Suboptimal prescribing in older inpatients and outpatients

This article examines the prevalence of suboptimal and inappropriate prescribing for patients over the age of sixty-five. Problems with prescribing medications for this population are associated with significant levels of mortality and can be prevented through the use of clinical pharmacy interventions and multidisciplinary teams in geriatric care.
