1. Implementation of Koha in managing the e-resources of the library

Abstract

In the digital era, libraries have been acquiring and subscribing to various types of digital resources. Each e-resource possesses distinct formats and search requirements, offers multiple access and authentication methods, and involves complex licensing agreements. Therefore, effectively managing these diverse e-resources necessitates a system that simplifies the processes of acquisition, access, and organization. An Electronic Resource Management System (ERMS) presents a potential solution for centralizing these operations. Numerous open source and commercial ERMS solutions are available and utilized in libraries worldwide. Nevertheless, numerous studies have revealed that libraries face financial constraints as well as limitations in terms of ICT infrastructure. An ideal solution should be both cost-effective and require minimal ICT infrastructure. Koha is one such software that has gained popularity in library automation, making it a viable option for managing e-resources. This study explores the implementation of Koha, version 22.11, for managing a library's e-resources. The study aims to investigate the existing features and functionalities of Koha in the context of libraries.

5. GC-MS Analysis of Phytoactive Compounds, Antioxidant and Antibacterial Activity of Citrullus lanatus Seeds

Abstract

The current study investigated the therapeutic potential of Citrullus lanatus seeds which are commonly discarded after eating the fruit. In this day and age, plant secondary metabolites are preferred therapeutic agents to manage a variety of diseases and disorders. The present study aimed to investigate the bioactive secondary metabolite profile of Citrullus lanatus seeds by investigating total phenolic and flavonoid content, antioxidant potential, and Gas Chromatography-Mass Spectrometry (GC-MS) analysis of bioactive compounds and anti-bacterial properties of four different crude extracts. Alkaloids, flavonoids, phenols, steroids, tannins, saponins, phytosterols, terpenoids, and glycosides were revealed in the seeds after qualitative phytochemical examination utilizing several solvents of varying polarity and established techniques of analysis. DPPH radical scavenging assay was used to assess the antioxidant potential and the total flavonoid and phenolic contents in seed extracts were determined using the spectrophotometric method. Methanolic extract revealed higher extractive yield, antioxidant potential, a higher total phenolic content (132.68 ± 0.861 mg of GAg), and higher total flavonoid content (48.13 ± 0.451 mg of Qg) as compared to other extracts. Gas Chromatography-Mass Spectrometry (GC-MS) analysis of all four seed extracts revealed the presence of 27 high and low molecular weight chemical entities in toto with varying amounts. These bioactive chemical substances have been revealed to be physiologically significant and essential from a pharmaceutical standpoint. This research demonstrates that the Citrullus lanatus seeds are high in bioactive secondary metabolites that are beneficial to human health, have a high antioxidant capacity, and antibacterial action against certain bacterial strains, indicating that these seeds have a lot of therapeutic value.

6. Imaging characteristics of phosphaturic mesenchymal tumors

Abstract

Background Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome associated with phosphaturic mesenchymal tumors (PMTs). Localization of the causative tumor in these cases is an arduous task since the culprit lesions are usually small, slow-growing, and can be located almost anywhere from head to toe. Purpose To describe the morphological characteristics of histologically proven PMTs on various radiological modalities. Material and Methods After institutional ethical approval, this retrospective study analyzed 20 cases with a histopathological evidence of PMT. Various imaging characteristics of the tumors on available computed tomography (CT) and magnetic resonance imaging (MRI) scans were evaluated. Descriptive statistical analyses were conducted. Results The tumors were located in diverse locations: lower extremities (n = 10); head and neck (n = 5); vertebral column (n = 3); pelvis (n = 1); and upper extremities (n = 1). Bone lesions seen on CT had variable morphology: sclerotic (n = 3/8, 37.5%); lytic (n = 3/8, 37.5%), and both lytic and sclerotic (n = 2/8, 25%) with presence of narrow zone of transition in all cases (n = 8/8) and amorphous internal matrix calcifications in 25% of cases (n = 2/8). Of the tumors, 68.4% (n = 13/19) were hypointense on T1 and all of them showed hyperintense signal on T2-weighted and STIR images (n = 19/19) and contrast enhancement (n = 16/16). Of the tumors, 66.7% (n = 6/9) showed restricted diffusion. DOTANOC PET/CT showed tumor uptake in all cases (n = 8/8). Conclusion PMTs may have variable and non-specific tumor appearances on various imaging modalities. However, in an appropriate clinical scenario and a background of suggestive biochemical work-up, the radiologist should keep a high index of suspicion.

