Abstract

Task-free functional connectivity in animal models provides an experimental framework to examine connectivity phenomena under controlled conditions and allows for comparisons with data modalities collected under invasive or terminal procedures. Currently, animal acquisitions are performed with varying protocols and analyses that hamper result comparison and integration. Here we introduce StandardRat, a consensus rat functional magnetic resonance imaging acquisition protocol tested across 20 centers. To develop this protocol with optimized acquisition and processing parameters, we initially aggregated 65 functional imaging datasets acquired from rats across 46 centers. We developed a reproducible pipeline for analyzing rat data acquired with diverse protocols and determined experimental and processing parameters associated with the robust detection of functional connectivity across centers. We show that the standardized protocol enhances biologically plausible functional connectivity patterns relative to previous acquisitions. The protocol and processing pipeline described here is openly shared with the neuroimaging community to promote interoperability and cooperation toward tackling the most important challenges in neuroscience. The authors pooled resources to identify best practices and develop a new standardized protocol for estimating functional connectivity in rats with magnetic resonance imaging.

Abstract

Objective: We evaluate nationwide perioperative outcomes of complex EVAR and assess the volume-outcome association of complex EVAR. Summary of Background Data: Endovascular treatment with fenestrated (FEVAR) or branched (BEVAR) endografts is progressively used for excluding complex aortic aneurysms (complex AAs). It is unclear if a volumeoutcome association exists in endovascular treatment of complex AAs (complex EVAR). Methods: All patients prospectively registered in the Dutch Surgical Aneurysm Audit who underwent complex EVAR (FEVAR or BEVAR) between January 2016 and January 2020 were included. The effect of annual hospital volume on perioperative mortality was examined using multivariable logistic regression analyses. Patients were stratified into quartiles based on annual hospital volume to determine hospital volume categories. Results: We included 694 patients (539 FEVAR patients, 155 BEVAR patients). Perioperative mortality following FEVAR was 4.5% and 5.2% following BEVAR. Postoperative complication rates were 30.1% and 48.7%, respectively. The first quartile hospitals performed

Abstract

This document provides DESIRA's third set of practice abstracts (PAs) which is a compilation of seven PAs. Those PAs are based on the experiences, lessons learned, project actions and reporting of the WP3 activities that aimed at the development of scenarios and showcasing of technologies building on the concept of digital game changers. Tasks 3.5 'Use Case development' and 3.6 'Showcase Technologies', are the main contributing project tasks that provided concrete results on which these seven PAs, cited in this report, are based. The first five PAs provide a concise description of five use cases that were developed during the second period of the DESIRA project. An array of conducted activities inside the boundaries of five preselected Living Labs, and with the participation of those LL's stakeholders, were planned so that WP3 could culminate in the development of five technology adoption use cases. The last two PAs of this report, supplement the five aforementioned use case PAs by showcasing two additional promising technology solutions that have the potential to contribute to sustainable digital transition pathways, as those are defined by the DESIRA's theoretical framework and as reflected by the examined agro-rural-forestry settings of this project. For a thorough and detailed analysis of the methodology, activities, and outcomes that contributed to the production of the use cases and showcase technology reports, it is recommended the reading of deliverables D3.3 'Use Cases Report' and D3.4 'Showcase Technology Report' which are the foundation documents on which this deliverable is based on.

Abstract

Rijkswaterstaat gebruikt het goederenvervoermodel BasGoed om lange termijn prognoses te maken van het goederenvervoer. Economische verwachtingen spelen hierbij een belangrijke rol. Het model BasGoed begint dan ook met een module waarin de economische groei bepaald wordt en vervolgens vertaald wordt naar de hoeveelheid vervoerde goederen. De vorige versie van de economie module was verouderd. Deze paper beschrijft de nieuw ontwikkelde module op basis van multiregionale input-output analyses, aansluitend bij de manier waarop het Planbureau voor de leefomgeving economische analyses maakt. De werking van de module wordt toegelicht, en de eerste resultaten worden vergeleken met de uitkomsten van de oude module.

