Background: Many complications may occur after placement of the ventriculoperitoneal shunt (VP shunt) for hydrocephalus, and delayed shunt insufficiency (DSI) is among the most common. It is often caused by abdominal adhesions, which increases the difficulty of diagnosis. This study aimed to explore the clinical value of dynamic plain abdominal radiography (DPAR) as a simple diagnostic method for patients with DSI due to terminal adhesion of the peritoneal shunt after VP surgery. Methods: A total of 30 patients with high suspicion of DSI due to abdominal adhesions after VP surgery were included. DPAR was used for prospective assessment. The interval between the first and second PAR was 4–6 h before surgery. If two plain abdominal radiography at different times indicated that the end of the shunt tube in the abdominal segment was fixed, it was diagnosed as DSI due to adhesion of the shunt tube at the abdominal end. The peritoneal end of the shunt tube was surgically repositioned. Postoperative DPAR was repeated to evaluate the distance of the shunt outlet within the abdominal segment. Results: All cases showed clinical symptoms or imaging findings of shunt insufficiency. The diagnostic accuracy of DPAR was 96.67% (29/30). The end of the shunt tube in the abdominal segment of the preoperative group was fixed with abdominal plain film twice with a mean difference of 1.74 ± 1.18 cm. The mean postoperative change in the position of the end of the shunt tube in the abdominal section was 9.36 ± 2.64 cm, showing a significant difference compared with the preoperative group (P < 0.001). The mean postoperative EVANs index (0.37 ± 0.08) was significantly lower than the preoperative (0.42 ± 0.08) (P = 0.007), Glasgow coma scale score (12.8 ± 2.69) was higher than the mean preoperative score (11.36 ± 2.43) (P = 0.013). Many physicians, particularly neurosurgeons, face malpractice claims during their career. The aim of this paper is to identify which factors increase the risk of litigation and influence legal outcomes relating to cerebrospinal fluid shunt management. Westlaw, an online legal database, was used to identify all medico-legal cases pertaining to cerebrospinal fluid shunts. Information regarding plaintiff demographics, defendant specialties, reasons for litigation, and trial outcomes were analysed. Thirty-six cases met criteria for inclusion. Most cases returned a defence verdict (44.4%), with delayed treatment or failure to appropriately treat patients with shunts the most common reason for litigation (66.7%). The average plaintiff verdict payout was $4,888,136.75 and average settlement $1,362,307.69. Delay or failure to treat resulted in the largest average payouts for plaintiff verdicts and settlements ($5,329,299.14 and $1,531,500.00, respectively). [ABSTRACT FROM AUTHOR]

Purpose: To develop and validate a noninvasive imaging technique for accurately assessing very slow CSF flow within shunt tubes in pediatric patients with hydrocephalus, aiming to identify obstructions that might impede CSF drainage. Theory and Methods: A simulation of shunt flow enhancement of signal intensity (shunt‐FENSI) signal is used to establish the relationship between signal change and flow rate. The quantification of flow enhancement of signal intensity data involves normalization, curve fitting, and calibration to match simulated data. Additionally, a phase sweep method is introduced to accommodate the impact of magnetic field inhomogeneity on the flow measurement. The method is tested in flow phantoms, healthy adults, intensive care unit patients with external ventricular drains (EVD), and shunt patients. EVDs enable shunt‐flow measurements to be acquired with a ground truth measure of CSF drainage. Results: The flow‐rate‐to‐signal simulation establishes signal–flow relationships and takes into account the T1 of draining fluid. The phase sweep method accurately accounts for phase accumulation due to frequency offsets at the shunt. Results in phantom and healthy human participants reveal reliable quantification of flow rates using controlled flows and agreement with the flow simulation. EVD patients display reliable measures of flow rates. Shunt patient results demonstrate feasibility of the method and consistent flow rates for functional shunts. Conclusion: The results demonstrate the technique's applicability, accuracy, and potential for diagnosing and noninvasively monitoring hydrocephalus. Background: Ventriculo-peritoneal shunt (VP shunt) is one of the commonest neurosurgical procedures. Post-operative shunt infection is associated with significant healthcare burden. Most of the shunt infections occur early within 30 days following surgery. Use of prophylactic intraventricular and topical vancomycin along with standard intravenous antibiotic prophylaxis have been reported to lower shunt infection rates in adult and children. Objective: To assess the efficacy of per-operative intraventricular and topical vancomycin in reducing early ventriculo-peritoneal shunt infection in children. Methods: This quasi-experimental study was carried out in the Department of Neurosurgery, Bangabandhu Sheikh Mujib Medical University from September 2022 to February 2024. Total 30 participants (n=30) were enrolled in this study who fulfilled the selection criteria. After obtaining informed written consent from participants/legal guardians, participant's data were recorded in a predetermined datasheet. Participants were divided into two groups- Group A- who underwent VP shunt surgery with per-operative intraventricular and topical vancomycin and Group B- who underwent VP shunt surgery without per-operative intraventricular and topical vancomycin. Results: On postoperative follow up, 04 (13.33%) participants presented with fever. Clinical symptoms and signs suspicious shunt infection were present among only (6.7%) participants (1 in group A and 1 in group B) out of 30 participants. Postoperative CSF analysis was done in these suspected participants only. No organism could be isolated from CSF culture in both groups. Out of 04 suspected participants, 02 (6.7%) participants (01 participants from each group) were confirmed of having shunt infection with no statistically significant difference between the two groups (p=1.00). Although ventriculoperitoneal shunt tip migration into an inguinal hernia is considered common in children, while in adults it is quite rare. In fact, only a handful of cases have been reported in the literature. We report a rare case whereby two catheter tips (one from a lateral ventricle shunt and a second from a 4th ventricular shunt) migrated into an inguinal hernia. This migration caused distal shunt malfunction, and only distal catheter shortening and suturing to the abdominal wall caused the two shunts to function again. [ABSTRACT FROM AUTHOR]