8. Impact of Structured Reporting of Skeletal Survey in Skeletal Dysplasia: A Single Institution Experience

Abstract

Background Structured reporting has the advantages of reducing ambiguity in written radiology reports with greater uniformity and comparability of reports amongst different institutes. It has multiple facets: structured format, structured content, and standardized language. While structured reporting initiative has been used in various radiology subspecialties such as oncology, cardiothoracic, abdominal and interventional radiology; skeletal dysplasia is a domain that remains largely untouched by this concept. Purpose To evaluate the impact of structured reporting in skeletal dysplasia. Methods and Materials This was an ethically approved pragmatic clinical trial. A defined number (75) of clinically diagnosed and/or genetically confirmed skeletal dysplasia radiographs were evaluated by two radiologists (reader A and reader B) with 5-and 7-years' experience in general radiology, respectively. A pre-defined structured reporting format for skeletal dysplasia was used as an interventional tool. Both the readers interpreted the radiographs before and after the training session. In addition to diagnosis, diagnostic confidence was noted using a semiquantitative scale. Improvement in diagnostic accuracy and diagnostic confidence after training were assessed. McNemar's test was used to assess the statistical significance of difference in proportion of correct diagnoses in pre- and post-education phases. An interrater reliability analysis using the Kappa statistic was performed to determine interobserver agreement between readers both in pre- and post-education phases. Results In the post-education phase, the proportion of accurate diagnosis improved from 48% (36/75) to 64% (48/75) for reader A, and from 44% (33/75) to 60% (45/75) for reader B as compared with the pre-education phase. Amongst the cases with a correct radiologic diagnosis, an increase in diagnostic confidence was noted in 18 cases for reader A, and 15 cases for reader B. In none of the cases, there was a reduction in diagnostic confidence after training. A McNemar's test determined that there was a statistically significant difference in the proportion of correct diagnoses in pre- and post-education phases, p Conclusion A structured reporting of skeletal survey can improve accuracy and confidence in diagnosing skeletal dysplasia.

9. A comparative study on the effect of intravenous Ringer lactate at 125 ml/hour versus 250 ml/hour on the duration of labour in nulliparous pregnant women

Abstract

Background: Several factors may influence the progression of normal labour. It has been postulated that the routine administration of intravenous fluids to keep women adequately hydrated during labour may reduce the period of contraction and relaxation of the uterine muscle, and may ultimately reduce the duration of the labour. However, the routine administration of intravenous fluids to labouring women has not been adequately studied although it is widely practiced, and there is no consensus on the type or volume of fluids that are required, or indeed, whether intravenous fluids are at all necessary. This study was conducted to study the effect of intravenous Ringer lactate at 125 ml/hour versus 250 ml/hour on the duration of labour. Methods: In the study, patients were divided in two groups of nulliparous pregnant women in spontaneous early labour (between 3-5 cm dilatation), at term with singleton pregnancy and cephalic presentation, where one group received intravenous Ringer lactate at 125 ml/hour and the other group received Ringer lactate at 250 ml/hour. The duration of labour, mode of delivery and caesarean rates were measured in both groups along with incidence of prolonged labour and need for oxytocin augmentation. Results: The incidence of prolonged labour and oxytocin augmentation were less in the group receiving intravenous RL at 250 ml/hour but was not statistically significant. Remaining outcomes were comparable in both the groups with no outcome reaching statistical significance. Conclusions: Rate of fluid administration is one of the many variables which affects the labour outcomes. The incidence of prolonged labour and oxytocin augmentation was less in the group which received intravenous RL 250 ml/hour. There was no statistically significant difference in both groups in respect of duration of labour, mode of delivery and caesarean rates.

