Metástasis extracraneal de glioblastoma. Reporte de un caso.

Abstract

BACKGROUND: Glioblastoma is the most common primary central nervous system neoplasm in adult patients. Extracranial metastases are not frequent and are estimated at 0.5% of cases. The most frequently reported anatomical sites are lung, lymph node, bone, and liver. Until now, only one case of extracranial metastasis has been reported in Latin America. CLINICAL CASE: A 26-year-old male patient with extracranial metastasis of glioblastoma, with no significant history of the current condition, who began 5 months prior to hospitalization with generalized tonic-clonic seizures. Imaging studies revealed an intraaxial lesion in the right temporal cerebral parenchyma, which affected the superior, middle, and inferior temporal gyrus, from the middle third of the temporal pole, reaching the lateral face of the occipital lobe, with a medial limit in the ventricular atrium. The evolution was poor, with deterioration of health and acute renal failure KDIGO III; loss of strength in the lower extremities and sphincter tone was added, in addition to complete spinal cord syndrome. Six months after the primary diagnosis of glioblastoma he passed away. Immunohistochemistry reported positivity for neurofilament, glial fibrillary acidic protein, IDH, and SOX10. CONCLUSIONS: The patient in this study represents the second reported case of extracranial metastasis in Latin America. Due to the lack of understanding about the mechanism by which these tumors originate, the mandatory study and reporting is necessary, as well as considering it at the time of the histopathological analysis.
