Granular activated carbon (GAC) and powdered activated carbon (PAC) are the two most common forms of activated carbon with varying particle size range. The goal of this study was to analyze the key factors (particle size and activation temperature) affecting the surface characteristics of GAC and PAC derived from palm kernel shell (PKS). The surface morphology suggested that the pore network in PKS-PAC is more developed than that in PKS-GAC owing to the presence of a micropore structure when prepared at lower activation temperatures. This study also demonstrated that activation performed on different PKS particle size range influenced the mesopore domain of the activated carbon produced. The PKS-GAC was found to have more mesopores than PKS-PAC, which made it more reliable for dye adsorption in water treatment. Overall, this study demonstrated that applying different particle size range of PKS during the activation process can have a significant influence on the surface characteristics, thus having a direct impact on the application of activated carbon. An accurate particle size range will ensure that the most cost-effective activated carbon is selected to achieve the desired performance objectives.

Quantum oscillations in resistivity and magnetization at high magnetic fields are a macroscopic fingerprint of the energy quantization due to the cyclotron motion of quasiparticles. In a thin Weyl semimetal, a unique thickness dependent Weyl-orbit quantum oscillation was proposed to exist, originating from a nonlocal cyclotron orbit via the electron tunneling between the top and bottom Fermi-arc surface states. Here, untwinned and high crystalline Weyl metal SrRuO$_3$ thin films with different thicknesses were grown on miscut SrTiO$_3$ (001) substrates. Magneto-transport measurements were carried out in magnetic fields up to 35 T, and quantum oscillations with different frequencies were observed and compared to the calculated band structure. In particular, we discovered a frequency $F \approx$ 30 T at low temperatures and above 3 T that corresponds to a small Fermi pocket with a light effective mass. Its oscillation amplitude appears to be at maximum for film thicknesses in a range of 10 to 20 nm, and the phase of the oscillation exhibits a systematic change with the film thickness. After isolating the well separated frequencies, the constructed Landau fan diagram shows an unusual concave downward curvature in the 1/$\mu_0H_n$-$n$ curve, where $n$ is the Landau level index. Based on the rigorous analysis of the thickness and field-orientation dependence of the quantum oscillations, the oscillation with $F \approx$ 30 T is attributed to be of surface origin, which is related to the Fermi-arc surface state originating from non-overlapping Weyl nodes projected on the film's surface plane. This research investigated the relationship between smartphone addiction (SA) and autism quotient, considering the indirect effect of theory of mind (ToM). This research successfully recruited 399 smartphone users to complete self-reported measures for SA, ToM, and autism quotient. Initial results with Pearson's correlation revealed a significant negative relationship between excessive smartphone use and ToM. A significant negative correlation was observed between ToM and the gauged autism quotient. However, the measured SA did not correlate significantly with the autism quotient. From the path analysis, the measured SA did not predict autism quotient and ToM significantly, whereas the measured ToM significantly predicted autism quotient. The negative relationship supports that individuals with established ToM are less likely to develop characteristics of autism. The estimated indirect effect was not significant, implying that ToM did not mediate the direct effect of SA on autism quotient. Implications and limitations are further discussed in this manuscript.

Fitting a time series model to the process data before applying a control chart to the residuals is essential to fulfill the basic assumptions of statistical process control (SPC). Autoregressive integrated moving average (ARIMA) model has been one of the well-established time series modeling approaches that is extensively used for this purpose and is widely recognized for its accuracy and efficiency. Nevertheless, the research community commented that its iterative stages are laborious and time-consuming. In addressing this gap, a novel time series modeling technique with its conceptual assumptions of attributes that was derived from the geometric Brownian motion (GBM) law was developed in this study. It was termed as the logarithmic return (LR) model. Then, the model was employed and tested on a real-world autocorrelated data, whereby the results were assessed and benchmarked with the ARIMA model. The findings for LR model reported a mean average percentage error that ranged between 1.5851% and 3.3793% (less than 10%), which were as accurate as the ARIMA model. The running time (in second of CPU time) taken by the LR model was at least 96.2% faster than the ARIMA model. Interestingly, the corresponding multivariate control chart constructed from the LR model also portrayed a similar general conclusion as that of its counterpart. The LR model was obviously parsimonious and easier to compute and took a shorter running time than the ARIMA model. Therefore, it possessed the potential as an alternative time series modeling methodology for the ARIMA model in the procedures of SPC.

Blastocystis sp. has been considered as an opportunistic intestinal parasite particularly in immunocompromised patient. Recent findings of the predominance of Blastocystis subtypes infection in the clinical disease with its interaction with gut microbiota in the hosts will be discussed accordingly. A total of 57 eligible studies, published from 2010 to October 2020 from a broad search in electronic databases were accessed. The studies showed that bloating, abdominal pain and diarrhoea were among the common symptoms in Blastocystis sp. infection in the immunocompromised patients. However, asymptomatic and healthy individuals were also infected by the Blastocystis sp. with higher prevalence among healthy individuals. Specifically, Blastocystis sp. ST3 were most frequently discovered among immunocompromised patients (IBS, cancer, transplant, HIV/AIDS, dengue) followed by ST1 and ST2. Despite most studies suggesting that Blastocystis sp. promote a healthy gut, a few studies had suggested otherwise. Blastocystis sp. colonisation may modify the gut microbiota with the reduction of beneficial bacteria phyla such as Firmicutes and Bacteroides. Although there was a positive association between Blastocystis sp. subtypes and clinical diseases, more studies are needed. To understand the pathogenicity of Blastocystis sp., their interactions with the gut microbiota communities in humans are properly discussed.