Survey included 2000 patient cases (1000 AOM 400; 1000 PP1M). Eligible patients were aged 18– 35 years, had been diagnosed with schizophrenia within 5 years, received AOM 400 or PP1M continuously for ≥ 6 months according to approved labels (mean durations: 1.6 and 1.7 years with AOM 400 and PP1M, respectively). Assessments included Global Assessment of Functioning (GAF) Scale, Personal and Social Performance Scale, Positive and Negative Syndrome Scale, and Quality of Life Scale. GAF assessments were done retrospectively and also at the time of survey.Results: Baseline mean GAF scores were 43.3 and 43.8 for AOM 400 and PP1M, respectively, indicating serious symptoms/functional impairment in both groups. Mean improvements following AOM 400 and PP1M therapy were 19.7 and 16.3 points, respectively (final scores in mild functional impairment category). Other measures assessing symptoms/functionality/quality of life demonstrated the benefits of long-term aLAI therapy.Conclusion: Schizophrenia patients with serious functional impairment prior to current aLAI treatment showed improvements in functional outcome after AOM 400 or PP1M therapy. Abstract

This systematic review and meta-analysis aimed to determine mean estimates of prevalence rates for fulfilling all diagnostic criteria of posttraumatic stress disorder (PTSD) or at least showing significant levels of posttraumatic stress (PTSS) in relation to the traumatic event of childbirth. For the first time, both mothers and fathers were included in the synthesis. Studies were identified through systematic database search and manual searches, irrespective of language. Meta-analyses of 154 studies (N = 54,711) applied a random-effects model to four data sets, resulting in pooled prevalence rates of 4.7% for PTSD and 12.3% for PTSS in mothers. Lower rates of 1.2% for PTSD and 1.3% for PTSS were found among fathers. Subgroup analyses showed elevated rates in targeted samples (those with a potential risk status) most distinctly for maternal PTSS. The significant amount of heterogeneity between studies could not be explained to a satisfactory degree through meta-regression. Given the substantial percentage of affected parents, the adoption of adequate prevention and intervention strategies is needed. As this field of research is evolving, attention should be broadened to the whole family system, which may directly and indirectly be affected by birth-related PTSD. Further studies on paternal PTSD/PTSS are particularly warranted.

Abstract

Purpose: Octogenarians are known to have less-favorable outcomes following ruptured abdominal aortic aneurysm (rAAA) repair compared with their younger counterparts. Accurate information regarding perioperative outcomes following rAAA-repair is important to evaluate current treatment practice. The aim of this study was to evaluate perioperative outcomes of octogenarians and to identify factors associated with mortality and major complications after open surgical repair (OSR) or endovascular aneurysm repair (EVAR) of a rAAA using nationwide, real-world, contemporary data. Methods: All patients that underwent EVAR or OSR of an infrarenal or juxtarenal rAAA between January 1, 2013, and December 31, 2018, were prospectively registered in the Dutch Surgical Aneurysm Audit (DSAA) and included in this study. The primary outcome was the comparison of perioperative outcomes of octogenarians versus non-octogenarians, including adjustment for confounders. Secondary outcomes were the identification of factors associated with mortality and major complications in octogenarians. Results: The study included 2879 patients, of which 1146 were treated by EVAR (382 octogenarians, 33%) and 1733 were treated by OSR (410 octogenarians, 24%). Perioperative mortality of octogenarians following EVAR was 37.2% versus 14.8% in non-octogenarians (adjusted OR=2.9, 95% CI=2.8–3.0) and 50.0% versus 29.4% following OSR (adjusted OR=2.2, 95% CI=2.2–2.3). Major complication rates of octogenarians were 55.4% versus 31.8% in non-octogenarians following EVAR (OR=2.7, 95% CI=2.1–3.4), and 68% versus 49% following OSR (OR=2.2, 95% CI=1.8–2.8). Following EVAR, 30.6% of the octogenarians had an uncomplicated perioperative course (UPC) versus 49.5% in non-octogenarians (OR=0.5, 95% CI=0.4–0.6), while following OSR, UPC rates were 20.7% in octogenarians versus 32.6% in non-octogenarians (OR=0.5, 95% CI=0.4–0.7). Cardiac or pulmonary comorbidity and loss of consciousness were associated with mortality and major complications in octogenarians. Interestingly, female octogenarians had lower mortality rates following EVAR than male octogenarians (adjusted OR=0.7, 95% CI=0.6–0.8). Conclusion: Based on this nationwide study with real-world registry data, mortality rates of octogenarians following ruptured AAA-repair were high, especially after OSR. However, a substantial proportion of these octogenarians following OSR and EVAR had an uneventful recovery. Known preoperative factors do influence perioperative outcomes and reflect current treatment practice.

