Abstract

Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. Conclusion: The PREFER in VTE registry will provide valuable insights into the characteristics of patients with VTE and their acute and mid-term management, as well as into drug utilization and the use of health care resources in acute first-time and/or recurrent VTE across Europe in clinical practice. Abstract

SummaryVenous thromboembolism (VTE) is a significant cause of morbidity and mortality in Europe. Data from real-world registries are necessary, as clinical trials do not represent the full spectrum of VTE patients seen in clinical practice. We aimed to document the epidemiology, management and outcomes of VTE using data from a large, observational database. PREFER in VTE was an international, non-interventional disease registry conducted between January 2013 and July 2015 in primary and secondary care across seven European countries. Consecutive patients with acute VTE were documented and followed up over 12 months. PREFER in VTE included 3,455 patients with a mean age of 60.8 ± 17.0 years. Overall, 53.0% were male. The majority of patients were assessed in the hospital setting as inpatients or outpatients (78.5%). The diagnosis was deep-vein thrombosis (DVT) in 59.5% and pulmonary embolism (PE) in 40.5%. The most common comorbidities were the various types of cardiovascular disease (excluding hypertension; 45.5%), hypertension (42.3%) and dyslipidaemia (21.1%). Following the index VTE, a large proportion of patients received initial therapy with heparin (73.2%), almost half received a vitamin K antagonist (48.7%) and nearly a quarter received a DOAC (24.5%). Almost a quarter of all presentations were for recurrent VTE, with >80% of previous episodes having occurred more than 12 months prior to baseline. In conclusion, PREFER in VTE has provided contemporary insights into VTE patients and their real-world management, including their baseline characteristics, risk factors, disease history, symptoms and signs, initial therapy and outcomes.

Abstract

Aims Opinions differ regarding the effectiveness of cardiac pacing in patients affected by reflex syncope. We assessed a standardized guideline-based algorithm in different forms of reflex syncope. Methods and results In this prospective, multi-centre, observational study, patients aged >40 years, affected by severe unpredictable recurrent reflex syncopes, underwent carotid sinus massage (CSM), followed by tilt testing (TT) if CSM was negative, followed by implantation of an implantable loop recorder (ILR) if TT was negative. Those who had an asystolic response to one of these tests received a dual-chamber pacemaker. Population: 253 patients, mean age 70 ± 12 years, median 4 (3–6) syncopes, 89% without or with short prodromes. Of these patients, 120 (47%) received a pacemaker and 106 were followed up for a mean of 13 ± 7 months: syncope recurred in 10 (9%). The recurrence rate was similar in 61 CSM+ (11%), 30 TT+ (7%), and 15 ILR+ (7%) patients. The actuarial total syncope recurrence rate was 9% (95% confidence interval (CI), 6–12) at 1 year and 15% (95% CI, 10–20) at 2 years and was significantly lower than that observed in the group of 124 patients with non-diagnostic tests who had received an ILR: i.e. 22% (95% CI, 18–26) at 1 year and 37% (95% CI, 30–43) at 2 years ( P = 0.004). Conclusion About half of older patients with severe recurrent syncopes without prodromes have an asystolic reflex for which cardiac pacing goes along with a low recurrence rate. The study supports the clinical utility of the algorithm for the selection of candidates to cardiac pacing in everyday clinical practice. Clinical Trial Registration . Abstract

Online social media are changing the news industry and revolutionizing the traditional role of journalists and newspapers. In this scenario, investigating the behaviour of users in relationship to news sharing is relevant, as it provides means for understanding the impact of online news, their propagation within social communities, their impact on the formation of opinions, and also for effectively detecting individual stances relative to specific news or topics.Our contribution is two-fold. First, we build a robust pipeline for collecting datasets describing news sharing; the pipeline takes as input a list of news sources and generates a large collection of articles, of the accounts that provide them on the social media either directly or by retweeting, and of the social activities performed by these accounts. Second, we also provide a large-scale dataset, built using the aforementioned tool, that can be used to study the social behavior of Twitter users and their involvement in the dissemination of news items. Finally we show an application of our data collection in the context of political stance classification and we suggest other potential usages of the presented resources.

