Abstract

Type 2 diabetes mellitus (T2DM) is traditionally viewed as a chronic and progressive disease in which delay of end-organ complications is the major treatment goal. Metabolic surgery offers a new horizon: complete disease remission. Scientific evidence demonstrates that certain operations, in special gastric band, duodenal-jejunal exclusion and gastric bypass improve glucose homeostasis through varied mechanisms. The trials that have studied T2DM patients who underwent metabolic surgery have shown favourable outcomes in improving beta-cell function, insulin sensitivity, insulin secresion and reducing insulin resistance modifying some hormones involved in satiety and glucose homeostasis such as leptin, ghrelin and adiponectin, glucagon-like peptide-1 and glucose-dependent insulinotropic polypeptid. These procedures have also been associated with remission of T2DM and better metabolic control in patients with the disease, making them a good alternative in the treatment of T2DM in selected patients.
