The persistent poor prognosis in cardiogenic shock: Insights from recent trials.

Abstract
- This article explores the challenges and poor prognosis associated with cardiogenic shock (CS), despite advancements in treatment for heart conditions. The high mortality rates of CS patients, with nearly 50% dying within 30 days, are discussed. The article highlights a recent trial that showed a survival benefit with a specific type of left ventricular assist device (LVAD) in a selected group of CS patients. The need for further research and understanding of CS, as well as the development of targeted treatment strategies, is emphasized. The article also discusses the importance of studying extracorporeal membrane oxygenation (ECMO) for CS treatment, including evaluating different cannulation strategies, optimizing venting approaches, and determining the best timing for transitioning to durable support. The distinction between restoring central hemodynamics and addressing oxidative stress and inflammation caused by non-pulsatile circulation is also emphasized.
