Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), can manifest in multiple organs. While the primary manifestations of COVID-19 occur in the respiratory system, other organ systems are also involved, including nervous systems that cause neurological symptoms. The aim of this study was to determine the neurological manifestations of COVID-19 patients and to assess the role of age and sex on neurological manifestation incidence. A cross-sectional study was conducted at Pelamonia Hospital, Makassar, Indonesia, among inpatient COVID-19 cases, using a total sampling method. Demographic data and neurological manifestations of the COVID-19 patients were collected. The associations between age and sex with the incidence of neurological symptoms were analyzed using the Chi-squared test. Out of 424 inpatients with COVID-19 cases, 62.3% were females, with the highest age group was 20–40 years (42.7%). The neurological symptoms were reported in 232 patients, accounting for approximately 54.7%. The most frequently reported neurological symptom was headache (n=104, 44.8%), followed by anosmia (n=44, 18.9%), ageusia (n=48, 20.6%), myopathy (n=14, 6%), stroke (n=10, 4.3%), seizure (n=5, 2.1%), and altered consciousness (n=7, 3%). An association was found between sex and the incidence of headache, myopathy, stroke, and altered consciousness. There was also an association between age and the incidence of headache and stroke. The study highlights that COVID-19 patients commonly exhibit neurological implications affecting the central nervous system and peripheral nervous system. Therefore, it is crucial for the early detection of neurological symptoms in COVID-19 cases to have better management.

Streptococcus equi, a Lancefield group C b-hemolytic streptococcus first identified in 2004, is a subspecies. Zooepidemicusrare in humans, unlike other streptococcus sp, this gram-negative agent is considered a normal commensal of the equine respiratory tract and is generally associated with some diseases. Streptococcus equi is able to evade phagocytes, this is specifically associated with a capsule of hyaluronic acid and surface proteins. Cell surface antigens contribute to virulence and can help serological diagnosis, the first case of human infection was reported in 2007, we report a previously healthy 69-yearold male patient who was admitted to the emergency room with neurological symptoms of hemiparesis drowsiness with need for mechanical ventilation with fever and chills, which in blood cultures showed growth of Streptococcus equi at 20 hours, relatives were questioned in contact with unpasteurized milk, treatment with culture-guided crystalline penicillin was interrupted, an echocardiogram was performed that showed an image suggestive of rounded vegetation with an area of 4 cm2, adhered to the anterior leaflet of the mitral valve. Subsequently, the patient became hemodynamically unstable, requiring vasopressor support at increasing doses, and died., Streptococcus equi, un estreptococo b-hemolítico del grupo C de Lancefield identificado por primera vez en 2004, es una subespecie. Los antígenos de superficie celular contribuyen a la virulencia y pueden ayudar al diagnóstico serológico, el primer caso de infección humana se reportó en 2007, reportamos un paciente masculino de 69 años previamente sano que ingresó a urgencias con síntomas neurológicos de hemiparesia somnolencia con necesidad por ventilación mecánica con fiebre y escalofríos, que en hemocultivos mostró crecimiento de Streptococcus equi a las 20 horas, se interrogó a familiares en contacto con leche no pasteurizada,se interrumpió tratamiento con penicilina cristalina guiada por cultivo, se realizó ecocardiograma que mostró imagen sugestiva de Vegetación redondeada de 4 cm2 de área, adherida a la valva anterior de la válvula mitral. Posteriormente, el paciente se tornó hemodinámicamente inestable, requiriendo soporte vasopresor a dosis crecientes, y falleció.

Between 2016 and 2017, several U.S. Diplomats in Havana, Cuba, experienced perplexing vestibular and neurological symptoms attributed to an unknown source. They presented with significant vestibular and headache symptoms similar to individuals who experience vestibular migraine (VM). As such, we hypothesize that VM may be a possible explanation for the Havana cohort's presenting symptoms. To evaluate this hypothesis, we compared symptoms reported by the affected individuals in Havana, Cuba, to symptoms reported by corresponding patients from a tertiary academic neurotology clinic with a chief complaint of vertigo or disequilibrium, who met the International Classification of Headache Disorders criteria for VM. The prevalence of the Havana subjects experiencing various symptomatic domains was compared with the VM cohort, leading to 26 unique domains and statistical comparisons between the cohorts. Of the 26 domains compared between the two cohorts, 18 were not significantly different. This included the two most important components of meeting criteria for VM, namely the co-existence of headache and vestibular symptoms. On regression analysis, the only feature which kept its significant difference between the two cohorts was acute intense head pressure (P = 0.007). The domains with similar occurrence ratios included dizziness, headache, light sensitivity, hearing reduction, and tinnitus. In other words, multiple headache and vestibular symptoms, consistent with VM criteria, were similar between the two cohorts. The considerable similarities across various domains between VM patients and Havana subjects could be due to migraine as a possible common etiology for both groups. We propose VM as a potential etiology for the symptomatology in the U.S. diplomats in Cuba.

Patients with essential thrombocythemia (ET) often suffer from neurological symptoms (NS) not ever resulting from previous thrombotic cerebral events (TCE). We reported NS occurred in 282 patients, in order to identify the factors influencing ET-related NS in the absence of TCE, and the response to therapy. Overall, 116 of 282 patients (41%) presented NS; 101 of them (87%) reported subjective transient and fluctuating NS, without concurrent TCE, which we defined as ET-related NS, by frequency: cephalalgia, chronic paresthesias, dizziness or hypotension, visual disturbances, and tinnitus. In univariate analysis, ET-related NS resulted more frequently in young people (P = 0.017) and in females (P = 0.025). We found a higher prevalence of JAK2V617F mutation in ET-related NS patients (P = 0.021). In multivariate analysis, gender (P = 0.024) and JAK2V617F mutation (P = 0.041) remained significantly associated with the development of ET-related NS, with a risk of about four times higher for JAK2V617F-mutated patients (OR = 3.75). Ninety-seven of 101 patients with ET-related NS received an antiplatelet (AP) agent at the time of NS, whereas only selected high-risk ET-related NS patients were treated with a cytoreductive drug, according to the published guidelines and similarly to patients without NS. We observed that only 32 of 97 (33%) patients with ET-related NS achieved a complete response after AP treatment. Among the 65 non-responder patients, 36 (55.4%) improved NS after the introduction of cytoreductive therapy; therefore, the addition of cytoreductive treatment should be considered in this setting.