Abstract

Objective: To find out the prevalence of sexually transmitted diseases (STDS) among the patients attending the tertiary care center through microscopic screening. Methods: This Cross Sectional study was conducted from April 2018 to December 2019 wherein 500 samples were collected from patients attending the STD clinic of Skin and VD-OPD, and integrated counseling and testing center (ICTC), having cervical or urethral discharge along with signs and symptoms of sexually transmitted infections (STI) and were screened microscopically for the same. Results: This study reported only 2 cases suggestive of N. gonorrhoeae on microscopy, wherein the Gram-stained smear showed the presence of a particular arrangement of Gram-negative coffee bean shape cocci, both intracellular and extracellular, and plenty of pus cells. In Direct Microscopy findings, 49.2% of samples showed normal flora, 20.4% Gram-positive cocci, 15.8% Gram-negative cocci, 4% Clue cells, 13.8% with mixed flora, 3% Budding yeast-like cells and 0.4% showed Gram-negative cocci. Conclusion: Such studies involving the laboratory and demographic data should be conducted regularly, which can help in estimating the disease burden, strengthening the diagnostic capacity, and formulating the requisite strategy for tackling this problem through a syndromic approach.

Abstract

Over a sixth of the global total does not have access to clean drinking water. People in poor countries face a significant burden of diarrheal disease and other debilitating, life-threatening diseases due to a lack of adequate water. Fresh-water demand will continue to rise dramatically throughout the globe in the next years to satisfy the demands of rising people, developing economies, changes in lifestyle, and altering consumption behavior. It will also put even more strain on already scarce natural resources and ecosystems. As a result of overuse and water contamination, clean water has recently become a scarce fundamental commodity. In addition, the importance of new microbes in drinkable water has grown. Because of the serious implications of a drinking water source being polluted with pathogenic bacteria, prevention is important. The objective of this study is to find the quality of water whether or not it is suitable for drinking on the basis of physicochemical and microbiological assessment. In this study, we determine the water quality in terms of contamination rate such as chemical and microbiological to assure that the water we are drinking is safe for us as well as for other people. The testing was done as per IS 10500:2012 (Indian Standard) and APHA, USEPA for chemical and bacteriological analysis. The maximum numbers of chemical parameters were within the limits whereas this was not in the case of bacteriological parameter. The result of this study was that physical appearance is agreeable and disagreeable in some areas of Dehradun and in chemical appearance the amount of calcium is more than the limit and there is microbial contamination found in all water samples. From these studies, it is recommended that drinking water should be filtered and boiled. The water storage tanks should be washed in timely manner with chlorine tablets and chlorine liquid to avoid the contamination of water.

Abstract

Retained surgical foreign bodies are unanticipated events culminating from inadvertent operating room errors and may cause severe medical and legal problems between the patient and the doctor. Here, we report detecting a surgical instrument fragment 13 years after an open abdominal hysterectomy in a quadragenarian during her evaluation of a month-old complaint of lower abdominal and right thigh pain. A computed tomography scan of the abdomen demonstrated a radio-opaque linear foreign body traversing the right obturator foramen with extension into the pelvis cranially and the adductor compartment of the right thigh caudally. The metallic foreign body, identified as a fragmented handle of a uterine tenaculum forceps with a slender sharp-tip hook, could be removed laparoscopically from the pelvis after a diagnostic laparoscopy, preventing significant complications. The minimally invasive approach enabled a smooth recovery, and the patient could go home on the second postoperative day.

It is sporadically linked to tumors, tuberculosis (TB) and lung cancer-causing agents like mycobacterial infections, autoimmune illnesses, etc. One of the most common occupational diseases in the world, silicosis poses serious health risks to employees, especially in developing nations like India.

