Foundations for the development of standardized dietetic assessments for inflammatory bowel disease and irritable bowel disease Evaluation of the quality criteria of standardized assessment- and outcome-instruments through COSMIN and linking to the ICF-Dietetics

Einleitung: In der momentanen diätologischen Praxis besteht kein einheitliches Diaetologisches Assessment im Fachbereich der Gastroenterologie. Diese Bachelorarbeit baut auf den jeweiligen Resultaten der vorangegangenen Bachelorarbeit „Grundlagen für die Entwicklung eines standardisierten diaetologischen Assessments für ausgewählte gastroenterologische Erkrankungen“ auf und befasst sich mit den Krankheitsbildern der Chronisch Entzündlichen Darmerkrankungen und des Reizdarmsyndroms. Methodik: Alle 36 der vorher erhobenen Assessment- und Outcome-Instrumente wurden auf ihre Validität, sowie Reliabilität nach COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments) überprüft. Anschließend wurden die passend bewerteten Assessment- und Outcome-Instrumente, sowie die ernährungsrelevanten Gesundheitsinformationen der in der vorangegangen Bachelorarbeit durchgeführten Delphi-Befragung von Expert*innen dem Linking-Prozess unterzogen. Ergebnisse: Die Ergebnisse der qualitativen Bewertung der Assessment- und Outcome-Instrumente mittels COSMIN und die gelinkten ICF-Diätetik-Kategorien wurden tabellarisch dargestellt und zusammengefasst. Aufgrund des Resultats der Qualitätsbewertung konnten insgesamt 22 beurteilte Assessment- und Outcome-Instrumente neben den Ergebnissen der Delphi-Befragung in die ICF-Diätetik gelinkt werden. Diskussion: Zukünftig wären standardisierte Assessment- und Outcome-Instrumente zu den einzelnen Krankheitsbildern im Fachbereich Gastroenterologie (wie Chronisch Entzündliche Darmerkrankungen und Reizdarmsyndrom) von Vorteil. Diese könnten in der Praxis zur leichteren und schnelleren Identifizierung von Ernährungsproblemen und zum Dokumentieren des Erfolgsnachweises im Zuge der diätologischen Behandlung implementiert werden. Darüber hinaus könnte das Berufsfeld aus der Entwicklung von ICF-Diätetik-Core-Sets zu gastrointestinalen Erkrankungen einen Nutzen ziehen, welche den Qualitätsstandard in der diaetologischen Praxis vereinfachen würden. Das effizientere Vorgehen aller in Österreich praktizierenden Diätolog*innen hätte die Professionalisierung der Berufsgruppe und einen wachsenden Stellenwert der Diätologie im gesamten Gesundheits-wesen zur Folge und ist jedenfalls erstrebenswert. Weitere Forschung zu den in dieser Arbeit nicht bearbeiteten Krankheitsbildern muss in den nachfolgenden Jahrgängen betrieben werden, damit eine Grundlage für ein standardisiertes Assessment- und Outcome-Instrument im Fachbereich der Gastroenterologie gelegt werden kann. Introduction: There are no standardized dietetic assessments for the field of gastroenterology in the current dietetic field. This thesis builds on the respective results of the previous thesis “Basic principles for the development of a standardized dietetic assessment for selected gastroenterological diseases“ and deals with the selected gastroenterological clinical pictures of inflammatory bowel disease and irritable bowel disease. Method: In total, all 36 of the previously found assessment and outcome instruments were reviewed for their validity and reliability according to COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments). Following that the appropriately evaluated assessment and outcome instruments, as well as the nutrition-related health items of the Delphi survey carried out with experts in the field, were subjected to the linking process. Findings: The results of the qualitative evaluation of the assessment and outcome instruments using COSMIN and the used ICF dietetics categories were displayed in charts and summarized in the following work. Based on the findings in the rating, a total of 22 evaluated assessment and outcome instruments and the results of the Delphi survey could be linked to the ICF dietetics. Discussion: Henceforward, standardized assessment and outcome instruments for each individual disease in the field of gastroenterology (such as inflammatory bowel disease and irritable bowel disease) could be of great value. These could be implemented as a custom in active practice to identify alimentary issues more easily and swiftly and to be able to document the success of said dietary treatment. In addition, the professional field could benefit from the development of ICF dietetics core sets on gastrointestinal diseases, which would simplify the quality standard in dietetic practice. This efficient approach of all dieticians practicing in Austria would result in the professionalization of the field and a growing importance of dietetics in the entirety of the health care system and is in any case desirable. Further research on the gastroenterological diseases not dealt with in this thesis must be carried out in the succeeding work from the following years, so that a foundation can be laid for a standardized assessment and outcome instrument in the field of gastroenterology.
