Abstract
- The article provides updates on medication errors. Three medication errors were reported in Europe involving the misadministration of bortezomib which happened when intrathecal chemotherapy or another intrathecal drug was schedule on the same day and same time as I.V. bortezomib therapy. A reorganization of parenteral products placed lookalike 10 mL vials of calcium gluconate and cupric sulfate next to each other leading to 2 near-miss errors occurring almost immediately.