Few studies have assessed the efficacy and safety of reconstruction of sternal infection using a pectoralis muscle flap combined with a rectus abdominis muscle (RAM) sheath fasciocutaneous flap. We report here our experience with this procedure to reconstruct the sternal defect in patients (n = 46) with a deep sternal wound infection (DSWI) after cardiac surgery. After wound reconstruction, the proportion of prolonged mechanical ventilation use and intensive care unit (ICU) stay were 17.4% (n = 8) and 21.7% (n = 10), respectively. The 30-day all-cause mortality was 15.2%; recurrence rate was 17.4%; postoperative complications were 15.2%; and median hospital stay was 31 (0-157) days. Multivariate logistic regression analysis revealed that hypertension (β = 21.32, 95%CI 4.955-37.68, P = .014), drainage-tube use (β = 0.944, 95%CI 0.273-1.614, P = .008), and prolonged intensive care unit stay (β = 53.65, 95%CI 31.353-75.938, P .001) were significantly correlated with hospital stay. In conclusion, a procedure including surgical debridement, sternal reconstruction with bilateral PM and RAM sheath flap, long-term antibiotics, and adequate drainage is a beneficial technique in the reconstruction of deep sternal wound infection after cardiac surgery. Duration of drainage tube use may be as an index for a hospital stay or wound healing.

This research investigated the relationships of the Dark Triad, impulsiveness, and social network sites (SNS) addiction with adolescents. In particular, this research examined the indirect effects of the measured motor, attentional, and nonplanning impulsiveness on the relationship of the Dark Triad and SNS addiction. To examine the possible antecedents of SNS addiction, 169 adolescents aged 14-17 were recruited for this research. After obtaining informed consents from their legal guardians, these adolescents completed the Dirty Dozen, the modified Bergen Facebook Addiction Scale, and the Barratt Impulsiveness Scale – version 11. Results showed that the Dark Triad composite score was positively correlated with SNS addiction and the different aspects of impulsiveness, while SNS addiction was positively correlated with motor impulsiveness. The indirect effect of motor impulsiveness on the relationship of the Dark Triad and SNS addiction was significant. The present findings provide theoretical insights to the development of SNS addiction.

IntroductionKawasaki disease (KD) is an acute idiopathic systemic vasculitis with a self- limiting course that predominantly affects children under 5 years old, particularly in the East Asian countries. Nevertheless, to date, the data on KD in Malaysia are limited. This study aimed to evaluate the epidemiology, clinical features, treatment, and outcomes of KD among the pediatric patients admitted to Hospital Canselor Tunku Muhriz (HCTM), Kuala Lumpur, Malaysia.MethodA retrospective cohort study of 66,500 pediatric patients presented at HCTM from the year 2004 to 2021 was conducted.Results62 KD cases out of 66,500 pediatric admissions were reported, with a male-to-female ratio of 1.58 to 1. Majority of KD patients (95.0%) were younger than 5 years old. Prior infection was reported in 5 KD patients (8.1%). Apart from the classical features, manifestations of various organ systems including cardiovascular (16.1%), gastrointestinal (43.5%), neurological (1.61%), musculoskeletal (1.61%), and genitourinary (17.7%) systems were observed. These findings highlighted the significant contribution of laboratory parameters to the prognosis of KD, prompting more in-depth research on the KD scoring systems and their relevance in this country.

Background and objectivesThe high transmissibility of SARS-CoV-2 has exposed weaknesses in our infection control and detection measures, particularly in healthcare settings. Aerial sampling has evolved from passive impact filters to active sampling using negative pressure to expose culture substrate for virus detection. We evaluated the effectiveness of an active air sampling device as a potential surveillance system in detecting hospital pathogens, for augmenting containment measures to prevent nosocomial transmission, using SARS-CoV-2 as a surrogate.MethodsWe conducted air sampling in a hospital environment using the AerosolSenseTM air sampling device and compared it with surface swabs for their capacity to detect SARS-CoV-2.ResultsWhen combined with RT-qPCR detection, we found the device provided consistent SARS-CoV-2 detection, compared to surface sampling, in as little as 2 h of sampling time. The device also showed that it can identify minute quantities of SARS-CoV-2 in designated “clean areas” and through a N95 mask, indicating good surveillance capacity and sensitivity of the device in hospital settings.ConclusionActive air sampling was shown to be a sensitive surveillance system in healthcare settings. Findings from this study can also be applied in an organism agnostic manner for surveillance in the hospital, improving our ability to contain and prevent nosocomial outbreaks.

BackgroundThis meta-analysis aimed at evaluating the efficacy of cognitive behavior therapy (CBT) against osteoarthritis-associated symptoms in patients with knee/hip osteoarthritis.MethodsMedline, PubMed, Cochrane Library, and EMBASE databases were searched from inception to July 2022 to identify randomized controlled trials (RCTs) comparing the efficacy of CBT with other treatment approaches in adults with confirmed knee/hip osteoarthritis. The pain intensity (primary outcome) and the secondary outcomes including insomnia severity, sleep efficiency, physical function as well as the severity of depression and fatigue were assessed at two time points (i.e., immediately after treatment and during the follow-up period). The effect size is expressed as standardized mean difference (SMD) with SMDs of < 0.2, 0.2–0.5, and 0.5–0.8, and > 0.8 representing negligible, small, medium, and large effect sizes, respectively.ResultsFifteen RCTs were included for analysis. Immediately after CBT intervention, meta-analysis showed similar treatment effect in pain severity [SMD = –0.46, 95% confidence interval (CI): –0.95 to 0.04, 11 studies, 1557 participants] and other symptoms including depression (SMD = –0.26, 95% CI: –0.58 to 0.06, five studies, 735 participants), fatigue (SMD = –2.44, 95% CI:–6.53 to 1.65, two RCTs, 511 participants), and physical function (SMD = –0.11, 95% CI:–0.25 to 0.02, five RCTs, 720 participants) between CBT and control groups, while there was an improvement in insomnia severity (SMD = –0.65, 95% CI: –1.06 to –0.24, four RCTs, 639 participants, medium treatment effect) and sleep efficiency (SMD = 0.32, 95% CI: 0.04 to 0.59, three RCTs, 352 patients, small treatment effect). During follow-up, CBT improved pain severity (SMD = –0.52, 95% CI: –1.03 to –0.01, eight studies, 1447 participants, medium treatment effect), insomnia (SMD = –0.43, 95% CI: –0.85 to –0.01, three RCTs, 571 participants, small treatment effect), and depression (SMD = –0.39, 95% CI: –0.59 to –0.18, four RCTs, 791 participants, small treatment effect). Nevertheless, sleep efficiency, fatigue, and physical function were not improved in the follow-up period.ConclusionOur results may suggest the durability of CBT-associated treatment benefits, supporting its role as a potential promising alternative or complementary intervention for patients with knee/hip osteoarthritis, especially against pain and insomnia. Background The COVID-19 pandemic has changed the epidemiology of upper respiratory tract infections (URTI) and the disease profile of patients attending the emergency department (ED). Hence, we sought to explore the changes in ED physicians’ attitudes and behaviours in four EDs in Singapore. Methods We employed a sequential mixed-methods approach (quantitative survey followed by in-depth interviews). Principal component analysis was performed to derive latent factors, followed by multivariable logistic regression to explore the independent factors associated with high antibiotic prescribing. Interviews were analysed using the deductive-inductive-deductive framework. We derive five meta-inferences by integrating the quantitative and qualitative findings with an explanatory bidirectional framework. Results We obtained 560 (65.9%) valid responses from the survey and interviewed 50 physicians from various work experiences. ED physicians were twice as likely to report high antibiotic prescribing rates pre-COVID-19 pandemic than during the pandemic (AOR = 2.12, 95% CI 1.32 to 3.41, p = 0.002). Five meta-inferences were made by integrating the data: (1) Less pressure to prescribe antibiotics due to reduced patient demand and more patient education opportunities; (2) A higher proportion of ED physicians self-reported lower antibiotic prescribing rates during the COVID-19 pandemic but their perception of the overall outlook on antibiotic prescribing rates varied; (3) Physicians who were high antibiotic prescribers during the COVID-19 pandemic made less effort for prudent antibiotic prescribing as they were less concerned about antimicrobial resistance; (4) the COVID-19 pandemic did not change the factors that lowered the threshold for antibiotic prescribing; (5) the COVID-19 pandemic did not change the perception that the public's knowledge of antibiotics is poor. Conclusions Self-reported antibiotic prescribing rates decreased in the ED during the COVID-19 pandemic due to less pressure to prescribe antibiotics. The lessons and experiences learnt from the COVID-19 pandemic can be incorporated into public and medical education in the war against antimicrobial resistance going forward. Antibiotic use should also be monitored post-pandemic to assess if the changes are sustained.