Abstract

The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135–15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359–5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138–5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184–5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598–9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090–6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286–5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p p = 0.018; 95% CI 0.138–0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143–0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible. Recent literature on managing traumatic duodenal injuries suggests the superiority of primary repair. We hypothesized that duodenal trauma repair by primary closure might not be a safe strategy in an environment dealing predominantly blunt injuries with limited resources.Data analysis was done from the prospectively maintained trauma registry. The study period chosen was from January 1, 2014 to December 31, 2018. Data of 63 patients were analyzed for demographics, injuries, management, and outcome. Logistic regression was used to identify mortality predictors.The most common mechanism of injury was blunt (56/63, 88.9%). Forty (63.5%) patients had associated intraabdominal injuries. The most common American Association for the Surgery of Trauma grade of injury to the duodenum was three in 21 patients. Univariate analysis showed that mortality was associated with hypotension on presentation, higher duodenal grade, associated abdominal vascular injuries, primary closure, and duodenal leak. Logistic regression showed associated associated abdominal vascular injuries, primary closure, and leak remained significant predictors of mortality.Primary repair was found to be an independent predictor of mortality. A patient's physiology is a critical determinant of the outcome. Liberal use of tube duodenostomy over primary repair seems reasonable for blunt duodenal injury management.

To find out the relative incidence and outcome of posterior fossa abnormality (PFA) in terms of survival at birth until 2 years of age.We conducted a prospective study; all fetuses diagnosed with posterior fossa abnormality were followed-up. The outcome was observed with respect to survival, the presence of associated anomalies, the existence of developmental delay after a telephonic interview.Out of 2703 children with congenital anomalies, 921 (34.1%) had a central nervous system defect; 76 cases of PFA were fully followed. Dandy-Walker malformation (DWM) was present in 50% (38/76), mega cisterna magna 18.4% (14/76), Blake pouch cyst 13.2% (10/76), vermian hypoplasia (VH) 13.2% (10/76) and arachnoid cyst 5.2% (4/76). The diagnosis was possible before 20 weeks in only 12 (15.8%) cases. The mean gestational age at delivery was 34.7 ± 6.7 weeks. Associated anomalies were seen in 35/76 (46.1%) cases. A total of 35/76 (46.1%) survived after 2 years; there was developmental delay in 9.2% of cases.There is a large variation in the outcome of PFA depending upon the type of anomaly. Associated anomalies are common in VH and DWM, making their prognosis worse.

Abstract

Increasing incidence of hepatocellular carcinoma (HCC) in India is a matter of concern and need for adequate profiling and streamlining management strategies cannot be over-emphasized.This is a prospective multi-centric observational cohort study comprising of an oncology center, one university tertiary hospital with specialized hepatology service, one public hospital with gastroenterology service, and a private liver transplant center located within a 3-km radius. The demographic and clinical parameters were recorded on a prospectively maintained database. The clinical profile, demographics, characteristics of HCC and the allocated treatment were noted and compared among the four centers.In total, 672 patients were enrolled from June 2016 till January 2020. Abdominal pain (64.3%) and weight loss (47.3%) were the most common symptoms. Most common identified etiology was hepatitis B (39%). The cancer center received lesser patients with hepatitis C and those with advanced stage of HCC. The private transplant center reported the highest proportion of NASH, which was also significantly higher in those belonging to higher socioeconomic strata, and lowest proportion of alcoholic cirrhosis. Metastasis was seen in almost one-fifth (19%) cases at diagnosis. Portal vein thrombosis was evident in 40%. Adherence to treatment guidelines was seen in three-fourth cases (76%).Hepatitis B is the most common underlying cause for HCC, whereas other causes like NASH are on the rise. Etiologic profile may vary with selective specialization of centers catering to patients with HCC. Adherence to guideline while allocating treatment was high among all centers with highest non-adherence in BCLC A.

Abstract

Calvarium and skull base can be affected by a variety of benign, tumor-like, and malignant processes. Skull metastases (SMs) may be located in any layer of the skull and may be incidental or present with neurological symptoms during the diagnostic workup. In the present study, we discuss the occurrence of SMs from various index malignancies and their myriad clinical presentation. This data-based study includes patients of bone metastases between June 2018 and July 2020. Patients with skull bone metastases were recognized, and location of primary site, their clinical presentation, and management strategy were noted. Ten patients with skull bone metastases were identified during this period. Four patients had skull base location with clinical manifestation as syndromes. Six patients had primary from breast cancer, three from Ewing's sarcoma, and one from lung cancer. Management varied according to the primary site and symptoms of each patient. SM, though not rare, is often diagnosed incidentally but presents diagnostic and management challenges in the patient with cancer.