Abstract

The recent COVID-19 pandemic came alongside with an “infodemic”, with online social media flooded by often unreliable information associating the medical emergency with popular subjects of disinformation. In Italy, one of the first European countries suffering a rise in new cases and dealing with a total lockdown, controversial topics such as migrant flows and the 5G technology were often associated online with the origin and diffusion of the virus. In this work we analyze COVID-19 related conversations on the Italian Facebook, collecting over 1.5M posts shared by nearly 80k public pages and groups for a period of four months since January 2020. On the one hand, our findings suggest that well-known unreliable sources had a limited exposure, and that discussions over controversial topics did not spark a comparable engagement with respect to institutional and scientific communication. On the other hand, however, we realize that dis- and counter-information induced a polarization of (clusters of) groups and pages, wherein conversations were characterized by a topical lexicon, by a great diffusion of user generated content, and by link-sharing patterns that seem ascribable to coordinated propaganda. As revealed by the URL-sharing diffusion network showing a “small-world” effect, users were easily exposed to harmful propaganda as well as to verified information on the virus, exalting the role of public figures and mainstream media, as well as of Facebook groups, in shaping the public opinion.

Abstract

Applications involving multi-robot systems have been increasing day by day, since they allow one to accomplish complex tasks otherwise impossible for a single unit. Common control approaches for these robot systems are based on distributed architecture, where each robot computes its own control input, only based on local information from onboard sensors or received from its neighbor robots. This means that the failure of one or more agents might jeopardize the task execution. For this reason, fault detection and isolation (FDI) strategies become crucial to accomplish the assigned task in the aforementioned case as well. This chapter presents a distributed fault diagnosis architecture aimed at detecting failures in a team of robots working in tight cooperation. The proposed approach relies on a distributed observer-controller scheme, where each robot estimates the overall system state by means of a local observer, and it uses such an estimate to compute the local control input to achieve a specific task. The local observer is also used to define a set of residual vectors aimed at detecting and isolating faults occurring on any robot of the team, even if there is no direct communication. The approach is validated through experiments where a heterogeneous team of three robots perform a cooperative task.

Abstract

Aplastic anemia (AA) is a serious hematological disorder, which is solely cured by hematopoietic stem cell transplantation (HSCT). Haploidentical HSCT is an emerging modality with encouraging outcomes in several blood conditions. The present study aims to comprehensively assess the feasibility and safety of haploidentical HSCT in patients with severe and very severe AA. It is a systematic review and meta-analysis of studies related to haploidentical stem cell transplantation in idiopathic AA investigating rates of successful engraftment, acute graft-versus-host disease (aGvHD), chronic GvHD (cGvHD), transplant-related mortality (TRM), and posttransplantation viral infections (including cytomegalovirus [CMV]) in patients with AA. The effects of reduced-intensity conditioning (RIC) and nonmyeloablative conditioning (NMA), as well as various GvHD prophylaxis regimens on these outcomes were evaluated. In total 15 studies were identified, (577 patients, 58.9% males), successful engraftment was observed in 97.3% of patients (95% CI, 95.9-98.7) while grades II-IV aGvHD and cGvHD were reported in 26.6% and 25.0%, respectively. The pooled incidence of TRM was 6.7% per year (95% CI, 4.0-9.4). RIC regimens were associated with higher proportions of successful engraftment (97.7% vs 91.7%, P = 0.03) and aGvHD (29.5% vs 18.7%, P = 0.008) when compared with NMA regimens with no differences in cGvHD or mortality incidence. When compared with methotrexate-containing regimens and other regimens, posttransplant cyclophosphamide-containing regimens reduced the rates of aGvHD (28.6%, 27.8%, and 12.8%, respectively, P = 0.02), CMV viremia (55.7%, 38.6%, and 10.4%, respectively, P < 0.001), and CMV disease in initially viremic patients (2.1%, 33.0%, and 0%, respectively, P < 0.001). We have concluded that Haploidentical HSCT was associated with promising outcomes in terms of successful engraftment and reduced complications. Future prospective trials are needed to identify the preferred conditioning regimen, GvHD prophylaxis, and graft source in the setting of haploidentical transplant for AA.