Type-2 diabetes mellitus (T2DM) is a medical condition in which oral medications avail to patients to curb their hyperglycaemia after failed dietary therapy. However, individual responses to the prescribed pharmacotherapy may differ due to their clinical profiles, comorbidities, lifestyles and medical adherence. One approach is to identify similar patients within the same community to predict their likely response to the prescribed diabetes medications. This study aims to present an evidence-based diabetes medication recommendation system (DMRS) underpinned by patient similarity analytics. The DMRS was developed using 10-year electronic health records of 54,933 adult patients with T2DM from six primary care clinics in Singapore. Multiple clinical variables including patient demographics, comorbidities, laboratory test results, existing medications, and trajectory patterns of haemoglobin A1c (HbA1c) were used to identify similar patients. The DMRS was evaluated on four groups of patients with comorbidities such as hyperlipidaemia (HLD) and hypertension (HTN). Recommendations were assessed using hit ratio which represents the percentage of patients with at least one recommended sets of medication matches exactly the diabetes prescriptions in both the type and dosage. Recall, precision, and mean reciprocal ranking of the recommendation against the diabetes prescriptions in the EHR records were also computed. Evaluation against the EHR prescriptions revealed that the DMRS recommendations can achieve hit ratio of 81% for diabetes patients with no comorbidity, 84% for those with HLD, 78% for those with HTN, and 75% for those with both HLD and HTN. By considering patients’ clinical profiles and their trajectory patterns of HbA1c, the DMRS can provide an individualized recommendation that resembles the actual prescribed medication and dosage. Such a system is useful as a shared decision-making tool to assist clinicians in selecting the appropriate medications for patients with T2DM.

The histone variant H2AZ is overexpressed in diverse cancer types where it facilitates the accessibility of transcriptional regulators to the promoters of cell cycle genes. However, the molecular basis for its dysregulation in cancer remains unknown. Here, we report that glioblastomas (GBM) and glioma stem cells (GSCs) preferentially overexpress H2AZ for their proliferation, stemness and tumorigenicity. Chromatin accessibility analysis of H2AZ2 depleted GSC revealed that E2F1 occupies the enhancer region within H2AZ2 gene promoter, thereby activating H2AZ2 transcription. Exploration of other H2AZ2 transcriptional activators using a customized “anti-H2AZ2” query signature for connectivity map analysis identified STAT3. Co-targeting E2F and STAT3 synergistically reduced the levels of H2AZ, histone 3 lysine 27 acetylation (H3K27ac) and cell cycle gene transcription, indicating that E2F1 and STAT3 synergize to activate H2AZ gene transcription in GSCs. Remarkably, an E2F/STAT3 inhibitor combination durably suppresses GSC tumorigenicity in an orthotopic GBM xenograft model. In glioma patients, high STAT3 signaling is associated with high E2F1 and H2AZ2 expression. Thus, GBM has uniquely opted the use of E2F1- and STAT3-containing “enhanceosomes” that integrate multiple signaling pathways to achieve H2AZ gene activation, supporting a translational path for the E2F/STAT3 inhibitor combination to be applied in GBM treatment.

An integrated and feasible approach was proposed using the underutilized grass fibre (stem) derived from Napier grass and sugarcane for paper production in this study. To enhance paper strength, pre-hydrolysis and beating techniques have been used to improve the chemical pulps and mechanical pulping process, respectively. Napier grass and sugarcane are promising non-wood sources for pulp production, owing to their high cellulose and low lignin and extractive content. With the additional mild alkaline pre-treatment to the mechanical pulping process, the lignin content was greatly reduced. The results reveal that the mechanical pulping with alkaline pre-treatment may indeed potentially replace the most prevalent pulping process (chemical pulping). As evidenced by the paper strength properties, mechanical pulping is far more suitable for grass-type biomass, particularly Napier grass, which had a folding endurance capability five times greater than chemical pulping. Furthermore, the remaining high hemicellulose content from mechanical pulping contributed to a high pulp yield, while also facilitating the fibrillation on the sugarcane’s laboratory paper handsheet. The findings also demonstrated that the additional beating process from chemical pulping causes the fibres to be drawn toward each other, resulting in a more robust fibre network that contributes to good paper strength. Consequently, this work sheds new light on the development of advanced paper derived from grass fibre.

