Abstract

While medication reconciliation at ICU admission and discharge are lower-tier approaches on the engagement ladder, reconciliation is an important patient safety activity ([3]). There is only a suggestion - and not a strong recommendation - for implementing medication reconciliation pertaining to ADE and medication error prevention within the Safe Medication Use Guideline due to a lack of rigorous evidence at the time of publication ([5]). There is now an opportunity to leverage recently published recommendations to expand inpatient pharmacist roles and link them to non-ICU acute inpatient, as well as, with outpatient care activities in support of patient and family centered care ([4]).