The standard surgical treatment for hydrocephalus is ventriculo-perioneal shunting with shunt failure being the most common complication. A rare and serious consequence is intestinal perforation. A malnourished 9-month-old boy presented with anal protrusion of the distal migrated ventriculoperitoneal (VP) shunt after 2 months of shunt placement. An abdominal X-ray and a brain CT scan were performed. Following the correction of malnourishment and electrolyte imbalance, the patient underwent surgical management, with uneventful discharge on the third day of the procedure. To avoid potentially fatal complications such as sepsis and meningitis, prompt diagnosis and treatment are crucial in these cases.

Background. Endoscopic septostomy is a secure and reliable endoscopic for treating multiloculated hydrocephalus. The ventriculoperitoneal (VP) shunt procedure is known as the gold standard. However, shunt malfunction often occurs. The combination of these has been reported to reduce the incidence of recurrent malfunction and reestablish communication between isolated intraventricular compartments making a special ventricular cavity. Case presentation. A one-year-old child who had previously communicated hydrocephalus complained of vomiting, appeared agitated, and had difficulty falling asleep. The patient had a history of congenital diseases. The patient had VP shunt Keen dextra and sinistra fitted to manage the hydrocephalus before, but one side of the VP shunt was found to be malfunctioning. Physical examination revealed a sunset eye phenomenon with a head circumference of 48 cm dry and clean ventricular parietal shunt surgery scars. The findings of the multiloculated cystic lesions on the endoscopic, agenesis of the septum pellucidum, and dysgenesis of the corpus callosum. Discussion. The patient was scheduled for endoscopic cyst fenestrations surgery, endoscopic third ventriculostomy (ETV), VP shunt Kocher dextra low pressure, and aff VP shunt Keen dextra that was evaluated with preoperative and postoperative CT scans which showed the sulcus and gyrus prominent had improved, slightly open in fissure Sylvie, reduced size in ventricular (the average decrease was 5 cm3) that decreased intracranial pressure which improved the patient’s quality of life. Background: The ventriculoperitoneal (VP) shunt redirects cerebrospinal fluid flow, with the selection of entry points crucial for optimal outcomes. Anatomical landmarks and specific entry points, such as Kocher's, Frazier's, Keen's, and Dandy's points, have been utilized for shunt catheter placement. This study investigates the impact of various entry points on outcomes, particularly the necessity for revision procedures, in patients undergoing VP shunt placement. Methods: In this retrospective cohort study, we analyzed data from patients in our center's database, collected from October 2017 to October 2022. Participants were classified based on ventriculoperitoneal shunt entry points. The study followed STROBE guidelines. Continuous variables were presented as means with standard deviations (SD) and categorical variables as frequencies and percentages. Linear Model ANOVA and Pearson's Chi-squared tests were used for comparisons. Data analysis was conducted using Jamovi software. Results: Our study included 94 patients who underwent shunt procedures. The patients were categorized into four treatment groups: Dandy point (10), Frazier point (21), Keen point (43), and Kocher point (20). Conclusion: Our study found no significant differences in age, FOHR, and indication for shunt placement among catheter entry point subgroups. However, gender distribution, catheter length, and catheter tip location significantly varied. The proportion of patients requiring revision surgery varied among the groups, with the highest rate in the Dandy point group and the lowest in the Keen group; however, the difference among the entry groups was insignificant.