BackgroundThe accumulation of fatigue leads to reduced physical, emotional, psychological, and social functions.ObjectivesFermented Prunus mume vinegar (PV) improves fatigue in animals; however, studies in humans have not been conducted. We aimed to examine the effects and safety of consuming fermented PV for 8 weeks on fatigue indices in adults with unexplained fatigue while considering the placebo effect.MethodsA randomized, double-blind, placebo-controlled trial was conducted in adults of >19 years, who were diagnosed with unexplained fatigue for at least 1 month. Eighty participants were randomly assigned to receive daily 70 mL of fermented PV (2.56 mg/g, chlorogenic acid, and 15.3 mg/g, citric acid) or a placebo for 8 weeks. At baseline and 4 and 8 weeks after treatment, the participants were visited for blood tests (liver enzyme, glucose, creatinine, lactate, malondialdehyde [MDA], and creatine kinase [CK]) and questionnaires (Fatigue Severity Scale [FSS], fatigue visual analog scale [VAS], Beck Depression Inventory [BDI], the Korean version of the Brief Encounter Psychosocial Instrument [BEPSI-K], EQ-5D-3L, and EQ-VAS]).ResultsFermented PV supplementation for 8 weeks did not remarkably improve the fatigue indices when compared to placebo. Additionally, differences in fatigue VAS, BDI, BEPSI-K, EQ-5D-3L, EQ-VAS, lactate, CK, and MDA concentrations between the groups were not observed. However, FSS had positively correlated with fatigue VAS, BDI, and BEPSI-K, whereas it was negatively correlated with EQ-5D-3L and EQ-VAS at the baseline and 8 weeks. None of the participants reported adverse events.ConclusionThe efficacy of fermented PV did not exceed the efficacy of placebo in adults with unexplained fatigue.Clinical trial registration[ClinicalTrials.gov], identifier [NCT04319692].

The oyster mushroom (Pleurotus species) is a popular and widely cultivated edible mushroom that can be found worldwide, including in Malaysia. However, its local production is unable to fulfil the market demand, partly due to the limited availability of rubber wood sawdust (RWS) as the conventional cultivation substrate. Furthermore, the palm oil industry in Malaysia generates large volumes of organic by-products that have caused environmental concerns. Therefore, the potential utilisation of oil palm waste-based substrates in order to develop a substitute RWS for Pleurotus ostreatus mushroom production is evaluated in this study, based on their agronomic performance and nutritional properties. Empty fruit bunches (EFBs), oil palm fronds (OPFs), and oil palm trunks (OPTs) were used to formulate the substrates. The control used was 100% RWS. Generally, 100% EFB showed a better agronomic performance, and mushroom growth was 1.9 times faster compared to the control, with a comparable mushroom yield. The crude protein and beta glucan content of mushrooms grown on oil palm by-product-formulated substrates were significantly higher than those grown using the control. Additionally, the number of fruiting bodies, crude protein, and beta glucan content of the mushrooms were positively correlated with potassium in the substrate. Therefore, 100% EFB could be used as a potential substitute for RWS for the cultivation and production of P. ostreatus.

IntroductionUlmus macrocarpa Hance extract (UME) has demonstrated an antilipidemic effect via upregulation of the adenosine monophosphate-activated protein kinase pathway and regulation of lipid metabolism in both laboratory and animal studies. Therefore, we examined the effects and safety of UME on plasma lipids in adults with untreated high, low-density lipoprotein cholesterol (LDL-C) concentrations.Materials and methodsIn the current double-blind placebo-controlled randomized clinical trial, 80 patients with untreated high LDL-C concentrations (130–190 mg/dl) were randomly allocated to either the “UME group” (received 500 mg UME as two capsules per day) or the “Placebo group” (received placebo containing cornstarch as two capsules per day) for 12 weeks. The primary outcome was the change in LDL-C concentration within the 12-week treatment period; secondary outcomes included changes in total cholesterol (TC), triglyceride, high-density lipoprotein cholesterol, apolipoprotein A1, and apolipoprotein B (ApoB) concentrations.ResultsUME over 12 weeks led to a greater decrease in LDL-C, TC, and ApoB concentrations than did the placebo as follows: by 18.1 mg/dl (P < 0.001); 23.3 mg/dl (P < 0.001); 9.3 mg/dl (P = 0.018), respectively. When LDL-C, TC, and ApoB concentrations were expressed as a lsmeans percentage of the baseline concentration, they after 12 weeks of UME had greater % differences compared to the placebo as follows: by 11.9% (P < 0.001); 10.0% (P < 0.001); 8.6% (P < 0.05), respectively. However, no significant inter- and intra-group changes in liver enzyme, free fatty acid, anti-inflammatory marker, and fasting glucose concentrations were observed. We propose a measurement method for sensitive and label-free detections of virus-like particles (VLPs) using color images of nanoplasmonic sensing chips. The nanoplasmonic chip consists of 5×5 gold nanoslit arrays and the gold surface is modified with specific antibodies for spike protein. The resonant wavelength of the 430-nm-period gold nanoslit arrays underwater environment is about 570 nm which falls between the green and red bands of the color CCD. The captured VLPs by the specific antibodies shift the plasmonic resonance of the gold nanoslits. It results in an increased brightness of green pixels and decreased brightness of red pixels. The image contrast signals of (green - red) / (red + green) show good linearity with the surface particle density. The experimental tests show the image contrast method can detect 100-nm polystyrene particles with a surface density smaller than 2 particles/µm2. We demonstrate the application for direct detection of SARS-CoV-2 VLPs using a simple scanner platform. A detection limit smaller than 1 pg/mL with a detection time less than 30 minutes can be achieved.

