Patient safety and quality of care are increasing concerns for healthcare internationally. This paper examines the spatial achievement of safety and wellbeing by healthcare staff, patients and their carers within UK primary care and Australian palliative care contexts. Two key socio-spatial modes of safety and wellbeing were found across these healthcare contexts. The technical mode was spatially managed by staff and driven by formal approaches to safety with a limited focus on wellbeing. In contrast, the relational mode was driven by attentiveness to the wellbeing and spatial engagement of staff, patients and carers that drew on informal elements of safety. Both modes extended across public, private, biomedical and administrative spaces, with technical and relational safety-wellbeing configurations often inhabiting the same spaces. Differences also existed across primary and palliative care contexts that reflected the unique pressures present within each context, and the ability of people and places to adapt to these demands. In the context of increasing workloads in healthcare internationally, this study highlights the benefits of attending as much to the relational dimensions of safety and quality of care as to the technical ones through increased focus on the safety and wellbeing of healthcare staff, patients and carers within and beyond traditional sites of care., Highlights • Two key modes of safety and wellbeing exist across primary and palliative care contexts. • The technical mode is spatially managed by staff and driven by formal approaches to patient safety. • The relational mode is driven by attentiveness to the wellbeing and spatial engagement of staff, patients and carers. • Both technical and relational modes extend across public, private, biomedical and administrative spaces. • Future improvement initiatives should focus on both technical and relational modes of safety and wellbeing.

Background: The handling of laboratory, imaging and other test results in UK general practice is a high-volume organisational routine that is both complex and high-risk. Previous research in this area has focused on errors and harm, but a complementary approach is to better understand how safety is achieved in everyday practice. This paper ethnographically examines the role of informal dimensions of test results handling routines in the achievement of safety in UK general practice and how these findings can best be developed for wider application by policymakers and practitioners.Methods: Non-participant observation was conducted of high-volume organisational routines across eight UK general practices with diverse organisational characteristics. 62 semi-structured interviews were also conducted with key practice staff alongside the analysis of relevant documents.Results: While formal results handling routines were described similarly across the eight study practices, the everyday structure of how the routine should be enacted in practice was informally understood. Results handling safety took a range of local forms depending on how different aspects of safety were prioritised, with practices varying in terms of how they balanced thoroughness (i.e. ensuring the high quality management of results by the most appropriate clinician) and efficiency (i.e. timely management of results) depending on a range of factors (e.g. practice history, team composition). Each approach adopted created its own potential risks, with demands for thoroughness reducing productivity and demands for efficiency reducing handling quality. Irrespective of the practice-level approach adopted, staff also regularly varied what they did for individual patients depending on the specific context (e.g. type of result, patient circumstances).Conclusions: General practices variably prioritised a legitimate range of results handling safety processes and outcomes, each with differing strengths and trade-offs. Future safety improvement interventions should focus on how to maximise practice-level knowledge and understanding of the range of context-specific approaches available and the safeties and risks inherent in each within the context of wider complex system conditions and interactions. This in turn has the potential to inform new kinds of proactive, contextually-appropriate approaches to intervention development and implementation focusing on the enhanced deliberation of the safety of existing high-volume routines.

Integrative Oncology refers to the combining of traditional and complementary medicine with conventional cancer healthcare. This report presents the results of a national Australian cross-sectional survey of healthcare organisations providing cancer services in 2016, focus group interviews with cancer survivors of Chinese, Vietnamese, Arabic and Anglo-European Australian backgrounds, and a community survey of cancer survivors. The purpose was to examine integrative oncology service provision across Australia, and identify barriers and facilitators to integrative oncology. The project was led by NICM at Western Sydney University, in partnership with South West Sydney Local Health District and Oncology Massage Ltd, a registered charity.

Accumulation of fructans was confirmed in asparagus tissues that had been cultured for 2 days on media supplemented with glucose. It is very common that fructans are biosynthesized from sucrose. We hypothesized however that fructans could also be biosynthesized from glucose. Stem tissues of in vitro-cultured asparagus were subcultured for 72 h on a medium containing 0.5 M of [1-C-13]glucose. A medium containing 0.5 M of normal (C-12) glucose was used as control. Carbohydrates were extracted from the tissues and analyzed using HPLC, MALDI-TOF MS and ESI-MS. HPLC results indicated that the accumulation of short-chain fructans was similar in both C-13-labelled and control samples. Short-chain fructans of DP =3-7 were detected using MALDI-TOF MS. The molecular mass of each oligomer in the C-13-labelled sample was higher than the mass of the natural sample by 1 m/z unit per sugar moiety. The results of ESI-MS on the HPLC fractions of neokestose and 1-kestose showed that these oligomers (DP =3) were biosynthesized from exogenous glucose added to the medium. We conclude that not only exogenous sucrose but glucose can induce fructan biosynthesis; fructans of both inulin type and inulin neoseries are also biosynthesized from glucose accumulated in asparagus tissues; the glucose molecules (or its metabolic products) were incorporated into fructans as structural monomers. Prescribing is the most common healthcare intervention, and is both beneficial and risky. An important source of risk in UK general practice is the management of ‘repeat prescriptions’, which are typically requested from and issued by nonclinically trained reception staff with only intermittent reauthorisation by a clinical prescriber. This paper ethnographically examines the formal and informal work employed by GPs and receptionists to safely conduct repeat prescribing work in primary care using Strauss’s (1985, 1988, 1993) concept of ‘articulation work’ across eight UK general practices. The analytical lens of articulation work provided an investigative framing to contextually map the informal, invisible resources of resilience and strength employed by practice team members in the achievement of repeat prescribing safety, where risk and vulnerability were continually relocated across space and time. In particular, the paper makes visible the micro-level competencies and collaborative practices that were routinely employed by both GPs and receptionists across different socio-cultural contexts, with informal, cross-hierarchical communication usually considered more effective than the formal structures of communication that existed (e.g. protocols). While GPs held formal prescribing authority, this paper also examines the key role of receptionists in both the initiation and safe coordination of the repeat prescribing routine.