Key Clinical Message: Cerebrofacial arteriovenous Metameric syndrome (CAMS) typically manifests as types I, II, or III, occasionally presenting as dual types. Our unique case underscores the coexistence of all three CAMS types in one patient. Neurocysticercosis (NCC) is a common parasitic disease of the central nervous system (CNS) in low- and middle-income countries. The infection is pleomorphic, caused by the larval form of the cestode, Taenia solium, and part of the heterogeneity of its clinical presentations is associated with the localization of the parasite within the CNS. Changes in the current epidemiological trends of NCC indicate that extra-parenchymal NCC is proportionally becoming more frequent. Extraparenchymal NCC is commonly accompanied by raised intracranial hypertension due to hydrocephalus, which is an emergency requiring cyst extirpation by surgical intervention to relieve the symptoms. Although less frequent, parenchymal cysts may also reach giant sizes requiring urgent surgical treatment. Finally, there is an advancement in the comprehension of the association between NCC and epilepsy—and patients with drug-resistant seizures are candidates for surgical treatment. Purpose: Neural tube defects (NTDs) are one of the most common congenital anomalies and a cause of chronic disability. The study was done to study outcomes of neural tube defects admitted at a tertiary level neonatal intensive care unit (NICU) from 2018 to 2022, a period of 4 years that also coincided with the COVID pandemic. The secondary outcome was to study the clinical presentation, associated anomalies and epidemiological features. Methods: It was a retrospective observational study; data of infants was obtained from medical records and analysis was done. Results: Thirty-four neonates were enrolled, of which there were 16 (47%) males and 18 (53%) females. History of pre-pregnancy maternal folate intake was present in 4 (11.7%) cases. 33 (97%) babies were diagnosed with meningomyelocele (MMC) and one each had anencephaly, iniencephaly and encephalocele, of which one had frontal and two had occipital encephalocele. The median age of surgery was 16 days of life with primary repair being the most common procedure followed by MMC repair with VP shunt. Twenty babies (58.8%) were discharged successfully, while 9 (26.5%) expired and 5 (14.7%) were discharged against medical advice; which can be attributed to the financial problems of the patients in a developing country. The overall deaths in our series were four (26.5%) which is slightly higher than other studies which may be due to the fact that this study was conducted during the COVID era with lesser rates of folate supplementation, reduced access to prenatal diagnosis coupled with poor follow-up and compliance of patients post-surgical repair. Conclusion: This study emphasizes the importance of periconceptional folic acid supplementation, prenatal diagnosis, early surgery and meticulous follow-up as being pivotal to improving outcomes in children with NTDs. Although neurosurgical treatment has evolved over time with ventriculoperitoneal shunt (VPS) as the standard of care, procedure-related complications and poor long-term cognitive and motor milestone outcomes are still considerable, justifying the need for safer alternatives. Endoscopic third ventriculostomy (ETV) has emerged as a promising prospect especially in non-communicating hydrocephalus (NCH). Aims and Objectives: To compare the beneficial effects of ETV versus ventriculoperitoneal shunt in patients of NCH. Materials and Methods: The present prospective study was conducted at the Department of Neurosurgery, Bangur Institute of Neurosciences, IPGMER and SSKM Hospital, Kolkata from July 2021 to December 2022 among 60 patients admitted with NCH. Out of 60 patients; 30 patients underwent ETV and the remaining 30 patients VP shunting. All included patients had their history taken and relevant clinical and radiological examination done pre-operatively. They were discharged on the third post-operative day or later depending on their clinical condition and recovery. Results: Aqueductal stenosis was the most common etiology for NCH in both the groups. Post-intervention, complications and reoperation rate were reported more in VP group as compared to ETV group. Overall 3 subjects died, out of which 2 belonged to VP group and 1 to ETV group. Success was found in 70% and 86.67% of the subjects in VP and ETV group respectively. Conclusion: In patients with non-communicating or obstructive hydrocephalus, ETV was reported to be superior to VPS in terms of reoperation and complication rate at 4th, 12th, and 24th weeks after the treatment.

Fossa posterior tumor removal procedure may have spesific problem including obstruction of cerebral spinal fluid pathway, development of hydrocephalus and an increased intracranial pressure. A 25 year old woman, 52 kgs, admitted to the hospital with major complaint of dizziness occurred approximatelly 5 hours prior to hospital admission. The patient also suffered from nausea, vomiting, buzzing hearings, and heartburn sensation. The non-contrast MSCT revealed a non-communicating hydrocephalus, tumor of the right cerebellar hemisphere, suspected as astrocytoma with differential diagnosis of medullablastoma. From axial, coronal and sagittal view of MSCT with contrast, a tumor mass was found in the posterior fossa, and most likely to be considered as a polycystic cerebellar astrocytoma with mild hydrocephalus. She was diagnosed with obstructive type of hydrocephalus and cerebellar tumor with increased intracranial pressure signs due to hydrocephalus, and planned for VP shunt prior to the tumor removal. To maintain brain perfusion as well as to ensure brain relaxation, anesthesia management was done with several methods such as hyperventilation, administration of mannitol 20%, while maintaining adequacy of MAP. The VP shunting was conducted in supine position, whilst the posterior fossa excision in conducted in prone position. Both procedures were done in 4.5 hours. Postoperative extubation was postponed until the following day at the intensive care unit. The patient was sedated and ventilated for 14 hours. No sign of venous air embolisms. Post extubation on the next day, the patient’s breathing and hemodynamic statuses were both stabile and adequate, with no significant neurological defect.

