Spontaneous coronary artery dissection is a rare cause of myocardial infarction but with potentially adverse outcome. It is more common in young women without conventional cardiovascular risk factors. Conservative medical strategy is the best therapeutic option with percutaneous coronary angioplasty reserved for high risk patients. We report a case of a young woman with spontaneous coronary artery dissection involving the left main coronary artery complicated by left ventricular thrombus.
