Objective: This retrospective longitudinal study analyzed the demographic profile, insulin usage pattern, and outcomes of insulin-naive adults with type 2 diabetes mellitus (T2DM) who initiated insulin glargine. Methods: The study included 1006 insulin naive T2DM individuals aged ≥ 18 years, treated with any insulin type between January 2016 and December 2018, using electronic medical records. Results: Majority of participants were men (55.8%) with a mean age of 59.8 ± 11.9 years and average T2DM duration of 12.0 ± 6.6 years. Insulin glargine was the most commonly used insulin (66.9%), followed by insulin aspart (16.4%), insulin degludec (15.1%), human insulin (11.1%), and insulin isophane (9.2%). At baseline, the mean glycated hemoglobin (HbA1c) was 8.9 ± 1.9%, mean fasting plasma glucose (FPG) was 190 ± 59 mg/dL, and mean post-prandial plasma glucose (PPG) was 264 ± 78 mg/dL. In the insulin glargine group, baseline HbA1c was 9.0 ± 1.7%, FPG was 196 ± 62 mg/dL, and PPG was 283 ± 81 mg/dL. Throughout the study, there was an improvement in HbA1c, FPG, and PPG levels in the insulin glargine group. Body weight remained relatively stable, and the number of hypoglycemic events was minimal and non-life-threatening. Conclusion: The REALITY study in India demonstrated that initiating basal insulin treatment in insulin-naive individuals with T2DM led to improved glycemic parameters over a 12-month period. Purpose: We aimed to investigate controversial pediatric urolithiasis issues systematically, integrating expert consensus and comprehensive guidelines reviews. Methods: Two semi-structured online focus group meetings were conducted to discuss the study's need and content, review current literature, and prepare the initial survey. Data were collected through surveys and focus group discussions. Existing guidelines were reviewed, and a second survey was conducted using the Delphi method to validate findings and facilitate consensus. The primary outcome measures investigated controversial issues, integrating expert consensus and guideline reviews. Results: Experts from 15 countries participated, including 20 with 16+ years of experience, 2 with 11–15 years, and 4 with 6–10 years. The initial survey identified nine main themes, emphasizing the need for standardized diagnostic and treatment protocols and tailored treatments. Inter-rater reliability was high, with controversies in treatment approaches (score 4.6, 92% agreement), follow-up protocols (score 4.8, 100% agreement), and diagnostic criteria (score 4.6, 92% agreement). The second survey underscored the critical need for consensus on identification, diagnostic criteria (score 4.6, 92% agreement), and standardized follow-up protocols (score 4.8, 100% agreement). Conclusion: The importance of personalized treatment in pediatric urolithiasis is clear. Prioritizing low-radiation diagnostic tools, effectively managing residual stone fragments, and standardized follow-up protocols are crucial for improving patient outcomes. Integrating new technologies while ensuring safety and reliability is also essential. Harmonizing guidelines across regions can provide consistent and effective management. The removal of pollutants from water bodies is crucial for the well-being of humanity and is a topic of global research. Researchers have turned their attention to green synthesized nanoparticles for wastewater treatment due to their eco-friendly nature, biocompatibility, and cost-effectiveness. This work demonstrates the efficient removal of organic dye and both gram-positive and gram-negative bacteria from water bodies using copper-doped cerium oxide nanoparticles synthesized with Murraya Koenigii extract. Characterized via various methods, the 15% copper doped cerium oxide nanoparticles (Cu 15% NPs) exhibited maximum Congo red dye adsorption (98% degradation in 35 min). Kinetic analysis favoured a pseudo-second-order model, indicating the chemical nature of adsorption. Equilibrium adsorption isotherms aligned with the Langmuir model, indicating homogenous monolayer dye adsorption on the doped adsorbent. The maximum uptake of adsorbate, Q m obtained from Langmuir model for Cu 15% NPs was 193 mg g−1. The study also showed enhanced antibacterial activity against Bacillus subtilis, Staphylococcus aureus, Escherichia coli and Pseudomonas aeruginosa for Cu-doped ceria, attributed to generation of reactive oxygen species (ROS) induced by the redox cycling between Ce3+ and Ce4+. This substantiated that the green synthesized copper doped cerium oxide nanoparticles are potential candidates for adsorptive removal of Congo red dye and as antibacterial agents. This investigation highlights the viability of utilizing recently synthesized solid-state polymer electrolyte thin films in the fabrication of solid-state dye-sensitized solar cells (DSCs). Specifically, composite solid polymer blend electrolytes comprising poly(ethylene glycol)/poly(vinylidene) fluoride/titanium dioxide (PEG/PVDF/TiO2) nanoparticles, as well as PEG/TiO2 nanoparticle composite solid polymer electrolytes, were made-up via a solution-casting method. To overcome the issues related to liquid and gel electrolyte consumption, leading to problems such as leakage and solvent evaporation, the utilization of solid electrolytes has been introduced for practical applications in dye-sensitized solar cells. The solid-state polymeric composite materials are proposed to substitute of TiO2 nanoparticles as fillers in the poly(ethylene glycol) host to augment the efficiency of solid electrolytes. The results recommend that the ionic conductivities of the solid-state electrolytes are comparable to those observed in PEG/TiO2 nanoparticle composite solid polymer electrolytes. This suggests that the blend polymer matrix itself may play a significant role in facilitating effective charge transport within the channels of the nanocomposite solid network. The application of pressing force enables effective penetration of the solid electrolyte into the TiO2 film, establishing a robust contact with the photoelectrode. The PEG/PVDF/TiO2 nanoparticle composite solid polymer blend electrolyte-based dye-sensitized solar cells show an open-circuit voltage of 0.641 V and solar cell efficiency increases to 5.5% better performance, as compared to the solid-state dye-sensitized solar cells with PEG/TiO2 nanoparticle composite solid polymer electrolytes. Considering different applications that require varied power and voltage conversion levels between AC grids and AC loads, AC–AC power conversion between AC grids has become an inevitable technology of energy management systems. An isolated converter for performing AC-to-AC