Manejo estomatológico de pacientes pediátricos con púrpura de Henoch-Schönlein. Presentación de un caso clínico.

Abstract

Introduction. Henoch-Schönlein purpura (PHS) described in 1875, it's multifactorial, involves previous infectious processes in the upper respiratory tract. Multiple clinical manifestations such as skin lesions, joint damage, abdominal, pulmonary, genitourinary, nephropathy, neurological and hematological. The diagnosis is made through physical examination, general examination of urine. In children, the prognosis is favorable. The stomatological exploration has to be individualized, focused on the elimination of infectious foci that can affect immunosuppressive treatment and predispose the patient to secondary infections. The use of drugs that are excreted via the kidney should be avoided and those that are excreted via the liver should be used. It is important to know that if immunosuppressants and non-steroidal anti-inflammatory drugs are used, they will produce hematological and immune changes, such as leukopenia, neutropenia, and thrombocytopenia, and will identify through laboratory studies that can help us decide if the prescription of antibiotic prophylaxis is necessary to perform oral rehabilitation. Clinical case. A 9-year-old male with PHS under observation by hematology, nephrology and rheumatology, laboratory studies showed normal parameters, so he was orally rehabilitated in two phases (hygienic-preventive and restorative-control). Stomatological management was specific according to the state of health in which he was at the time of oral rehabilitation. Conclusions. It is important to establish an interconsultation with the treating medical specialists in order to assess at all times the general systemic condition of the patient, taking into account specific stomatological considerations.
