Abstract

Neurogenic pulmonary edema is a life threatening clinical syndrome characterized by the acute onset of pulmonary edema following a significant central nervous system injury. The most common causes are subarachnoid hemorrhage, head trauma, epileptic seizure, cervical spine trauma, multiple sclerosis, encephalitis, hemorrhagenic and ischemic strokes and also intracranial tumors. The pathophysiologic mechanism is unclear but it probably involves an adrenergic response to the central nervous system injury which leads to increased level of catecholamine, pulmonary hydrostatic pressure and lung capillary permeability. Clinical symptoms are nonspecific and often include dyspnea, tachypnea, tachycardia, hypoxemia, pink-frothy secretion and bilateral pulmonary infiltrates. These symptoms start within minutes or hours and usually resolves 48-72 hours later. Basic principles of treatment include reducing intracranial pressure, controlled ventilation with suplemental oxygen and positive end expiratory pressure, diuresis and surgical decompression.