FCN-437c was escalated from 50 mg (once daily [QD] on days 1–21 of 28-day cycles) to the MTD/RP2D. In the dose-expansion phase, patients with CDK4/6i-treated breast cancer, or KRAS-mutant (KRASmut) non-small-cell lung cancer (NSCLC) received the MTD. Twenty-two patients were enrolled. The most common tumors in the dose-escalation phase (n = 15) were breast, colorectal, and lung (each n = 4 [27.3%]). The dose-expansion phase included five (71.4%) patients with breast cancer and two (28.6%) with KRASmut NSCLC. Twenty (90.9%) participants experienced FCN-437c–related adverse events. Dose-limiting toxicities occurred in two (33.3%) participants (200-mg dose, dose-escalation phase): grade 3 neutropenia and grade 4 neutrophil count decreased. Due to toxicities reported at 150 mg QD, the MTD was de-escalated to 100 mg QD. One (4.5%) participant (KRASmut NSCLC, 100-mg dose) achieved a partial response lasting 724+ days, and five (22.7%) had stable disease lasting 56+ days. In conclusion, FCN-437c was well tolerated with encouraging signs of antitumor activity and disease control. Further exploration of FCN-437c in aSTs is warranted.

Ishige okamura (IO) is a genus of brown algae and plays a role in anti-diabetes, anti-obesity, and myogenesis. However, the effect of IO extract (IOE) on human muscle strength and mass is unclear. Therefore, we will examine the impact and safety of consumption of IOE for 12 weeks on muscle strength and mass in middle-aged and old-aged adults with relatively low skeletal muscle mass.Materials and methodsA randomized controlled trial is conducted on 80 adults aged 50–80. A total of 80 participants will be enrolled in this study. Participants assign IOE-taking group (n = 40) and placebo taking group (n = 40). The growth of SrRuO $$_3$$ 3 (SRO) thin film with high-crystallinity and low residual resistivity (RR) is essential to explore its intrinsic properties. Here, utilizing the adsorption-controlled growth technique, the growth condition of initial SrO layer on TiO $$_2$$ 2 -terminated SrTiO $$_3$$ 3 (STO) (001) substrate was found to be crucial for achieving a low RR in the resulting SRO film grown afterward. The optimized initial SrO layer shows a c(2 $$\times $$ × 2) superstructure that was characterized by electron diffraction, and a series of SRO films with different thicknesses (ts) were then grown. The resulting SRO films exhibit excellent crystallinity with orthorhombic-phase down to $$t \approx $$ t ≈ 4.3 nm, which was confirmed by high resolution X-ray measurements. From X-ray azimuthal scan across SRO orthorhombic (02 ± 1) reflections, we uncover four structural domains with a dominant domain of orthorhombic SRO [001] along cubic STO [010] direction. The dominant domain population depends on t, STO miscut angle ( $$\alpha $$ α ), and miscut direction ( $$\beta $$ β ), giving a volume fraction of about 92 $$\%$$ % for $$t \approx $$ t ≈ 26.6 nm and $$(\alpha , \beta ) \approx $$ ( α , β ) ≈ (0.14 $$^{\mathrm{o}}$$ o , 5 $$^{\mathrm{o}}$$ o ). On the other hand, metallic and ferromagnetic properties were well preserved down to t $$\approx $$ ≈ 1.2 nm. Residual resistivity ratio (RRR = $$\rho ({\mathrm{300 K}})$$ ρ ( 300 K ) / $$\rho ({\mathrm{5K}})$$ ρ ( 5 K ) ) reduces from 77.1 for t $$\approx $$ ≈ 28.5 nm to 2.5 for t $$\approx $$ ≈ 1.2 nm, while $$\rho ({\mathrm{5K}})$$ ρ ( 5 K ) increases from 2.5 $$\upmu \Omega $$ μ Ω cm for t $$\approx $$ ≈ 28.5 nm to 131.0 $$\upmu \Omega $$ μ Ω cm for t $$\approx $$ ≈ 1.2 nm. The ferromagnetic onset temperature ( $$T'_{\mathrm{c}}$$ T c ′ ) of around 151 K remains nearly unchanged down to t $$\approx $$ ≈ 9.0 nm and decreases to 90 K for t $$\approx $$ ≈ 1.2 nm. Our finding thus provides a practical guideline to achieve high crystallinity and low RR in ultra-thin SRO films by simply adjusting the growth of initial SrO layer.

The efficacy of additional water-soluble additives was studied relative to the physical and mechanical properties of particleboards produced from oil palm empty fruit bunch (OPEFB). Polyethylene glycol, acrylamide, and acrylic resin were selected as water-soluble additives for use in the OPEFB particleboard production process. The effects of the three additives at two different concentrations (2% and 4% of dry OPEFB mass) on the particleboard properties were evaluated. Addition of water-soluble additives increased the performance of the OPEFB particleboard. The additive concentration has a significant effect on the properties of the particleboard. With the increase of additive concentration, the internal bonding and modulus of rupture value increased while the thickness swelling and water absorption decreased. Particleboards with an additional 4% of acrylamide or polyethylene glycol achieved the highest modulus of rupture (22 MPa), highest internal bonding strength (1 N/mm2), and lowest thickness swelling (9%). All the particleboards produced with 4% of water-soluble additive achieved the standard requirements of JIS A 5908:2003 for physical and mechanical properties.

The development of low-environmental-impact technologies for the elimination of biological damage is one of the vital goals of the wood protection industry. The possibility of utilizing pyroligneous acid as a wood preservative can be a great solution to extend the application of the currently fast-growing timber species, which has lower natural durability against biological damage. In this study, the effectiveness of pyroligneous acid as a wood preservative was evaluated by impregnating rubberwood with pyroligneous acid using vacuum-pressure treatment, and the treated woods were exposed to mould fungi, wood-decay fungi and termite attacks under laboratory conditions. Pyroligneous acids produced from rubberwood (RWPA) and oil palm trunk (OPTPA) at different pyrolysis temperatures were evaluated. To fully understand the effectiveness of pyroligneous acids as wood preservatives, different concentrations of pyroligneous acids were impregnated into rubberwood. Concentrations of 50% RWPA and 30% OPTPA were sufficient against mould and decay fungi on rubberwood. Rubberwood impregnated with pyroligneous acid acted as a slow-acting toxic bait to cause a high termite mortality rate due to toxic feeding and does not serve as a good repellent to prevent termites from feeding on the wood. In general, OPTPA has better biological durability compared to RWPA.

