早期肠内营养联合腹腔微创引流对重症急性胰腺炎免疫功能和营养状态的影响

Abstract

Objective To investigate the effect of early enteral nutrition support combined with minimally invasive percutaneous drainage of abdominal cavity on the immune function and nutritional status in the patients with severe acute pancreatitis (SAP). Methods 40 cases of SAP in our hospital from August 2012 to February 2015 were selected as the research subjects and randomly divided into the observation group and the control group according to the odd and even number of admission, 20 cases in each group. The observation group was given early enteral nutrition support combined with minimally invasive percutaneous drainage of abdominal cavity based on the traditional comprehensive treatment, while the control group was given the traditional comprehensive treatment. The immune function (CD4/CD8 ratio, IgG level) before and after treatment, serum albumin and prealbumin change, WBC, blood amylase level on 1, 7, 14, 21 d after admission, mortality after treatment, hospitalization time and treatment cost were also observed. Results The serum CD4/CD8 ratio and IgG level after treatment in the observation group were significantly improved compared with the before treatment and the control group after treatment,the difference was statistically significant (P<0.05); WBC and blood amylase levels on 7,14, 21 d of treatment in the observation group were decreased compared with the control group, the serum prealbumin level was significantly higher than before treatment, moreover significantly increased compared with the control group after treatment, the difference was statistically significant (P<0.05); the mortality in the observation group was 5.0% (1/20), which was significantly lower than 25.0% (5/20) in the control group, the hospitalization time and treatment costs were significantly less than those in the control group, the difference was statistically significant (P<0.05). Conclusion Early enteral nutrition support combined with intraperitoneal drainage in the treatment of SAP can significantly improve the nutritional status and immune function with low treatment costs, short treatment time, low mortality and definite effect.
