Search

Patient and Public Involvement (PPI) in research should focus on the engagement of patients/carers in every stage of the research cycle. However, the inconsistent level of education and research experience may compromise this involvement and training and support are recommended. In Portugal, consistent training to empower patients in their health decisions, or to be involved in research is neither available nor adapted for the patients and public. Our main goal is to develop and deliver the first high-quality and comprehensive online educational programme for adult patients with asthma and their carers, based on patients’/carers’ needs and preferences. The project includes four tasks. In Task 1, we will identify and organise the most important topics on asthma and health research, based on the identification of educational needs of adult patients/carers with asthma and the expertise of asthma specialists, using Delphi methodology. In Task 2, we will design and implement a high-quality, free, online self-learning programme based on reliable sources of information and on patients’/carers’ preferences. In Task 3, we will assess its feasibility, satisfaction and impact. Finally, Task 4 will be centred on the management and dissemination. Patients and carers will be actively engaged in all stages of this project as co-researchers, including the writing of this protocol. By the end of this project, we expect to have implemented the first high-quality, free-of-charge, online self-learning programme on asthma and health research, adapted for adult patients with asthma and their carers. It will be available for all patients/carers not only from Portugal but also from all Lusophone countries. This programme will empower patients and carers to increase their knowledge about the disease, develop new skills and have the opportunity to learn about research. Ultimately, it will introduce the knowledge patients/carers need to represent themselves and influence health policies in Portugal successfully.

Asthma, rhinitis, and atopic dermatitis (AD) are interrelated clinical phenotypes that partly overlap in the human interactome. The concept of “one-airway-one-disease,” coined over 20 years ago, is a simplistic approach of the links between upper- and lower-airway allergic diseases. With new data, it is time to reassess the concept. This article reviews (i) the clinical observations that led to Allergic Rhinitis and its Impact on Asthma (ARIA), (ii) new insights into polysensitization and multimorbidity, (iii) advances in mHealth for novel phenotype definitions, (iv) confirmation in canonical epidemiologic studies, (v) genomic findings, (vi) treatment approaches, and (vii) novel concepts on the onset of rhinitis and multimorbidity. One recent concept, bringing together upper- and lower-airway allergic diseases with skin, gut, and neuropsychiatric multimorbidities, is the “Epithelial Barrier Hypothesis.” This review determined that the “one-airway-one-disease” concept does not always hold true and that several phenotypes of disease can be defined. These phenotypes include an extreme “allergic” (asthma) phenotype combining asthma, rhinitis, and conjunctivitis.

Background Previous studies have demonstrated the feasibility of using an asthma app to support medication management and adherence but failed to compare with other measures currently used in clinical practice. However, in a clinical setting, any additional adherence measurement must be evaluated in the context of both the patient and physician perspectives so that it can also help improve the process of shared decision making. Thus, we aimed to compare different measures of adherence to asthma control inhalers in clinical practice, namely through an app, patient self-report and physician assessment. Methods This study is a secondary analysis of three prospective multicentre observational studies with patients (≥13 years old) with persistent asthma recruited from 61 primary and secondary care centres in Portugal. Patients were invited to use the InspirerMundi app and register their inhaled medication. Adherence was measured by the app as the number of doses taken divided by the number of doses scheduled each day and two time points were considered for analysis: 1-week and 1-month. At baseline, patients and physicians independently assessed adherence to asthma control inhalers during the previous week using a Visual Analogue Scale (VAS 0–100). Results A total of 193 patients (72% female; median [P25–P75] age 28 [19–41] years old) were included in the analysis. Adherence measured by the app was lower (1 week: 31 [0–71]%; 1 month: 18 [0–48]%) than patient self-report (80 [60–95]) and physician assessment (82 [51–94]) (p 0.05). Objective We aimed to compare patient's and physician's ratings of inhaled medication adherence and to identify predictors of patient-physician discordance. Design Baseline data from two prospective multicentre observational studies. Setting 29 allergy, pulmonology and paediatric secondary care outpatient clinics in Portugal. Participants 395 patients (≥13 years old) with persistent asthma. Measures Data on demographics, patient-physician relationship, upper airway control, asthma control, asthma treatment, forced expiratory volume in one second (FEV 1) and healthcare use were collected. Patients and physicians independently assessed adherence to inhaled controller medication during the previous week