Objective This study aims to develop a convolutional neural network-based learning framework called domain knowledge-infused convolutional neural network (DK-CNN) for retrieving clinically similar patient and to personalize the prediction of macrovascular complication using the retrieved patients. Materials and Methods We use the electronic health records of 169 434 patients with diabetes, hypertension, and/or lipid disorder. Patients are partitioned into 7 subcohorts based on their comorbidities. DK-CNN integrates both domain knowledge and disease trajectory of patients over multiple visits to retrieve similar patients. We use normalized discounted cumulative gain (nDCG) and macrovascular complication prediction performance to evaluate the effectiveness of DK-CNN compared to state-of-the-art models. Ablation studies are conducted to compare DK-CNN with reduced models that do not use domain knowledge as well as models that do not consider short-term, medium-term, and long-term trajectory over multiple visits. Results Key findings from this study are: (1) DK-CNN is able to retrieve clinically similar patients and achieves the highest nDCG values in all 7 subcohorts; (2) DK-CNN outperforms other state-of-the-art approaches in terms of complication prediction performance in all 7 subcohorts; and (3) the ablation studies show that the full model achieves the highest nDCG compared with other 2 reduced models. Discussion and Conclusions DK-CNN is a deep learning-based approach which incorporates domain knowledge and patient trajectory data to retrieve clinically similar patients. It can be used to assist physicians who may refer to the outcomes and past treatments of similar patients as a guide for choosing an effective treatment for patients.

In the current double-blind placebo-controlled randomized clinical trial, 80 patients with untreated high LDL-C concentrations (130-190 mg/dl) were randomly allocated to either the "UME group" (received 500 mg UME as two capsules per day) or the "Placebo group" (received placebo containing cornstarch as two capsules per day) for 12 weeks. The accumulation of fatigue leads to reduced physical, emotional, psychological, and social functions.FermentedA randomized, double-blind, placebo-controlled trial was conducted in adults of19 years, who were diagnosed with unexplained fatigue for at least 1 month. Eighty participants were randomly assigned to receive daily 70 mL of fermented PV (2.56 mg/g, chlorogenic acid, and 15.3 mg/g, citric acid) or a placebo for 8 weeks. At baseline and 4 and 8 weeks after treatment, the participants were visited for blood tests (liver enzyme, glucose, creatinine, lactate, malondialdehyde [MDA], and creatine kinase [CK]) and questionnaires (Fatigue Severity Scale [FSS], fatigue visual analog scale [VAS], Beck Depression Inventory [BDI], the Korean version of the Brief Encounter Psychosocial Instrument [BEPSI-K], EQ-5D-3L, and EQ-VAS]).Fermented PV supplementation for 8 weeks did not remarkably improve the fatigue indices when compared to placebo. Additionally, differences in fatigue VAS, BDI, BEPSI-K, EQ-5D-3L, EQ-VAS, lactate, CK, and MDA concentrations between the groups were not observed. However, FSS had positively correlated with fatigue VAS, BDI, and BEPSI-K, whereas it was negatively correlated with EQ-5D-3L and EQ-VAS at the baseline and 8 weeks. Previous studies have explored the associations of retinal vessel calibre, measured from retinal photographs or fundus images using semi-automated computer programs, with cognitive impairment and dementia, supporting the concept that retinal blood vessels reflect microvascular changes in the brain. Recently, artificial intelligence deep-learning algorithms have been developed for the fully automated assessment of retinal vessel calibres. Therefore, we aimed to determine whether deep-learning-based retinal vessel calibre measurements are predictive of risk of cognitive decline and dementia. We conducted a prospective study recruiting participants from memory clinics at the National University Hospital and St. Luke’s Hospital in Singapore; all participants had comprehensive clinical and neuropsychological examinations at baseline and annually for up to 5 years. Fully automated measurements of retinal arteriolar and venular calibres from retinal fundus images were estimated using a deep-learning system. Cox regression models were then used to assess the relationship between baseline retinal vessel calibre and the risk of cognitive decline and developing dementia, adjusting for age, gender, ethnicity, education, cerebrovascular disease status, hypertension, hyperlipidemia, diabetes, and smoking. A total of 491 participants were included in this study, of whom 254 developed cognitive decline over 5 years. In multivariable models, narrower retinal arteriolar calibre (hazard ratio per standard deviation decrease = 1.258, P = 0.008) and wider retinal venular calibre (hazard ratio per standard deviation increase = 1.204, P = 0.037) were associated with increased risk of cognitive decline. Among participants with cognitive impairment but no dementia at baseline (n = 212), 44 progressed to have incident dementia; narrower retinal arteriolar calibre was also associated with incident dementia (hazard ratio per standard deviation decrease = 1.624, P = 0.021). In summary, deep-learning-based measurement of retinal vessel calibre was associated with risk of cognitive decline and dementia.

The tongue plays a crucial role in the swallowing process, and impairment can lead to dysphagia, particularly in motor neuron diseases (MNDs) resulting in hypoglossal-tongue axis degeneration (e.g., amyotrophic lateral sclerosis and progressive bulbar palsy). This study utilized our previously established inducible rodent model of dysphagia due to targeted degeneration of the hypoglossal-tongue axis. This model was created by injecting cholera toxin B conjugated to saporin (CTB-SAP) into the genioglossus muscle of the tongue base for retrograde transport to the hypoglossal (XII) nucleus via the hypoglossal nerve, which provides the sole motor control of the tongue. Our goal was to investigate the effect of high-repetition/low-resistance tongue exercise on tongue function, strength, and structure in four groups of male rats: (1) control + sham exercise (n = 13); (2) control + exercise (n = 10); (3) CTB-SAP + sham exercise (n = 13); and (4) CTB-SAP + exercise (n = 12). For each group, a custom spout with adjustable lick force requirement for fluid access was placed in the home cage overnight on days 4 and 6 post-tongue injection. For the two sham exercise groups, the lick force requirement was negligible. For the two exercise groups, the lick force requirement was set to ∼40% greater than the maximum voluntary lick force for individual rats. Following exercise exposure, we evaluated the effect on hypoglossal-tongue axis function (via videofluoroscopy), strength (via force-lickometer), and structure [via Magnetic Resonance Imaging (MRI) of the brainstem and tongue in a subset of rats]. Results showed that sham-exercised CTB-SAP rats had significant deficits in lick rate, swallow timing, and lick force. In exercised CTB-SAP rats, lick rate and lick force were preserved; however, swallow timing deficits persisted. MRI revealed corresponding degenerative changes in the hypoglossal-tongue axis that were mitigated by tongue exercise. These collective findings suggest that high-repetition/low-resistance tongue exercise in our model is a safe and effective treatment to prevent/diminish signs of hypoglossal-tongue axis degeneration. The next step is to leverage our rat model to optimize exercise dosing parameters and investigate corresponding treatment mechanisms of action for future translation to MND clinical trials.