She is a known case of scar epilepsy and underwent VP shunting due to hydrocephalus post‐meningoencephalitis 16 years ago. The patient subsequently underwent symptomatic treatment and was later diagnosed with OM during her management. Histopathology of the biopsy following wound debridement revealed invasive fungal growth causing necrosis and erosion of the buccal mucosa and palate with no evidence of malignancy. The presentation of OM is an infrequent and exclusively rare entity. Our study aims to present the possible circumstances under which people suffer from this deteriorating condition in juxtaposition to scar epilepsy. This case report highlights the significance of prompt medicinal intervention and debridement coupled with preventative measures undertaken for a better prognosis and longevity. Objective: To evaluate the safety and outcomes of cochlear implantation (CI) in patients with ventriculoperitoneal (VP) shunts to inform clinical practice. Study Design: Historical cohort study. Setting: Tertiary referral centers. Methods: A multi‐institutional historical cohort of patients with VP shunts and CI was identified and analyzed. Results: A total of 46 patients (median age 8 years [interquratile range, IQR: 2‐46]) with VP shunts and CI were identified. Of these, 41 (89%) patients had a VP shunt prior to CI. Based on institutional preference and individual patient factors, CI was performed contralateral to a pre‐existing VP shunt in 24 of these 41 cases (59%) and ipsilateral in 17 (41%). Furthermore, pre‐CI relocation of the VP shunt was performed in 3 cases (7%), and 2 patients (5%) underwent planned revision of their VP shunt concurrent with CI. In total, 2 of 27 pediatric patients (7%) required unanticipated revision shunt surgery, both contralateral to CI device placement, given VP shunt malfunction. One of 19 adult patients (5%) required shunt revision during CI due to shunt damage noted intraoperatively. Among 43 patients with available follow‐up, 38 (88%) are regular CI users, with a median consonant‐nucleus vowel‐consonant word: score of 58% (IQR: 28‐72). Conclusion: CI can be performed at low risk, either contralateral or ipsilateral, to a VP shunt, and does not mandate shunt revision in most cases. Additional considerations regarding CI receiver‐stimulator placement are necessary with programmable shunts to mitigate device interaction. Treatment for pediatric hydrocephalus aims not only to shrink the enlarged ventricle morphologically but also to create an intracranial environment that provides the best neurocognitive development and to deal with various treatment-related problems over a long period of time. Although the primary diseases that cause hydrocephalus are diverse, the ventricular peritoneal shunt has been introduced as the standard treatment for several decades. Nevertheless, complications such as shunt infection and shunt malfunction are unavoidable; the prognosis of neurological function is severely affected by such factors, especially in newborns and infants. In recent years, treatment concepts have been attempted to avoid shunting, mainly in the context of pediatric cases. In this review, the current role of neuroendoscopic third ventriculostomy for noncommunicating hydrocephalus is discussed and a new therapeutic concept for post intraventricular hemorrhagic hydrocephalus in preterm infants is documented. To avoid shunt placement and achieve good neurodevelopmental outcomes for pediatric hydrocephalus, treatment modalities must be developed.

Histopathology of the biopsy following wound debridement revealed invasive fungal growth causing necrosis and erosion of the buccal mucosa and palate with no evidence of malignancy. The presentation of OM is an infrequent and exclusively rare entity. Our study aims to present the possible circumstances under which people suffer from this deteriorating condition in juxtaposition to scar epilepsy. This case report highlights the significance of prompt medicinal intervention and debridement coupled with preventative measures undertaken for a better prognosis and longevity.

Introduction: Tuberculous meningitis (TBM) is becoming a rare illness in industrialized nations, but it is still widespread in underdeveloped nations. Ventriculoperitoneal shunt surgery is the GOLD Standard and is time tested procedure. Shunts, however, are associated with multiple problems. Material and Method: This is an observational study that compares the outcome of Endoscopic Third Ventriculostomy (ETV) with Ventriculoperitoneal shunt in patients of TBM with hydrocephalus at a 6-month follow-up. This study was done at a single tertiary care center. Data was collected from the patient records as per selection criteria. 100 consecutive cases in each ETV and Shunt group fulfilling selection criteria were included in the study. The two procedures were compared for their clinical outcome based on modified Vellore grade at 6 months of follow-up. Discussion: Evaluation of outcome at 6 months revealed an overall success rate of 68.5%. The success rate was higher for shunt (71%) as compared to ETV (66 %). However, the difference between the two groups was not significant statistically. For children with advanced TBM and elevated ICP, shunt surgery may not be necessary. While some authors have considered the potential of delaying surgery for these kids, others have argued that even a single positive outcome warrants shunt placement for every kid with tuberculous hydrocephalus. Conclusion: In pediatric cases of TBM with Hydrocephalus ETV should be considered as a first-line treatment in lower grades of TBM. Though for definitive comparison, a long-term randomized study is needed, and the study to look into factors responsible for the failure of ETV in TBM cases needs to be identified to formulate an ETV success score. Latar belakang. Anak hidrosefalus dengan VP shunt dapat mengalami penurunan kualitas hidup (QoL) akibat berbagai faktor yang hingga kini belum banyak diteliti. Tujuan. Menilai luaran QoL anak hidrosefalus dengan VP shunt dan faktor yang memengaruhinya. Metode. Anak berusia 5-18 tahun dengan hidrosefalus yang dipasang VP shunt tahun 2010-2015 diidentifikasi melalui rekam medis. Orangtua kemudian diwawancarai dengan panduan kuesioner HOQ (hydrocephalus outcome questionnaire). Hasil meliputi skor QoL pada aspek fisik, kognitif, dan keseluruhan. Skor 0 menunjukkan luaran terburuk dan skor 1 terbaik. Dilakukan analisis perbedaan skor berdasarkan faktor risiko (usia saat pemasangan, etiologi, adanya kejang dan komplikasi shunt) dan analisis kategorik berdasarkan pencapaian skor standar penelitian terdahulu (0,69). Hasil. Terdapat 95 anak yang diidentifikasi, 18 meninggal dan 46 dengan data tidak lengkap sehingga terdapat 31 anak yang dianalisis. Skor HOQ yang didapatkan, yaitu fisik 0,75±0,27, sosial-emosional 0,78±0,22, kognitif 0,66±0,31, dan keseluruhan 0,73±0,25. Skor HOQ keseluruhan lebih rendah pada anak dengan komplikasi. Kejang (RR 2,52) dan komplikasi shunt (RR 4,85) berhubungan dengan luaran buruk. Analisis multivariat menunjukkan hanya komplikasi yang berhubungan dengan QoL buruk (adjusted OR 15,11). Kesimpulan. Luaran QoL ditemukan lebih baik dibandingkan penelitian sebelumnya. Kejang dan komplikasi shunt dapat berpengaruh negatif terhadap luaran QoL anak hidrosefalus dengan VP shunt.

