Abstract

Objective To investigate the epidemiological characteristics and high -risk factors for chronic obstructive pulmonary disease (COPD) among residents aged 40 and above in Gansu Province, to provide reference for guiding regional COPD prevention and control efforts. Methods From May to December 2019, a multi -stage stratified cluster sampling method was used to select 1 836 residents aged 40 and above in three monitoring sites in Gansu Province: Dunhuang city, Cheng County in Longnan city, and Zhengning County in Qingyang city, and questionnaire surveys, physical examinations, and pulmonary function tests were conducted. The chi-square test was used for univariate analysis, and the high-risk factors affecting COPD were analyzed using a multivariate logistic regression model. Results The prevalence of COPD among residents aged 40 and above in Gansu Province was 11.9%. The results of the multivariate logistic regression analysis showed that compared to females and the age group of 40 to 49 years, being male (OR=2.130, 95%CZ: 1.515-2.994) and being in the age group of 60 to 69 years (OR= 2452, 95%CIi 1.533-3.922) or 70 years and above (OR=2.351, 95%CI: 1.248-4.430) were high-risk factors for COPD among residents. Compared to Dunhuang city, residing in Cheng County (0-=1.928, 95%CI: 1.321-2.815) was a high-risk factor for COPD. Having a BMI <18.5 kg/m2 was also identified as a high-risk factor for COPD, Furthermore, having simultaneous exposure to risk factors, chronic respiratory symptoms, and a personal history of chronic respiratory diseases (OR =3.481, 95%CI: 1.729-7.005) were high-risk factors for COPD among residents. Conclusion The prevalence of COPD among residents aged 40 and above in Gansu Province is relatively high. Therefore, special attention should be paid to male individuals, the elderly, individuals with low body weight, and those with simultaneous exposure to risk factors, chronic respiratory symptoms, and a personal history of chronic respiratory diseases in COPD prevention and control efforts.
