption comportementale sont bons et qu’il existe au moins quatre sous-groupes différents dans la population caractérisés par différents facteurs associés à l'adoption de comportements préventifs. Enfin, pour répondre à la troisième question, une approche multicritère délibérative a été utilisée pour examiner les stratégies de gestion du virus du Nil occidental au Québec, dans le cadre théorique d’une transmission accrue, et les stratégies de gestion du paludisme au Burkina Faso, dans le cadre de la transmission actuelle. De manière analogue au modèle développé pour la priorisation des maladies, cette comparaison entre les deux régions et les contextes de maladie ont permis de confirmer l’existence de préoccupations générales partagées. Cette thèse a permis de démontrer la pertinence des approches d'aide à la décision pour explorer des stratégies de gestion efficaces basées sur l'expérience des intervenants et les meilleures preuves scientifiques disponibles., Evidence is accumulating on the ongoing effects of climate change. Given the anticipated health implications, notably vector-borne disease impacts, there is a need to design effective and tailored strategies to adapt to vector-borne disease risk. Climate change is a complex problem, involving multiple stakeholders and many unknowns. As such, approaches that can embrace this complexity are needed to inform adaptation research with evidence - both scientific and stakeholder-informed. Evidence-informed approaches are being increasingly sought in health policy and decision-making in order to improve population health. Evidence-informed approaches have arisen in reaction to ad-hoc, crisis-driven responses to health problems. They recognize the need to be systematic and transparent, and make use of the best available evidence. Deliberative multicriteria decision aid is one such approach. The objective of this thesis was to study key decision concerns of importance in adapting to vector-borne disease risk under climate change in two contexts: Quebec, where vector-borne diseases are present but not the main burden of disease, and Burkina Faso, where vector-borne diseases contribute to the primary burden of disease. The theoretical underpinnings of this study are rooted in post-normal science, climate change adaptation, and a multicriteria decision analysis approach. Climate change adaptation research is aimed at informing policies to reduce risks and impacts associated with climate change. Within this framework, and in the context of vector-borne disease adaptation, three key questions arise: 1) what diseases are we concerned about? 2) who is most vulnerable and at risk to these diseases? and 3) what are recommended measures to adapt to these diseases? This thesis contributes to these three dimensions to inform adaptation to vector-borne disease. With regards to the first question, we identified concerns of importance for disease prioritization under climate change in both Quebec and Burkina Faso using a deliberative multi-criteria decision aid approach. The results showed that general concerns are shared among these contrasting contexts while specific disease priorities differ as a result of context-informed evidence – both scientific and stakeholder-shared. With regards to the second question, current knowledge and readiness of the Quebec population relative to West Nile virus was explored as a preliminary and integral step to assessing adaptive capacity to mosquito-borne disease risk. Here, response to perceived mosquito-borne disease risk constitutes a form of adaptation. The results showed that overall knowledge and behavioural adoption levels are good though at least four different subgroups exist within the population with different factors associated with preventive behaviour adoption. Finally, with respect to the third question, a deliberative multi-criteria approach was used to examine management strategies for West Nile virus in Quebec and malaria in Burkina Faso. West Nile virus strategies under current and theoretical increased transmission were explored in Quebec, and malaria management strategies under current transmission were explored in Burkina Faso. Analogously to the model developed for disease prioritization, shared general concerns were found between the contrasting country and disease contexts lending support to the practical applications of decision-aid approaches for exploring effective management strategies informed by stakeholder experience and the best available scientific evidence.

