Abstract

Tiivistelmä Bronkioliitti on viruksen aiheuttama pienten hengitysteiden tulehdussairaus, joka johtaa ensimmäiseen uloshengitysvaikeuskohtaukseen tiukan määritelmän mukaisesti alle 6 kuukauden ikäisellä ja laajan määritelmän mukaisesti alle 2-vuotiaalla lapsella. RSV-bronkioliitti ja ”rinovirus-atopiavinkuna” ovat erilaiset sairaudet, joiden riskitekijät, patogeneesi, taudinkulku, hoito ja ennuste eroavat toisistaan. Variability in the definitions of bronchiolitis and and the degree to which they take into account the risk factors for asthma have caused confusion in clinical practice. There is no longer a single common entity bronchiolitis. Instead, three clinically distinct clusters of bronchiolitis can be identified: 1) respiratory syncytial virus-induced bronchiolitis, characterized by young age and mechanical obstruction of the airways due to mucus and cell debris; 2) rhinovirus-induced wheezing, which is associated with atopic characteristics and high risk of asthma development; and 3) illness due to other viruses, which is usually less frequent and less severe. Clinically, it is important to distinguish between these three main clusters of bronchiolitis since their clinical picture and prognosis are different. Although there are few controlled trials, there is some indication that they also differ in their response to acute treatments.