Contains fulltext : 251573.pdf (Publisher’s version ) (Open Access) OBJECTIVE: The Dutch Surgical Aneurysm Audit (DSAA) initiative was established in 2013 to monitor and improve nationwide outcomes of aortic aneurysm surgery. The objective of this study was to examine whether outcomes of surgery for intact abdominal aortic aneurysms (iAAA) have improved over time. METHODS: Patients who underwent primary repair of an iAAA by standard endovascular (EVAR) or open surgical repair (OSR) between 2014 and 2019 were selected from the DSAA for inclusion. The primary outcome was peri-operative mortality trend per year, stratified by OSR and EVAR. Secondary outcomes were trends per year in major complications, textbook outcome (TbO), and characteristics of treated patients. The trends per year were evaluated and reported in odds ratios per year. RESULTS: In this study, 11 624 patients (74.8%) underwent EVAR and 3 908 patients (25.2%) underwent OSR. For EVAR, after adjustment for confounding factors, there was no improvement in peri-operative mortality (aOR [adjusted odds ratio] 1.06, 95% CI 0.94 - 1.20), while major complications decreased (2014: 10.1%, 2019: 7.0%; aOR 0.91, 95% CI 0.88 - 0.95) and the TbO rate increased (2014: 68.1%, 2019: 80.9%; aOR 1.13, 95% CI 1.10 - 1.16). For OSR, the peri-operative mortality decreased (2014: 6.1%, 2019: 4.6%; aOR 0.89, 95% CI 0.82 - 0.98), as well as major complications (2014: 28.6%, 2019: 23.3%; aOR 0.95, 95% CI 0.91 - 0.99). Furthermore, the proportion of TbO increased (2014: 49.1%, 2019: 58.3%; aOR 1.05, 95% CI 1.01 - 1.10). In both the EVAR and OSR group, the proportion of patients with cardiac comorbidity increased. CONCLUSION: Since the establishment of this nationwide quality improvement initiative (DSAA), all outcomes of iAAA repair following EVAR and OSR have improved, except for peri-operative mortality following EVAR which remained unchanged.

Abstract

A velocity map imaging setup has been designed and fabricated at the Max Planck Institute for Nuclear Physics in Heidelberg to study multiphoton ionization. This setup is placed in an optical cavity to amplify a frequency comb used to generate the photoelectrons. A rotatable wave plate can be used to change the polarization of the laser, by changing the polarization it is possible to view the electron momentum distribution from different angles. In this work, first measurements with this setup were investigated, and the resulting electron momentum distribution was reconstructed as a 3D model using the Abel transform and computed tomography.

Abstract

Voor de aanleg van een scheepvaartverbinding tot het Maasvlakte 2 gebied - in het meest westelijk gelegen havengebied van Rotterdam - zal de Yangtzehaven worden verdiept en in westelijke richting worden verlengd. Dit werk wordt uitgevoerd door het Havenbedrijf Rotterdam N.V. De geplande havenverdieping ligt in het huidige Maasvlakte 1 gebied en maakt onderdeel uit van het project Maasvlakte 2. Bij de uitvoering van dit werk zullen Vroeg en Laat Holocene lagen verstoord worden die archeologische waarden kunnen bevatten. Het betreft de lagen op een diepte van 17 - 22 m -NAP. Uit het archeologisch vooronderzoek komt naar voren dat er twee archeologische verwachtingscategorieën met betrekking tot de bedreigde diepere ondergrond (17 en 22 m - NAP) kunnen voorkomen: - Archeologische waarden uit de steentijd in de top van de rivierduinzanden en de basis van de Vroeg Holocene delta-afzettingen. - Scheepswrakken in de Subatlantische geulen. In Bijlage 14 zijn met een arcering de meest kansrijke gebieden aangegeven voor wat betreft de steentijd archeologie en het mogelijk voorkomen van scheepswrakken. In deze studie wordt aanbevolen om deze kansrijke gebieden nader archeologisch en paleolandschappelijk te onderzoeken. Met deze aanvullende kennis kan de beste veldstrategie bepaald - voor beide categorieën - worden met betrekking tot het archeologische vervolgonderzoek in het Yangtzehaven gebied.

