LESSONS LEARNED on the Road to Population Health.

The article discusses the challenges of population health management and value-based contracting. It also presents the difficulties experienced by four health systems in the U.S. including out-of-network cost utilization for North Shore-LIJ Health System, utilization of population management tools by doctors for Heartland Regional Medical Center in Illinois, and extension of pediatric network to rural areas for UH Rainbow Care Network.