West Nile virus (WNV) and Rift Valley Fever virus (RVFV) are two emerging arboviruses that have never been reported in Lebanon. They can be transmitted by Culex pipiens mosquito species including two biotypes: pipiens and molestus. During this project, we assessed the circulation of WNV among mosquitoes, human, horse and chicken populations in Lebanon. Moreover, we evaluated, under experimental conditions, the capacity of local Cx. pipiens biotypes to transmit both viruses.Adult mosquitoes were collected, identified and tested to detect WNV RNA. Besides, human, horse and chicken blood samples were collected and screened for WNV antibodies using an in-house ELISA and then confirmed by neutralization assay. Moreover, local Cx. pipiens specimens were experimentally infected with WNV lineage 1 or RVFV Clone 13 strain. The viral infection, dissemination and transmission were then estimated at different days post infection.The vector competence was compared between Cx. pipiens biotypes.Entomological results revealed that 87.2% of collected adult mosquitoes were Cx. pipiens. Screened mosquitoes were negative for WNV. Seroprevalence rates were 1.01% and 1.98% among humans and horses respectively. Besides, local Cx. pipiens were highly competent for WNV transmission and to a lesser extent to RVFV. The molestus biotype was able to transmit WNV earlier than pipiens biotype.The present study provides new evidence of a low circulation of WNV among human and horses in Lebanon. Cx. pipiens is the suspected vector and is experimentally competent to ensure transmission. Therefore, there is a need to establish surveillance program to predict and prevent potential outbreaks.; Le Virus du Nil Occidental (VNO) et le Virus de la Fièvre de la Vallée du Rift (VFVR) sont deux arbovirus transmis par le moustique Culex pipiens comprenant deux biotypes: pipiens et molestus. Au cours de ce projet, nous avons évalué la circulation du VNO au Liban dans des populations de moustiques, des humains, des chevaux et des poulets. Nous avons aussi évalué la compétence vectorielle des populations locales de Cx. pipiens à transmettre le VNO et le VFVR.Des moustiques ont été récoltés et testés pour la présence d’un gène spécifique du VNO. En plus, des sérums humains, de chevaux et de poulets ont été analysés pour rechercher des anticorps spécifiques par ELISA puis confirmés par neutralisation. En outre, des spécimens de Cx. pipiens ont été infectés avec la lignée 1 du VNO ou la souche de VFVR Clone 13. Ensuite, les taux d’infection, de dissémination et de transmission ont été déterminés à différents jours après infection des moustiques. La compétence vectorielle a été comparée entre les différents biotypes.Les résultats entomologiques ont révélé que Cx. pipiens est dominant (87.2%). Tous les moustiques analysés étaient négatifs pour le VNO. Les taux de séroprévalence étaient de 1.01% et 1.98% parmi les humains et les chevaux respectivement. De plus, Cx. pipiens s’est révélé bien plus compétent pour transmettre le VNO que le VFVR. Le biotype molestus est capable de transmettre le VNO plus tôt que celui de pipiens. Cette étude présente des preuves sur une faible circulation du VNO au Liban. Cx. pipiens s’est révélé compétent pour assurer cette transmission. Ainsi, il est essentiel d'établir des programmes de surveillance pour prévenir les éventuelles épidémies.

Les arboviroses représentent un fardeau sanitaire considérable et croissant à l’échelle mondiale. La complexité des facteurs biotiques et abiotiques qui interviennent dans la transmission de ces arbovirus pose un défi de taille aux scientifiques qui tentent de comprendre, de modéliser ou d’anticiper leur transmission ainsi qu’aux intervenants de santé publique qui ont la responsabilité de surveiller, d’évaluer et gérer le risque que posent les arbovirus pour la santé des populations. Cette étude visait à estimer et caractériser le risque d’exposition à plusieurs arbovirus suspectés d’être actifs et émergents au Québec mais dont la distribution avait peu ou n’avait pas été étudiée au Québec : le virus du Nil occidental (VNO), le virus de l’encéphalite équine de l’est (VEEE) et deux virus du sérogroupe de la Californie (VSGC), le virus de Jamestown Canyon (VJC) et le virus du lièvre d’Amérique (VLA). Basée notamment sur l’hypothèse selon laquelle les animaux d’espèces différentes qui partagent un environnement commun partagent également un risque environnemental commun, cette étude visait également à évaluer si les populations d’animaux de compagnie pouvaient aider à estimer et caractériser le risque d’infection arbovirale chez l’humain. L’échantillonnage sérologique de populations humaines, canines et équines du sud-ouest du Québec a permis d’évaluer et de comparer la