Abstract

Objective: To analyze the nursing diagnosis focusing on the problem and risks documented in clinical practice by nurses caring of patients in Intensive Care Unit. Methods: Quantitative, transversal, documentary and descriptive study. 122 medical records were selected in the medical file sector, by simple random sampling, with a significance level of 99%, over a one-year time frame, 2016 to 2017. Medical records with a history and diagnosis filled out by nurses from the Intensive Care Unit or by nursing students signed by the responsible nurse were chosen in the first 24 hours after the patients' admission. Medical records with erasures and / or absence of pages in their chronological sequence were excluded. Results: 809 titles of nursing diagnosis were identified, of these 375 (46.3%) with a focus on risks and 434 (53.7%) with a focus on the problem. The diagnosis stood out with the highest percentages: unbalanced nutrition, less than body needs (98.4%); deficit in self-care (96.7%); risk of infection (92.6%); impaired gas exchange (71.3%); risk of impaired skin integrity (54.9%). Conclusions: The identification of nursing diagnosis in the Intensive Care Unit enables nurses to recognize the demands of everyday care, for planning and decision making focused, resolutive and effective in intensive care.