transmission is proposed with minimal components for reduced losses and enhanced system efficiency. Single-phase direct buck-boost AC to AC converter with minimum components constituted with two dual IGBT control units (IGBT 1–IGBT 4), inductor (Lf), and capacitor (Cf) is proposed in this work. The MATLAB/Simulink platform is used to provide in-depth analysis of the circuit and components along with the design guidelines, and simulation outcomes of this proposed model. The voltage gains of G = 2.13, power factor of 0.97, and overall efficiency of 98% are achieved in the proposed system with minimum components of 4 switches, 2 conductors, and 1 capacitor and inductor respectively. Soil salinity is a major abiotic stress that adversely affects plant growth, and productivity. About 20% of irrigated lands are affected by salinity worldwide, In India, there are 6.74 million hectares of salt-affected lands. Salt-tolerant Plant Growth Promoting (PGP) microorganisms can enhance the growth of plants in such salt-stressed areas. Therefore, this study aimed to investigate the diversity of beneficial fungal communities, screen for their ability to support plant growth, and evaluate the production of various essential compounds in view of plant growth in salt-stressed lands. A total of 68 fungal colonies were isolated from 5 different agroforestry plantation sites in Karur, Tamil Nadu, South India at quarterly intervals. The isolates were screened for sodium chloride (NaCl) tolerance (0%, 5%, 10%, 15%, and 20% concentration). A total of 7 isolates showed considerable salt tolerance and were tested qualitatively in-vitro, for PGP traits such as phosphate, potassium, and zinc solubilization, nitrogen fixation, hydrogen cyanide production, siderophore production, and ACC deaminase production. Finally, 5 isolates with maximum values for PGP properties under 20% NaCl concentration were tested for the quantity of Indole-3-Acetic Acid (IAA) and Exo-polysaccharide (EPS) production. All 5 isolates were identified up to the species level using 18S rRNA gene sequencing. To the best of our knowledge, this is the first report on isolating saline-tolerant PGP Fungi (PGPF) from the rhizosphere region of Casuarina equisetifolia and Eucalyptus camaldulensis in Karur, Tamil Nadu, India. Background: A significant proportion of diabetic kidney disease (DKD) experience a rapid decline in eGFR, leading to end-stage kidney disease (ESKD) within months. This single-centered retrospective cohort study aimed to assess the prevalence, clinical profile, and predictors for rapid progression in type 2 diabetes mellitus (T2DM) patients with DKD. Method: Three hundred fifty-nine T2DM patients with DKD between January 2018 and 2022 were included and those with superimposed non-diabetic kidney disease, chronic kidney disease 5, and < 6 months follow-up were excluded. They were classified as rapid and non-rapid progressors based on the annual eGFR decline of > 5 ml/min/1.73 m2/year. The primary outcome analyzed was the progression to ESKD. The secondary outcomes were the onset of microvascular and macrovascular complications and predictors for rapid progression as well as ESKD. Results: In a median follow-up of 3.5 years, 61.3% were rapid progressors (mean eGFR decline of 15.4 ml/1.73m2/year) and 38.7% were non-rapid progressors (mean eGFR decline 1.8 ml/1.73m2/year. Among rapid progressors, 61.4% reached ESKD. Severe proteinuria, the presence of retinopathy, and acute kidney injury (AKI) episodes were strong predictors of rapid progression. Cardiovascular disease and diabetic retinopathy (microvascular complications) were significantly higher among rapid progressors and had a mortality rate of 7.2%. Conclusion: The majority of type 2 DKD patients were rapid progressors and two-thirds of them developed ESKD. The prevalence of hypertension, cardiovascular disease, diabetic retinopathy, AKI episodes, and mortality was higher in rapid progressors. Dysarthria is a speech communication disorder that is associated with neurological impairments. To detect this disorder from speech, we present an experimental comparison of deep models developed based on frequency domain features. A comparative analysis of deep models is performed in the detection of dysarthria using scalogram of dysarthric speech. Also, it can assist physicians, specialists, and doctors based on the results of its detection. Since dysarthric speech signals have segments of breathy and semi-whispery, experiments are performed only on the frequency-domain representation of speech signals. Time-domain speech signal is transformed into a 2-D scalogram image through wavelet transformation. Then, the scalogram images are applied to pre-trained convolutional neural networks. The layers of pre-trained networks are tuned for our scalogram images through transfer learning. The proposed method of applying the scalogram images as input to pre-trained CNNs is evaluated on the TORGO database and the classification performance of these networks is compared. In this work, AlexNet, GoogLeNet, ResNet 50 and two pre-trained sound CNNs, namely VGGish and YAMNET are considered deep models of pre-trained convolutional neural networks. The proposed method of using pre-trained and transfer learned CNN with scalogram image feature achieved better accuracy when compared to other machine learning models in the dysarthria detection system. This paper presents a new nonlinear fractional-order model identification of a two-level voltage source inverter fed induction motor (VSI-IM). The nonlinearities present in inverter and IM cause the failure of the sensorless controls and model predictive controls, which mainly depends on the VSI-IM model. In the proposed work, an attempt has been made to identify the exact nonlinear behavior of the VSI-IM model using the PWM signal injection method. The identified fractional-order VSI-IM model is helpful to provide more insight into the dynamic behavior of the IM drive in the speed controller design (sensorless and sensored) and in predictive controls (model-based and model-free). The fractional VSI-IM models are identified at low, medium, and high frequencies. The best fit of these models is compared to the integer-order identified VSI-IM models. The experimental study clearly shows that the fractional-order VSI-IM identified model is superior to the integer-order VSI-IM model in terms of best fit with the actual hardware speed response. Memristive devices being applied in neuromorphic computing are envisioned to significantly improve the power consumption and speed of future computing platforms. The materials used to fabricate such devices will play a significant