La psychothérapie centrée sur le lien transférentiel dans la clinique des addictions sexuelles.

L'article vise à partager des constats théoriques, cliniques et techniques dans la prise en charge des sujets qui présentent une addiction sexuelle sévère. À partir de recherches théoriques et d'une pratique clinique confrontée à la clinique des addictions sexuelles, nous examinons des modèles (PFT) qui nous ont amené à concevoir un dispositif de psychothérapie psychanalytique centré sur le lien transférentiel intégrant les travaux de Kernberg, Fonagy, Bateman, mais également ceux mis en perspectives par Anzieu, Bouvet et Roussillon en France. Si ces modalités de cadre et d'interprétation techniques ont été d'abord pensées pour une population de sujets borderline, nos constats cliniques empiriques tendent à vérifier la valeur performative d'un dispositif thérapeutique psychodynamique centré sur le lien auprès des sujets présentant une addiction sexuelle sévère. Si l'efficacité des traitements pharmacologiques, des thérapies cognitives ou de groupes a été démontrée dans la prise en charge des sujets présentant une addiction sexuelle sévère, aucune étude randomisée contrôlée pour l'heure n'existe afin de vérifier l'efficacité d'une psychothérapie analytique centrée sur le lien auprès de cette population. L'article livre ici des résultats qualitatifs empiriques et tend à ouvrir le dialogue avec d'autres méthodologies psychothérapeutiques pour penser une offre diversifiée de modèles de soins adaptés à cette population. The article aims to share theoretical, clinical, and technical findings on the treatment of subjects with severe sexual addiction. Based on theoretical research and clinical practice with sex-addicted patients, we examine the models (PFT) which led us to conceive of a psychoanalytical psychotherapy centered on the transferential link integrating the works of Kernberg, Fonagy, Bateman, but also those put into perspective by Anzieu, Bouvet, and Roussillon in France. If these modalities of framework and technical interpretation were first developed for a population of borderline subjects, our empirical clinical findings tend to verify the performative value, for subjects presenting a severe sexual addiction, of a psychodynamic therapeutic process centered on the link. Although the efficacy of pharmacological treatments, cognitive therapies, or group therapies has been demonstrated in the treatment of subjects with severe sexual addiction, no randomized controlled study currently exists to verify the efficacy of a bonding-focused analytical psychotherapy with this population. The article provides empirical qualitative results and opens a dialogue with other psychotherapeutic methodologies in order to think about a diversified offer of care models adapted to this population.