Introduction: The ventriculoperitoneal shunt is one of the most commonly performed neurosurgical procedures. One of the avoidable complications of shunt surgery is shunt infection. This PRISMA-compliant meta-analysis analysed the effectiveness of topical and/or intraventricular antibiotics in preventing shunt infections in patients undergoing shunt surgery. Methods: Four databases were searched from inception to 30th June 2021. Only original articles comparing the rate of shunt infection with and without antibiotics were included. Random-effects meta-analysis was used to compare the effect of the use of antibiotics in preventing infection and subgroup analysis for finding differences in various antibiotics. Results: The rate of shunt infection was 2.24% (53 out of 2362) in the topical antibiotic group in comparison to 5.24% (145 out of 2764) in the control group (p = 0.008). Subgroup analysis revealed that there is no significant difference between the antibiotics used. Conclusions: Our meta-analysis found that the risk of shunt infection is significantly reduced with the use of topical and intraventricular antibiotics without any serious adverse effect. No side effects of topical or intraventricular antibiotics were reported in the included studies. Further prospective studies are required to establish the safety and optimal dosage of topical antibiotics for them to be used routinely in neurosurgical practice. Introduction: Posterior Fossa arachnoid cysts (PFAC) are rare lesions in the childhood. Owing to the wide variety of cystic malformations within the posterior cranial fossa, PFAC have long attracted the attention of pediatric neurosurgeons because they can produce not only neurological symptons but also behaviro and learning deficts, in addition to presenting major surgical challenges. Surgical treatment is warranted in symptomatic patients. Nowadays, neuroendoscopic techniques have established themselves as the treatment of choice of these lesions. Patients and Methods: We present four cases of symptomatic PFAC which were treated using endoscopic neurosurgery. Endoscopic approaches were defined by the presence of hydrocephalus, and discussed in each case. Results : Two cases of quadrigeminal plate cysts, predominantly occupying the infratentorial compartment , one case of supracerebellar cyst and one case of cerebellopontine angle were treated with good results. However, one case of quadrigeminal plate cyst developed a communicating hydrocephalus, requiring placement of cyst-ventricle peritoneal shunt, guided by the neuroendoscope. Discussion and Conclusion : A literature review was performed. The clinical features, the advantages and disadvantages of several surgical treatments are discussed. Symptomatic PFAC should be treated. The relatively good results using endoscopic fenestration buttresses our use as a first-line surgery.

Background: Shunt revisions constitute a major neurosurgical hurdle. The subject has been thoroughly investigated, and recently the term preventable shunt revision rate has been introduced focusing on early malfunctions. We aim to investigate the differences between early and late malfunctions and identify further preventable causes of revision. Methods: The study was based on two databases of shunt surgery from Copenhagen University Hospital and Aarhus University Hospital. We extracted details on shunt surgery, including cause of revision and time to revision. In this study, we presented a Kaplan-Meier curve and made a subdivision in early and late revisions. Causes for early and late revisions were subsequently compared. Results: A total of 622 patients with mean follow-up time of 1.58 years were included. During follow-up 243 (39.1%) patients had at least one shunt revision. The Kaplan-Meier shunt survival curve could be mathematically described as biphasic with an early and a late curve section. The intersection between the two sections was at 0.2 years. Shunt survival was 77.7% at 0.2 years and 59.5% at two years. The causes for early and late revisions differed significantly in terms of infections (early revision), valve defects, and disconnections (late revisions). Conclusions: We found that early shunt revisions are predominant, and that causes for early shunt revisions in some terms differ from late shunt revisions. In early revisions infections are overrepresented, while valve defects and disconnections are overrepresented in late revisions. Our findings suggest that reduction of early and late shunt revisions could require different strategies.

Ventriculo-peritoneal (VP) shunt is the typical and most common procedure for the treatment of hydrocephalus. Many complications have been associated with VP shunts, migration of the distal end of the VP tube into the scrotum is a rare one. We report the presentation and management of a case of 3 month age infant who had scrotal swelling primarily diagnosed as hydrocele. Investigations proved the presence of shunt migration. The possibility of shunt migration should be considered. Early diagnosis and management of such complications is easy and can prevent subsequent serious sequelae.

Abstract Background Ventriculoperitoneal (VP) shunts are commonly performed procedures for a variety of disorders and are mostly long standing. These patients when present for non-neurological surgeries like gastrointestinal, urology, or caesarean section, there are several concerns like difficult abdominal surgery due to peritoneal adhesions, chances of shunt infection with potential retrograde infective meningoencephalitis, and ventriculitis or shunt failure with recurrent hydrocephalus. Case presentation A 35-year-old male, known case of third ventricular tumour with functional left-sided ventricular peritoneal shunt was scheduled to undergo cholecystectomy for gall bladder stone. Intraoperatively optic nerve sheath diameter was measured as an indicator of raised intracranial pressure. Intraoperative was uneventful. Efforts were taken to prevent rise in intracranial pressure perioperatively. Patient was discharged on third postoperative day. Conclusion A vigilant perioperative care along with adequate team work go a long way in achieving success in patients of ventriculoperitoneal shunts presenting for non-neurological surgeries.