role in their viability. Graphene is a promising material, with superb electrical properties and the ability to be produced in large volumes. In this paper, we demonstrate that a graphene-based memristive device could potentially be used as synapses within spiking neural networks (SNNs) to realise spike timing-dependant plasticity for unsupervised learning in an efficient manner. Specifically, we verify the operation of two SNN architectures tasked for single-digit (0–9) classification: (i) a single layer network, where inputs are presented in 5 × 5 pixel resolution, and (ii) a larger network capable of classifying the dataset. Our work presents the first investigation and large-scale simulation of the use of graphene memristive devices to perform a complex pattern classification task. In favour of reproducible research, we will make our code and data publicly available. Sea vessels and artificial sea‐based structures are severely affected by biofouling, i.e., the formation of deposits of living and dead marine organisms that belong to different species and range in size from unicellular bacteria to multicellular seaweed and mussels. This is a significant engineering problem since they essentially alter the geometry of the hull, increasing friction and reducing the speed of vessels, thus increasing the cost and environmental footprint of transportation. Given the scale of global transportation reaches several billion tons per year, the socioeconomic consequences of the reduction in transit speed and increased consumption of fuel continue to drive researchers and engineers to develop strategies to combat the processes of marine biofouling. Many types of antifouling paints, coatings, and materials that have been designed and tested, and in some instances used commercially, suffer from shortcomings ranging from environmental toxicity to limited efficiency and durability. In this review article, a brief overview of the traditional antifouling materials is presented and recent achievements in the design of advanced antifouling materials based on such nanomaterials as graphene, nanotubes, nanoparticles, and more complex nanostructures are discussed. Purpose: The purpose of this study is to evaluate the adenosine pharmacological stress-induced electrocardiogram (ECG) changes and their association with stress-induced ischemic defects on myocardial perfusion scintigraphy (MPS) in the evaluation of coronary artery disease (CAD) and to evaluate event‑free survival among patients with positive and negative ECG/MPS image findings. Methods: A total of 100 patients were examined using stress MPS from March 2020 to August 2021. Stress-induced ECG changes during adenosine infusion were evaluated. The summed stress score (SSS) was evaluated to identify ischemic defects in myocardium. Association of stress ECG changes and scintigraphic results was evaluated. Results: Out of 100 patients, stress ECG changes during adenosine infusion were seen among 34 patients, whereas 66 patients had normal ECG findings. Positive stress MPS findings with SSS >3 were seen in 22 patients, whereas 78 patients had SSS ≤3. There was no agreement between stress ECG changes and MPS findings with Cohen’s kappa coefficient (κ) = −0.023, whereas there was mild agreement between stress ECG changes and SSS >7 with κ = 0.105. Median follow-up of 11 months showed more events among patients with positive ECG changes than negative ECG changes. Conclusion: Adenosine, pharmacological stress is safe to use, but few patients might experience some minor and transient symptoms. Adenosine may induce ECG changes in patients with or without positive MPS findings. Patients with concordant positive findings need aggressive cardiac care, whereas patients with small or no defects on MPS need close monitoring. [ABSTRACT FROM AUTHOR]

The generation of waste is significantly influenced by the increase in population and industrialization, thereby compelling the increased demand for waste management and resource recovery. This paper investigates the potential opportunities presented by the utilization of microwave-assisted pyrolysis for processing plastic and organic waste materials, with a particular focus on industrial hemp leaves, hurds, and root materials, and other feedstocks. Drawing from a range of published studies, it is suggested that microwave-assisted pyrolysis has the potential to achieve energy neutrality or even energy generation, if all byproducts are used. Depending on factors such as recoverable volumes and the associated recovery costs of commercially significant chemicals like vinegars and bio-oils, the microwave-assisted pyrolysis of industrial hemp leaves, hurds, and root materials may prove to provide high return of the yield and profits. Additionally, this paper explores the production of other valuable byproducts such as syngas and biochar from alternative feedstocks, particularly when data related to hemp processing is not readily available. Introduction The quality control (QC) procedures for positron emission tomography (PET) scanners are covered by National Electrical Manufacturers Association and International Electrotechnical Commission. QC must be carried out at regular intervals according to the specifications of the scanner manufacturer. Daily and weekly QC plays a valuable role in monitoring positron emission tomography (PET) scanner performance changes. This study shares operational and performance experience of QC procedures that do not require a radioactive Ge-68 source to perform daily QC and experience with fluorodeoxyglucose F18 (18 F-FDG) as a substitute for germanium-68/sodium-22 (Ge-68/Na-22) source for weekly QC. Method This study was performed on an uMI550 digital positron emission tomography-computed tomography (PET-CT) scanner. In this scanner daily QC checks system temperature and humidity, system count rate, data link status, and voltage. QC was performed at the console control, the position of the scanner table was in the home position pulled out from the gantry, and the room was closed during the quick QC. Weekly full QC check items include look-up table drift, energy drift, time-of-flight status, C-map status, temperature and humidity, and voltage. Weekly full QC was performed with a 18 F-FDG source in a rod phantom source. Results Over 200 daily QC tests without a radioactive source Ge-68 phantom and 50 full weekly QC tests using a 18 F-FDG rod phantom were performed with this scanner according to the manufacturer's instructions and a test report was generated. No daily QC errors or warnings were observed during this period. Conclusion The new approach for the daily PET QC does not expose