using a 100 mm Visual Analogue Scale (VAS). Discordance was defined as classification in distinct VAS categories (low 0-50; medium 51-80; high 81-100) or as an absolute difference in VAS scores ≥10 mm. Correlation between patients' and physicians' VAS scores/categories was explored. A multinomial logistic regression identified the predictors of physician overestimation and underestimation. Results High inhaler adherence was reported both by patients (median (percentile 25 to percentile 75) 85 (65-95) mm; 53% VAS>80) and by physicians (84 (68-95) mm; 53% VAS>80). The Portuguese Severe Asthma Registry (Registo de Asma Grave Portugal, RAG) was developed by an open collaborative network of asthma specialists. RAG collects data from adults and pediatric severe asthma patients that despite treatment optimization and adequate management of comorbidities require step 4/5 treatment according to GINA recommendations. In this paper, we describe the development and implementation of RAG, its features, and data sharing policies. The contents and structure of RAG were defined in a multistep consensus process. A pilot version was pretested and iteratively improved. The selection of data elements for RAG considered other severe asthma registries, aiming at characterizing the patient's clinical status whilst avoiding overloading the standard workflow of the clinical appointment. Features of RAG include automatic assessment of eligibility, easy data input, and exportable data in natural language that can be pasted directly in patients' electronic health record and security features to enable data sharing (among researchers and with other international databases) without compromising patients' confidentiality. RAG is a national web-based disease registry of severe asthma patients, available at asmagrave.pt. It allows prospective clinical data collection, promotes standardized care and collaborative clinical research, and may contribute to inform evidence-based healthcare policies for severe asthma. publishersversion published

Trabalho final de mestrado integrado em Medicina (Pneumologia), apresentado á Faculdade de Medicina da Universidade de Coimbra A Síndrome da Apneia Obstrutiva do Sono (SAOS) consiste numa síndrome clínica caracterizada pela ocorrência de episódios repetitivos de apneias e/ou hipopneias de carácter obstrutivo durante o sono, secundárias à obstrução e colapso das vias aéreas superiores. A SAOS representa o mais comum dos Distúrbios Respiratórios do Sono, e a sua prevalência no sexo masculino é cerca de 2 vezes superior à verificada no sexo feminino. Estudos recentes sugerem que não só o sexo feminino possui efeito protector no desenvolvimento da SAOS, como esta é largamente negligenciada na Mulher, não sendo diagnosticada nem instituída terapêutica. O principal objectivo deste Artigo de Revisão consiste em descrever as principais especificidades da SAOS no Sexo Feminino, em virtude da necessidade premente de caracterizar este distúrbio respiratório na Mulher, explicitando diferenças entre géneros na Apresentação Clínica e Polissonográfica, possíveis Factores Predisponentes, Mecanismos Patogénicos, Comorbilidades e Adesão e Eficácia da Terapêutica da SAOS. Mulheres com SAOS manifestam mais frequentemente sintomas atípicos, e, comparativamente com os Homens, são geralmente mais obesas, apresentam apneia obstrutiva de menor severidade, com predominância de eventos apneicos em sono REM e menor eficácia do sono. A Menopausa e o Envelhecimento representam os principais factores de risco de SAOS no género feminino, e a Hipertensão Arterial a principal comorbilidade. A implementação de tratamento com CPAP em Mulheres com SAOS está associada a uma diminuição da procura dos Cuidados de Saúde. A presente Revisão Bibliográfica revela que a SAOS no sexo feminino se reveste de particularidades que podem estar na base no subdiagnóstico desta patologia do sono na Mulher. Obstructive Sleep Apnea Syndrome (OSAS) consists in a clinical syndrome characterized by repetitive episodes of obstructive apneas and/or hypopneas due to upper airway narrowing and collapse. OSAS is the most common expression of Sleep Disordered Breathing and its prevalence in men is two times higher than in women. Recent studies suggest that not only the female gender represents protection for developing OSAS but this syndrome is also overly neglected in women and is not diagnosed nor any treatment is prescribed. The main purpose of this review article is to describe the main specifics of OSAS in the female gender in light of the great necessity to properly characterize this sleep disturbance in women, regarding gender differences in Clinical and Polysomnographic Presentation, possible Risk Factores and Pathogenic Mechanisms, Comorbidities and Compliance and Efficiency of OSAS Treatment. Women with OSAS have a higher prevalence of atypical symptoms and when compared to men are usually more obese, have a tendency to develop less severe sleep apnea with a higher ratio of apneas during REM sleep and have higher impairment of sleep efficiency. Menopause and Aging are the main risk factors for OSAS in the female gender and Hypertension the most common of Comorbidities. Treatment with CPAP is related to a decrease of health-care utilization by women with OSAS. As revealed in this current review of the literature, OSAS in women as its own specifics which may be the answer to misdiagnose this disease in the female gender.

