ICD triple counting leading to a lethal inappropriate shock.

The article presents a case study of a 67‐year old patient with ischemic cardiomyopathy had been implanted a single chamber implantable cardioverter defibrillator (ICD) for primary prevention in another center 1 month before the index event and possibly caused by lead dislocation to the level of the tricuspid annulus.
