Algorithme diagnostique et alternatives devant une hypersensibilité retardée aux bêtalactamines.

Résumé Les hypersensibilités retardées aux bêtalactamines (BL) ne sont pas forcément allergiques. Ainsi, leur exploration est primordiale pour ne pas contre-indiquer à tort cette classe d'antibiotiques très utile. L'exploration des hypersensibilités retardées aux BL comporte plusieurs étapes : tout d'abord l'interrogatoire minutieux avec réalisation d'une frise chronologique des prises médicamenteuses depuis 2 mois. Puis la réalisation de tests cutanés : des patch-tests plus ou moins des intradermoréactions à lecture retardée (en cas d'anamnèse compatible avec patch-tests négatifs) voire des tests biologiques (en cas de diagnostic difficile). En cas de tests négatifs, on discutera la réalisation d'un test de réintroduction. En cas de tests cutanés positifs, on recherchera les alternatives thérapeutiques au sein des bêtalactamines en réalisant une batterie de tests puis des réintroductions. Abstract Delayed hypersensitivity to beta-lactams (BL) is not necessarily allergic. It is thus essential to explore such cases of hypersensitivity to avoid unwarranted contraindication of this class of antibiotics. Exploring delayed hypersensitivity to BL involves a number of steps. First, careful questioning is called for with recording of all drugs taken in the previous two months. Skin tests must then be performed consisting of patch tests, and if necessary, intradermal tests (in the case of a compatible history but negative patch tests), or possibly laboratory tests (in the event of diagnostic difficulties).