Trabalho final de mestrado integrado em Medicina (Pneumologia), apresentado á Faculdade de Medicina da Universidade de Coimbra One of the current designs of medicine, given the aging of the population and the increase in chronic pathologies, is to improve the patients’ quality of life and try to optimize resources. Chronic obstructive pulmonary disease - COPD - is the 4th leading cause in worldwide mortality, and represents a major public health problem. When approaching this pathology, exercise intolerance is a strong indicator of the overall damage to the individual, predictive of a prognosis, in such way that evaluating physical condition may be considered as important as evaluating the pulmonary function. Walking tests – the six minute walk test (6MWT) and the shuttle walk test (SWT) - and the cardiopulmonary exercise test (CPET) are widespread in clinical practice. The aim of this review is to compare the different tests in order to draw conclusions about the applicability and the necessity of CPET in evaluating DPOC. When assessing the variables that help characterize the severity of the disease, evaluate the prognosis, and therapeutic setting, walking tests allowed a good approximation to the values obtained with the CPET. They provide satisfying information for evaluating performance, prognosis and established therapy, and present a good reproducibility, if performed rigorously. The simplicity of implementation in clinical practice is a big advantage over the CPET. On the other hand, CPET allows the identification of comorbidities, co-existing conditions, or diagnoses alternatives, as a cause to exercise intolerance. Thus, we conclude that CPET in clinical practice is not necessary in the primary approach to COPD and may be reserved for situations with no clear information after the routine assessment and the results from the walking tests Um dos desígnios actuais da medicina, face ao envelhecimento populacional e aumento da prevalência de patologias crónicas, é melhorar a qualidade de vida destes doentes, procurando optimizar a gestão de recursos. A doença pulmonar obstructiva crónica – DPOC – é 4ª. Causa de mortalidade mundial, e representa um importante problema de saúde pública. Na abordagem desta patologia, a intolerância ao exercício é um forte indicador do dano global do indivíduo, preditora de prognóstico, de tal modo que a avaliação da capacidade física pode considerar-se tão importante como a avaliação da função pulmonar. Na prática clínica estão difundidos sobretudo os testes de marcha – prova dos seis minutos marcha (six minute walk test - 6MWT) e a prova de shuttle (shuttle walk test - SWT) - e a Prova de esforço cardiorespiratória (cardiopulmonary exercise test - CPET). Esta revisão pretendeu comparar os diferentes testes para concluir acerca da aplicabilidade e da necessidade da CPET na avaliação da DPOC. Na avaliação das variáveis que auxiliam na caracterização da severidade da doença, avaliação do prognóstico, e definição da terapêutica, os testes da marcha permitiram uma boa aproximação aos valores obtidos pela CPET. Fornecem informação satisfatória na avaliação da performance, prognóstico, e terapêutica instituída, e apresentam boa reproductibilidade, se realizados com rigor. Apresentam como grande vantagem face à CPET a simplicidade de execução na prática clínica. A CPET, por sua vez, permite identificar comorbilidades, condições co-existentes, ou alternativas diagnósticas, como causa de intolerância ao esforço. Assim, conclui-se que na prática clínica a CPET poderá reservar-se para situações específicas e não elucidadas após avaliação rotineira e dos resultados dos testes da marcha, não sendo necessária na abordagem primária da DPOC.

Trabalho final de mestrado integrado em Medicina (Pneumologia), apresentado á Faculdade de Medicina da Universidade de Coimbra A DPOC é uma das mais prevalentes e debilitantes doenças no mundo inteiro, ocupando actualmente um lugar de destaque nas principais causas de mortalidade e morbilidade da população global. Em Portugal, 14,2% da população sofre de DPOC. Os doentes com este diagnóstico devem ser englobados em planos terapêuticos cuidados que, para além do tratamento farmacológico optimizado e personalizado a cada doente, englobam também outras estratégias não farmacológicas. Muitas das terapias não farmacológicas provaram já o seu valor na abordagem destes doentes sendo os Programas de Reabilitação Pulmonar o protótipo desta abordagem. São inquestionáveis as evidências da sua acção sobre a sintomatologia dos doentes, reduzindo a dispneia e a fadiga, melhorando consideravelmente a qualidade de vida dos doentes, aumentando a sua capacidade física e melhorando o prognóstico. Munindo-se de uma equipa multidisciplinar e de diversas estratégias, tendo como base o exercício físico, os Programas de Reabilitação Pulmonar devem ser considerados ferramentas fundamentais no arsenal do tratamento da DPOC, merecendo a atenção dos prestadores de cuidados de saúde para a implementação de políticas públicas que incluam estes programas na rotina dos serviços de saúde COPD is one of the most prevalent and debilitating diseases worldwide, currently occupying a proeminent place in the leading causes of mortality and morbidity of the global population. In Portugal, 14.2% of the population suffers from COPD. Patients with this diagnosis should be submitted to forethought therapeutic plans which, in addition to an optimized and customized pharmacological treatment for each patient, include other nonpharmacological strategies. Many of the non-pharmacological therapies have proven their value and the Pulmonary Rehabilitation Programs are the prototype of this approach. The evidence of the benefits on the patient’s symptoms is unquestionable. It has been determined Reabilitação Pulmonar na DPOC 2 that these Programs reduce dyspnea and fatigue, improving the patient’s quality of life and improving their prognosis. Using a multidisciplinary team and different strategies, where physical exercise are the baseground, Pulmonary Rehabilitation Programs should be considered important tools for the treatment of COPD. Health care providers should implement public policies including such programs in the routine of health care facilities.

