•Increased serum ferritin is a frequent situation that requires thorough evaluation.•Clinician should avoid unnecessary exploration and inaccurate diagnosis.•Magnetic resonance imaging is the reference method to assess iron burden.•Second line genetic testing are discussed taking into account the whole evaluation.