operators to radiation. This translates into commercial and operational merits with consistent performance and results. Background: Uremic pruritus has an impact on the quality of life and sleep of hemodialysis patients, but the majority of cases go unreported and untreated unless severe, due to a lack of awareness. The purpose of this study is to determine the prevalence, associated factors, and impact on health-related quality of life (HR-QOL) and sleep in hemodialysis patients. Methodology: A single-center observational study of 3 months wherein 120 adults on maintenance hemodialysis were included. Baseline characteristics, dialysis-related factors, and lab parameters influencing uremic pruritus were recorded. Those with uremic pruritus completed "12-item pruritus severity scale (12-PSS)", "SKINDEX10", and "Itch-MOS" questionnaires to evaluate severity, impact on HR-QOL, and sleep respectively. Results: Sixty seven over one hundred twenty (55.83%) patients had pruritus and majority were mild (40.83%) as per 12-PSS. Those with pruritus (n=67) had a mean age of 56.5±11.3 years, most were males (82%), chronic glomerulonephritis (29.1%) was the commonest cause of end-stage kidney disease, 3 active smokers, and 4 seropositive. 65(97%) patients were on twice-weekly dialysis, 36/67 had <5 years' dialysis vintage and acceptable adequacy. There was no significant association between uremic pruritus and dialysis-related/laboratory parameters. Patients with uremic pruritus demonstrated significantly worse "HR-QOL" (p<0.001) on the "SKINDEX-10", and patients' "Itch-MOS" scores demonstrated a significant decline in sleep quality with increasing pruritus severity (p<0.001). Conclusion: The majority of patients on maintenance hemodialysis experience uremic pruritus. None of the clinical characteristics, dialysis-related factors, and laboratory parameters affected uremic pruritus. Uremic pruritus patients had the worst HR-QOL & their sleep quality significantly declined as pruritus severity escalated. KY boost converter (KBC) finds wide application in photovoltaic (PV) and single-phase inverters. In this study, a novel KBC containing seven levels of a multilevel inverter (MLI) is presented. Here, KBC between the direct current (DC) source and the inverter is proposed. Moreover, the authors considered a closed-loop response of the KBC-MLI system with a proportional integral (PI) controller and a fractional-order PID (FOPID) Controller. The main purpose of this suggested technique is to match the output current, total harmonic distortion (THD), and dynamic response of closed-loop PI and FOPID-controlled KBC-MLI systems. The proposed model is simulated with the MATLAB software, and the attained outcome illustrated an enhanced dynamic performance by using the FOPID-controlled KBC-MLI system. The simulation results of the KBC-MLI technique have been evaluated against the theoretical models. Finally, extensive experiments were carried out to validate the theoretical results. The fungus Trametes versicolor, a basidiomycete, is well recognized as a source of industrial enzymes including laccase. Laccase productivity of T. versicolor NFCCI 4361 was determined at different concentrations of molasses, copper (Cu), guaiacol (Gua), gallic acid (GA), and 2, 2´-azino bis (3-ethylbenzthiazoline-6-sulphonic acid) (ABTS) in Potato Dextrose Broth (PDB). Copper with molasses synergistically increased the laccase productivity of T. versicolor NFCCI 4361 from 291.59 ± 30.92 to 1228.96 ± 87.52 U/L. The GA and Gua had negligible effects, while ABTS (1 mM) greatly increased the laccase productivity, both on its alone (495.4 ± 83.47 U/L) and when combined with copper (921.71 ± 41.58 U/L). Following the study of laccase productivity, the percentage of spent wash decolorization was determined by using powdered activated carbon (AC), living, dead, and activated carbon immobilized mycelial biomass (ACIMB) of T. versicolor NFCCI 4361, comparatively. The maximum percentage of decolorization was obtained by ACIMB (44%), followed by AC (40%), living (30%), and dead (16%) mycelial biomass of T. versicolor NFCCI 4361 after 144 h of interaction time. As data processing volume increases, the limitations of traditional computers and the need for more efficient computing methods become evident. Neuromorphic computing mimics the brain's low‐power and high‐speed computations, making it crucial in the era of big data and artificial intelligence. One significant development in this field is the memristor, a device that exhibits neuromorphic tendencies. The performance of memristive devices and circuits relies on the materials used, with graphene being a promising candidate due to its unique properties. Researchers are investigating graphene‐based memristors for large‐scale, sustainable fabrication. Herein, progress in the development of graphene‐based memristive neuromorphic devices and circuits is highlighted. Graphene and its common fabrication methods are discussed. The fabrication and production of graphene‐based memristive devices are reviewed and comparisons are provided among graphene‐ and nongraphene‐based memristive devices. Next, a detailed synthesis of the devices utilizing graphene‐based memristors is provided to implement the basic building blocks of neuromorphic architectures, that is, synapses, and neurons. This is followed by reviewing studies building graphene memristive spiking neural networks (SNNs). White Spot Syndrome Virus (WSSV) is a major virus found in shrimp that causes huge economic loss in shrimp farms. A selective diagnostic approach for WSSV is required for the early diagnosis and protection of farms. This work proposes a novel recognition method based on improved Convolutional Neural Network (CNN) namely Dense Inception Convolutional Neural Network (DICNN) for diagnoses of WSSV disease. Initially, the process of data acquisition and data augmentation is carried out. The Inception structure is then used to improve the performance of multi-dimensional feature extraction. As a result, the proposed work has the highest accuracy of 97.22% when compared to other traditional models. The proposed work is targeted to Litopenaeus Vannamei (LV), and Penaeus Monodon (PM) diversities for major threats detection of White Spot Syndrome (WSS). In this article, a novel Euclidean distance mapping (EDM) technique for space vector pulse width modulated (SVPWM) voltage source inverter (VSI) is presented. The EDM technique processes space vector diagram (SVD) as an image, which simplifies execution of SVPWM. In the proposed technique, Euclidean distance calculation between all eight switching vectors and the tip of the revolving