Abstract

A recent study conducted in Portland, Oregon examined the outcomes of different treatment strategies for patients with synchronous colorectal liver metastases. The study compared the liver-first approach to the primary-first/simultaneous approach and found that patients who underwent liver-first resection had inferior recurrence-free survival. The liver-first approach was associated with shorter recurrence-free survival, particularly in terms of extrahepatic recurrence. The study suggests that further research is needed to determine the oncologic risk associated with leaving the primary tumor in place and recommends multidisciplinary treatment sequencing and enhanced postoperative surveillance for patients undergoing liver-first resection.