séroprévalence aux arbovirus étudiés chez chacune de ces trois espèces. Les estimations de séroprévalence ont révélé un niveau d’activité arbovirale significative pour chacun des arbovirus. Des différences ont été remarquées quant au pourcentage de sujets séropositifs chez chacune des espèces. Les facteurs environnementaux ayant une influence sur le risque d’infection par le VEEE ont été modélisés à partir de données sérologiques et cliniques chez les chevaux. Les milieux humides boisés ont été identifiés comme les principaux environnements à risque pour le VEEE au Québec alors que les zones agricoles ont été identifiées comme des environnements protecteurs. Les facteurs environnementaux ayant un impact sur le risque d’infection par le VNO ont été modélisés à partir des données sérologiques chez le chien et des données cliniques agrégées chez l’humain. Cette modélisation a suggéré un risque singulièrement plus élevé en zone agricole chez le chien et un risque plus faible en zone forestière chez l’humain, des facteurs rarement identifiés dans la littérature Nord-Américaine. Les facteurs environnementaux et individuels ayant un impact sur le risque d’infection par les VSGC chez l’humain et le chien ont par la suite été modélisés à partir des données sérologiques chez ces deux espèces. D’après nos modèles, le risque d’infection par ces virus serait supérieur en zone forestière et le degré d’exposition aux piqures de moustiques serait un facteur déterminant du risque d’infection chez les deux espèces. Cette étude a permis de bonifier de façon substantielle le portrait de l’activité arbovirale au Québec. Elle a permis de caractériser la distribution du risque et a fourni des données probantes pouvant soutenir la recherche ainsi que la planification des interventions en santé publique. La méthodologie utilisée dans le cadre de cette étude supporte la pertinence de l’approche « One Health » pour l’étude des maladies vectorielles émergentes., Arboviral infections represent a considerable and growing health burden globally. The complexity of biotic and abiotic factors involved in the transmission of these arboviruses pose a challenge to scientists trying to understand, model or anticipate arboviral transmission as well as to public health authorities who have the responsibility to monitor, assess and manage the public health risk posed by arboviruses. This study aimed at estimating and characterizing the risk of exposure to several arboviruses suspected of being active and emerging in Québec but whose distribution had not been studied thoroughly in Québec: West Nile virus (WNV), eastern equine encephalitis virus (EEEV) and two viruses of the California serogroup (CSG), Jamestown Canyon virus (JCV) and Snowshoe hare virus (SHV). Based on the assumption that animals of different species sharing a common environment also share similar environmental risk, this study also aimed to assess whether some populations of domestic animals could help to estimate and characterize the risk of arboviral infection in humans. Serological sampling of human, canine and equine populations from southwestern Québec was used to evaluate and compare the seroprevalence to the selected arboviruses in each of these three species. Seroprevalence estimates showed a significant level of arboviral activity for all arboviruses. Differences were noted in the percentage of seropositive individuals in each species. Environmental factors that influence the risk of infection by EEEV were modeled based on serological and clinical data in horses. Wooded wetlands were identified as the main risk environments for EEEV in Québec while agricultural areas were identified as protective environments. Environmental factors affecting the risk of WNV infection were modeled based on serological data in dogs and aggregated clinical data in humans. These models suggested a higher risk in agricultural areas in dogs and a lower risk in forest areas in humans, two factors rarely identified in the North American literature. Environmental and individual factors affecting the risk of infection by CSGV in humans and dogs have subsequently been modeled based on serological data in these two species. According to our models, the risk of infection with these viruses would be higher in forested areas and the degree of exposure to mosquito bites would be a risk factor for infection in both species. This study substantially enhanced the comprehension of arboviral activity in Québec. It allowed for characterizing the distribution of risk and provided evidence that may support research and planning of public health interventions. The methodology used in this study supports the relevance of the "One Health" approach for the study of emerging vector-borne diseases.

