重型颅脑损伤气管切开患者不同吸痰方式效果观察.

Abstract

Objective To observe the clinical effect of different methods of sputum suction in severe craniocerebral injury patients with tracheotomy. Methods A total of 80 severe craniocerebral injury patients with tracheotomy,who were all unconscious without mechanical ventilation,in Department of Neurosurgery in the hospital from May 2016 to May 2017 were selected and divided into the control group and the research group according to tracheotomy sequence,40 cases in each group. The control group took the sputum suction method according to the demand,while the research group took the sputum suction method with percussion on back,and the effect of sputum suction in the two group was compared. Results The proportion of saturation of pulse oximetry rising to 90% or more in 30 min after sputum suction and the sputum interval time in the research group[84.0%(42/50),(3.60±0.66)h ] were more than those in the control group[66.0%(33/50),(1.65±0.53)h],and the degree of artificial airway patency in the research group was superior to that in the control group. The occurrence rate of hypostatic pneumonia and tracheal mucosa damage in the research group [22.5%(9/40),15.0%(6/40)] were remarkably lower than those in the control group [45.0%(18/40),35.0%(14/40)],with statistically significant difference(P<0.05). Conclusion For the severe craniocerebral injury patients with tracheotomy,sputum suction with percussion on back can restore saturation pulse oximetry quickly,decrease the occurrence of hypoxia,prolong sputum interval time,reduce the incidence of hypostatic pneumonia and tracheal mucosa damage.