Introduction To date, there are no standard practice guidelines available and no universal consensus regarding treatment protocol in management of tuberculous meningitis (TBM) with hydrocephalus. Over the years, diverse views have existed in neurosurgical management of TBM with hydrocephalus. Some authors advocate ventriculo-peritoneal (VP) shunt, while others suggest that external ventricular drainage (EVD) may be the preferable neurosurgical procedure for a poor-grade patient. Method We systematically reviewed published literature and presented our institution's experience. We performed a retrospective case study in our Sarawak neurosurgical center from 2018 to 2020. We tabulated the outcome according to preoperative classifications, which were Vellore Grading (VG), Modified Vellore Grading (MVG), British Medical Research Council Classification (MRC), and others: author-defined. Result In our center, there were 20 cases of TBM with hydrocephalus treated by EVD and VP shunt from 2018 to 2020. We systematically searched published medical literature, and 23 articles were retrieved and analyzed. Poor outcomes were observed in poor-grade patients, especially VG/MVG 3/4 and MRC 3, from both institution and systemic review data. Shunt complication rate was lower in our center as compared with published literature. Conclusion Unfortunately, morbidity and mortality were approximately twofold higher in poor-grade as compared with good-grade patients. However, about one-third of poor-grade patients achieved a good outcome. Cerebrospinal fluid (CSF) diversion would be an unavoidable treatment for hydrocephalus. Poor-grade patients tend to have cerebral infarcts in addition to hydrocephalus. An extended duration of EVD placement could be a potential measure to assess Glasgow coma scale recovery and monitor serial CSF samples. [ABSTRACT FROM AUTHOR]

Background: Abdominal access during ventriculoperitoneal (VP) shunt insertion has historically been obtained by neurosurgeons via an open abdominal approach. With recent advances in laparoscopy, neurosurgeons frequently consult general surgery for aid during the procedure. The goal of this study is to identify if laparoscopic assistance improves the overall outcomes of the procedure. Methods: This retrospective study included all patients who underwent open or laparoscopic VP shunt placement between September 2012 and August 2020 at our tertiary referral hospital. Patient demographics, comorbidities, prior history of abdominal surgery, open vs. laparoscopic insertion, operation time, and complications within 30 days were obtained. Results: Neurosurgery placed 107 shunts using an open abdominal technique and general surgery placed 78 using laparoscopy. The average OR time in minutes was 75.5 minutes for the open cohort and 61.8 for the laparoscopic cohort (p = 0.006). In patients without a history of abdominal surgery, the average OR time in minutes was 79.4 in the open cohort and 57.1 in the laparoscopic cohort (p = 0.015). The postoperative shunt infection rate was 10.2% in the open group and 3.8% in the laparoscopic group (p = 0.077). Discussion: Laparoscopic placement of VP shunts is a reasonable alternative to open placement and results in shorter OR times. There is also a trend toward few infections in the laparoscopic placement. There appears to be an advantage with a team approach and laparoscopic placement of the peritoneal portion of the shunt. Long-term risks and survival times of ventriculoperitoneal (VP) shunts implanted due to hydrocephalus (HC) after craniotomy for brain tumors are largely unknown. The aim of this study was to establish the overall VP shunt survival rates during a decade after shunt insertion and to determine risks of shunt failure after brain tumor surgery in the long-term period. In this population-based cohort from a well-defined geographical region, all adult patients (> 18 years) from 2004 to 2013 who underwent craniotomies for intracranial tumors leading to VP shunt dependency were included. Our brain tumor database was cross-linked to procedure codes for shunt surgery (codes AAF) to extract brain tumor patients who became VP shunt dependent after craniotomy. The VP shunt survival time, i.e. the shunt longevity, was calculated from the day of shunt insertion after brain tumor surgery until the day of its failure. A total of 4174 patients underwent craniotomies, of whom 85 became VP shunt dependent (2%) afterwards. Twenty-eight patients (33%) had one or more shunt failures during their long-term follow-up, yielding 1-, 5-, and 10-year shunt success rates of 77%, 71%, and 67%, respectively. Patient age, sex, tumor location, primary/repeat craniotomy, placement of external ventricular drainage (EVD), ventricular entry, post-craniotomy hemorrhage, post-shunting meningitis/infection, and multiple shunt revisions were not statistically significant risk factors for shunt failure. Median shunt longevity was 457.5 days and 21.5 days for those with and without pre-craniotomy HC, respectively (p < 0.01). This study can serve as benchmark for future studies. ObjectiveThere is no general consensus on the placement of preoperative and intraoperative external ventricular drainage (EVD) in patients with lateral ventricular tumors (LVTs). The aim of this study was to identify the predictors of postoperative acute and persistent hydrocephalus need for postoperative cerebrospinal fluid (CSF) drainage and guide the management of postoperative EVD in patients with LVTs.MethodsWe performed a single-institution, retrospective analysis of patients who underwent resection of LVTs in our Department between January 2011 and March 2021. Patients were divided between one group that required CSF drainage and another group without the need for CSF drainage. We analyzed the two groups by univariate and multivariate analyses to identify the predictors of the requirement for postoperative CSF drainage due to symptomatic intracranial hypertension caused by hydrocephalus.ResultsA total of 97 patients met the inclusion criteria, of which 31 patients received preoperative or intraoperative EVD. Ten patients without prophylactic EVD received postoperative EVD for postoperative acute hydrocephalus. Eleven patients received postoperative ventriculoperitoneal(VP) shunt subsequently. Logistic regression analysis showed that tumor invasion of the anterior ventricle (OR = 7.66), transependymal edema (OR = 8.76), and a large volume of postoperative intraventricular hemorrhage (IVH) (OR = 6.51) were independent risk factors for postoperative acute hydrocephalus. Perilesional edema (OR = 33.95) was an independent risk factor for postoperative VP shunt due to persistent hydrocephalus.ConclusionPostoperative hydrocephalus is a common complication in patients with LVTs. These findings might help to determine whether to conduct earlier interventions.