Trabalho final de mestrado integrado em Medicina área científica de Pneumologia, apresentado á Faculdade de Medicina da Universidade de Coimbra Introdução A asma é uma doença crónica e inflamatória das vias aéreas com elevada prevalência na população geral, em que o conhecimento, educação e envolvimento dos doentes é crucial para o sucesso no controlo e tratamento. Objetivos Com este estudo pretendeu-se avaliar a eficácia de sessões de educação e esclarecimento de indivíduos com diagnóstico de asma, no controlo da doença e na qualidade de vida destes doentes. Adicionalmente, avaliar a eficácia da sessão na aquisição de conhecimentos com uso de questionário validado “Questionário para Avaliação dos Conhecimentos de Doentes Asmáticos sobre a sua Doença”. Métodos Trata-se de um estudo prospetivo de cohort numa população de doentes asmáticos não completamente controlados, selecionados na consulta externa de Pneumologia dos HUC-CHUC. Os doentes foram incluídos se score de ACT (Asthma Control Test) ≤ 20, sob terapêutica otimizada. A qualidade de vida e controlo da doença foram avaliados em dois momentos, antes da sessão e às seis semanas, usando os questionários validados ACT, ALQ (Asthma Life Quality) e CARAT (Control of Rhinitis and Asthma Test). A avaliação dos conhecimentos adquiridos foi avaliada com o uso do questionário referido, antes e depois das sessões. A análise estatística foi realizada através do programa de software SPSS Statistics versão 20.0.0 e R versão 2.13.1. Variáveis categóricas foram descritas como frequências absolutas e relativas e as variáveis contínuas foram reportadas como média e desvio-padrão. A comparação de variáveis quantitativas foi baseada no uso de testes não-paramétricos (teste de Wilcoxon). O teste de Qui-quadrado foi usado para comparar variáveis categóricas. Resultados O teste de Wilcoxon para amostras emparelhadas mostrou que existem diferenças estatisticamente significativas entre os scores ACT antes e depois das sessões (p = 0.022), assim como entre os scores CARAT (p = 0.015), ALQ (p = 0.034) e Q (número de respostas corretas ao questionário de avaliação de conhecimentos) (p = 0.036) nesse mesmo intervalo. Através de análise de correlação bivariada verificou-se que os valores de ACT2 e CARAT2 não apresentam correlação estatisticamente significativa (coeficiente de Spearman) com os valores de ALQ2 (r = - 0.64, p = 0.121) e (r = -0.45, p = 0.3075), respetivamente. Observou-se uma variação percentual superior dos scores de CARAT ( ̅ = 44.55%) e ACT ( ̅ = 36.74%) comparativamente aos scores ALQ ( ̅ = 29,41%). Os valores de CARAT2 e ALQ2 não apresentaram correlação estatisticamente significativa com o tempo de evolução da doença (r = 0.56, p = 0.1875) e (r = -0.74, p = 0.0566), respetivamente, contrariamente ao observado para os valores de ACT2 (r = 0.88, p = 0.0091). Discussão A presença de variação significativa dos valores de ACT, CARAT e ALQ antes e depois das sessões caracteriza melhorias significativas nos parâmetros de controlo da doença e de qualidade de vida. A obtenção de diferenças estatisticamente significativas entre os valores Q demonstrou o aumento dos conhecimentos por parte dos doentes. Conclusão Concluiu-se que a educação para a asma, tendo em vista a transmissão de informação e o ensino de práticas de auto-monitorização, é eficaz na melhoria clínica e no aumento da qualidade de vida num grupo de doentes asmáticos. Para além disso, demonstrou ser capaz de aumentar o conhecimento sobre a doença de forma significativa Introduction Asthma is a chronic inflammatory disorder of the airways with a high prevalence in the general population, in which knowledge, education and patient involvement are crucial for obtaining success in both control and treatment. With this study we intend to evaluate the effectiveness of educational and clarification sessions for subjects diagnosed with asthma in the control of the disease and in the quality of life of these patients. Additionally, evaluate the effectiveness of the session on the acquirement of knowledge by using the validated questionnaire “Questionário para Avaliação dos Conhecimentos de Doentes Asmáticos sobre a sua Doença”. Methods This is a prospective cohort study in a population of asthmatic patients not completely controlled, selected at the Pulmonology’s outpatient service at HUCCHUC. The patients were indicated if the ACT (Asthma Control Test) score ≤ 20, under optimized therapy. Quality of life and asthma control were evaluated in two moments, before the session and six weeks later, using the validated questionnaires ACT, ALQ (Asthma Life Quality) and CARAT (Control of Rhinitis and Asthma Test). The evaluation of knowledge acquisition was evaluated using the aforementioned questionnaire, before and after the sessions. Statistical analysis was performed using SPSS Statistics software