Abstract

Wetland plants transport oxygen to belowground tissues to survive in anoxic sediments, and simultaneously conduct methane (CH4) from the sediment to the atmosphere. Although plant-mediated transport is the main CH4 emission pathway in vegetated wetlands, the contribution of vegetated areas to total emissions in wetlands remains uncertain. To accurately quantify these emissions, understanding the physiological processes driving plant-mediated CH4 transport is crucial. This review describes the state of the art understanding of CH4 transport through trees, emergent, floating-leaved, and submerged freshwater macrophytes. Gas transport mechanisms in plants include diffusion, pressurized flow, and transpiration-driven flow. Pressurized flow in the gas-filled aerenchyma leads to higher gas transport rates than diffusion, and mostly occurs in plants standing in deeper water. Transpiration-driven flow occurs in the xylem tissue of trees, whereby dissolved CH4 is transported by sap flow. Pressurized flow and transpiration-driven flow both result in diel cycles in CH4 emission, with higher emissions during the day than at night. The total CH4 emission through a wetland plant depends on its growth stage, transport mechanisms and the balance between sediment and in-plant CH4 production and oxidation. Although plants contribute substantially to total CH4 emissions, soil carbon content, soil temperature, nutrient availability, and water depth are often stronger driving factors than plant species. Nevertheless, accurate quantification of emissions from vegetated wetlands requires standardization of measurement protocols which capture diurnal and seasonal variation in emissions. Knowledge on CH4 transport through trees and submersed and free-floating macrophytes is scarce and warrants further research.

Abstract

Nonalcoholic steatohepatitis (NASH) is a chronic liver disease that increases cardiovascular disease risk. Indoleamine 2,3-dioxygenase-1 (IDO1)-mediated tryptophan (Trp) metabolism has been proposed to play an immunomodulatory role in several diseases. The potential of IDO1 to be a link between NASH and cardiovascular disease has never been investigated. UsingApoe−/−andApoe−/−Ido1−/−mice that were fed a high-fat, high-cholesterol diet (HFCD) to simultaneously induce NASH and atherosclerosis, we found thatIdo1deficiency significantly accelerated atherosclerosis after 7 weeks. Surprisingly,Apoe−/−Ido1−/−mice did not present a more aggressive NASH phenotype, including hepatic lipid deposition, release of liver enzymes, and histopathological parameters. As expected, a lower L-kynurenine/Trp (Kyn/Trp) ratio was found in the plasma and arteries ofApoe−/−Ido1−/−mice compared to controls. However, no difference in the hepatic Kyn/Trp ratio was found between the groups. Hepatic transcript analyses revealed that HFCD induced a temporal increase in tryptophan 2,3-dioxygenase (Tdo2)mRNA, indicating an alternative manner to maintain Trp degradation during NASH development in bothApoe−/−andApoe−/−Ido1−/mice−. Using HepG2 hepatoma cell and THP1 macrophage cultures, we found that iron, TDO2, and Trp degradation may act as important mediators of cross-communication between hepatocytes and macrophages regulating liver inflammation. In conclusion, we show thatIdo1deficiency aggravates atherosclerosis, but not liver disease, in a newly established NASH and atherosclerosis comorbidity model. Our data indicate that the overexpression ofTDO2is an important mechanism that helps in balancing the kynurenine pathway and inflammation in the liver, but not in the artery wall, which likely determined disease outcome in these two target tissues.

Abstract

Voor de aanleg van een toegang tot de 2e Maasvlakte zal de Yangtzehaven worden aangelegd door de gemeente Rotterdam. In het kader van de op te stellen milieueffectrapportage (M.E.R.) en het bestemmingsplan voor dit grote infrastructurele project is aan Deltares, afdeling Bodem en Grondwatersystemen gevraagd (tot 1 januari 2008 onderdeel van TNO BenO) een geo-archeologisch vooronderzoek uit te voeren naar de archeologische verwachting van de ondergrond die door het werk verstoord zal gaan worden. Het vooronderzoek reconstrueert begraven landschappen op basis van geologische gegevens, dit is een geogenetische benadering. Van de ondergrond van het Yangtzehavengebied zal een geologisch lagenmodel worden gemaakt. Per laag wordt op basis van het afzettingsmilieu (paleolandschap) de archeologische verwachting vastgesteld. Op basis van dit verwachtingsmodel zullen de archeologische waarden in het archeologisch vervolgonderzoek worden opgespoord.