Risks and survival times of ventriculoperitoneal (VP) shunts implanted due to hydrocephalus after craniotomies for brain tumors are largely unknown. The purpose of this study was to determine the overall timing of VP shunting and its failure after craniotomy for brain tumors in adults. The authors also wished to explore risk factors for early VP shunt failure (within 90 days). A population-based consecutive patient cohort of all craniotomies for intracranial tumors leading to VP shunt dependency in adults (> 18 years) from 2004 to 2013 was studied. Patients with pre-existing VP shunts prior to craniotomy were excluded. The survival time of VP shunts, i.e., the shunt longevity, was calculated from the day of shunt insertion post-craniotomy for a brain tumor until the day of shunt revision requiring replacement or removal of the shunt system. Out of 4774 craniotomies, 85 patients became VP shunt-dependent (1.8% of craniotomies). Median time from craniotomy to VP shunting was 1.9 months. Patients with hydrocephalus prior to tumor resection (N = 39) had significantly shorter time to shunt insertion than those without (N = 46) (p < 0.001), but there was no significant difference with respect to early shunt failure. Median time from shunt insertion to shunt failure was 20 days (range 1–35). At 90 days, 17 patients (20%) had confirmed shunt failure. Patient age, sex, tumor location, primary/secondary craniotomy, extra-axial/intra-axial tumor, ventricular entry, post-craniotomy bleeding, and infection did not show statistical significance. Purpose  In this study, we analyzed the utility of intracranial pressure (ICP) monitoring intraoperatively for deciding height reduction and need for cerebrospinal fluid (CSF) diversion during cranial vault remodeling in children with multisutural craniosynostosis (CS). Methods  This is a retrospective observational study of children who underwent surgery for CS and ICP monitoring during surgery. The ICP was monitored using an external ventricular drainage catheter. The ICP monitoring was continued during the entire procedure. Results  A total of 28 (19 boys) children with the involvement of two or more sutures underwent ICP monitoring during surgery. The commonest pattern of suture involvement was bicoronal seen in 16 (57.1%) children followed by pancraniosynostoses in eight (28.6%) cases. The mean opening ICP was 23 mm Hg, which dropped to 10.9 mm Hg after craniotomy. The ICP increased transiently to 19.5 mm Hg after height reduction, and the mean ICP at closure was 16.2 mm Hg. The ICP recordings helped in undoing the height reduction in two children and ventriculoperitoneal shunt after surgery in two children. Conclusions  Intraoperative monitoring of ICP helps in deciding the type of cranial vault remodeling and the need for CSF diversion after surgery. [ABSTRACT FROM AUTHOR]

Abstract Background Bowel perforation caused by the ventriculo-peritoneal shunt is a rare occurrence with an estimated incidence rate of 0.1% to 1.0% among all cases of VP shunt displacement. This is an unusual report of spontaneous trans-anal extrusion of caudally migrated ventriculo-peritoneal shunt tip in a child. Literature was reviewed to find out therapeutic strategies. Case presentation An asymptomatic 8 months old boy presented with spontaneous trans-anal extrusion of caudally migrated left-sided Chhabra type of ventriculo-peritoneal (VP) shunt for last 1 day, following surgery for hydrocephalus initially done 3 months ago. He had no features of peritonitis or encephalitis. Digital X-ray of the whole abdomen in postero-anterior view in erect posture was only evident of the expulsion of radio-opaque distal catheter tip through the anus into the exterior. Noncontrast-enhanced computed tomography scan (NCCT) of brain showed proximal catheter in the lateral ventricle of the brain. Under sedation, the distal part of the VP shunt catheter was resected out, aseptically, over the abdomen and pulled out gently through the anus. The proximal catheter part along with the reservoir was removed through a separate incision in the neck and sent for bacteriological study, which came out later to be negative. Postoperatively, the child was put on a prophylactic antibiotic and 3 weeks later another VP shunt was placed in the contralateral side. Conclusions Spontaneous trans-anal extrusion of VP shunt tip is a surgical emergency. The whole catheter must be removed aseptically in such a way that both contamination of the cerebral cavity and spillage into the peritoneum can be avoided. Awareness of this unusual complication among surgeons is needed for early recognition, management, and timely intervention to minimize morbidity.