Objectives: Extant research on cost-sharing finds no impact on health care utilization when the amount is insubstantial. This research investigates the effects on nonacute outpatient services for schoolchildren with refractive errors in Taiwan and discusses the potential harm caused by cost sharing and relevant cost containment policies. Methods: Longitudinal claims data from the National Health Insurance database are employed. District demographic information is also used for aggregate-level analyses. Interventional modeling is conducted on pooled individual-level data with a Poisson model and negative binomial models. Generalized least square modeling is performed on aggregate district-level data to elucidate the impacts of cost sharing and the reimbursement rate with controls for patient and institutional characteristics, district socioeconomic factors, and competitiveness among institutions. Results: The findings of this study show that cost sharing does not significantly affect children’s utilization of outpatient services in the patient-level analyses. However, it significantly decreases the service volume based on the results of district aggregate analyses. There are potentially marginal patients in society, and they are more likely to be girls in poorer families, whose chances of seeking medical care significantly decrease when cost sharing increases. Conclusions: The gap in health inequity can be widened when stringent cost-containment policies are implemented. The offset effect caused by delayed care may also result in higher health care expenditures later. Cost sharing for children should be separately and prudently designed to better protect them from deprivations caused by changes in health policies.

ObjectiveCurrent knowledge on the global burden of infant sepsis is limited to population-level data. We aimed to summarize global case fatality rates (CFRs) of young infants with sepsis, stratified by gross national income (GNI) status and patient-level risk factors.MethodsWe performed a systematic review and meta-analysis on CFRs among young infants < 90 days with sepsis. We searched PubMed, Cochrane Central, Embase, and Web of Science for studies published between January 2010 and September 2019. We obtained pooled CFRs estimates using the random effects model. We performed a univariate analysis at patient-level and a meta-regression to study the associations of gestational age, birth weight, onset of sepsis, GNI, age group and culture-proven sepsis with CFRs.ResultsThe search yielded 6314 publications, of which 240 studies (N = 437,796 patients) from 77 countries were included. Of 240 studies, 99 were conducted in high-income countries, 44 in upper-middle-income countries, 82 in lower-middle-income countries, 6 in low-income countries and 9 in multiple income-level countries. Overall pooled CFR was 18% (95% CI, 17–19%). The CFR was highest for low-income countries [25% (95% CI, 7–43%)], followed by lower-middle [25% (95% CI, 7–43%)], upper-middle [21% (95% CI, 18–24%)] and lowest for high-income countries [12% (95% CI, 11–13%)]. Factors associated with high CFRs included prematurity, low birth weight, age less than 28 days, early onset sepsis, hospital acquired infections and sepsis in middle- and low-income countries. Study setting in middle-income countries was an independent predictor of high CFRs. We found a widening disparity in CFRs between countries of different GNI over time.ConclusionYoung infant sepsis remains a major global health challenge. As the products made from wood–plastic composites (WPCs) become more sophisticated and required more detail profiles, the injection moulding processing method with wood pulp as the reinforcing material is the answer to cater to the rapid change and demands of composite products. The general objective of this study was to study the effects of the material formulation, as well as the injection moulding process parameters, on the properties of a polypropylene composite reinforced with chemi-thermomechanical pulp from oil palm trunks (PP/OPTP composite) via the injection moulding process. The PP/OPTP composite with a material formulation of 70% pulp/26% PP/4% Exxelor PO produced using injection moulding at 80 °C as the mould temperature and with 50 tonnes of injection pressure exhibited the highest physical and mechanical properties. The increment loading of pulp increased the water absorption capacity of the composite. Higher loading of the coupling agent effectively reduced the water absorption capacity and increased the flexural strength of the composite. The increase in mould temperature from unheated to 80 °C prevented excessive heat loss of the flowing material, which enabled the molten material to flow better and filled up all cavities in the mould. The increased injection pressure slightly improved the physical properties of the composite, but the effect on the mechanical properties was insignificant. For the future development of WPCs, further studies should be focused on the viscosity behaviour, as a greater understanding of the processing parameters’ effects on the PP/OPTP’s viscosity behaviour will lead to improved product design and enable great potential usage of WPCs.

Social media posts expressing mental illness such as depression are trending in today's society. The aesthetics of dark and depressing posts are highlighted in blogs or social media accounts of young individuals. Consequently, this study investigated why young people use mental illness as an aesthetic on social media. On top of that, this study contributes to the limited body of research on this subject. This study applied a method known as netnography, an analysis of social interactions online. Furthermore, the actor-network theory (ANT) was employed as an analytical tool to explore insights and draw conclusions. Seven mediators were identified as possible causes that prompted mental illness aesthetic use, including Tumblr, media, attention seekers, coping mechanism, victimhood, emo culture, and mental health awareness. These findings provided insights into developing interventions and systematic methods to discuss mental illness properly. This study also identified current challenges in social media research regarding mental illness detection.