reference voltage vector for every subcycle is carried out. The identification of nearest three voltage vectors (NTV) and switching sequence designs are done on the basis of these Euclidean distances. The EDM-SVPWM technique achieves improvement in the inverter performance in the context of the factors like stator flux ripple over a subcycle, distortion factor (DF), total harmonics distortion (THD) of line voltage, and line current ripple of the inverter. The significant quantities of food waste that require disposal have a high environmental impact, and the depletion of non-renewable fuel sources has heightened the need to investigate sustainable and efficient methods of biomass conversion into energy. This research focuses on utilising pumpkin peel as a feedstock for energy recovery through microwave pyrolysis under different operating conditions. The study demonstrated that a higher biochar yield (11 wt%) was achieved at 0.9 kW. However, results revealed that superior quality biochar was obtained at 1.2 kW, characterized by high carbon content (70.33%), low oxygen content (23%), and significant pore formation in the carbon surface area. Optimal operating conditions, such as 1.2 kW, resulted in superior quality biochar and higher bio-oil generation. The pumpkin peel demonstrated the potential for CO2 (carbon dioxide) sequestration, with a rate of 14.29 g CO2 eq/kg. The research findings contribute to the exploration of sustainable solutions for biomass conversion and emphasize the importance of utilizing food waste for energy production while mitigating environmental impacts. This study offers Ensemble methods for Cloud-based Network Intrusion Detection and Classification. The main means through which Ensemble Learning enhances each Machine Learning result Using algorithms and a strong classifier. In starting, the redundant data is removed from the collected raw data and the data cleaning is performed as the pre-processing phase. From these pre-processed data, modified holoentropy, correntropy, information gain, and statistical features are extracted. An ensemble technique that uses models like the neural network (NN), support vector machine (SVM), random forest (RF), and optimized deep belief network (DBN) performs the final detection of the presence or absence of attacks. The process of fine-tuning is supervised, and in order to increase the training's accuracy, the weights received from the RBM pre-training are modified. A new hybrid model called Moth Logarithmic Assisted Deer Hunting Optimization performs this fine-tuning (MLADHO). The conceptual fusion of the traditional Moth Flame Optimization (MFO) and the Deer Hunting Optimization Algorithm (DHO) yields the suggested algorithm. Purpose Rapid and effective revascularization is very important in the treatment of acute ischemic stroke (AIS). Endovascular treatment is a promising modality in the management of AIS in young patients. Methods We evaluated the clinical and imaging records in 14 patients younger than 18 years presenting within 6 hours of AIS. They received endovascular therapy (ET) either by mechanical thrombectomy, thromboaspiration, or both (Solumbra) between July 2017 and June 2021. The National Institute of Health Stroke Scale (NIHSS) score was calculated on admission and before the discharge of all patients. The 90-day modified Rankin Scale (mRS) score on disability-free outcome was also evaluated. Results The mean preprocedure NIHSS score was 10.78 ± 2.11 that improved to 4.5 ± 1.88 after the procedure. Thrombolysis in cerebral infarction (TICI) grade 2b and 3 blood flow could be established in 12 (85.72%) patients. One patient had TICI 2a flow and one patient had recurrent occlusion despite repeated recanalization (TICI grade 0). The disability-free outcome, mRS score at 90 days was 0 to 1 in 12 (85.72%) patients, mRS score 2 in one (7.14%) patient, and mRS score 3 in one patient (7.14%). We did not have any major complication related to the procedure. Conclusion ET provides high rates of arterial recanalization and favorable disability-free outcomes in young patients with AIS. A laboratory setup is developed to experimentally reinforce the student's understanding of multilevel neutral point clamped inverter operated with various multicarrier and modulation reference signals. Sixteen combinations of carrier and reference signals out of many more possible are studied in this setup. The lab module is project-based and is designed to perform experiments for the 'advanced power electronics and drives' course at the undergraduate level. Background and Objective: For 1.3 billion population in India, there are only scarce reports on disability of epilepsy using disability-adjusted life year (DALY) as a measure. Our objective was to estimate DALY using real-life data over a period of time for a cohort of people with epilepsy (PWE) admitted to an Epilepsy Monitoring Unit (EMU) of a tertiary care epilepsy center. Materials and Methods: We ascertained survival status as on December 31, 2016 of all eligible admissions to the EMU between 01/01/2005 and 12/31/2015. We examined the medical records of randomly selected 200 of the 1970 survivors and all the expired PWE (n = 40) for clinical characteristics. The cumulative real-life DALY (cr-DALY) for individual was calculated as the sum of the years lost to disability (YLD) and the years of life lost (YLL). Annual population-based DALY (p-DALY) was estimated from the cr-DALY, total patient-years of follow-up, and regional population prevalence. Results: The cr-DALY per PWE was 17.63 (generalized seizures only). The cr-DALY increased by 23.7% when all seizure types were considered (23.12). PWE with epilepsy onset <10 years of age, focal epilepsy (particularly, extratemporal lobe epilepsy), and premature death had significantly higher cr-DALY. Those who underwent surgery for epilepsy or achieved remission had significantly lower cr-DALY. The computed p-DALY was 583/1,00,000 population (generalized epilepsy contributed 165/1,00,000 population; focal epilepsy contributed 418/1,00,000 population). Conclusion: Our study had identified, for the first time, several determinants that reduced DALY significantly. Real-life DALY, rather than prevalence-based DALY, captures the cumulative disability of affected individuals. The present study was carried out to investigate the phytochemical and in vitro antioxidant activity of whole plant of Henckelia humboldtiana. Qualitative phytochemical screening of methanol and ethanol extracts of Henckelia humboldtiana revealed the presence of alkaloids, catechins, flavonoids, phenols, quinones, saponins, steroids, tannins, terpenoids, sugars, glycosides