Abstract
- Pharmacists and nurses who teach patients on how to care for themselves after discharge helps lower the number of hospital readmissions. A program for improved patient education, developed by an Agency for Healthcare Research and Quality study, calls for the help of nurses in assisting patients for arranging follow-up appointments, understanding their diagnoses, and confirming drug routines.