International audience; West Nile virus is an arthropod-borne zoonosis transmitted by a large number of mosquito species, and birds play a key role as reservoir of the virus. Its distribution is largely widespread over Africa, Asia, the Americas and Europe. Since 1978, it has frequently been reported in Madagascar. Studies described a high seroprevalence level of the virus in humans in different areas of the island and a human fatal case of WNV infection was reported in 2011. Despite these reports, the epidemiology of WNV in Madagascar, in particular , viral circulation remains unclear. To explore the transmission of WNV in two rural human populations of Madagascar, we investigated local mosquitoes and poultry for evidence of current infections, and determined seroprevalence of candidate sentinel species among the local poultry. These 2 areas are close to lakes where domestic birds, migratory wild birds and humans coexist. Serological analysis revealed WNV antibodies in domestic birds (duck, chicken, goose, turkey and guinea fowl) sampled in both districts (Antsalova 29.4% and Mitsinjo 16.7%). West Nile virus nucleic acid was detected in one chicken and in 8 pools of mosquitoes including 2 mosquito species (Aedeomyia madagascarica and Anopheles pauliani) that have not been previously described as candidate vectors for WNV. Molecular analysis of WNV isolates showed that all viruses detected were part of the line-age 2 that is mainly distributed in Africa, and were most closely matched by the previous Malagasy strains isolated in 1988. Our study showed that WNV circulates in Madagascar amongst domestic birds and mosquitoes, and highlights the utility of poultry as a surveillance tool to detect WNV transmission in a peri-domestic setting.

International audience; Long-term neurocognitive and functional impairments following West Nile virus (WNV) disease are poorly understood. We assessed quality-of-life indices and neurocognitive performance in a cohort of 54 persons recovering from one of three WNV disease syndromes (fever [WNF], meningitis [WNM], or encephalitis [WNE]) approximately 1.5 years following acute illness. We compared findings between the three syndromic groups; the study cohort and a demographically similar group of 55 controls from a study of chronic fatigue syndrome (CFS); and the study cohort and a `normative' control population based on cognitive test data. Persistent symptoms, diminished quality of life, and functional impairment were reported by 50% of WNF patients, and 75% each of WNM and WNE patients. Overall, objective neurocognitive performance did not differ significantly between the three syndromic groups, or between the study cohort and the CFS controls or the normative controls. In some neurocognitive subtests, the study cohort scored below the 15th percentile when compared with normative control data. Most persons who returned to independent living following hospitalization for WNV illness had persistent subjective complaints, but had normal cognitive function. However, a minority displayed subtle neurocognitive deficits more than 18 months following acute disease.

Live-attenuated measles vaccine is one of the best human vaccines available. During the 30 last years, it has been given to hundreds of million of children and shown to be highly efficacious and safe. This vaccine induces a life-long immunity to reinfection after a single or two low-dose injections. It is easily produced on a large scale in most countries and can be distributed at low cost. Reversion to pathogenicity has never been observed. For these favorable characteristics, measles vaccine might be a very promising paediatric vector to immunize children against both measles and other viral infections like AIDS or flaviviroses in the developing world. To this end, we developed a replicating vector derived from the Schwarz attenuated vaccine strain of measles virus. We showed the capacity of this vector to strongly and stably express proteins from Human Immunodeficiency virus (HIV) or West Nile virus (WNV). This vector induces specific immune reponses against the heterologous antigens in vivo that, in the case of West Nile virus, are able to protect the animals from an experimental challenge. This work led to the setting of phase I/II clinical trials intended to evaluate the capacities of this vector to induce specific HIV immune reponses in adults with pre immunity against measles.; Le vaccin antirougeoleux compte parmi les meilleurs vaccins disponibles. Au cours des trente dernieres années, il a été adminis- tré à des centaines de millions d'enfants et a montré son efficacite et son innocuité. Ce vaccin vivant attenué induit une immunité vie après une seule ou deux injections. II est facilement produit grande échelle dans de nombreux pays et il est distribue et bas prix. Ces caracteristiques favorables nous ont conduits à envisager son utilisation comme vecteur pediatrique pour simultanément proteger les enfants de la rougeole et les immuniser contre d'autres infections virales comme le sida ou les flaviviroses. Dans ce but, nous avons développé un vecteur vivant derive de la souche vaccinale attenuee Schwarz du virus de la rougeole. Nous avons montre que ce vecteur pouvait exprimer de maniere forte et stable des protéines du virus de I'immunodéficience humaine (VlH) et du virus du Nil occidental. Ce vecteur permet I'induction in vivo de reponses immunes specifiques capables, dans le cas du virus du Nil occidental, de proteger d'une épreuve expérimentale. Ces travaux ont amene à la mise en projet d'essais cliniques de phase I et IIII destinés à tester chez I'homme les capacit6s de ce vecteur à induire des réponses immunes spécifiques du VlH chez des adultes prealablement immunises contre la rougeole.