and xanthoproteins. Total phenolic and flavonoid contents were determined. Antioxidant activity have been tested using various antioxidant model systems viz. DPPH, hydroxyl, superoxide, ABTS and reducing power. Ethanol extract of H. humboldtiana is found to possess higher DPPH, superoxide and ABTS radical scavenging activity while methanol extracts is found to possess higher hydroxyl radical scavenging activity. Ethanol extract of whole plant of Henckelia humboldtiana shows the highest reducing ability. This study indicates the significant free radical scavenging potential of Henckelia humboldtiana whole plant. This is the first report on the antioxidant property of this plant. Background: Reverse distribution is a pattern that is occasionally observed with 99mTc sestamibi myocardial stress perfusion scintigraphy. The actual cause for this kind of pattern to occur is still under evaluation with no specific aetiology identified. Methods: We retrospectively studied 1195 patients who underwent 99mTc sestamibi myocardial stress perfusion scintigraphy. There was no history of acute myocardial infraction 1 month before the scan; no prior percutaneous transluminal coronary angioplasty was performed in any of these patients. Twenty-seven patients (2.2%) were found to have reverse distribution pattern based on the criteria after ruling out artefacts. Results: Their mean age was 53.5 ± 10.4 years; there were 18 females. Their mean body mass index (BMI) was 27.6 ± 3.4 kg/m². Six patients had a history of hypertension, 4 patients had a history of diabetes mellitus and 2 patients had both before the scan. Scan findings suggested low probability for coronary artery disease (CAD) in 24 patients and high probability for CAD in three patients. Left anterior descending artery (LAD) territory, posterior descending artery and left circumflex (LCX) artery territory were involved in 22, 8 and 1 patient respectively. The mean time of follow-up was 36.3 ± 4.3 months. None of the patients later had any recurrent episode of chest pain or worsening of symptoms or sentinel events such as death after the scan. Conclusions: Reverse distribution pattern is sometimes observed with 99mTc-MIBI stress MPI. Background: Ultrasonography (US) is the first-line diagnostic tool used to assess fetal musculoskeletal (MSK) anomalies. Associated anomalies in other organ systems may benefit from evaluation via Magnetic Resonance Imaging (MRI). In this study, we compared the diagnostic accuracy of US and MRI to diagnose fetal MSK (primary objective) and non-MSK anomalies (secondary objective). We describe additional findings by low-dose computerized tomography (CT) in two cases incompletely characterized via US and MRI. Materials and Methods: This was an IRB-approved retrospective study of consecutive patients with suspected fetal MSK anomalies examined between December 2015 and June 2020. We compared individual MSK and non-MSK anomalies identified via US, MRI, and CT with postnatal outcomes. Sensitivity and specificity for US and MRI were calculated and compared. Results: A total of 31 patients with 112 MSK and 43 non-MSK anomalies were included. The sensitivity of MRI and US for MSK anomalies was not significantly different (76.6% vs. 61.3%, p = 0.3). Low-dose CT identified eight additional skeletal anomalies. MRI diagnosed a higher number of non-MSK anomalies compared to US (81.4% vs. 37.2%, p < 0.05). Conclusions: Fetal MRI and US have comparable sensitivity for MSK anomalies. In selected cases, low-dose CT may provide additional information. Fetal MRI detected a larger number of non-MSK anomalies in other organ systems compared to US. Multimodality imaging combining all the information provided by MRI, US, and CT, if necessary, ultimately achieved a sensitivity of 89.2% (95% CI: 83.4% to 95.0%) for the diagnosis of musculoskeletal anomalies and 81.4% for additional anomalies in other organs and systems. Purpose: This all-comers registry aimed to assess safety and early efficacy of venous embolization in patients with venogenic erectile dysfunction due to venous leak in an unselected cohort. Methods: Between October 2019 and September 2022, patients with venogenic erectile dysfunction resistant to phosphodiesterase-5-inhibitors were treated with venous embolization using ultrasound-guided anterograde access via a deep dorsal penile vein in a single center. A mix of ethiodized oil and modified cyanoacrylate-based glue n-butyl 2 cyanoacrylate (NBCA) monomer plus methacryloxy-sulpholane monomer (Glubran-2, GEM, Italy) was used as liquid embolic agent. Prior to embolization, venous leak had been verified based on penile duplex sonography and computed tomography cavernosography. Procedural success was defined as technically successful and complete target vein embolization. The primary safety outcome measure was any major adverse event 6 weeks after the procedure. The primary feasibility outcome measure was IIEF-15 (International Index of Erectile Function-15) score improvement ≥ 4 points in ≥ 50% of subjects on 6 weeks follow-up post intervention. Results: Fifty consecutive patients (mean age 61.8 ± 10.0 years) with severe erectile dysfunction due to venous leak underwent venous embolization. Procedural success was achieved in 49/50 (98%) of patients with no major adverse events on follow-up. The primary feasibility outcome measure at 6 weeks was reached by 34/50 (68%) of patients. Conclusion: Venous leak embolization via deep dorsal penile vein access using a liquid embolic agent was safe for all and efficacious in the majority of patients with severe venogenic erectile dysfunction on 6 weeks follow-up. Atmospheric pressure microwave plasma has the lead in synthesizing freestanding and scalable graphene within seconds without the need for high vacuum and temperature. However, the process is limited in utilizing chemical sources for synthesis, such as methane and ethanol. Herein, the usage of an extract of a sustainable precursor, that is, Melaleuca alternifolia, commonly known as tea tree, is for the first time reported to synthesize graphene nanosheets in atmospheric pressure microwave plasma. The synthesis is carried out in a single step at a remarkably low microwave power of 200 W. The morphology, structure, and electrochemical properties of graphene are studied using state‐of‐the‐art characterization techniques such as Raman spectroscopy, X‐ray diffraction, transmission electron microscopy (TEM) and electrochemical impedance spectroscopy. The TEM images reveal the presence of a combination of nanostructures such as nano‐horns, nano‐rods, or nano‐onions consisting of multi‐layer graphitic architectures. An