Oysters possess an excellent nutritional profile containing γ-aminobutyric acid (GABA). Previous data suggest that GABA is a potent bioactive compound for improving muscle health. Lactic acid fermentation is thought to increase GABA content. However, the effect of oral supplementation of fermented oyster extracts (FO) on human muscle strength and mass is unclear. Therefore, we tested the effects and safety of consumption of FO combined with regular walking for 12 weeks on muscle strength and mass in older adults with relatively low muscle mass.A randomized controlled trial was conducted on 54 adults between 50 and 78 years of age. Participants were randomized to receive either placebo or 1,200 mg FO daily for 12 weeks. By fermentation withOf the 54 participants, 49 completed the trial without reporting adverse effects. FO supplementation over 12 weeks did not cause any increase in knee or grip strength compared to the control group. Also, no differences were observed in the muscle mass, growth hormone, muscle biomarkers, anti-inflammatory markers, and antioxidative markers between the two groups. None of the participants experienced adverse events. Optical response of crystalline solids is to a large extent driven by excitations that promote electrons among individual bands. This allows one to apply optical and magneto-optical methods to determine experimentally the energy band gap - a fundamental property crucial to our understanding of any solid - with a great precision. Here we show that such conventional methods, applied with great success to many materials in the past, do not work in topological Dirac semimetals with a dispersive nodal line. There, the optically deduced band gap depends on how the magnetic field is oriented with respect to the crystal axes. Rigidoporus microporus, which causes white root rot disease (WRD) in Hevea brasiliensis, is a looming threat to rubber plantation in Malaysia. The current study was conducted to determine and evaluate the efficiency of fungal antagonists (Ascomycota) against R. microporus in rubber trees under laboratory and nursery conditions. A total of 35 fungal isolates established from the rubber tree rhizosphere soil were assessed for their antagonism against R. microporus by the dual culture technique. Trichoderma isolates can inhibit the radial growth of R. microporus by 75% or more in the dual culture test. Strains of T. asperellum, T. koningiopsis, T. spirale, and T. reesei were selected to assess the metabolites involved in their antifungal activity. Results indicated that T. asperellum exhibited an inhibitory effect against R. microporus in both volatile and non-volatile metabolite tests. All Trichoderma isolates were then tested for their ability in producing hydrolytic enzymes such as chitinase, cellulase and glucanase, indole acetic acid (IAA), siderophores production, and phosphate solubilization. From the positive results of the biochemical assays, T. asperellum and T. spirale were selected as the biocontrol candidates to be further tested in vivo against R. microporus. The nursery assessments revealed that rubber tree clone RRIM600 pretreated with only T. asperellum or with the combination of T. asperellum and T. spirale was able to reduce the disease severity index (DSI) and exert higher suppression of R. microporus compared to other pretreated samples, with the average DSI below 30%. Collectively, the present study demonstrates that T. asperellum represents a potential biocontrol agent that should be further explored to control R. microporus infection on rubber trees.

The sludge in this study was obtained from refinery crude oil storage tanks. It contained a high proportion of hydrocarbon composition and harmful substances (such as polycyclic aromatic hydrocarbons and benzene). Through the microwave irradiation treatment process, the harmful substances were removed from the sludge which was then recycled and combined with agricultural waste mushroom substrates to produce refuse derived fuel (RDF). The results showed that the calorific value of RDF was 7279 cal/g when the blending ratio (wt/wt) of oil sludge and mushroom substrates was 5:5. On the other hand, when the portion of the mushroom substrates was increased, the sludge became easier to ignite with better combustion reaction. When the blending ratio (wt/wt) was changed from 8:2 to 5:5, the ignition index and comprehensive performance index were increased by 51.9 and 50.2%. Therefore, mixing the sludge with agricultural waste mushroom substrates is in line with the concept of waste recycling and circular economy.

Conventional cancer chemotherapies are not fully efficacious and do not target tumors, leading to significant treatment-related morbidities. A number of genetically attenuated cancer-targeting bacteria are being developed to safely target tumors in vivo. Here we report the toxicological, tumor-targeting, and efficacy profiles of Salmonella enterica serovar Typhimurium CRC2631 in a syngeneic and autochthonous TRAMP model of aggressive prostate cancer. CRC2631 preferentially colonize primary and metastatic tumors in the TRAMP animals. In addition, longitudinal whole genome sequencing studies of CRC2631 recovered from prostate tumor tissues demonstrate that CRC2631 is genetically stable. Moreover, tumor-targeted CRC2631 generates an anti-tumor immune response. Combination of CRC2631 with checkpoint blockade reduces metastasis burden. Collectively, these findings demonstrate a potential for CRC2631 in cancer immunotherapy strategies.

The growth and yield of Pleurotus florida mushroom were evaluated in media with corn husk and wood sawdust substrates. Five formulations of substrates, namely 0%, 25%, 50%, 75%, and 100% of wood sawdust substituted with corn husk, were tested with 0% corn husk or 100% wood sawdust serving as the control. The total number of fruiting bodies, the number of effective fruiting bodies, the total fresh weight and dry weight of the mushroom, and biological efficiency were significantly increased with as low as 25% corn husk in the substrate, and they showed a significant increasing trend as the composition of corn husk in the substrate increased. Superior yield was produced by P. florida cultivated in 100% corn husk, where the total number of fruiting bodies, the number of effective fruiting bodies, the total fresh weight, and the total dry weight of the mushroom were 4.8 times, 5.4 times, 4.6 times, and 5.4 times greater than the control, respectively. The biological efficiency of P. florida increased gradually from 8.8% in the control to 51.37% in the 100% corn husk substrate. Therefore, corn husk could be exploited as a substitute or alternative substrate to wood sawdust for more sustainable production of P. florida.

While microbiomes in industrialized societies are well characterized, indigenous populations with traditional lifestyles have microbiomes that are more akin to those of ancient humans. However, metagenomic data in these populations remains scarce and the association with soil-transmitted helminth infection status is unclear. Here, we sequenced 650 metagenomes of indigenous Malaysians from 5 villages with different prevalence of helminth infections. Individuals from villages with higher prevalence of helminth infections have more unmapped reads and greater microbial diversity. Microbial community diversity and composition were most strongly associated with different villages and the effects of helminth infection status on the microbiome varies by village. Longitudinal changes in the microbiome in response to albendazole anthelmintic treatment was observed in both helminth infected and uninfected individuals. Inference of bacterial population replication rates from origin of replication analysis identified specific replicating taxa associated with helminth infection. Our results indicated that helminth effects on the microbiota was highly dependent on context and effects of albendazole on the microbiota can be confounding for the interpretation of deworming studies. Furthermore, a substantial quantity of the microbiome remains undescribed and this large dataset from indigenous populations associated with helminth infections should facilitate characterization of the disappearing microbiome from developed industrialized societies.