Abstract

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is curative for bone marrow failure in patients with Fanconi anemia (FA), but the presence of a malignant transformation is associated with a poor prognosis and the management of these patients is still challenging. We analyzed outcome of 74 FA patients with a diagnosis of myelodysplastic syndrome (n = 35), acute leukemia (n = 35) or with cytogenetic abnormalities (n = 4), who underwent allo-HSCT from 1999 to 2016 in EBMT network. Type of diagnosis, pre-HSCT cytoreductive therapies and related toxicities, disease status pre-HSCT, donor type, and conditioning regimen were considered as main variables potentially influencing outcome. The 5-year OS and EFS were 42% (30-53%) and 39% (27-51%), respectively. Patients transplanted in CR showed better OS compared with those transplanted in presence of an active malignant disease (OS:71%[48-95] vs 37% [24-50],P = .04), while none of the other variables considered had an impact. Twenty-two patients received pre-HSCT cytoreduction and 9/22 showed a grade 3-4 toxicity, without any lethal event or negative influence on survival after HSCT(OS:toxicity pre-HSCT 48% [20-75%] vs no-toxicity 51% [25-78%],P = .98). The cumulative incidence of day-100 grade II-IV a-GvHD and of 5-year c-GvHD were 38% (26-50%) and 40% (28-52%). Non-relapse-related mortality and incidence of relapse at 5-years were 40% (29-52%) and 21% (11-30%) respectively, without any significant impact of the tested variables. Causes of death were transplant-related events in most patients (34 out of the 42 deaths, 81%). This analysis confirms the poor outcome of transformed FA patients and identifies the importance of achieving CR pre-HSCT, suggesting that, in a newly diagnosed transformed FA patient, a cytoreductive approach pre-HSCT should be considered if a donor have been secured.

Abstract

In this paper a scheme for sensor fault diagnosis in chemical batch reactors is proposed. The scheme is based on a bank of two observers aimed at generating a set of residuals sensitive to faults occurrence. The observers are designed via a H∞ approach, so as to guarantee faults detection and isolation even in the presence of external disturbances and modeling errors. The heat released by the reaction is estimated via an universal approximator, based on radial basis functions, whose weights are adjusted on-line. Finally, the estimates provided by the observers and the sensor measures are processed to provide information about the faulty sensor as well as an healthy measure. Abstract

Aim The aim of this study was to determine the safety and the efficacy of paediatrician-administered propofol in children undergoing different painful procedures. Methods We conducted a retrospective study over a 12-year period in three Italian hospitals. A specific training protocol was developed in each institution to train paediatricians administering propofol for painful procedures. Results In this study, 36 516 procedural sedations were performed. Deep sedation was achieved in all patients. None of the children experienced severe side effects or prolonged hospitalisation. There were six calls to the emergency team (0.02%): three for prolonged laryngospasm, one for bleeding, one for intestinal perforation and one during lumbar puncture. Nineteen patients (0.05%) developed hypotension requiring saline solution administration, 128 children (0.4%) needed O2 ventilation by face mask, mainly during upper endoscopy, 78 (0.2%) patients experienced laryngospasm, and 15 (0.04%) had bronchospasm. There were no differences in the incidence of major complications among the three hospitals, while minor complications were higher in children undergoing gastroscopy. Conclusion This multicentre study demonstrates the safety and the efficacy of paediatrician-administered propofol for procedural sedation in children and highlights the importance of appropriate training for paediatricians to increase the safety of this procedure in children.

30. Sensor fault detection and isolation for robot manipulators

Abstract

This paper deals with the problem of detecting and isolating sensor faults in industrial robot manipulators. To the purpose, an analytical redundancy approach has been pursued, based on a bank of state observers for residual generation. An extended H∞ approach is adopted to build the bank of residual generators; the compensation of poorly known dynamics in each observer is improved by the use of a neural network. The synthesis of the observer gains is achieved by solving an LMI feasibility problem, where constraint on the position of the estimation error linearized dynamics poles in the complex plane are taken into account. Finally, in order to test the effectiveness of the proposed approach, a case study is presented, based on experimental data collected on a six-degree-of-freedom Comau Smart-3 S industrial manipulator. ©2005 IEEE.
