Abstract

The tyrosine-protein phosphatase nonreceptor type 11 (PTPN11) is an important regulator of RAS signaling and frequently affected by mutations in patients with acute myeloid leukemia (AML). Despite the relevance for leukemogenesis and as a potential therapeutic target, the prognostic role is controversial. To investigate the prognostic impact of PTPN11 mutations, we analyzed 1529 adult AML patients using next-generation sequencing. PTPN11 mutations were detected in 106 of 1529 (6.93%) patients (median VAF: 24%) in dominant (36%) and subclonal (64%) configuration. Patients with PTPN11 mutations were associated with concomitant mutations in NPM1 (63%), DNMT3A (37%), and NRAS (21%) and had a higher rate of European LeukemiaNet (ELN) favorable cytogenetics (57.8% vs 39.1%; P < .001) and higher white blood cell counts (P = .007) compared with PTPN11 wild-type patients. In a multivariable analysis, PTPN11 mutations were independently associated with poor overall survival (hazard ratio [HR]: 1.75; P < .001), relapse-free survival (HR: 1.52; P = .013), and a lower rate of complete remission (odds ratio: 0.46; P = .008). Importantly, the deleterious effect of PTPN11 mutations was confined predominantly to the ELN favorable-risk group and patients with subclonal PTPN11 mutations (HR: 2.28; P < .001) but not found with dominant PTPN11 mutations (HR: 1.07; P = .775), presumably because of significant differences within the rate and spectrum of associated comutations. In conclusion, our data suggest an overall poor prognostic impact of PTPN11 mutations in AML, which is significantly modified by the underlying cytogenetics and the clonal context in which they occur.

Abstract

The Infectious Diseases Working Party (AGIHO) of the German Society of Hematology and Oncology (DGHO) here presents its updated recommendations for the treatment of documented fungal infections. Invasive fungal infections are a main cause of morbidity and mortality in cancer patients undergoing intensive chemotherapy regimens. In recent years, new antifungal agents have been licensed, and agents already approved have been studied in new indications. The choice of the most appropriate antifungal treatment depends on the fungal species suspected or identified, the patient’s risk factors (e.g., length and depth of neutropenia), and the expected side effects. This guideline reviews the clinical studies that served as a basis for the following recommendations. All recommendations including the levels of evidence are summarized in tables to give the reader rapid access to the information.

Abstract

Der erste Teil der Habilitationsschrift widmet sich zytogenetischen Veränderungen in hochaufgereinigten aberranten Plasmazellen sowie der Distribution und dem Homing verschiedener Zell-Kompartimente beim Multiplen Myelom und MGUS. Die Koexistenz von aberranten Plasmazellen mit und ohne rekurrierende IGH-Translokationen wie z.B. t(11;14) lässt hierbei annehmen, dass auch diese chromosomalen Veränderungen entgegen der bisherigen Hypothese im Krankheitsverlauf sekundär auftreten können. Weiterhin konnten wir zeigen, dass die Expression von CD117 und CXCR4 auf der Oberfläche von aberranten Plasmazellen mit einer wesentlich alterierten Distribution myeloischer und lymphatischer Zell-Kompartimente bei verschiedenen Plasmazell-Dyskrasien einhergeht. Die dargelegten Untersuchungsergebnisse zu neuen Therapiemöglichkeiten bei Plasmazell-Dyskrasien zeigen, dass das neue Alkylphospholipid Perifosin in vitro Apoptose induziert und deshalb in klinischen Studien zur Therapie des Multiplen Myeloms weiter evaluiert werden sollte. Eine Konditionierung mit Fludarabin Treosulfan vor allogener Stammzelltransplantation ist bei Patienten mit Multiplem Myelom prinzipiell machbar und geht meist mit einem frühen kompletten hämatopoietischen Chimärismus einher. Virale Infektionen und Erkrankungen - insbesondere durch CMV - stellen jedoch auch heutzutage eine häufige Komplikation bei Patienten nach allogener Stammzelltransplantation dar., The first section of the presented work describes cytogenetic abnormalities in highly purified aberrant plasma cells and the distribution of different cell subsets in multiple myeloma and MGUS. The coexistence of aberrant plasma cells with and without recurrent IGH translocations (e.g. t(11;14)) led to assume that also these cytogenetic abnormalities might correspond to secondary genetic events, which contrasts the current hypothesis. Furthermore, we found that the expression of CD117 and CXCR4 on the surface of aberrant plasma cells is associated with a markedly altered distribution of different lymphatic and myeloid cell compartments in plasma cell dyscrasias. The presented studies on novel treatment approaches in multiple myeloma suggest that the novel alkylphospholipid perifosine induces apoptosis in vitro and thus should be evaluated further in clinical trials as a treatment option for multiple myeloma. Conditioning with fludarabine and treosulfan prior to allogeneic stem cell transplantation is feasible and results frequently in an early complete hematopoietic chimerism. However, viral infections and diseases, mainly caused by CMV, are still a frequent complication in patients undergoing allogeneic stem cell transplantation.
