Abstract
- The advances in embryo culture and cryopreservation techniques associated with the potential deleterious effects of ovarian stimulation (SO) on endometrial receptivity and the risk of ovarian hyperstimulation syndrome (SHSO) has allowed the emergence of the "freeze all" strategy. It consists in delaying the transfer of embryos during a cycle without SO and therefore not exposed to supra physiological levels of hormones. Theoretically, this strategy would improve the outcome in terms of efficacy and safety for all patients who undergo OS for IVF/ ICSI. However, according to the recent publications, it appears that, in clinical practice, the strategy of freeze all would be beneficial only for some subgroups of patients: women at risk of OHSS, hyper-responders and candidates for preimplantation genetic diagnosis. In these situations, the freeze all strategy is justified and even recommended. In other situations, freeze all does not seem to bring any improvement in clinical or obstetric or neonatal outcomes.
