•CRE PCNSI patients: 28-day mortality rate = 50.0% (14/28).•ST11-KL64 CRKP with blaKPC-2: main CRE cause post-craniotomy.•Poor prognosis in CRE PCNSIs linked to CSF protein, chloride, and lumbar drainage.•Effective treatment: ceftazidime/avibactam or amikacin combo, intrathecal amikacin.