Hydrocephalus is often treated with CSF diversion via ventriculoperitoneal (VP) shunting. We present the unique case of a 33-year-old female with a history of infiltrating astrocytoma and consequent obstructive hydrocephalus necessitating shunt placement. She later presented with non-specific symptoms prompting shunt evaluation. Ultimately, while a cause for her symptoms was not identified, imaging revealed distal catheter migration into the pleural space. The patient remained asymptomatic during two years of follow-up without surgical intervention. Background: Endoscopic Third Ventriculostomy (ETV) is increasingly being accepted as the treatment of choice in place of Ventriculo-Peritoneal (VP) Shunt for hydrocephalus. However, their differences in cognitive and Quality of Life (QOL) scores have not been studied much in children.Objective: To compare the outcome, cognitive function, and QOL between ETV and VP shunt.Methods: Patients of non-tumor hydrocephalus treated with ETV or/and VP shunt underwent cognitive assessment (using modified child MMSE standardized as per the age group) and QOL (using PedsQL as per the age group in Physical, Emotional, Social, and School Functioning domains) in addition to the outcome of not requiring additional intervention.Results: Out of 139 patients, there were 29 infants and 40 children upto 14 years. Among these children, ETV was the primary intervention in 45, VP shunt in 24, and could be studied for a mean follow-up of 1.7 years. Though ETV required lesser additional intervention than VP shunt (19.2% vs. 28.6%) in toddlers and older children, there was no overall significant difference. Subnormal cognitive scores were noted in 25%, 40%, and 50% after ETV, single shunt procedure, and multiple shunt procedures, respectively, with no statistically significant difference. Among the different domains of QOL, the child reported scores in the social domain were significantly better after ETV than VP shunt (475[+13] vs. 387[+43], P value 0.03), whereas most other scores were non-significantly better following ETV.Conclusion: Patients who underwent ETV show a trend for better clinical outcome, cognitive function, and QOL with significantly better child-reported QOL scores in the social domain. Background: Post-infective hydrocephalus (PIH) arises as a complication of any CNS infection, and can be either communicating or noncommunicating.Objective: The aim of this article is to study the various causes of PIH and its pathophysiology and treatment.Material and Methods: The literature was searched for articles describing the causes of PIH.Results: Common causes of PIH are CNS tuberculosis (TB), neurocysticercosis, and perinatal or neonatal infection. TBM is most likely to result in hydrocephalus out of all these manifestations of CNS TB, and hydrocephalus is more likely to occur early in the course, typically 4-6 weeks after the onset of TBM, and is more common among children as compared to adults. A trial of medical management (antitubercular therapy, steroids, and decongestants) can be given to patients with communicating hydrocephalus. Ventriculoperitoneal shunt is the most employed method of CSF diversion in these patients. Though traditionally considered contraindicated, many recent studies have found ETV to be a reasonable option in patients with PIH. HCP in patients with neurocysticercosis can be associated with intraventricular cysts and racemose cysts in the basal subarachnoid cisterns. Surgical intervention is required either for cyst removal or CSF diversion. Endoscopic approaches can be used to remove the intraventricular cysts, which takes care of the HCP. PIH in infants can result either from antenatal infections (TORCH infections) or postnatal infections such as meningitis.Conclusions: Management of PIH can be challenging. Background: The use of ventriculoperitoneal (VP) shunts has become ubiquitous in neurosurgery for the treatment of hydrocephalus. VP shunts work by creating a conduit for cerebrospinal fluid (CSF) to flow from the cerebral ventricles to the peritoneum and thus relieving pressure. Although typically safe, VP shunt complications are extremely common, occurring in up to 29% of adult cases and approximately half of pediatric cases. These complications may require patients to undergo several revisions throughout their lifetimePurpose: We describe a man who developed scalp necrosis overlying his VP shunt. We also summarize the potential complications of VP shunts. We discuss the presentation and pathogenesis of scalp necrosis in these patients.Materials and methods: A PubMed search of the following terms was performed and relevant citations were assessed: ventriculoperitoneal shunt, VP shunt, shunt complications, scalp necrosis, skin necrosis, ventriculoperitoneal shunt induced scalp necrosis, ventriculoperitoneal shunt induced skin necrosis.Results: A 73-year-old man developed VP shunt-induced scalp necrosis leading to loss of skin overlying the shunt valve. The patient was emergently referred to neurosurgery and admitted for revision of the shunt due to the significant risk for infection. The VP shunt was replaced with an external drain and a skin flap was used to repair the defect.Conclusion: VP shunts are devices that are frequently used for reducing intracranial pressure associated with hydrocephalus. The insertion of the shunt beneath the scalp sets the stage for potential pressure-induced scalp necrosis. The early recognition and diagnosis of VP shunt-induced scalp necrosis is essential to prevent infections and future complications.

Introduction: Ventriculoperitoneal shunt is an established and efficient treatment for hydrocephalus. Peritoneal abscess is a, rare, serious complication which should be managed aggressively. There is no general consensus for managing peritoneal abscess after ventriculo-peritoneal (VP) shunt. Here we present our stepwise protocol of management for a series of five cases. Case presentations: We present a series of five cases diagnosed with peritoneal abscess after VP shunting. Patients were fully investigated on admission. Abscess was confirmed by pelvic-abdominal U/S and diagnostic aspiration. Conservative management was started with antibiotics according to culture and sensitivity initially in all cases. Shunt externalization and apo-cath insertion was done for non-responsive cases. Further non-responsive cases were managed by shunt removal and external ventricular drain (EVD) until subsidence of infection. The study included 3 males and 2 female patients. One case responded to conservative measures, while the other four needed more invasive procedures: apo-cath, shunt removal and EVD with intra-ventricular antibiotics. Conclusion: Abdominal infection following VPS insertion is a serious complication associated with high morbidity and mortality. Patients should be managed in collaboration with general surgery department staff. We propose a management protocol that includes a systematic stepwise approach according to the general condition and neurological status of each patient. The protocol starts with conservative treatment and progresses to more invasive and aggressive methods according to the response of the patient.