version 20.0.0 and R version 2.13.1. Categorical variables were described as absolute and relative frequencies and continuous variables were reported as the mean and standard deviation. Comparison of quantitative variables was based on use of non-parametric tests (Wilcoxon test). Chi-squared test was used to compare the categorical variables. Results The Wilcoxon test for matched samples showed significant statistical differences between the ACT scores before and after the sessions (p = 0.022), as well as for the CARAT (p = 0.015), ALQ (p = 0.034) and Q (number of correct answers in the questionnaire for knowledge evaluation) (p = 0.036) scores in that same interval. Through bivariate correlation analysis it was verified that the ACT2 and CARAT2 values do not admit significant statistical correlation (Spearman coefficient) with the ALQ2 values (r = -0.64, p = 0.121) and (r = -0.45, p = 0.3075), respectively. A higher percentage variation was found for the CARAT ( ̅ = 44.55%) and ACT ( ̅ = 36.74%) scores comparatively to the ALQ scores ( ̅ = 29.41%). The CARAT2 and ALQ2 values do not show significant statistical correlation with the time course of the disease (r = 0.56, p = 0.1875) and (r = -0.74, p = 0.0566), respectively, contrarily to the observed ACT2 values (r = 0.88, p = 0.0091). Discussion The presence of significant statistical variation for the ACT, CARAT and ALQ values before and after the sessions characterizes significant improvement in both asthma control and quality of life parameters. The attainment of significant statistical differences between Q values demonstrated the increase in knowledge by the patients. Conclusion We concluded that asthma education owing to information transmission and self-monitoring practices is effective in clinical improvement and quality of life increase for a group of asthmatic patients. Furthermore, it demonstrated to be capable of enhancing knowledge about the disease significantly.

Trabalho final de mestrado integrado em Medicina área científica de Pneumologia, apresentada á Faculdade de Medicina da Universidade de Coimbra A Síndrome de Apneia Obstrutiva do Sono (SAOS) é uma doença crónica, caracterizada por episódios recorrentes de obstrução parcial ou completa da via aérea superior durante o sono, associados a hipóxia intermitente, fragmentação do sono e sintomas de roncopatia e hipersonolência diurna. A SAOS é um distúrbio respiratório do sono com elevada prevalência e tem vindo a ser associada a diversas patologias. É reconhecida como fator de risco de patologia cardiovascular e evidências recentes revelam uma relação com a Síndrome Metabólica. A Síndrome Metabólica é caracterizada por um conjunto de alterações metabólicas e cardiovasculares que incluem obesidade central, dislipidemia aterogénica, hipertensão arterial e intolerância à glicose. Aparentemente, a SAOS ativa vias fisiopatológicas que conduzem a alterações integradas na Síndrome Metabólica. Todavia, a obesidade constitui um fator de confundimento importante no estudo desta associação. A eficácia do tratamento com Continuos Positive Airway Pressure (CPAP), atual tratamento gold-standard da SAOS, na modificação dos parâmetros que integram a Síndrome Metabólica e na prevalência desta, permanece por esclarecer de um modo completo. O presente trabalho visa a revisão bibliográfica, aprofundada e atualizada, da associação entre SAOS e Síndrome Metabólica, bem como do potencial efeito terapêutico do tratamento CPAP nos parâmetros que definem Síndrome Metabólica. Obstructive Sleep Apnea Syndrome (OSAS) is a chronic clinical condition, characterized by recurrent episodes of partial or complete upper airway collapse and obstruction during sleep, associated with intermittent hypoxia, sleep fragmentation and symptoms of disruptive snoring and excessive daytime sleepiness. OSAS is a highly prevalent disorder and has been associated with multiple conditions. OSAS is regarded as a risk factor for cardiovascular disease and substantial evidence suggests a link between this condition and the Metabolic Syndrome. The Metabolic Syndrome is a cluster of metabolic and cardiovascular risk factors that include abdominal obesity, atherogenic dyslipidemia, elevated blood pressure and glucose intolerance. Apparently, OSAS activates pathophysiological mechanisms that lead to metabolic dysfunction. However, obesity persists as a main confounding factor in the study of this association. The efficacy of Continuous Positive Airway Pressure (CPAP), current goldstandard treatment for OSAS, on the modification of the metabolic components of the Metabolic Syndrome and on its prevalence, remains unclear. The present review aims to summarize the association between OSAS and Metabolic Syndrome, as well as the potential therapeutic effect of CPAP in the metabolic parameters that presently define Metabolic Syndrome.