29. Comparative evaluation of gingival displacement produced by three different gingival retraction materials

Abstract

Aim: To compare and evaluate the gingival displacement produced by three different gingival retraction materials. Materials and Methods: A study was conducted to evaluate the gingival displacement produced by three gingival retraction materials. 12 subjects were selected for the study. T-stat retraction paste system (Nexobio co.Ltd,Korea), 3M ESPE Retraction Paste (3M Deutschland GmbH, Germany), Roeko Stay- put retraction cord (Coltene Whaledent Pvt. Ltd.) were used in the study. Results: Out of the three materials used, lateral displacement was maximum with the Stay-put retraction cord followed by 3M retraction paste and least by T-Stat retraction paste. All the materials produced acceptable amount of vertical gingival retraction. When compared Stay-put retraction cord was found to be most effective among the three materials. On comparison of the cordless retraction materials, it was found that the material which was more viscous in consistency (3M retraction paste) was able to produce more lateral gingival displacement than the material having less viscosity (T-stat retraction paste) even though both the cordless materials provided almost similar amount of vertical gingival displacement. The overall gingival retraction produced by the Stay-put cord was the highest followed by 3M retraction paste and least by the T-stat retraction paste.

Abstract

Most power is generated using fossil fuels like coal, natural gas, and diesel. The contribution of coal to power generation is very high compared to other sources. Almost all thermal power plants use coal as a fuel for power generation. Such sources of fossil fuels are limited and thus the cost of power generation increases. At the same time, the induced toxic gases due to these fossil fuels pollute the environment. The objective of this work is to solve the economic emission dispatch problem. Economic emission dispatch helps to find out how to operate power plants at the minimum cost and induce the minimum emissions at a thermal power plant. Economic emission dispatch with constraints is a nonlinear optimization problem. For the solution of such nonlinear economic emission load dispatch problems, this work considers a new particle swarm optimization technique. The proposed new PSO gives the best solution for economic emission load dispatch and handles the constraints. For the testing of the proposed new PSO algorithm, this work considered a case study of a system of six generating units, and it was tested for load demands of 700 MW, 800 MW, and 1000 MW. The results of the new PSO for the three load demands considered give the minimum generation cost, minimum emission, and minimum total cost compared to other optimization algorithms. The proposed techniques are effective, and they can help obtain the minimum generation cost and minimize emissions.

Abstract

The fundamental objective of economic load dispatch is to operate the available generating units such that the needed load demand satisfies the lowest generation cost and also complies with the various constraints. With proper power system operation planning using optimized generation limits, it is possible to reduce the cost of power generation. To fulfill the needs of such objectives, proper planning and economic load dispatch can help to plan the operation of the electrical power system. To optimize the economic load dispatch problems, various classical and new evolutionary optimization approaches have been used in research articles. Classical optimization techniques are outdated due to many limitations and are also unable to provide a global solution to the ELD problem. This work uses a new variant of particle swarm optimization techniques called modified particle swarm optimization, which is effective and efficient at finding optimum solutions for single as well as multi-objective economic load dispatch problems. The proposed MPSO is used to solve single and multi-objective problems. This work considers constraints like power balance and power generation limits. The proposed techniques are tested for three different case studies of ELD and EELD problems. (1) The first case is tested using the data of 13 generating unit systems along with the valve point loading effect; (2) the second case is tested using 15 generating unit systems along with the ramp rate limits; and (3) the third case is tested using the economic emission dispatch (EELD) as a multi-objective problem for 6 generating unit systems. The outcomes of the suggested procedures are contrasted with those of alternative optimization methods. The results show that the suggested strategy is efficient and produces superior optimization outcomes than existing optimization techniques.