Abstract

Aims: \ud Hypertensive disorders occur in women with peripartum cardiomyopathy (PPCM). How often hypertensive disorders co-exist, and to what extent they impact outcomes, is less clear. We describe differences in phenotype and outcomes in women with PPCM with and without hypertensive disorders during pregnancy.\ud \ud Methods: \ud The European Society of Cardiology PPCM Registry enrolled women with PPCM from 2012-2018. Three groups were examined: 1) women without hypertension (‘PPCM-noHTN’); 2) women with hypertension but without pre-eclampsia (‘PPCM-HTN’); 3) women with pre-eclampsia (‘PPCM-PE’). Maternal (6-month) and neonatal outcomes were compared.\ud \ud Results: \ud Of 735 women included, 452 (61.5%) had PPCM-noHTN, 99 (13.5%) had PPCM-HTN and 184 (25.0%) had PPCM-PE. Abstract

In november 2018 is een archeologische begeleiding uitgevoerd in plangebied De Nieuwe Loet in Castricum. Ten behoeve van de herontwikkeling van het mannenpaviljoen de Loet op het terrein van het voormalig psychiatrisch ziekenhuis Duin en Bosch, wordt onder andere een ondergrondse parkeergarage (ca. 1584 m²) gerealiseerd. Uit vooronderzoek was gebleken dat in de ondergrond resten aanwezig kunnen zijn uit de periode van de Late-IJzertijd tot de Vroege-Middeleeuwen. Resultaten Tijdens de archeologische begeleiding zijn tot op de maximale ontgravingsdiepte (ca 0,75 m + NAP) uitsluitend afzettingen aangesneden die zijn geïnterpreteerd als afzettingen van de Jonge Duinen, die werden gevormd vanaf de Vroege-Middeleeuwen. Ook zijn vegetatiehorizonten aangetroffen op een diepte van tussen 1.50 en 2 m + NAP, die zijn gedateerd in de Nieuwe tijd, en één kuil of natuurlijk spoor, dat eveneens in de Nieuwe tijd gedateerd kan worden. Vondsten zijn niet aangetroffen. Dit betekent dat in onderhavig onderzoek, in tegenstelling tot wat werd verwacht op basis van vooronderzoek, eventuele resten uit vroegere periodes niet zijn bereikt. Het is evenwel mogelijk dat deze zich bevinden op een niveau onder de ontgravingsdiepte die is bereikt tijdens onderhavig onderzoek.

47. Tamoxifen for the treatment of polycystic liver disease

Abstract

Rationale: Polycystic liver disease is a rare disease characterized by the growth of numerous cysts in the liver. The liver function remains well preserved, but liver volumes can grow very large, and some patients ultimately need a liver transplantation. Other treatment options are limited and there is an unmet need for new therapeutic options. Patient concerns: We describe a 59-year-old patient with pain in the abdomen, especially when bending forward. Five years ago, she was diagnosed with breast cancer and as an incidental finding a couple of large liver cysts were diagnosed, explaining her abdominal pain. Diagnosis: Polycystic liver disease with several large liver cysts. Interventions: The patient was treated with tamoxifen, an estrogen receptor modulator, as treatment for her hormone receptor positive breast cancer. One of the liver cysts was aspirated. Outcomes: In the 4.6 years after the start of tamoxifen treatment, 20 mg once daily, the volume of her liver cysts decreased remarkably. There was a reduction of combined cyst volume from 311 mL to 22 mL without percutaneous drainage. Lessons: Epidemiological as well as experimental evidence supports a pivotal role for estrogens as a driver for growth of polycystic livers. Estrogen antagonism has often been proposed as a therapeutic target, but supporting evidence is lacking in the literature. We hypothesize that the decrease in cyst size in this patient was caused by tamoxifen therapy, suggesting an in vivo antagonistic effect on cystic cholangiocytes. This is an important finding because tamoxifen could be a promising new treatment option for polycystic liver disease.