É provável que o tratamento efetivo das complicações pulmonares na esclerose sistémica dependa do diagnóstico precoce. As provas funcionais respiratórias são usadas na avaliação do envolvimento pulmonar, mas questiona-se o seu valor clínico na esclerose sistémica. O objetivo deste trabalho de revisão bibliográfica foi esclarecer a importância do diagnóstico precoce do dano pulmonar na esclerose sistémica e o papel do estudo funcional respiratório nesse contexto. Fez-se uma pesquisa na Pubmed para artigos publicados do ano 2000 até ao presente. A revisão efetuada destacou a importância do diagnóstico precoce do envolvimento pulmonar da esclerose sistémica. Apesar da melhor estratégia de diagnóstico e monitorização do envolvimento pulmonar na esclerose sistémica permanecer controversa, as provas funcionais respiratórias fornecem informação clinicamente útil nessa investigação, devendo ser feitas a todos os doentes diagnosticados com esclerose sistémica. São necessários mais estudos nesta área de forma a esclarecer a validade de alguns dos testes de avaliação pulmonar, e seus parâmetros, na estratificação do risco, diagnóstico precoce e monitorização do envolvimento pulmonar. Pulmonary involvement in systemic sclerosis is the leading cause of disease-related morbidity and mortality. It appears likely that effective treatment of pulmonary complications related to systemic sclerosis is dependent upon early detection. Pulmonary function tests are used to evaluate pulmonary complications but questions remain concerning its clinical significance in systemic sclerosis. The aim of this review was to clarify the significance of the early detection of lung damage in systemic sclerosis and to evaluate the role of pulmonary function tests in that context. A Pubmed search for articles published from 2000 to present was conducted. This review emphasized the importance of early diagnosis of lung involvement related to systemic sclerosis. Although the best way for diagnosis and monitoring remains controversial, pulmonary function testing provide clinically useful information in that investigation and, therefore, it should be performed in every newly diagnosed patient with systemic sclerosis. More studies are needed in this area to clarify the validity of some of these pulmonary function tests, and its parameters, in risk stratification, early diagnosis and management of lung involvement.

Nos últimos anos, tem-se verificado que a DPOC está associada a diferentes comorbilidades e que a inflamação não está apenas restrita aos pulmões, como também é visível a nível sistémico. Por esse motivo, a DPOC é agora considerada síndrome crónica de inflamação sistémica. A coexistência da DPOC e de outras doenças extrapulmonares, como o Síndrome Metabólico (SM), assim como a presença de fatores comuns entre elas, sugerem a hipótese de partilha de vias fisiopatológicas que poderão contribuir para a inflamação sistémica. Este estudo teve como objetivo analisar a evidência científica atual, referente à associação DPOC-SM, visto que a compreensão da base fisiopatológica poderá ser útil na modificação da história natural dos doentes com DPOC e na identificação de novos objetivos para o tratamento. Para isso, foi efetuada uma pesquisa sistemática da literatura entre os anos de 2005 e de 2012, através de uma base de dados médicos, PubMed [MEDLINE]. Os dados recolhidos sugerem que os doentes com DPOC apresentam um ou mais componentes do SM, sendo esta associação estatisticamente significativa nos estadios mais avançados da doença. Esta coexistência depende de vários fatores relacionados com a obesidade abdominal e com a presença de inatividade física. Contudo, serão necessários mais estudos para elucidar a associação entre a DPOC e o SM de modo a formular estratégias terapêuticas para este fenótipo da doença Chronic Obstructive Pulmonary Disease (COPD) is one of the most prevalent diseases and will be the third leading cause of death by 2020. In the past few years, there has been increasing evidence that COPD is associated with different co-morbidities and that the inflammation is no longer restricted to the lungs, but is also seen systemically. Therefore COPD is now considered to be a chronic systemic inflammatory syndrome. The coexistence of COPD and other chronic extra pulmonary diseases, such us Metabolic Syndrome (MetS), as well as the recognition of common factors between these conditions, supports the hypothesis of a shared pathophysiological pathway that may contribute to the systemic inflammation. The purpose of this study was to analyze the current scientific evidence on the association COPD-MetS, since the understanding of the pathophysiological basis might be helpful to modify the natural history in patients with COPD and to identify new targets for therapy. Therefore a literature review was performed for the years 2005 to 2012 in the database PubMed [MEDLINE]. Data collected suggest that patients with COPD have one or more components of MetS, and this association is statistically significant in the more advanced stages of the disease. The coexistence COPD and MetS depends on several factors related to abdominal obesity and the presence of physical inactivity. However, further studies are required to elucidate the association between COPD and MetS in order to formulate therapeutic strategies for this disease phenotype