excellent sensing capability of as‐synthesized graphene is demonstrated through the detection of diuron herbicide. A classical symmetrical carrier‐based digital Average Current Mode (ACM) control has two sampling choices, peak point and valley point, to control the average inductor current. If it is employed to regulate the DC‐DC boost converter, there will be a difference in the dynamic performance at peak and valley point sampling instants because of discontinuous output voltage ripple due to Equivalent Series Resistance (ESR) of the output capacitor. Therefore, this paper investigates the effect of sampling instant on the dynamic performance of the PWM DC‐DC boost converter in consideration of capacitor ESR. The small‐signal transfer functions of a boost converter for both sampling positions are prominent to analyze each sampling impact on its dynamics. They are derived by using discrete‐time modeling. Further, the choices of sampling instant are investigated related to the RHP zero elimination, voltage stability, and peak‐to‐peak ripple current of the DC‐DC boost converter. The boost converter has improved stability performance by synchronizing the ADC sampling at starting position (valley point) of the symmetrical carrier signal, as evident by the analytical and hardware results of both sampling techniques working from 25% to 100% load working conditions. The global burden of diabetic kidney disease (DKD) is escalating, and it remains as a predominant cause of the end-stage renal disease (ESRD). DKD is associated with increased cardiovascular disease and morbidity in all types of diabetes. Prediction of progression with albuminuria and eGFR is challenging in DKD, especially in non-proteinuric DKD patients. The pathogenesis of DKD is multifactorial characterized by injury to all components of the nephron, whereas albuminuria is an indicator of only glomerular injury. The limits in the diagnostic and prognostic value of urine albumin demonstrate the need for alternative and clinically significant early biomarkers, allowing more targeted and effective diabetic treatment, to reduce the burden of DKD and ESRD. Identification of biomarkers, based on multifactorial pathogenesis of DKD can be the crucial paradigm in the treatment algorithm of DKD patients. Polling sequences perform resource scheduling based on load requirements, and various methodologies have evolved in recent times for better efficiency. Load adaptive sequence arrangement (LASA) is one of the familiar and efficient methodologies adopted in various methodologies. However, the power consumption and idle time of optical network units are high which should be reduced to attain optimal network performances. The modified LASA is obtained by integrating the first-in-last-out polling sequence such that the proposed arrangement provides a better coverage radius, minimal power cost, maximum access reach, maximum energy saving, and minimal delay and energy cost. Automated synthesis of practical images from the text could be useful and interesting; however, present AI systems are yet far from this objective. Nevertheless, in current years, powerful and generic Recurrent Neural Network (RNN) structures were introduced to train discriminative text feature representation. In the meantime, Deep Convolutional GANs have started producing highly convincing images of specified categories, like room interiors, album covers, and faces. In this research work, we plan to develop a new model for text-to-image synthesis, which contains three important phases: (i) feature extraction, (ii) text encoding, and (iii) optimal image synthesis. Initially, the text features such as improved TF–IDF, bag of words, and N-gram are extracted from the text and they are trained by Bi-LSTM. During the encoding of an image from text, cross-modal feature grouping is performed. Further, the image is synthesized using modified GAN (MGAN) with a new loss function. Here, for precise synthesis of images, the weights of GAN are optimized using Self-improved Social Ski-Driver (SI-SSD) optimization algorithm. Summary: This paper presents the digital realization of the fractional‐order PI (FOPI) controller fractional integrator element using the improved constant phase approximation method. A new pole‐zero rationalization approach is proposed within the bandwidth of the Bode phase plot. The voltage source inverter fed induction motor fractional‐order model has been identified. A novel mathematical FOPI controller design procedure for the identified FO plant model has been proposed. The designed FOPI controller fractional integrator element is implemented using the constant phase approximation method. A detailed comparative performance study of the proposed approximation method with the existing Charef, Oustaloup, and refined Oustaloup methods has been carried out using the Bode, Nyquist, and Nichols plots. The proposed FOPI controller is placed in the speed feedback loop of the rotor field‐oriented control algorithm of IM. To confirm the proposed FOPI controller approximation method's robustness, a hardware study is carried out on the laboratory hardware‐in‐loop (HIL) platform. A single-stage matrix converter (MC) is preferred for the conversion of a single-phase $({1\emptyset })$ supply of fundamental frequency (50 Hz) to the multiphase $({m\emptyset })$ power supply in a limited operating frequency range (5–15 Hz). These converters possess a limitation on account of voltage and current distortions when the converter is used up to fundamental or higher frequency operating loads. To resolve this problem, this letter proposes a dual-stage $1\emptyset \ \text{to}\ m\emptyset $ MC, which ensures improved power quality across the output voltage and current. This converter has a three-leg front-end side rectifier stage (RS) consisting of six numbers of common emitter configured bidirectional power semiconductor switches. A filter circuit is installed in between $1\emptyset $ supply and RS enabling to build positive average dc voltage across the virtual dc link while maintaining the source current profile. The second stage is an inversion stage having a conventional voltage source inverter structure. The proposed dual-stage MC topology is having a simple structure with the ability to convert the $1\emptyset $ supply to any number of desired output phases not only at the lower frequency but also at the fundamental and higher frequency. Purpose: This paper aims to look into the role of transformational leadership and transactional leadership as predictors of employee creativity and organisational innovation. Employee creativity is examined as a potential mediator in the leadership