Creating an interprofessional team and discharge planning guide to decrease hospital readmissions for COPD

An interprofessional team was created to address high hospital readmission rates for chronic obstructive pulmonary disease (COPD). The team developed a discharge planning guide to ensure inpatient care was less fragmented. Educational efforts focused on improving nurse knowledge of COPD and appropriate technique for inhaler use.