Trabalho final de mestrado integrado em Medicina área científica de Pneumologia, apresentado à Faculdade de Medicina da Universidade de Coimbra Introdução: A Fibrose Quística (FQ), o mais frequente distúrbio genético da raça caucasiana, é uma doença multissistémica cuja severidade é primordialmente condicionada pela progressão da doença pulmonar crónica. A doença do refluxo gastroesofágico (DRGE) poderá ser tanto uma das manifestações clínicas da FQ quanto um dos factores agravantes da progressão da doença e deterioração da função pulmonar. O refluxo gastroesofágico (RGE) encontra-se aumentado na FQ contudo, a sua prevalência, características, mecanismos e repercussões pulmonares, encontram-se ainda pobremente caracterizadas. Objectivos: A presente revisão visa a sistematização do conhecimento actual sobre esta associação, estudando causas de exacerbação pulmonar na FQ, factores de risco, prevalência e mecanismos fisiopatológicos da DRGE, no sentido de compreender o seu papel no declínio da função pulmonar e agravamento da sintomatologia respiratória. Foi objectivo secundário apresentar a perspectiva real do serviço de Pneumologia dos HUC, elaborando a casuística dos doentes com FQ aqui assistidos. Desenvolvimento: Os doentes com FQ apresentam elevada prevalência de RGE, estimada actualmente entre 35% e 81%. Os mecanismos primários envolvidos incluem o aumento do número de relaxamentos transitórios, a hipersecreção ácida e o atraso no esvaziamento gástrico. Os desequilíbrios de pressão toraco-abdominal e a sintomatologia pulmonar também contribuem para o RGE. A DRGE poderá condicionar agravamento da função respiratória por aspiração pulmonar de conteúdos gástricos, ou pela broncoconstrição secundária à agressão da mucosa esofágica, embora seja difícil a demonstração de causalidade directa entre as duas entidades, dado que o agravamento pulmonar é multifactorial. A casuística elaborada no grupo dos 23 doentes com FQ assistidos no serviço de Pneumologia dos HUC, com uso de questionário (adaptação do Mayo GER Questionnaire) revelou a presença de sintomas de RGE nos últimos 12 meses (em 91,30% da amostra), tendo-se estabelecido diferença estatisticamente significativa entre os grupos sob terapêutica com Omeprazol versus sem terapêutica com Omeprazol, no que diz respeito à gravidade dos valores de CVF, sendo que os mesmos foram piores no grupo tratado. Conclusão: É inquestionável a relevância da DRGE na progressão da FQ, pelo que estudos clínicos com ênfase no efeito da terapêutica anti-refluxo deverão ser efectuados, a fim de esclarecer as suas repercussões pulmonares. Embora escassa a evidência do impacto da DRGE na deterioração da função pulmonar, a sua influência na malnutrição e a possibilidade de melhoria do outcome com a terapêutica com IBP, têm vindo a encorajar o tratamento precoce e agressivo da DRGE. Porém, com base na nossa casuística, o tratamento com IBP poderá ser insuficiente, revestindo-se esta temática de actualidade e importância científica Background: Cystic fibrosis (CF), the most common genetic disturb among Caucasian populations, is a multisystem disease, severely involving respiratory tract, which continues to be the major cause of morbidity and mortality in these patients. Gastroesophageal reflux disease (GORD) might contribute to the decline of lung function and progression of pulmonary disease in these patients. Despite the increased gastroesophageal reflux (GOR) symptoms in CF patients, its prevalence and association with gastric aspiration and respiratory impact are not well characterized. Aim: The purpose of this study is to establish the state of the actual knowledge about this association, assessing the causes of impaired lung disease, as the risk factors, frequency and phatological mechanisms of GORD and the correlation between GOR, incidence of respiratory symptoms and decline of pulmonary function in CF. The second aim is to assess the prevalence of GORD and demonstrate its association with severity of lung disease in CF among real cases provided by the Pneumology Unit of our hospital. Actual knowledge: The prevalence of increased acid GOR in CF is currently estimated between 35-81%. Several primary mechanisms might be involved, including increased number of lower oesophageal sphincter relaxations, gastric hiperacidity and delayed gastric emptying. It is commonly suggested that pulmonary symptoms and increased toracoabdominal pressure might also contribute to GOR. Reflux gastric contents may potencially impair pulmonary function through microaspiration and reflex bronchospasm induced by acid aggression in oesophageal mucosa. However, it is difficult to demonstrate a direct relationship between GOR and pulmonary damage, due to multifactorial nature of the impairment of lung disease in CF. In a study with 23 CF patients attended at the Pneumology Unit of Coimbra’s University Hospital who completed a modified Mayo GER Questionnaire, 21 patients (91,30%) reported GOR symptoms during the preceding 12 months. A statistically significant difference was established between the group under Omeprazol therapy versus the group without Omeprazol therapy, referring to the severity of FVC values, which were more severe in the first group. Conclusion: Although our findings provide some indication for the relevance of GOR in patients with CF, further prospective investigations and outcome studies with anti-reflux therapies are needed to confirm the deleterious role of reflux in CF progression. Despite the limited evidence that GERD adversely impacts lung function, the fact that GERD can complicate malnutrition in CF and the possibility that acid suppression improves the outcome of this patients, leads to a strong recommendation for aggressive treatment of GERD. However, according to our study, acid suppression treatment might be insufficient, which is a greatly contemporary and scientifically important subject