styles–organisational innovation relationships. Design/methodology/approach: A cross-sectional, quantitative design was adopted and structural equation modelling (SEM) techniques were used to analyse data collected from 369 employees working in 39 public coffee enterprises in Vietnam. Findings: Transformational leadership and transactional leadership were significant predictors of employee creativity and organisational innovation. Specifically, transformational leadership was instrumental to employee creativity and organisational innovation while transactional leadership was detrimental to these two variables. Additionally, employee creativity partially mediated the relationships between the two leadership styles and organisational innovation. Practical implications: Results of this study benefit the management of organisations and policy makers by providing an insight of which leadership style will effectively suit public enterprises to promote employee creativity and foster organisational innovation. Originality/value: While there is a lack of studies investigating organisational innovation in organisational methods and that the interrelationships between leadership styles, employee creativity and organisational innovation are not fully understood, this study pioneers in examining relationships between leadership styles and organisational innovation that is being mediated by employee creativity. Figuring out that organisational innovation is more likely to be fostered by the positive influence of leadership behaviours and the improvement of employee creativity, in particular, the significant role of employee creativity represents important contributions of the current study. Congenital talipes equinovarus is one of the most prevalent birth defects, affecting approximately 0.6 to 1.5 children per 1000 live births. Currently, the Ponseti method is the gold-standard treatment for idiopathic clubfeet, with good results reported globally. This literature review focuses on common errors encountered during different stages of the management of idiopathic clubfeet, namely diagnosis, manipulation, serial casting, Achilles tenotomy, and bracing. The purpose is to update clinicians and provide broad guidelines that can be followed to avoid and manage these errors to optimize short- and long-term outcomes of treatment of idiopathic clubfeet using the Ponseti method. A literature search was performed using the following keywords: "Idiopathic Clubfoot" (All Fields) AND "Management" OR "Outcomes" (All Fields). Databases searched included PubMed, EMBASE, Cochrane Library, Google Scholar, and SCOPUS (age range: 0–12 months). A full-text review of these articles was then performed looking for "complications" or "errors" reported during the treatment process. A total of 61 articles were included in the final review: 28 from PubMed, 8 from EMBASE, 17 from Google Scholar, 2 from Cochrane Library, and 6 from SCOPUS. We then grouped the errors encountered during the treatment process under the different stages of the treatment protocol (diagnosis, manipulation and casting, tenotomy, and bracing) to facilitate discussion and highlight solutions. While the Ponseti method is currently the gold standard in clubfoot treatment, its precise and intensive nature can present clinicians, health care providers, and patients with potential problems if proper diligence and attention to detail is lacking. Recurrent respiratory papillomatosis (RRP) is one of the most common laryngeal lesions in children. A 14-year-old female presented with complaints of a change in voice and noisy breathing. She underwent excision of laryngeal papillomatosis thrice before. Fibreoptic laryngoscopy revealed that the lesion was occluding the entire glottic chink except for a small opening through which she was breathing. As there was a risk of seeding of the distal airway following tracheostomy, the initial plan of tracheostomy was deferred. Anaesthesia was induced with sevoflurane. C-MAC™ laryngoscopy revealed that the entire glottic chink was occupied with the papillomatous lesion, and no opening was visualised. The lesion could not be negotiated with a 4 mm endotracheal tube and started bleeding. Immediately, surgeon was asked to perform a tracheostomy. With each recurring incidence of RRP, the probability of the papilloma's non-yielding nature may increase, making the negotiation of the lesion impossible. [ABSTRACT FROM AUTHOR]

Multilevel inverters play a vital role in the industrial and renewable energy sectors due to their flexibility in synthesizing sinusoidal waveforms using a low-pass filter with a medium voltage range. It has several drawbacks, such as a higher number of power component requirements and voltage balancing problems. In this study, a new structure for multilevel inverters has been developed to offer good power quality with minimum number of switching devices and gate driver circuits. The proposed structure is configured to operate in symmetrical and asymmetrical configurations for single/three-phase versions. A simulation study using MATLAB/Simulink has been utilized to study the operating characteristics under both configurations. Chronic pain is a frequent, intricate, and adverse condition that has a considerable influence on individuals and society at large. In India, its prevalence is around 20%. Although a spectrum of conservative treatment modalities is available, a significant proportion of patients with chronic pain syndromes remain refractory and require surgical intervention. In these groups of patients, radiofrequency ablation (RFA) techniques are safe minimally invasive treatments and provide significant and durable pain relief. Thus, we aimed to formulate the Indian Society for the Study of Pain (ISSP) guidelines for the management of chronic pain syndromes with various RFA techniques. An in-depth literature review by experts in Pain Medicine practising in India, was used to produce 16 statements across 4 common chronic pain syndromes, including knee pain, headache and facial pain, lumbar facet joint pain, and sacroiliac joint pain. The quality of evidence was assessed with the Third US Preventive Service Task Force guidance document and the strength of the recommendation was determined by the Delphi consensus process. The level of evidence for most of the statements was I. Moreover, for most statements, the level of agreement between the experts was good (≥80% of the experts). The ISSP guidelines for the management of chronic pain syndromes are developed by experts in pain medicine. For most of the statements, the highest level of evidence was available and inter-expert agreement was good.