Abstract

Background Studies from high income countries suggest improved survival for females as compared to males following trauma. However, data regarding differences in trauma outcomes between females and males is severely lacking from low- and middle-income countries. The objective of this study was to determine the association between sex and clinical outcomes amongst Indian trauma patients using the Australia-India Trauma Systems Collaboration database. Methods A prospective multicentre cohort study was performed across four urban public hospitals in India April 2016 through February 2018. Bivariate analyses compared admission physiological parameters and mechanism of injury. Logistic regression assessed association of sex with the primary outcomes of 30-day and 24-hour in-hospital mortality. Secondary outcomes included ICU admission, ICU length of stay, ventilator requirement, and time on a ventilator. Results Of 8,605 patients, 1,574 (18.3%) were females. The most common mechanism of injury was falls for females (52.0%) and road traffic injury for males (49.5%). On unadjusted analysis, there was no difference in 30-day in-hospital mortality between females (11.6%) and males (12.6%, p = 0.323). However, females demonstrated a lower mortality at 24-hours (1.1% vs males 2.1%, p = 0.011) on unadjusted analysis. Females were also less likely to require a ventilator (17.3% vs 21.0% males, p = 0.001) or ICU admission (34.4% vs 37.5%, p = 0.028). Stratification by age or by ISS demonstrated no difference in 30-day in-hospital mortality for males vs females across age and ISS categories. On multivariable regression analysis, sex was not associated significantly with 30-day or 24-hour in-hospital mortality. Conclusion This study did not demonstrate a significant difference in the 30-day trauma mortality or 24-hour trauma mortality between female and male trauma patients in India on adjusted analyses. A more granular data is needed to understand the interplay of injury severity, immediate post-traumatic hormonal and immunological alterations, and the impact of gender-based disparities in acute care settings.

Abstract

Enterococci are Gram-positive, ovoid cocci that can be found as diplococci or in short chains. These species are found in the typical microbiota of the intestine, oral cavity, vagina, and other areas. Despite being commensals with moderate virulence, these species have lately emerged as important nosocomial infections with increasing drug resistance particularly to vancomycin. This has resulted in their changing patterns of infection that are resistant to conventional antimicrobial therapy. The main objective of this study was to assess the frequency of the VanA genotype among Vancomycin Resistant Enterococci, as well as the isolation, identification, speciation, and antibiotic sensitivity pattern of Enterococci from various clinical specimens. In this six-month study, 500 different clinical specimens such as urine, blood, and pus were collected aseptically from patients suffering from urinary tract infection (UTI), septicemia, and pyogenic illnesses, and a total of 94 Enterococci strains were identified. These isolates were identified and speciated using traditional tests and biochemical processes. Following culture, the disc diffusion technique was used to determine their antibiotic susceptibility pattern, as suggested for common antibiotics. RCR was also used to evaluate the prevalence of the vanA gene among VRE isolates. 88.29% were E. faecalis, 7.88% were E. faecium, 2.12% were E. durans, 1.06 % was E. casseliflavus, and 1.06 % was E. gallinarum, out of a total of 94 isolates. Urine (70.21 %) yielded the most isolates, followed by blood (17.02 %) and pus (17.02 %). Female patients had a higher isolation rate of 73.40 % (69/94) than male patients, who had a rate of 26.59 % (25/94), and the majority of the Enterococcal isolates were from inpatients (62.77 %) rather than outpatients (37.23 %). 30 % of the isolates were hemolytic, whereas 70% were non-hemolytic. The isolates sensitivity patterns revealed that they were resistant to antibiotics such as ampicillin, ciprofloxacin, and gentamicin. The susceptibility test also included 30g discs of vancomycin. Vancomycin resistance is greatest in E. faecium (28 %) 6/83, followed by E. faecalis (8 %) 2\7. All other isolates were vancomycin-susceptible. Vancomycin-resistant isolates made up 8.51 % of the total. Barely urine isolates were tested for nitrofurantoin resistance, which was found to be only 5%. Linezolid sensitivity was found in all of the Enterococci. According to the PCR test for the vanA gene, among total VRE isolates 5 E. faecalis and 1 E. faecium isolates had vanA genotypes with bands of 473bp. A number of studies have revealed an increment in the infection rate and antimicrobial resistance of Enterococci. Elevated isolation rate of VRE has posed threat to therapeutic strategies as now only few antibiotics are susceptible to such isolates. It is thus imperative to execute measures that could stable the VRE augmentation lest a normal commensal would get the status of SARS-CoV-2