This chapter explores the emergence of the social investment market in Australia. It begins by introducing the social investment phenomenon, differentiating it from other investment strategies, and noting the international context of its development. A review of extant literature reveals limited empirical data exploring the social investment phenomenon. The Biggart and Delbridge (2004) ‘systems of exchange’ typology is adopted as a useful lens to explore the emergence of social investment and to investigate the multi-faceted nature of exchange and collaboration within the field. Australia is highlighted as an interesting context to explore the emergence of social investment given rapid and significant policy initiatives to develop the market. Policy documents and in-depth interviews with practitioners are used to assess the nature of investor rationalities and social relations within the emerging market. The exploratory research suggests that social investment may be emerging as a ‘moral system of exchange,’ with policy implications and future research directions discussed.

This thesis is an exploration of the concepts of knowledge, sociality and relatedness amongst the Nivacle indigenous people of the Paraguayan Chaco, concentrating particularly on the community of Jotoicha in the Mennonite Colonies of the Central Chaco region. A central issue in this thesis is the concept of "knowledge" as a relational capacity and the ways in which knowledgeable behaviour can be constitutive of aesthetically pleasing forms of sociality. Such practices can be generative of increased similarity between individuals over time. The thesis begins with an exploration of Nivacle understandings of "knowledge" and shows it to be an eminently social concept that is created in an organ located in the stomach known as the cachi. "Knowledge" for the Nivacle is the basis for an individual's social conscience, their productive and reproductive skills, as well as their inter-personal relationships. "Knowledge" is also a central aspect of Nivacle understandings of relatedness. Rather than being based on a static genealogical structure, relatedness is best understood within the context of lived relationships that are constantly evolving. These relationships are generated through the practices of feeding and caring for one's "close" kin, with such practices also being generative of sociality itself. However,"close" relationships are neither static nor geographically bounded and are always open to the possibility of re-activation through visiting and gift giving. The Nivacle have been inserted into the market economy for several decades. Wage labour is conceptualised alongside other "subsistence activities" and productive activities are generative both of gender relations and sociality itself. In the final chapter I discuss Nivacle notions of reciprocity within the context of team-based sporting events. I show that whilst such community-based divisions appear to be premised on relations of "sameness" and "difference," they are best understood within the context of an overarching desire for people to generate similarity between different kinds of people in a variety of contexts.

Background: The systems-based management of laboratory test ordering and results handling is a known source of error in primary care settings worldwide. The consequences are wide-ranging for patients (e.g. avoidable harm or poor care experience), general practitioners (e.g. delayed clinical decision making and potential medico-legal implications) and the primary care organization (e.g. increased allocation of resources to problem-solve and dealing with complaints). Guidance is required to assist care teams to minimize associated risks and improve patient safety.Objective: To identify, develop and build expert consensus on ‘good practice’ guidance statements to inform the implementation of safe systems for ordering laboratory tests and managing results in European primary care settings.Methods: Mixed methods studies were undertaken in the UK and Ireland, and the findings were triangulated to develop ‘good practice’ statements. Expert consensus was then sought on the findings at the wider European level via a Delphi group meeting during 2013.Results: We based consensus on 10 safety domains and developed 77 related ‘good practice’ statements (≥ 80% agreement levels) judged to be essential to creating safety and minimizing risks in laboratory test ordering and subsequent results handling systems in international primary care.Conclusion: Guidance was developed for improving patient safety in this important area of primary care practice. We need to consider how this guidance can be made accessible to frontline care teams, utilized by clinical educators and improvement advisers, implemented by decision makers and evaluated to determine acceptability, feasibility and impacts on patient safety.

The field of co-operative development is replete with invocations to practice participatory management. Furthermore, Moreau and Mertens (2013) have argued that participatory management should be part of the core curriculum for social enterprise (SE) management education. This study examines a participatory management approach called OPERA to establish its contribution to critical appreciative processes (CAPs) within an appreciative inquiry (AI). We participated in four OPERA sessions involving 75 co operative and SE educators, consultants and managers between July 2014 and April 2016. We retrospectively examined the results of the first two session, made reflective diaries of the final two sessions and then practised OPERA in our own educational practice to authenticate findings. We found that OPERA: improves the perceived quality of meetings; elicits a broad range of topics from participants; assists non-hierarchical management practice, and; promotes direct democracy. Our study obtained credible evidence that OPERA contributes to the discovery, dream and design parts of an AI cycle, but not that it promotes critical appreciation.
