Program Evaluation: A Patient Transition of Care Model for Chronic Obstructive Pulmonary Disease

A program evaluation was completed using the framework from the Centers for Disease Control and Prevention. A retrospective quantitative data analysis demonstrated the transition of care program effectively met its intended objectives of reducing the Medicare readmission rate and penalty for pulmonary patients. Health care organizations should embrace this nurse-driven, interprofessional model, which is a valuable, evidence-based intervention that enhances quality, cost-effective care.
