Competing Interests: Competing interests: V.J. has received consulting fees or honoraria from Bayer, Astra Zeneca, Boehringer Ingelheim, Vera, Visterra, Otsuka, Novartis, Chinook, Biocryst and Alpine, under the policy of all payments going to the George Institute for Global Health. T.K. has received consultancy fees from Visterra, Otsuka and AstraZeneca as a country investigator, and is currently a recipient of a grant from the National Research Council of Thailand and received speaking honoraria from AstraZeneca, Alexion, Fresenius Medical Care and Baxter Healthcare.