Trabalho final de mestrado integrado em Medicina (Pneumologia), apresentado à Faculdade de Medicina da Universidade de Coimbra Estima-se que a Doença Pulmonar Obstrutiva Crónica (DPOC) se torne na terceira principal causa de morte em 2020. Dados recentes apontam para uma prevalência de 14,2%. Perante a inexistência de um tratamento eficaz, torna-se indispensável encontrar biomarcadores que se relacionem com a biopatologia desta doença, permitindo a sua melhor abordagem e monitorização. Identificar biomarcadores que indiquem a eficácia dos fármacos nos componentes do processo inflamatório, e assim descobrir um novo e adequado tratamento, é fundamental para travar o impacto sócio-económico e a morbimortalidade desta patologia. Com este trabalho de revisão pretende-se estudar os biomarcadores disponíveis, a sua relação com parâmetros clínicos, nomeadamente, o grau de progressão da doença, as alterações dos valores laboratoriais, a co-existência de hábitos tabágicos ou de comorbilidades e o valor preditivo nas exacerbações. Espera-se que o estudo aprofundado nesta área possa ter importância no conhecimento do rebate sistémico desta patologia inflamatória crónica e que, num futuro próximo, os biomarcadores se tornem úteis na prática clínica, no que refere ao diagnóstico, terapêutica e prognóstico da DPOC. Este trabalho foi elaborado segundo um levantamento equilibrado dos desenvolvimentos mais recentes sobre a importância dos biomarcadores na DPOC, não só pela lacuna científica no que diz respeito aos seus papéis na referida doença, mas também pela actualidade e emergência desta temática. Deste modo, foi feita uma pesquisa alargada na PubMed, com o termo “biomarkers in COPD”. Para melhor compreensão da fisiopatologia, foi realizada uma pesquisa com os termos “pathophysiology”, “imunopathogenesis” e “exacerbation”. Adicionalmente, foi realizada uma pesquisa na direcção electrónica da Sociedade Portuguesa de Pneumologia recorrendo, através desta, a revistas médicas disponíveis. Como modo de conclusão deste trabalho refere-se que, apesar do esforço farmacêutico e biotecnológico e de ensaios clínicos realizados, poucos são os biomarcadores usados rotineiramente na clínica. Além disso, os já identificados através da análise de amostras biológicas, permanecem longe de validação, isto é, de significado clínico. A realização de mais estudos clínicos e o recurso à proteómica vão permitir avançar nesta área, identificando-se novos e robustos biomarcadores, validando os já existentes Recently was estimated that Chronic Obstructive Pulmonary Disease (COPD) will become the third main cause of death in 2020. Recent data point to a predominance of 14,2%. As there is no effective treatment, urges the need to find biomarkers related with the biopathology of this disease, allowing better surveillance and approach. Identify biomarkers that indicate the efficiency of drugs in the inflammatory process components, in order to achieve a better treatment, is crucial to stop both the social-economical and morbimortality impact of this disease. The goal with this review is to study the available biomarkers, their relation with several parameters as clinical ones, disease progression rate, alterations of laboratorial values, co-existance of tobacco habits or any kind of comorbility and the predictive value on exacerbations. It is expected that deep study in this subject may be relevant on understanding the systemic modifications of this chronic inflammatory pathology and that, in a near future, biomarkers become useful in clinical practice when it comes to diagnostic, treatment and prognosis of COPD. This theory was elaborated after a balanced gathering of the most recent developments on how important biomarkers are in COPD, not just because of the scientific gap referring to their role in the disease, but also because of it is an emergent and current thematic. A large PubMed research was made with the words “biomarkers in COPD”. To better understand the physiopathology, a research was made using the words “pathophysiology”. “imunopathogenesis” and “exacerbation”. Additionally, we ran the “Sociedade Portuguesa de Pneumologia” website for medical available magazines. Concluding, despite the pharmaceutical and biotechnological effort and the many clinical studies, only a few biomarkers are currently used in clinical practice. Besides that, those which have been identified by biological samples analysis, are far from validation and this means they still don’t have clinical significance. More clinical studies and proteomic use may allow new advances in this area, identifying new and useful biomarkers and validating the existing ones.