Abstract

Plants and herbs have been used as sources of safest compounds in the management of diseases as well as in the restoration and fortification of body structures in ancient medical systems such as Ayurvedic, Unani, and traditional Chinese medicine. The major motive for incorporating herbs in healthcare is to develop a healthy relationship with the body's chemistry while eschewing off-target and unwanted side effects produced by synthetic medications. A surge in the human population, an insufficient supply of drugs, a non-manageable cost of treatments, and a surge in antimicrobial resistance to presently used drugs have forced the pharmacognostic researchers to trace out novel plant-based bioactive compounds to be used as therapeutic agents against innumerous kinds of diseases and disorders. In this study, we include two different samples of Mentha arvensis also known as Pudina or wild mint procured from two different locations in Uttar Pradesh region of India. One sample was taken near the edges of flowing stream in Meerut (Sample 1) and the other sample was taken from hilltop near Dehradun (Sample 2). The major objectives of this study were to analyse the effect of continuous water exposure as cold stress on the phytochemical properties, comparative antioxidant potential as well as antibacterial action of the Mentha arvensis leaves procured from Summit and Rivulet. The dried and powered leaves were extracted via four solvent systems of varied polarity viz, Methanol [M], Chloroform [C], Acetone [A], and Water [AQ]. Comparative phytochemical analysis including total phenolic content, total flavonoid content, and antioxidant potential along with antibacterial activity of both samples was evaluated from all extracts. Our results revealed that continuous exposure of water as in Sample 1 has a prominent impact on the phytochemical properties, antioxidant potential and anti-bacterial effect of the Mentha arvensis leaves. GC-MS studies of Menta arvensis essential oil revealed the presence of various bioactive compounds most of which are secondary metabolites, and we attribute antioxidant and anti-bacterial activity of Menta arvensis leaves to these compounds.

Abstract

The orbital structures are ideally suited for ultrasound examination due to their superficial location and cystic composition of the eye. However, orbital ultrasound remains an underutilised modality due to preference for other cross-sectional modalities in general practice.In this article, we review the basic principles, clinical uses and technique of orbital ultrasound in peadiatric patients.The clinical utility of orbital ultrasound in peadiatric patients is demonstrated using selected cases.Ultrasound is useful in the diagnosis of various posterior segment pathologies, especially in conditions causing opacification of light-conducting media of the eye. It is also beneficial in diagnosing various orbital pathologies, particularly in differentiating solid from cystic lesions.The added advantages of its use in children include lack of ionising radiation and reduced requirement of sedation or general anesthesia. Ultrasound is the most practical initial investigation in cases where ophthalmoscopy is limited by opacification of ocular media. The addition of color Doppler on ultrasound can give additional information about the vascularity of the lesion.Use of ultrasound can be streamlined into the workup of various orbital and ocular pathologies either as an initial investigation or as a problem-solving tool in cases with a diagnostic dilemma on other modalities.

This study evaluated the cost-effectiveness of AcrySof IQ Vivity intraocular lens (DFT015) compared with standard aspheric monofocal intraocular lens (SN60WF), from a private health fund perspective in Australia.Methods: A Markov model was developed using the following health states: well, need for spectacles (near/distance/bifocal/varifocal), very bothersome visual disturbances (glare/haloes/starbursts) – with/without spectacles, and death. Model inputs were sourced from a randomized clinical study (NCT03010254), published literature, prostheses list and clinical opinion. A lifetime horizon (up to 30 years) was considered, and cost and health outcomes were discounted at 5% per annum. Model outcomes included incremental cost–effectiveness ratio defined as incremental cost per quality adjusted life year gain. Sensitivity and scenario analyses were also conducted.Results: Bilateral implantation of DFT015 intraocular lens provided quality adjusted life year gain of 0.16 at an incremental cost of AU$307 compared to bilateral SN60WF, leading to an incremental cost-effectiveness ratio of AU$1908/quality adjusted life year, well below the cost-effectiveness threshold (Range: AU$45,000-AU$75,000) typically used by Medical Services Advisory Committee in Australia. Results were most sensitive to intraocular lens costs, post-operative spectacle dependence, and disutility due to wearing glasses. Robustness of the results was further confirmed by probabilistic sensitivity analysis and scenario analyses.Conclusion: AcrySof IQ Vivity intraocular lens is a highly cost-effective treatment strategy with improved vision-related quality of life outcomes for presbyopic cataract surgery patients.Keywords: cataract surgery, ATIOL, EDoF, presbyopia, ICER, quality of life

