Abstract

Aim. Double-barrel free vascularised fibular graft provides good bone augmentation and restoring. We aimed to evaluate the fesability of free vascularized double barrel fibular graft with skin island flap for reconstruction of large diaphyseal tibial defect. Material and methods. We report a case of posttraumatic tibial large defect and the clinical results of vascularized double barrel fibular graft monitorized by a skin flap for reconstruction. Results. Bony reconstruction was achieved and union was achieved in 6 months. The vascular status was daily monitorized by the skin island and there was not any sign of thrombosis or necrosis. The patient didn't Tratamentul fracturilor complexe de tibie cu lambou fibular dedublat vascularizat transferat microchirurgical The treatment of complex tibial fractures with double barrel free vascularised fibular graft have postoperative stress fractures of the fibula graft as the mechanical stress to the graft increase. Osteosynthesis was done with external fixator changed with a plate 3 months postoperatory. Conclusions. Vascularized fibula transfer is a suitable procedure for long tibial defects, and a skin island-monitoring flap is a simple, extremely useful, and reliable method for assessing the vascular status of vascularized bone.
