The author offers his insights on the innovative program toward a totally electronic medical record (EMR) in the U.S. He shares his own decision of advancing to electronic in his profession as a physician. He considers his template-driven notes inaccurate as patients to not speak template. However, he realizes his task to accurately and comprehensively understand his patients rather than merely documenting them.