Cancer therapeutics cause various treatment-related changes that may impact patient follow-up and disease monitoring. Although atypical responses such as pseudoprogression may be misinterpreted as treatment nonresponse, other changes, such as hyperprogressive disease seen with immunotherapy, must be recognized early for timely management. Radiation necrosis in the brain is a known response to radiotherapy and must be distinguished from local tumor recurrence. Radiotherapy can also cause adverse effects such as pneumonitis and local tissue toxicity. Systemic therapies, like chemotherapy and targeted therapies, are known to cause long-term cardiovascular effects. Thus, there is a need for robust biomarkers to identify, distinguish, and predict cancer treatment–related changes. Radiomics, which refers to the high-throughput extraction of subvisual features from radiologic images, has been widely explored for disease classification, risk stratification, and treatment-response prediction. Lately, there has been much interest in investigating the role of radiomics to assess oncologic treatment–related changes. We review the utility and various applications of radiomics in identifying and distinguishing atypical responses to treatments, as well as in predicting adverse effects. Although artificial intelligence tools show promise, several challenges—including multi-institutional clinical validation, deployment in health care settings, and artificial-intelligence bias—must be addressed for seamless clinical translation of these tools.

Abstract

Coronary artery calcium (CAC) scores obtained from CT scans have been shown to be prognostic in assessment of the risk for development of cardiovascular diseases, facilitating the prediction of outcome in asymptomatic individuals. Currently, several methods to calculate the CAC score exist, and each has its own set of advantages and disadvantages. Agatston CAC scoring is the most extensively used method. CAC scoring is currently recommended for use in asymptomatic individuals to predict the risk of developing cardiovascular diseases and the disease-specific mortality. In specific subsets of patients, the CAC score has also been recommended for reclassifying cardiovascular risk and aiding in decision making when planning primary prevention interventions such as statin therapy. The progression of CAC scores on follow-up images has been shown to be linked to risk of myocardial infarction and cardiovascular mortality. While the CAC score is a validated tool used clinically, several challenges, including various pitfalls associated with the acquisition, calculation, and interpretation of the score, prevent more widespread adoption of this metric. Recent research has been focused extensively on strategies to improve existing scoring methods, including measuring calcium attenuation, detecting microcalcifications, and focusing on extracoronary calcifications, and on strategies to improve image acquisition. A better understanding of CAC scoring approaches will help radiologists and other physicians better use and interpret these scores in their workflows.

Abstract

Background The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approaches reported by 67.3% of the participants, especially in uncomplicated appendicitis, cholecystitis and multiple-recurrent diverticulitis. Conclusions The SARS-CoV-2 pandemic still significantly impacts on care and outcome of patients in ES. Well-known problems with in-hospital logistics are not sufficiently resolved by now; however, medical staff shortages and reduced capacities have been dramatically aggravated over last two pandemic years.

Abstract

One of the family i.e. Celastraceae, consists of around 90-100 genera and 1300-1350 species of vines, shrubs, herbs and small trees. It is commonly reported and found mainly in tropical regions. According to this family, scientists reported various species which showed various medicinal uses or properties and these are already described or mentioned in folk/traditional type of medicine. Most of the bioactive derived compounds from this family have been reported or described in the literature as well. In this study, we focused on traditional medicinal plant i.e. Gymnosporia montana of Celastraceae family and studied its phytochemical analysis in leaf extract using different solvent system i.e. aqueous, methanol and ethyl acetate and also analysing it’s in vitro anti-inflammatory and antimicrobial activity. The results showed there is slight variation in phytochemical analysis of leaf extract and showed in vitro anti-inflammatory and antimicrobial activities at higher doses. Overall, this revealed the companionship of phytoconstituents for instance flavonoids, alkaloids and so on.
